대상관계 회복을 위한 놀이치료 사례연구

본 연구는 대상관계 회복을 위한 놀이치료 사례연구이다. 본 연구대상은 연구 참여에 동의한 남자아동 2명(5세, 6세)이다. 연구기간은 2021년 4월부터 2022년 5월까지 주 1회 진행하였으며, A아동은 총 31회기, B아동은 총 48회기를 진행하였다. 자료분석은 Strauss와 Corbin(1998)의 분석단계인 개방코딩, 축코딩, 선택코딩의 세가지 과정으로 분석하였다. 아동은 놀이를 통해 세상은 경험한다. 본 연구는 근거이론을 적용하여 놀이치료 과정에서 놀이주제를 살펴보았다. 아동들은 자신이 경험한 의미를 부여하고, 그 의미를 어떻게 이해하는지를 심층적으로 탐색하였다. 연구대상은 아동 2명으로 놀이주제를 체계적으로 탐색하였다. 놀이치료 과정에서 아동들의 대상관계와 놀이주제는 어떠한지를 밝혀 현장에서 아동의 문제행동을 이해하고자 한다. 놀이를 통해 자신의 좌절과 결핍, 긍정적인 양육, 자기회복, 안전한 기지를 경험하였다. 즉, 놀이치료 과정에서 나타나는 놀이가 아동들의 심리적 회복과 욕구를 충족하고 회복시킬 수 있다는데 본 연구의 의의가 있다. This study is a case study of play therapy for object relationship recovery. The subjects of this study were two male children (aged 5 and 6) who agreed to participate in the study. The study period was conducted once a week from April 2021 to May 2022, and child A had a total of 31 sessions and child B had a total of 48 sessions. Data analysis was analyzed in three stages: open coding, axial coding, and selective coding, which are the analysis steps of Strauss and Corbin (1998). Children experience the world through play. This study applied grounded theory to examine the play theme in the process of play therapy. Children gave the meaning they experienced and explored in depth how to understand the meaning. The subjects of the study were two children, and the theme of play was systematically explored. In the process of play therapy, we try to understand the problem behaviors of children in the field by revealing how children's object relationships and play topics are. Through play, they experienced their own setbacks and deficiencies, positive parenting, self-recovery, and a safe base. In other words, the significance of this study is that play in the process of play therapy can satisfy and restore children's psychological recovery and needs.

간판매체에 반영된 주변화 양상과 지역인의 의식

본 연구는 정관 신도시 개발에 따라 중심지역에서 주변지역화한 덕산마을을 중심으로 간판매체에 반영된 주변화 양상과 그 과정에서 나타나는 지역인의 의식을 살펴보았다. 덕산마을의 상점 간판에 반영된 주변화 양상은 다음과 같다. 첫째, 체인점 간판이 많은 신도시 지역에 비해, 덕산마을에는 소규모 단독 자영업 간판이 많다는 점인데, 이는 과거 중심상권을 형성하던 덕산마을 상가가 자본의 규모면에서 주변화한 것을 보여준다. 둘째, 신도시 지역 간판이 현재의 사회적 흐름을 반영하고 있는데 비해, 덕산마을의 간판은 오히려 1980~1990년대 흐름을 그대로 유지하고 있다. 셋째, 신도시 지역 간판이 젊은 구매층과 현재의 소비문화적 수요를 담고 있는 반면, 덕산마을의 간판은 장노년층의 소비문화적 수요를 담고 있다. 이러한 주변화 양상과 함께 주변화 과정을 통해 덕산마을 주민들은 피해의식과 좌절감을 느낄 뿐만 아니라 신도시지역에 대한 선망과 질시의 감정을 동시에 경험하고 있는 것으로 나타났다. 그러나 다른 한편으로는 공동체 의식을 바탕으로 한 소비행위를 통해 주체적인 삶의 방식을 보여주고 있었다. The aim of this study is to examine the marginalization trends reflected on signboards around Deoksan village marginalized from central area due to the development of Jeonggwan new town and to investigate the native’s cognition in the process. The marginalization trends reflected on the store signboard in Deoksan village are as follows; First, there are a lot of small independent self-employment store signboards, compared with the new town where many chain store signboards are established. This shows the marginalization of Deoksan village in the scale of capital. Second, while the signboards in the new town exhibit the current social tendency, those in Deoksan village have maintained landscapes of the 1980s~1990s. Third, while the signboards in the new town contain the young buyer’s and current consumption cultural requirements, those in Deoksan village include the consumption cultural demands of middle and old aged people. It was found that through this marginalization, natives in Deoksan village are not only frustrated and damaged psychologically, but also feel envious at and keep a jealous eye on residents in the new town. On the other hand, however, They show the way of subjective life through the consumption behavior based on the sense of community.

