        Orthologous Allergens and Diagnostic Utility of Major Allergen Alt a 1

        Purpose: Hypersensitivity to fungi is associated with rhinoconjunctivitis and asthma. For some fungi, such as Alternaria alternata (A. alternata), the symptoms of asthma are persistent, increasing disease severity and the risk of fatal outcomes. There are a large number of species of fungi but knowledge of them remains limited. This, together with the difficulties in obtaining adequate standardized extracts, means that there remain significant challenges in the diagnosis and immunotherapy of allergy associated with fungi. The type of indoor fungi related to asthma/allergy varies according to geographic, climatic, and seasonal factors, making their study difficult. The aim of this study was to determine hypersensitivity to indoor fungi in a population from Cuenca, Spain. Methods: Thirty-five patients with symptoms compatible with rhinitis or asthma who showed clear worsening of their symptoms in their homes or workplace were included. In vivo and in vitro tests were made with a battery of fungal allergens, including the species isolated in the home or workplace. Results: Ulocladium botrytis (U. botrytis) and A. alternata were the most representative species as a source of home sensitization. These species showed very high concordance in skin tests, specific IgE, and histamine release. The allergen Alt a 1, which was recognized in all patients, was detected in A. alternata, U. botrytis, and Stemphylium botryosum (S. botryosum). Conclusions: U. botrytis and A. alternata were the most representative species as a source of home sensitization. Alt a 1 was recognized in all patients and may be considered a non-species-specific allergen that could be used as a diagnostic source of sensitization to some species of the Pleosporaceae family.

        Perception of Discipline According to Gender, Type of School, Sport Activity and Interest in Physical Education in Spanish Students

        The purpose of this study was to compare the differences between different groups (student`s gender, type of school, interest in physical education (PE) and sport activity outside school hours), based on the student`s disciplined/undisciplined behaviour in the PE class. The sample was formed by 456 students aged between 14 and 16, from the 3rd and 4th ESO (compulsory secondary education) course in the Region of Murcia (Spain). The study was performed in the months of the third school term and the questionnaire was handed out in the PE hall. The scales used for the study were the Reasons for Discipline Scale (RDS) and Strategies to Sustain Discipline Scale (SSDS), both created by Papaioannou (1998). The results obtained from the univariate analyses showed that girls were more disciplined and perceived more intrinsic reasons in their teachers for maintaining discipline than boys, who demonstrated more undisciplined behaviour and perceived more reasons of indifference or introjection in their teachers for maintaining discipline. On the other hand, students that liked PE and did sport were more disciplined and perceived more intrinsic reasons in their teachers for maintaining discipline.

        Goal Orientations, Motivational Climate, Discipline and Physical Self-Perception Related to the Teacher`s Gender, Satisfaction and Sport Activity of a Sample of Spanish Adolescent Physical Education Students

        The aim of this research was to find possible relationships between goal orientations, motivational climate, physical self-perception and discipline and the teacher`s gender, satisfaction with physical education and sport activity outside school hours in Spanish physical education students. The sample consisted of 565 students aged between 12 and 16. Participants completed the following scales: Goal Orientations, Motivational Climate in Physical Education Classes, Disciplined-Undisciplined Behaviour and Physical Self-Perception Profile. Correlations were raised among the discipline-indiscipline behaviours, perceptions of the motivational climate, goal orientations and physical self-perceptions. The results showed that students who have a female teacher have higher task-involving motivational climate, discipline, body attractiveness, sport competence, physical condition and self-confidence than those that have a male teacher. Those that are satisfied with physical education have higher task-involving motivational climate, task orientation, discipline, body attractiveness, sport competence, physical condition and self-confidence than those that are not satisfied. Exercisers have higher ego orientation, task orientation, body attractiveness, sport competence, physical condition and self-confidence than non-exercisers.

