        This study is intended to describe nursing autonomy and to provide nursing administrators and nursing educators with basic data for increasing professional autonomy of nurses. For this task, the author collected the data with questionnaire from November 11, 1991 to November 16, 1991. The subjects of this study were 133 nurses working at one university hospital in Seoul and the contents of questionaire were professional atuonomy, personal characteristics, and job related characteristics. The autonomy score was mid level of professional autonomy (mean= 135.3, S.D.= 12.49). There was significant difference in autonomy score according to working department (F= 4.0825, P < .01). There were significant correlation between autonomy score and head nurse's leadership style (r= .2847, P < .001) and internal Lucus of control (r= -.2794, P < .01). As a conclusion, change is necessary in working conditions and head nurse's leadership style for increasing professional autonomy.

        국선도 프로그램이 노인의 신체수행능력 및 낙상효능감에 미치는 영향

        The Purpose of this study is to investigate the effect of Kouksundo program on elderly women’s SPPB (Short Physical Performance Battery) and FES (fall efficacy scale). Objects for study were exercise group (n=16) and control group (n=12) of women aged 65 and over; Kouksundo program was conducted on the exercise group (60 min/day, 3 days/week, for 12 weeks). Using SPSS (23ver.), repeated measured ANOVA), paired ttest and independent t-test were implemented. The conclusion o fthis research is as follows; First, accordin g to physical performance ability test, it was found that gait seped showed simple effect significantly increasing in the exercise group (p<.05) but between measuring points, there were significant differences in repeated chair stands (p<.01) and total SPPB (p<.001). Major effect showed significant increase in the repeated chair stands (p<.01) and total SPPB (p<.001) only in the exercise group. Second, according to measurement of FES, it was found that daily FES showed simple effect significnatly increasing in the exercise group (p<.05) but activity FES and total FES showed interaction effect (p<.05). Major effects showed significant increase (p<.001) in the activity FES and the total FES only in the exercise group, and activity FES showed a significant difference between groups (p<.05). As seen above, it is revealed that Kouksundo program is very useful to improve physical performance ability and fall efficacy scale o fthe elderly women and therefore, it is expected that Kouksundo program, a traditional psychosomatic discipline, will give a help to prevent a fall of the elderly women. Accordingly, it is suggested that Kouksundo program, ar atditional psychosomatic discipline, will also be utilized as kinesiology in addition to general exercise when planning a preventive project to improve physical performance ability and fall efficacy scale of the elderly people.

Benner uses an interpretive approach to identifying competencies, by that many problems in nursing education and nursing practice can be solved.

