증례 : 호흡기 ; 결핵성 흉막염을 동반한 갑상선 유두암의 폐전이 1예

저자들은 70세 여자 환자가 흉수소견으로 내원하여 흉수 천자 및 늑막 조직검사를 시행하여 결핵성 흉막염이 진단되었으나, 아데노신 탈아미노효소가 2.4 IU/L로 낮았고, 흉부 전산화단층촬영상 우하엽에 폐종괴 소견과 양측 미세결절이 관찰되었다. 경흉 세침흡입 세포검사에서 5년 전 전절제술을 받았던 갑상선 유두암의 폐전이가 진단되어, 결핵성 흉막염을 동반한 갑상선 유두암의 폐전이 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Papillary thyroid carcinoma is the most prevalent type of thyroid cancer. Metastasis of this carcinoma commonly occurs in the lung and has been reported to present 5 to 30 years after the initial diagnosis of thyroid cancer. Pleural effusion can mask this metastatic spread. In patients with pleural effusion, laboratory measurement of the adenosine deaminase (ADA) level in the pleural fluid can be effective in identifying and ranking pulmonary metastasis in the differential diagnosis. A 70-year-old female patient visited our hospital with dyspnea. She presented with a considerable amount of pleural effusion. A pleural biopsy was performed and the Ziehl-Neelsen stains revealed a few acid-fast bacilli, but the ADA level in the pleural fluid was 2.4 IU/L. After drainage of the pleural effusions, we discovered pulmonary nodules by computed tomography (CT), which were later diagnosed through histologic examination as pulmonary metastasis from papillary thyroid carcinoma. 우주 구조물을 위협하는 여러 요소들 중 MMOD(MicroMeteoroid and Orbital Debris)는 약 8~70km/s의 속도로 우주 구조물과 충돌하여 큰 피해를 주고 있다. 이러한 피해로부터 우주 구조물을 보호하기 위해서 현재 다양한 위플 쉴드가 연구, 적용되고 있다. 위플 쉴드에는 알루미늄이 주로 사용되고 있지만, 복합재료의 사용도 증가하고 있어 본 연구에서는 알루미늄과 복합재료의 초고속 충돌 특성을 유한 요소 해석을 통해 비교하였다. 충격체는 직경 5.5mm인 알루미늄 2017-T4의 구를 사용하였고, 알루미늄 평판은 6061-T6, CFRP 평판은 T300/5208을 사용하였다. 충격체의 초기 충돌 속도는 1km/s이다. 충돌 후 충격체의 운동에너지는 알루미늄 평판의 경우 약 64J 감소하였고, CFRP 평판의 경우 약 63J 감소하였다. 비슷한 충돌 에너지 흡수 정도를 보이고 있지만, 밀도를 비교해 보았을 때 CFRP가 약 1.7배 가볍기 때문에 방탄 특성이 더 좋다고 할 수 있다. Among the factors that threaten spacecraft, Micrometeoroid and Orbital Space Debris(MMOD) cause damage to spacecraft and impact velocity is about 8~70km/s. Nowadays, various Whipple Shield are studied and applied to protect spacecraft. As the materials used to Shielding System, aluminum is usually used but composite is also used increasingly. So this study compared characteristics of hypervelocity impact of Aluminum and composites through finite element analysis. The Projectile was a spherical shape using Aluminum 2017-T4, and aluminum plate was using Aluminum 606l-T6, CFRP plate was using T300/5208. Initial impact velocity of projectile was 1km/s. As a result, kinematic energy of projectile decreased to about 64J and about 63J for aluminum plate and CFRP plate, respectively after impact. Although both results is almost same about the absorption of impact energy, you can think the CFRP has good ballistic characteristic, because CFRP is lighter about 1.7 times compared with density of aluminum.

The co-existence of mouth and genital ulceration with iritis was first defined as a syndrome by Behcet. It is now known to be a chronic and relapsing systemic disease, which involves many other systems. Neurogenic bladder dysfunction due to Behcet's disease, however, is extremely rare, and only a limited number of cases have been reported in the international literature. Voiding disfunction in Behcet's disease can be due to neurological or direct bladder involvement and can be related to bladder and sphmctehc components in both phases of micturition. We report on a 35-year old patient with a neuropathic bladder secondary to Behcet's disease.
뇌졸증 후유증 환자에 대한 Idebenone의 효과와 안전성에 관한 연구

To evaluate the clinical efficacy and safety of Idebenone in 19 patients with the sequelae of the Cerebrovascular accidents(CVA). we conducted an open label trial of Idebenone for 8 weeks. The protocol consists of baseline. 4 weeks and 8 weeks evaluation. We evaluated psychic signs. subjective symptoms. activities of daily life and neurologic signs. The results were as follows : 1. Eight weeks after the trial. the majority of subjects(73.7%) were improved slightly or better. 2. At final evaluation, the slight improvement or better of psychic signs, subjective symptoms, activities of daily life and neurologic signs were 75.0%, 60.0%, 79.7% and 70.0%, respectively. 3. The adverse reactions were observed in 2 cases. one was headache and the other Gl trouble. There also were noticed transient elevation of SGOT/SGPT in one case. According to these results, it was concluded that administration of Idebenone after the acute phase of CVA is effective and safe.
