        항인지질 증후군과 연관된 허혈성 대장염

        There are various causes of ischemic colitis. It can be largely divided into occlusive and non-occlusive causes: thrombus formation in the superior or inferior mesenteric arteies secondary to atherosclerotic vascular lesion is one of the most frequent occlusive causes; hypotensive state due to causes such as hemorrhage is one of common examples of non-occlusive causes. Antiphospholipid antibody is associated with numerous thromboembolic phenomena. We report a case of elderly male who presented with ischemic colitis and was diagnosed as having antiphospholipid syndrome later. He was positive for lupus anticoagulant, showed mesenteric venous thrombosis on abdominal CT, and experienced acalculous cholecystitis during the hospital course. The patient`s clinical, laboratory, and pathologic findings are consistent with ischemic colitis associated with antiphospholipid syndrome. (Korean J Gastroenterol 2003;41:504-508)

        행정조직에서 조직문화 및 하위문화의 비교

        본 연구의 목적은 행정조직에서 조직문화 및 하위문화의 존재와 하위문화 특성간의 차이 여부를 밝히는 데 있다. 조직문화 및 하위문화를 개인주의·집단주의, 권위주의·민주주의 차원으로 분석한 결과, 현재의 조직문화는 집단주의적이고 권위주의적이라는 평가가 강하나, 신념 차원에서 보면 현재의 조직문화에 대한 평가보다는 개인주의적이고 민주주의적 신념을 보이고 있다는 것이다. 우리나라의 행정문화 연구는 그간 문화적 특성을 단일한 것으로 가정하여 왔기 때문에 본 연구에서 다양한 하위문화가 존재하는가를 살펴본 결과 연령, 직렬, 직급별로 분명한 문화적 차이가 있는 것으로 나타났다. 즉, 기관내 집단간 하위문화 존재 여부를 규명한 결과 개인주의·집단주의 문화차원에서 연령, 직렬, 직급별로 유의한 차이를 보이고 있으나, 기관별로는 차이를 보이지 않았다. 또한 권위주의·민주주의 차원에서는 연령, 직렬, 직급별로는 유의한 차이가 없으나, 기관별로는 차이를 보이고 있었다. The purpose of this study is to determine whether organization culture and subculture exist in administrative organizations, and whether there are differences between the characteristics of organizational cultures and subcultures. Our studies of the culture of administration have focused on assuming that cultural characteristics are straightforward and simple. Thus, the study tried to examine whether various subcultures exist, and the results were as follows. As for the cultural aspects of individualism and collectivism, there were significant differences by age, place of duty, and official post; there was no difference by place of employment. As for the cultural aspects of authoritarianism and democracy, significant differences were seen among the variables as well.

        담낭의 운동장애로서 과잉수축의 임상적 의의는 ?

        N/A At present, the gallbladder dysfunction implies a disorder of decreased gallbladder contractility. Other motor disorder such as overactive gallbladder which shows excessive contraction cannot be excluded in the motility disorder of gallbladder. Thus, this study was done to define the diagnostic criteria and to develop the techniques to induce the excessive contraction of gallbladder. Methods: CCK-op at 20 ng/kg by slow continuous infusion for 30minutes, that is known as most physiologic method for gallbladder contraction, was given for assessment of gallbladder emptying in 12 normal volunteers. Also, rapid bolus injection of cholecystokinin octapeptide (CCK-op) at 20 ng/kg or 40 ng/kg was performed to induce the excessive contraction of gallbladder. Gallbladder contractility was represented as the ejection fraction (GBEF) measured by cholecystokinin- cholescintigraphy. Results: 1. With a slow continuous infusion of CCK-op, the mean GBEF was 78.2±5.6% (mean±SD). 2. With a rapid bolus injection of CCK-op, GBEF showed variable results (10-86%) among subjects who had normal gallbladder. 3. Based on the results obtained by slow continuous infusion of CCK-op in normal volunteers, overactive gallbladder was defined when GBEF approached more than 70% within 15minutes after bolus injection of CCK-op. The overactive gallbladder was noted in 6 (50%) subjects who received rapid bolus injection of CCK-op (40 ng/kg). 4. Abdominal pain developed only in high-dose (40 ng/kg) bolus injection group (6/12, 50%), concomitantly with increased bowel movements, irrespective of excessive gallbladder contractility. Conclusion: Excessive gallbladder contraction had no clinical significance in the experimentally induced clinical model.(Korean Journal of Gastrointestinal Motility 2002;8:37-43)