        6주간의 이완프로그램이 여성노인의 수면장애, 정신적 스트레스 및 염증 지표에 미치는 영향

        The purpose of this study was to investigate the effect of six-week relaxation program on sleep disorder, mental stress, and inflammatory markers in older female adults. The subjects participated in this study were females aged 65 years or more; they were divided exercise group (n=16) and control group (n=12). For 6 weeks, the subjects in the exercise group practiced the relaxation program for 60 minutes per each session, three times a week. Data analysis of this study was processed by paired t-test, independent t-test, Pearson’s Correlation Analysis using computerized SPSS 23.0 statistical program. First, the results of on completion of the practice of relaxation program, the degree of stress and CRP decreased in the temporal perspective, and the subjects in the exercise group showed significant differences in terms of sleep disorder (p<.01) and TNF- α (p<.05); the group of subjects practiced exercises also rendered significant intergroup difference in TNF-α (p<.01). Second, the results of all subjects in both the exercise group and the control group manifested significant negative correlations between the degrees of sleep disorder and stress(p<.05). The practice of relaxation program was found to have positive effects on improving sleep disorder, stress, and alleviation of inflammation; reduced degree of stress was also found positively facilitated the improvement of sleep disorder. 본 연구는 6주간의 이완프로그램이 여성노인의 수면장애, 정신적 스트레스 및 염증 지표에 미치는 향을 살펴 보기 위하여 실시 되었다. 본 연구 대상자는 65세 이상 여성노인으로 운동군(n=16), 대조군(n=12)이고, 운동군 은 이완프로그램을 6주 동안 주 3회, 회당 60분씩 실시하다. 자료처리는 SPSS 23.0 통계프로그램을 이용하여 대응표본 t검정(paired t-test), 독립표본 t검정(independent t-test), 상관관계분석을 실시하으며, 그 결과는 다음 과 같다. 첫째, 이완프로그램 실시 후 시기 간에서 스트레스와 CRP는 감소하으나 유의한 차이가 나타나지 않 았고, 운동군에서만 수면장애(p<.01)와 TNF-α(p<.05)에서 유의한 차이가 나타났으며, 집단 간에서는 운동군에 서만 TNF-α에 유의한 차이가 나타났다(p<.01). 둘째, 상관관계에서는 운동군과 대조군 모두 수면장애와 스트 레스가 유의한 음의 상관관계를 나타내었다(p<.05). 따라서 이완프로그램은 여성노인들의 수면장애, 스트레스, 염증완화에 긍정적인 향을 미치고, 스트레스 감소가 수면장애를 낮추는데 도움을 주는 것으로 나타났다.

        국선도 프로그램이 여성노인의 기능 체력, 근골격계 통증, 삶의 질 및 우울에 미치는 영향

        The Purpose of this study was to investigate the effects of Kouksundo Program on fuctional fitness, musculoskeletal pain, quality of life and depression in elderly women. Objects of this research were 28 elderly women over 65 years old living in Y-gun, Gyeonggi-do composed of 16 women in Exercise Group(EG) and 12 women in Control Group(CG). EG conducted Kouksundo program for total 12 weeks of 60 minutes and 3 times a week. Data analysis of this study was processed by paired t-test, independent t-test using computerized SPSS 23.0 statistical program. First, the results of functional fitness strength in EG during measurement period showed significant increase in strength of lower extremities, the upper limbs' flexibility, the lower extremities' flexibility, and total body endurance while the lower extremities' flexibility solely showed significant difference. Second, the results of musculoskeletal pain strength in EG during measurement period showed significant decrease in neck, shoulder, and waist while neck and shoulder showed significant differences between groups. Third, the results of quality of life in EG showed significant increase in periods while significant difference appeared between groups according to independent verification through change amount in quality of life. Fourth, the results of depression test results, EG showed significant decrease in periods while significant difference appeared between groups according to independent verification through change amount in depression. Taking above results together, Kouksundo program for 12 weeks improved increase in functional fitness and reduced musculoskeletal pain, it seems to be a program helpful for improvement of psychological variables in elderly women and it is expected to be utilized as a method of exercise intervention to enhance health of elderly women and to improve quality of life in the future. 본 연구는 국선도 프로그램이 여성노인의 기능 체력, 근골격계 통증, 삶의 질과 우울에 미치는 영향을 알아보기 위해 경기도 Y군에 거주하는 여성노인 28명(운동군 16명, 대조군 12명)을 대상으로 하였으며 운동군은 국선도 프로그램을 60분씩, 주 3회, 총 12주 동안 실시하였다. 본 연구의 자료처리는 SPSS 23.0 통계프로그램을 이용하여 대응표본 t검정(paired t-test), 독립표본 t검정(independent t-test)을 실시하였으며 연구 결과는 다음과 같다. 첫째, 기능 체력에 대한 결과 운동군은 측정 시기 간에 하지근력(p<.05), 상․하지유연성(p<.01), 전신지구력에서 유의하게 증가하였고(p<.05), 집단 간에는 하지유연성에서 유의한 차이가 나타났다(p<.05). 둘째, 근골격계 통증에 대한 결과 운동군은 측정 시기 간에 목, 어깨(p<.05), 허리(p<.01)에서 유의하게 감소하였고, 집단 간에는 목, 어깨에서 유의한 차이가 나타났다(p<.05). 셋째, 삶의 질에 대한 결과 운동군은 시기 간에 유의하게 증가하였고(p<.001), 삶의 질 변화량도 집단 간에 유의한 차이가 나타났다(p<.01). 넷째, 우울에 대한 결과 운동군에서 시기 간에 유의하게 감소하였고(p<.01), 우울 변화량도 집단 간에서 유의한 차이가 나타났다(p<.01). 이상의 결과를 종합해 볼 때 12주간의 국선도 프로그램은 여성노인들의 기능 체력을 향상시키고, 근골격계 통증을 감소시키며, 삶의 질 및 우울 개선에 도움을 줄 수 있는 프로그램이라 사료된다. 추후 여성노인들의 건강한 일상생활 영위와 삶의 질을 향상 시킬 수 있는 운동중재의 방법으로 활용되길 기대해본다.