        Patient-Reported Outcomes of Minimally Invasive versus Open Transforaminal Lumbar Interbody Fusion for Degenerative Lumbar Disc Disease: A Prospective Comparative Cohort Study

        Background: Comparative outcomes of minimally invasive transforaminal lumbar interbody fusion (MI-TLIF) and traditional open TLIF (O-TLIF) for degenerative lumbar disc disease have been poorly studied. The purpose of this study was to prospectively compare the outcomes between MI-TLIF and O-TLIF for patients with a degenerative disc disease, focusing on the functional capacity of patients in daily life. Methods: A prospective cohort study was performed, comparing 54 patients who underwent O-TLIF and 55 patients who underwent MI-TLIF with a follow-up of 4 years. Clinical evaluation was performed using the Oswestry Disability Index (ODI), 36-item short form survey (SF-36), and a visual analog scale for pain (VAS pain). Radiological evaluation was also performed. Results: At the final follow-up, compared with O-TLIF, MI-TLIF was associated with significantly better intraoperative results, including similar operative time (p = 0.246), lower estimated blood loss (p = 0.001), and shorter hospital stay (p = 0.001). The final ODI score was significantly better in the MI-TLIF group (p = 0.031). The SF-36-physical (p = 0.023) and VAS pain (p = 0.024) scores were significantly better in the MI-TLIF group. There was no significant difference in the fusion rate (p = 0.747). Conclusions: The MI-TLIF technique is an effective and safe procedure for degenerative lumbar disc disease. Compared to traditional O-TLIF, MI-TLIF was associated with less disability and higher quality of life, with a low rate of intraoperative and postoperative complications.

        Effect of cholesterol and triglycerides levels on the rheological behavior of human blood

        Important public health problems worldwide such as obesity, diabetes, hyperlipidemia and coronary diseases are quite common. These problems arise from numerous factors, such as hyper-caloric diets, sedentary habits and other epigenetic factors. With respect to Mexico, the population reference values of total cholesterol in plasma are around 200 mg/dL. However, a large proportion has higher levels than this reference value. In this work, we analyze the rheological properties of human blood obtained from 20 donors, as a function of cholesterol and triglyceride levels, upon a protocol previously approved by the health authorities. Samples with high and low cholesterol and triglyceride levels were selected and analyzed by simple-continuous and linear-oscillatory shear flow. Rheometric properties were measured and related to the structure and composition of human blood. In addition, rheometric data were modeled by using several constitutive equations: Bautista-Manero-Puig (BMP) and the multimodal Maxwell equations to predict the flow behavior of human blood. Finally, a comparison was made among various models, namely, the BMP, Carreau and Quemada equations for simple shear rate flow. An important relationship was found between cholesterol, triglycerides and the structure of human blood. Results show that blood with high cholesterol levels (400 mg/dL) has flow properties fully different (higher viscosity and a more pseudo-plastic behavior) than blood with lower levels of cholesterol (tendency to Newtonian behavior or viscosity plateau at low shear rates)

        Usability and Evaluation of a Deployed 4G Network Prototype

        This article presents a field evaluation of an IP-based architecture for heterogeneous environments that has been developed under the aegis of the Moby Dick project, covering UMTS-like (universal mobile telecommunications system) TD-CDMA (time division-code division multiple access) wireless access technology, wireless and wired LANs. The architecture treats all transmission capabilities as basic physical and data-link layers, and replaces all higher-level tasks by IP-based strategies. The Moby Dick architecture incorporates mobile IPv6, fast handovers, AAA-control (authentication, authorisation, accounting), charging and quality of service (QoS) in an integrated framework. The architecture further allows for optimised control on the radio link layer resources. It has been implemented and tested by expert users, and evaluated by real users on field trials with multiple services available.

        Periodontal granulation tissue preservation in surgical periodontal disease treatment: a pilot prospective cohort study

        Purpose: The aim of this study was to evaluate the clinical outcomes of periodontal granulation tissue preservation (PGTP) in access flap periodontal surgery. Methods: Twenty patients (stage III–IV periodontitis) with 42 deep periodontal pockets that did not resolve after non-surgical treatment were consecutively recruited. Access flap periodontal surgery was modified using PGTP. The clinical periodontal parameters were evaluated at 9 months. The differences in the amount of granulation tissue width (GTw) preserved were evaluated and the influence of smoking was analyzed. Results: GTw >1 mm was observed in 97.6% of interproximal defects, and the granulation tissue extended above the bone peak in 71.4% of defects. At 9 months, probing pocket depth reduction (4.33±1.43 mm) and clinical attachment gain (CAG; 4.10±1.75 mm) were statistically significant (P<0.001). The residual probing depth was 3.2±0.89 mm. When GTw extended above the interproximal bone peak (i.e., the interproximal supra-alveolar granulation tissue thickness [iSUPRA-GT] was greater than 0 mm), a significant CAG was recorded in the supra-alveolar component (1.67±1.32 mm, P<0.001). Interproximal gingival recession (iGR) was significant (P<0.05) only in smokers, with a reduction in the interdental papillary tissue height of 0.93±0.76 mm. In non-smokers, there was no increase in the iGR when the iSUPRA-GT was >0 mm. The clinical results in smokers were significantly worse. Conclusions: PGTP was used to modify access flap periodontal surgery by preserving affected tissues with the potential for recovery. The results show that preserving periodontal granulation tissue is an effective and conservative procedure in the surgical treatment of periodontal disease.

