        추체아전절제술과 피하 이식형 골도 보청기(BonebridgeTM)을 동시에 시행한 편측성 난청 환자 1예

        Subtotal petrosectomy (STP) is an effective treatment modality for recurrent suppurative otitismedia (CSOM) and cochlear implant (CI) may be combined for auditory rehabilitation. An activetranscutaneous bone conduction implantation system, known as BonebridgeTM (BB) (MED-EL),is indicated for conductive or mixed hearing loss as well as for single-side deafness, but no casesof BB implantation during STP have been reported. A 37-year-old woman who had previouslyundergone radical mastoidectomy visited our clinic. The left side was deaf and the CT scanshowed total ossification of the cochlea, indicating that CI was not possible. STP was performedand bone conduction-floating mass transducer was placed at a sinodural angle. A month later,the sound processor was applied successfully without any complications. This case suggeststhat BB implantation during STP may be another effective therapeutic option for CSOM patientswho have difficulty undergoing CI procedure.

        Verruca vulgaris of the larynx (VVL) is an extremely rare disease that can be easily misdiagnosed as papillary keratosis, squamous papilloma or verrucous carcinoma, which would have led to needless operation. We experienced a case of VVL in a 66-year-old man with 1-year history of progressive hoarseness. Whitish lesions in both vocal cords were observed by using indirect laryngoscopy and completely removed via microlaryngeal surgery with $CO_2$ laser.

        Contralateral Routing of Signals 보청기 사용자의 보청기 기능이득 확인을 위해이비인후과 진료 현장에서 적용 가능한 음장검사 방법: 탐색적 연구

        Background and Objectives The hearing in noise test (HINT) or sound localization test(SLT) is used as the standardized assessment for contralateral routing of signals (CROS) andbilateral microphones with contralateral routing of signals (BiCROS) hearing aids (HAs). However, implementing HINT or SLT can be challenging in most ENT clinics. In this study,we propose a new feasible method specifically designed for conducting a sound field hearingtest for CROS and BiCROS HA users. Subjects and Method CROS/BiCROS HA users who visited Seoul St. Mary’s hospital be-tween 2019 and 2021 were enrolled for the study. SLT was conducted in a sound-proof roomequipped with two speakers positioned at a 45° angle from the patient on each side: noise stim-uli were presented to the better ear the headphone patient was wearing while signals were pre-sented to the worse ear via the speakers in the room. To validate the sound field test, a correla-tion analysis was conducted using the Bern benefit in single-sided deafness and the Koreanversion of the International Outcome Inventory for Hearing Aids (K-IOI-HA) Questionnaires. Results The aided speech discrimination score was significantly higher in the signal 60 dBHL/masking 50 dB HL condition than 50 dB HL/masking 50 dB HL condition ( p<0.05). Asignificant correlation was found between the gain of sound field speech audiometry and theK-IOI-HA questionnaire ( p<0.05). Conclusion Our novel sound field hearing test for CROS/BiCROS HA users, which pres-ents appropriate noise stimuli via headphones to the better ear, appears to be a feasible ap-proach for CROS/BiCROS HA users.

        합성곱 신경망 기반 채점 모델 설계 및 적용을 통한 운동학 그래프 답안 자동 채점

        This study explores the possibility of automated scoring for scientific graph answers by designing an automated scoring model using convolutional neural networks and applying it to students’ kinematics graph answers. The researchers prepared 2,200 answers, which were divided into 2,000 training data and 200 validation data. Additionally, 202 student answers were divided into 100 training data and 102 test data. First, in the process of designing an automated scoring model and validating its performance, the automated scoring model was optimized for graph image classification using the answer dataset prepared by the researchers. Next, the automated scoring model was trained using various types of training datasets, and it was used to score the student test dataset. The performance of the automated scoring model has been improved as the amount of training data increased in amount and diversity. Finally, compared to human scoring, the accuracy was 97.06%, the kappa coefficient was 0.957, and the weighted kappa coefficient was 0.968. On the other hand, in the case of answer types that were not included in the training data, the s coring was almos t identical among human s corers however, the automated scoring model performed inaccurately. 본 연구는 합성곱 신경망을 활용한 자동 채점 모델을 설계하고학생의 운동학 그래프 답안에 적용함으로써, 과학 그래프 답안에 대한 자동 채점의 가능성을 탐색하였다. 연구자가 작성한 2,200개의답안을 2,000개의 훈련 데이터와 200개의 검증 데이터로 데이터셋을구성하고, 202개의 학생 답안을 100개의 훈련 데이터와 102개의 시험데이터로 데이터셋을 구성하여 연구를 진행하였다. 먼저, 자동 채점모델을 설계하고 성능을 검증하는 과정에서는 연구자가 작성한 답안데이터셋을 활용하여 그래프 이미지 분류에 최적화되도록 자동 채점모델을 완성하였다. 다음으로 자동 채점 모델에 훈련 데이터셋을 여러 유형으로 학습시키면서 학생의 시험 데이터셋에 대한 채점을 수행하여 훈련 데이터의 양이 많고 다양할수록 자동 채점 모델의 성능이향상된다는 것을 확인하였고, 최종적으로 인간 채점과의 일치율은97.06%, 카파 계수는 0.957, 가중 카파 계수는 0.968을 얻었다. 한편, 훈련 데이터로 학습되지 않은 유형의 답안의 경우 인간 채점자들 간에는 채점이 거의 일치하였으나, 자동 채점 모델은 일치하지 않게채점하는 것을 확인하였다.