        여대생의 운동참여 빈도와 스트레스가 월경특성 및 월경곤란증에 미치는 영향

        The purpose of this study was to investigate the influence on menstruation and dysmenorrhea by female college students' exercise participation frequency and stress level. For this, the data was analyzed after classifying the 232 female college students into a non-exercise group(n=158) and exercise group(passive exercise group n=25, 1~2 times per week, and active exercise group n=49, more than 3 times per week). For data progressing, a frequency analysis, descriptive statistics, a cross tabulation analysis, multiple response analysis, ANOVA, and a multiple response analysis were performed with use of the SPSS 23.0 statistical program. The results are as follows. Firstly, as a result of analyzing subject's characteristics according to the exercise participation frequency, there was no difference in general and menstrual characteristics except exercise characteristics. Secondly, as a result of variation analysis by exercise participation frequency and stress level, there was no main effect on the exercise participation frequency, and there was no effective interaction between exercise participation frequency and stress level, but in case of stress level, there was a significant effective interaction on sleep duration(p<.001) and dysmenorrhea according to stress level(p<.001). Thirdly, as a result of the correlations between major variables, exercise participation frequency had no correlations with major variables, but stress level had a negative correlation with sleep duration(p<.001). The menstrual period had no positive correlation with the age of the first period(p<.05), but menstrual period(p<.05). The Dysmenorrhea had a positive correlation with stress level(p<.001) and menstrual pain(p<.001), but it had a negative correlation with sleep duration(p<.01). Finally, based on the above results, it is necessary to search for exercise intervention methods that can alleviate stress in developing programs of prevention and relief from menstrual pain and dysmenorrhea. 본 연구는 여대생의 운동참여 빈도와 스트레스가 월경특성 및 월경곤란증에 미치는 영향을 알아보기 위하여 실시하였으며, 이를 위해 여대생 232명을 운동 비참여군(n=158)과 운동참여군(소극적 참여군 n=25, 1~2일/ 주, 적극적 참여군 n=49, 3일 이상/주)으로 분류하여 분석하였다. 자료처리는 SPSS 23.0 통계프로그램을 이용하여 빈도분석, 기술통계, 교차분석, 다중응답분석, ANOVA, 상관관계분석을 실시하였으며, 그 결과는 다음과 같다. 첫째, 운동참여 빈도에 따른 대상자의 특성 분석결과, 운동특성을 제외한 일반적 특성과 월경특성에서는 차이가 나타나지 않았다. 둘째, 운동참여 빈도와 스트레스에 따른 분석결과, 운동참여 빈도의 주효과 및운동참여 빈도와 스트레스에 따른 상호작용효과는 나타나지 않았으나 스트레스의 경우, 수면시간(p<.001)과월경곤란증(p<.001)에 유의한 주효과가 나타났다. 셋째, 주요 변인 간의 상관관계 결과, 운동참여 빈도는 주요변인과 상관관계가 나타나지 않았으나, 스트레스는 수면시간과 부적상관(p<.001)이 나타났다. 월경기간은 월경주기와 정적상관(p<.05), 초경나이와 부적상관(p<.05)이 나타났고 월경곤란증은 스트레스(p<.001)와 월경통증(p<.001)과 정적상관이 나타났으며, 수면시간(p<.01)과는 부적상관이 나타났다. 이상의 결과를 종합해 볼 때추후 월경통증 및 월경곤란증 예방 및 완화 운동 프로그램 개발 시 스트레스를 완화 시킬 수 있는 운동 중재방법을 모색할 필요가 있을 것으로 사료된다.

