상지 굴곡근과 신전근 근력운동이 유방암으로 인한 림프부종에 미치는 영향

The purpose of this study was to investigate the effect of strengthening exercise on volume of swelling, circumference of upper limb for patients with breast cancer-related lymphedema. 45 volunteers with diagnosed BCRL were involved in the study, The all group was conducted with complex decongestive physiotherapy for 5 days a week. Additionally first study group was conducted with flexor muscle strengthening exercise, second study group conducted with extensor muscle strengthening exercise for 5 days a week, The period of study conducted a total of 2 weeks, before and after the experiment. The assessment tools for lymphedema are; the circumference of the upper limb and the volume of swelling. This data were analyzed using measured one-way ANOVA(Tukey’s post hoc test) & paired t-test by SPSS 20.0 for windows program. The result of this study indicated that; 1) Before and after the experiment, the circumference of the upper limb and the volume of swelling were significantly different(p<.05), 2) Extensor muscle strengthening exercise group were statistically significant in the proximal 10㎝ area(p<.05), 3) Extensor muscle strengthening exercise group showed significant difference than other groups(p<.05). Consequently strengthening exercise has effects on reducing volume and circumference of lymphedema in patients with lymphedema of upper limb associated with mastectomy, Especially in groups of patients who were trained strengthening of extensor muscles showed more reduction of lymphedema than other groups, In group of patients who trained strengthening of extensor muscles had more effect on reduction of lymphedema in proximal upper limb.
유방암 절제술 후 상지 림프부종과 견갑골 기능이상의 상관관계

The purpose of this study was to investigate the most effective and comprehensible method for the assessment of lymphedema and scapular dyskinesis in breast cancer-related lymphedema (BCRL). 20 volunteers with diagnosed BCRL were involved in the study. all assessment of the patient’s edema sides (ES) and non-edema sides (NES) were evaluated. the assessment tools for lymphedema are; the circumference of the upper limb for the volume. scapular dyskinesis was used resting scapular position (RSP) and range of motion (ROM). RSP are; 1) scapular index, 2) 8th thoracic spinous process (T8S) to inferior angle of scapular (IAS) distance, 3) standing PM distance, 4) PM index (PMI). ROM are; 1) shoulder flexion, 2) shoulder extesion. the paired t-test was used to compare ES and NES. pearson correlation analysis was used to confer a relationship between the volume of arm, RSP, and ROM. The result of this study indicated that; 1) the volume of arm statistically significant (p<.05), 2) all the variable between the ES and NES for scapular dyskinesis were statistically significant (p<.05), 3) the standing PM distance showed the strongest relationship in the correlation analysis between RSP and ROM (p<.05, r=.87). Therefore, it can be concluded that there is a relationship between the standing PM distance and ROM and it is suggested that an assessment tool using the standing PM distance to evaluate scapular dyskinesis in BCRL would be clinically effective.
상지림프부종환자의 견갑골안정화운동이 안정시 견갑골 위치에 미치는 영향

PURPOSE: The purpose of this study was to confirm the effect of exercise combined with scapular stabilizing on resting scapular position (RSP) in breast cancer-related lymphedema patients. METHODS: A total of 20 patients with lymphedema after mastectomy participated in the study. All assessments of the patients edema sides (ES) and non-edema sides (NES) were evaluated. The assessment tools used wad RSP. RSP are; 1) scapular index, 2) 8th thoracic spines process (T8S) to inferior angle of scapular (IA) distance, 3) standing pectoralis minor (PM) distance, and 4) PM index (PMI). All patients carried out a scapular stabilizing exercise seven times a week for 8 weeks. The collected data were analyzed with PASW 18.0. The statistical significance (α) was 0.05. RESULTS: According to the results, all the variable between the ES and NES for RSP were statistically significant (p<0.05) in the pre-test. After the exercise, the differences in T8S to IA distance and the PMI between the ES and NES weren``t statistically significant. The results of the RSP showed a significant improvement in T8S to IA distance, standing PM distance, and PMI. CONCLUSION: The results of this study showed that, performing the scapular stabilizing exercise had a significant effect on improving RPS in breast cancer-related lymphedema patients.