        미로내 신경초종 절제술 이후 순차적 이식형 골도 보청기 삽입술 시행 1예

        Intralabyrinthine schwannoma (ILS) is a rare benign tumor that arises within the membranouslabyrinthine. Since hearing loss is inevitable after tumor removal in most cases, an appropriaterehabilitation method should be considered. A 41-year-old male, who was diagnosedwith ILS underwent tumor removal via translabyrinthine approach, has subsequently experiencedright-side deafness as a result. Seventeen months after the tumor removal, a surgery forhearing rehabilitation was performed. Since cochlear implantation was not doable due to cochlearossification, an active transcutaneous bone conduction implant (BonebridgeTM, MEDEL)was placed at the sinodural angle. To the best of our knowledge, this is the first report describinghearing rehabilitation with BonebridgeTM implantation after ILS removal.

        Surgical Outcomes of Sigmoid Sinus Resurfacing for Pulsatile Tinnitus: The Predictive Value of the Water Occlusion Test and Imaging Studies

        Objectives. Sigmoid sinus resurfacing (SS-R) is one of the most effective surgical treatments for patients with pulsatile tin-nitus (PT) originating from anatomical variants of the vascular walls in the temporal bone. This study aimed to pro-vide updated information on the clinical outcomes of SS-R and evaluate the efficacy of the water occlusion test(WOT) as an additional diagnostic modality. Methods. We retrospectively reviewed medical records including tinnitus questionnaires, temporal bone computed tomog-raphy (TBCT) scans, audiologic tests, and preoperative WOT results from patients who underwent SS-R. Results. In total, 26 patients were included in the study. The mean age was 44 years, and the mean symptom duration was3 months. Fourteen patients (53.8%) were completely cured, seven (26.9%) significantly improved, and five (19.2%)remained stable. The mean visual analog scale loudness score decreased from 5.26 to 1.34 (P <0.001) and the meanTinnitus Handicap Inventory score also improved from 50.23 to 5.5 (P <0.001) after SS-R surgery. In 10 patients withdiscrepancies between the preoperative TBCT and intraoperative findings, WOT showed a significant additive effectin predicting surgical outcomes. No patients experienced severe intraoperative or postoperative complications duringthe entire follow-up period. Conclusion. SS-R provides significant symptom improvement in patients with sigmoid sinus dehiscence who had a positiveWOT without other sigmoid sinus variants. A combined diagnostic approach with TBCT, WOT, and intraoperativefindings is crucial for achieving better surgical outcomes in patients with PT caused by sigmoid sinus variants.

        Effect of Hearing Rehabilitation Therapy Program in Hearing Aid Users: A Prospective Randomized Controlled Study

        Objectives. Despite sufficient hearing gains, many patients with hearing loss have difficulty using hearing aids due to poor word recognition ability. This study was performed to introduce our hearing rehabilitation therapy (HRT) program for hearing aid users and to evaluate its effect on hearing improvement. Methods. In this prospective randomized case-control study, 37 participants with moderate or moderate-severe sensorineural hearing loss who had used bilateral hearing aids for more than 3 months with sufficient functional hearing gain were enrolled in this study. Nineteen participants were randomly assigned to the control group (CG) and 18 patients were assigned to participate in our HRT program once a week for 8 consecutive weeks (hearing rehabilitation therapy group [HRTG]). Their hearing results and questionnaire scores for hearing handicap and hearing aid outcomes were prospectively collected and compared between the two groups. Results.         접형동 점액낭종으로 인한 좌측 활차신경 단독 마비

        Paranasal sinus mucoceles are an uncommon cause of isolated palsies of cranial nerves III, IV, and VI. The trochlear nerve has been reported to be less frequently affected than the abducens and oculomotor nerves. Isolated sphenoid sinus diseases may cause serious complications by involving adjacent vital structures such as the optic nerve, cavernous sinus, internal carotid artery, and cranial nerves III–VI. We report a case of a 76-year-old woman who presented to our emergency department with a chief complaint of acute double vision and headache. Her diplopia was diagnosed as left trochlear nerve palsy. Brain CT and MRI revealed expanding cystic lesions in both sphenoid sinuses with bony erosion of the left sinus wall. The patient underwent an endoscopic intranasal sphenoidotomy and recovered completely from diplopia at postoperative 2 months.