        Effect of Facial Chuna Manual Therapy Combined with Korean Medicine Treatmeant for Bell’s Palsy

        The effect of combined Korean medicine treatment including facial chuna manual therapy was retrospectively investigated in patients with Bell’s palsy. There were 16 patients (14 males) who received 2 months outpatient treatment following > 14 days of hospitalization. The Yanagihara score and House- Brackmann grade were used to measure the severity of symptoms and recovery. The patients (mean age 58.5 ± 24.74 years) were categorized based on their medical history of hypertension (n = 10), diabetes millitus (n = 10), or oral intake of steroids early in the diagnosis of Bell’s palsy (n = 10). There was no difference between patients who had or did not have a previous history of diabetes mellitus or oral steroid. Both the Yanagihara-score and House-Brackmann grade improved significantly in the total patient sample after treatment (p < 0.001). Combined Korean medicine including facial chuna manual therapy may have a beneficial effect on patient symptoms and recovery.

      • 규칙적인 운동이 여성노인의 WHR, VAS, 유연성 및 삶의 질에 미치는 영향

        The purpose of this study was to analyze the effect of regular exercise on WHR, VAS, flexibility, and quality of life of elderly women. The subjects were 145 elderly women over 60years old who participated in the program using Kouksundo for 12weeks (twice a week, 90 minutes). In this study, pearson's correlation and multiple regression analysis(enter method) were performed using the SPSS(version 23) statistical program. All statistical significance levels were α=. 05. First, the paired t-test results of the elderly women participating in the exercise program showed a significant decrease in WHR and VAS(p<.001), and flexibility and quality of life were significantly increased(p<.001). Second, the correlation results showed that WHR had negative correlation with flexibility(p<.001), negative correlation with VAS with flexibility(p<.01), and quality of life(p<.001). Flexibility was positively correlated with quality of life (p<.01). Third, multiple regression analysis showed that VAS(p<.001) and flexibility(p<.05) were significant variables for quality of life. These results indicate that regular exercise affects obesity, back pain, physical fitness and quality of life in elderly women. 본 연구는 규칙적인 운동이 여성노인의 복부비만률, 요통지수, 유연성 및 삶의 질에 미치는 영향을 구명하는데 그목적이 있다. 연구대상자는 국선도를 활용한 프로그램에 12주(주2회, 90분)동안 참여한 60세 이상 여성노인 145명이었다. 본 연구의 자료 처리는 SPSS(version 23) 통계 프로그램을 이용하여 대응표본 t검정, pearson's의 상관관계와다중회귀분석을 수행하였으며 모든 통계적 유의수준은 α=.05로 하였다. 첫째, 운동프로그램에 참여한 여성노인들의시기 간 차이 분석결과 WHR, VAS는 유의하게 감소하였고(p<.001) 유연성과 삶의 질은 유의하게 증가하였다(p<.001). 둘째, 각 변인들 간의 상관관계 결과 WHR은 유연성(p<.001)과 부적상관관계, VAS는 유연성(p<.01), 삶의질(p<.001)과 부적상관관계, 유연성은 삶의 질(p<.01)과 정적상관관계를 나타냈다. 셋째, 대상자 삶의 질의 상대적영향력 분석을 위해 다중회귀분석을 실시한 결과 VAS(p<.001), 유연성(p<.05)이 삶의 질에 유의한 영향을 미치는변수로 나타났다. 이상의 결과를 볼 때 규칙적인 운동은 여성노인에 있어 비만, 요통, 체력, 삶의 질에 영향을 미친다는것을 확인하였다.