        행정조직문화와 성과관리의 관계

        본 연구의 목적은 행정조직에서 조직문화의 존재와 성과관리간 관계를 밝히는데 있다. 조직문화를 개인주의·집단주의, 권위주의·민주주의 차원으로 분석한 결과, 현재의 조직문화는 집단주의적이고 권위주의적이라는 평가가 강하나, 신념 차원에서 보면 현재의 조직문화에 대한 평가보다는 개인주의적이고 민주주의적 신념을 보이고 있다는 것이다. 다음으로 행정조직문화와 성과관리의 관계를 분석한 결과를 보면, 현재의 조직은 집단주의적, 권위주의적이면서 하향평가, 인간관계중심의 평가, 결과중심의 평가를 시행하고 있고, 개인보다는 팀단위의 성과를 기준으로 한 성과급지급을 지지하며, 연고기준의 승진제도를 지지하고 있다. 그러나, 개인주의적, 민주주의적 신념을 가질수록 다면평가, 업적중심의 평가, 평가점수의 공개, 개인능력 기준의 성과평가를 선호하며, 개인단위의 성과급 지급을 선호하고, 성과기준의 승진제도를 선호하고 있었다. 따라서 향후에는 개인주의, 민주주의 차원에서 성과관리를 설계하여 도입하여야 할 것으로 생각된다. The purpose of this study was to clarify relationship between characteristics of organizational culture in government organization and performance management. This study surveyed characteristics of culture with dividing it into two views Individualism-Collectivism, Authoritarianism-Democracy, and performance management with dividing it into three views performance evaluation, merit pay and promotion. In results of the analysis, respondents judged existing public organizational culture had tendency to Collectivism and Authoritarianism. On the contrary, their faith showed that tendency to Individualism and Democracy was stronger than those of existing organizations. The results of analysis on relationship between characteristics of administrative organizational culture and performance management are as follows. In results of analysis on relationship between culture of administrative organization and performance management, respondents judged existing public organizational culture was collectivism and authoritarianism and existing public organization performed top-down evaluation, human relation-based evaluation, results-based evaluation, team-based merit pay system, personal connection-based promotion. On the contrary, respondents who had faith in individualism and democracy preferred firstly public opening of evaluation score and criterion of individual ability in factor of performance evaluation, secondly merit pay based on unit on individual in factor of merit pay and finally criterion of performance in factor of promotion.

        H. pylori 제균으로 완전 관해된 위의 고도 MALT 림프종 1예 : a case report

        위의 저도 MALT 림프종은 H. pylori 제균 후에 60∼90%가 관해를 보이는 것으로 알려져 있으나 고도 MALT 림프종은 H. pylori 제균 만으로는 치료가 부족한 것으로 알려져 있다. 그러나 최근에 위의 고도 MALT 림프종에서도 H. pylori 제균 치료를 통해 관해에 이른 사례들이 보고 되었고, 저자들도 고도 MALT 림프종 환자에서 H. pylori 제균 치료를 통하여 내시경적 및 조직학적 완전 관해에 도달한 증례를 경험하여 이를 보고하는 바이다. Low grade MALT lymphoma of stomach is associated with H. pylori infection in more than 90% of cases, and eradication of H. pylori leads to regression of the low grade MALT lymphoma in 60∼90% of cases. On the contrary, high grade MALT lymphoma is thought to be independent from H. pylori for growth and usually is thought to require antitumor chemotherapy. However, there have been recent reports of high grade MALT lymphoma regressing after H. pylori eradication. We experienced and are reporting a case of high grade MALT lymphoma arising in the background of low grade MALT lymphoma that showed complete regression after H. pylori eradication.