      • Expectational Stability in Multivariate Models

        This paper shows that the concept of Expectational stability (E-stability) in a multivariate framework is inherently model-dependent. Whereas a Rational Expectations equilibrium (REE) is subject to model-specific parameter restrictions from the economic model at hand, a perceived law of motion (PLM) is postulated without such restrictions because economic agents are not likely to know the restrictions a priori. Therefore, an unrestricted PLM is in general overparameterized relative to an REE of interest in multivariate models even when the functional form is the same as the REE. Since E-stability necessarily reflects model-specific extents of overparameterization, it is model-dependent as well. Consequently, E-stability is not directly comparable across multivariate models or different representations of a given model. This implies that one may draw different conclusions on E-stability of an REE to one model under alternative representations of the same model and the REE. Our finding is independent of the information structure, the stationarity of solutions or the determinacy of a given model. We provide several economic examples analyzed in the literature. In particular, we show that a fundamental solution to a canonical New-Keynesian model can be E-stable or not depending on representations even in the case of determinacy.

        Identification and Tuning Methods for PI Control Systems Based on Symmetric Send-on-delta Sampling

        Two procedures for the definition of an autotuning algorithm for event-based proportional-integral (PI) control systems are proposed in this contribution. Firstly, a method for the identification of the most common transfer functions used for tuning PI controllers is explained. The rationale of the identification method is based on the limit cycles that the event-based sampler and a convenient tuning of the PI controller can produce in the closed loop. The identification method is designed for controller tuning, that is, fitting the behaviour of the process in the range of frequencies between ω−180◦ and ω−135◦ . Secondly, a new method for tuning the event-based PI controller is explained. The main control objective of the new tuning rule is to generate a set of parameters that introduces the process in a limit cycle at a user-specified inter-event time. However, by introducing an oscillation margin in the tuning rule, the method generalizes to produce controller parameters that avoid limit cycles with a certain robustness margin. Simulations demonstrate the effectiveness of the tuning rule to force the system to oscillate at a specified frequency as a consequence of the events triggered by the event-based sampler.

        Drug Treated Schizophrenia, Schizoaffective and Bipolar Disorder Patients Evaluated by qEEG Absolute Spectral Power and Mean Frequency Analysis

        Objective: Research of electroencephalograph (EEG) power spectrum and mean frequency has shown inconsistent results inpatients with schizophrenic, schizoaffective and bipolar disorders during medication when compared to normal subjects thus;the characterization of these parameters is an important task. Methods: We applied quantitative EEG (qEEG) to investigate 38 control, 15 schizophrenic, 7 schizoaffective and 11 bipolar disordersubjects which remained under the administration of psychotropic drugs (except control group). Absolute spectral power (ASP),mean frequency and hemispheric electrical asymmetry were measured by 19 derivation qEEG. Group mean values were comparedwith non parametrical Mann-Whithney test and spectral EEG maps with z-score method at p<0.05. Results: Most frequent drug treatments for schizophrenic patients were neuroleptic+antiepileptic (40% of cases) or 2 neuroleptics(33.3%). Schizoaffective patients received neuroleptic+benzodiazepine (71.4%) and for bipolar disorder patients neuroleptic+antiepileptic (81.8%). Schizophrenic (at all derivations except for Fp1, Fp2, F8 and T6) and schizoaffective (only at C3)show higher values of ASP (+57.7% and +86.1% respectively) compared to control group. ASP of bipolar disorder patients didnot show differences against control group. The mean frequency was higher at Fp1 (+14.2%) and Fp2 (+17.4%) in bipolar disorderpatients than control group, but no differences were found in frequencies between schizophrenic or schizoaffective patientsagainst the control group. Majority of spectral differences were found at the left hemisphere in schizophrenic and schizoaffectivebut not in bipolar disorder subjects. Conclusion: The present report contributes to characterize quantitatively the qEEG in drug treated schizophrenic, schizoaffectiveor bipolar disorder patients.