        코로나-19 상황에서 여대생의 신체활동량이 우울 및 자기효능감에 미치는 영향

        The purpose of this study is to explore the effect of physical activity of female college students on the depression and self-Efficacy in COVID-19 pandemic. They were conducted with data collected from 170 female students attending a university located in Seoul. Subjects were classified into low, medium, and high activity groups(LG, MG, HG), and the results are as follows. First, for depression, HG showed significantly lower average in physical and behavior slowdown(p<.05), positive emotions (p<.01), interpersonal relationships(p<.05), and depression total points(p<.05). Second, for subfactors of self-efficacy, self-control and task challenge did not show any significant differences, but the HG showed significantly higher average self-confidence factor and self-efficacy following average points compared to the LG, and MG (p<.05). Third, physical activity showed statistically significant positive correlation with self-efficacy total points and task challenge, and showed negative correlation with physical and behavior slowdown factors for depression. In other words, higher levels of physical activity correlated with higher levels of self-efficacy and lower depression lovel. Therefore in pandemic situations such as COVID-19, it is necessary to actively seek ntervention methods that can safely increase the amount of physical activity of female university students. 본 연구는 코로나-19 상황에서 여대생들의 신체활동량이 우울 및 자기효능감에 미치는 영향을 알아보고자 서울 소재 대학교에 재학 중인 여대생(n=170)을 대상으로 분석하였다. 대상자들을 기준에 따라 저강도, 중강 도, 고강도 활동군(LG, MG, HG)으로 분류하였으며, 그 결과는 다음과 같다. 첫째, 우울의 경우, 하위요인 중 신체 및 행동둔화(p<.05), 긍정적 감정(p<.01), 대인관계(p<.05), 우울 총점(p<.05)에서 모두 HG가 유의하게 낮은 평균값을 보였다. 둘째, 자기효능감의 경우, 하위요인 중 자기조절과 과업도전은 유의한 차이가 나타나 지 않았으나 자신감요인과 자기효능감 총점에서 HG가 MG와 LG보다 유의하게 높은 평균 점수를 보였다 (p<.05). 셋째, 신체활동량은 자기효능감 총점과 과업도전에서 유의한 정적 상관관계가 나타났고 우울에서는 신체 및 행동둔화 요인에서 부적 상관관계가 있는 것으로 나타났다. 즉 신체활동량이 많을수록 자기효능감 이 높고 우울 수준이 낮음을 부분적으로 확인할 수 있었다. 따라서 코로나-19와 같은 팬데믹 상황에서 여대 생의 자기효능감을 높이고 우울감을 감소시킬 수 있도록 신체활동량을 안전하게 증가시킬 수 있는 운동 중 재방법이 적극적으로 모색되어야 할 것이다.

        Pulmonary Mucinous Cystadenocarcinoma: Report a Case and Review of CT Findings

        A pulmonary mucinous cystadenocarcinoma is an extremely rare tumor that is considered to be a cystic variant of mucinproducing lung adenocarcinoma. We present a case of pulmonary mucinous cystadenocarcinoma in a 54-year-old woman. Chest CT scans showed a 4.3-cm-sized, lobulated, well-defined, and homogeneous mass in the right middle lobe with peripheral stippled calcifications that demonstrated low-attenuation with no enhancement after contrast administration; 18Ffluorodeoxyglucose (FDG) PET/CT demonstrated mild heterogeneous FDG uptake. The mass was diagnosed as adenocarcinoma with mucin production by transbronchial lung biopsy. Right middle lobectomy was performed, and the pathologic examination disclosed a pulmonary mucinous cystadenocarcinoma.