Background and Objectives: Gastric cancer is still the leading cause of cancer death in Korea, and surgery remains the only method of treatment that offers the potential for a cure. Purpose: The aims of this study are to examine the outcome of the patients with gastric adenocarcinoma who underwent curative resection and to identify the prognostic factors leading to improved survival rates in these patients. Materials and Methods: The medical records of 278 patients with gastric adenocarcinoma, treated curatively at the Chosun University Hospital between January 1990 and December 1994, were reviewed retrospectively from January 2000 to March 2000. Follow-up was obtained for 250 patients(89.9%). The survival rates were calculated by the Kaplan-Meier method, and the significance of differences between patient factors(age and sex), tumor factors(location, size, gross type, tumor depth, lymph node metastasis, and stage) and treatment factors(operation method and reconstruction type) were evaluated statistically by the log-rank test. Results: The overall five-year survival probability was 59.6%. There was no survival difference according to age and sex. The 5-year survival rates according to the tumor depth were 97.0% for Tl, 74.5% for T2, 39.6% for T3, and 23.1% for T4(p=0.0000); to the lymph node metastasis were 81,4% for N0, 51.9% for N1, and 21.2% for N2(p=0.0000); to the TNM stage were 96.7% for stage I a, 79.5% for stage I b, 68.8% for stage II, 52.9% for stage III a, 25.9% for stage III b, and 7.5% for stage IV(p=0.0000). Other statistically significant differences were shown in tumor location, size, gross type, operation method, and reconstruction type after subtotal gastrectomy. Conclusion: The most definitive prognostic factors affecting the survival rate of gastric adenocarcinoma were tumor depth, lymph node metastasis, and tumor stage. The keys to successful outcome includes early diagnosis and curative resection with radical lymph node dissection at early stage as possible.

오렌지 껍질로부터 분리된 프라보노이드의 화학요법 감작효과

Background and Objectives : The overexpression of P-glycoprotein (Pgp) or multidrug resistanceassociated protein (MRP) confers multidrug resistance (MDR) to cancer cells. MDR cells could be sensitized to anticancer drugs when treated concomitantly with a chemosensitizer. In this study, orange peel have been screened for the development of chemosensitizers reversing MDR. Materials and methods : Expression of Pgp in AML-2/WT and AML-2/D100 were determined using western blot analysis and RT-PCR. Candidates for chemosensitizers were isolated, purified from orange peel using sephadex LH-20 column chromatography and silica-gel column chromatography, whose structure was determined using ^1H-NMR, ^13C-NMR, ^1H-^1H-COSY, 13C-NMR, EI-Mass, IR. Chemosensitizing effect of NP-28 on AML-2/DX100 over expressing Pgp and MRP was determined by the MTT assay. Membrane toxicity was evaluated by the RBC hemolysis test. Antioxidant effects were examined by using a fluorescence probe (2’-7’-dichlorofluorescein diacetate). Results : A potent candidate, NP-28, has a flavonoid nucleus with some methoxy moiety. NP-28 is believed to have high therapeutic index, being non-transportable inhibitor and possibly, of inhibiting other efflux pumps such as MRP and antioxidant effects. NP-28 is also unlikely to be a substrate of Pgp. NP-28 did not increase Pgp levels. Conclusion : NP-28 has a Chemosensitizing effect to reverse Pgp-mediated MDR by increasing the intracellular accumulation of drugs. Furthermore, they do not increase expression of Pgp, which can allow a long term use without the concern about Pgp activation. NP-28 is anticipated as an ideal second-generation flavonoid chemosensitizer and/or possibly other therapeutic agents by further investigation.

Exploring Multiple Feedback Methods in a Korean EFL Writing Class

The purpose of this study was to investigate effective and efficient feedback methods in an EFL writing classroom. The participants of the study were 24 students enrolled in an advanced English composition class at a university in Korea. The students exchanged peer feedback in small groups and each group was asked to write an argumentative essay on the same topic and exchange both written and oral feedback with their peers based on the guidelines provided on the peer feedback form. Then, the participants received both written and oral feedback from the instructor. The participants wrote three drafts altogether: first draft, second drafts based on the peer feedback, and the final versions revised according to teacher feedback. A paired t-test indicated that after receiving feedback from peers, the participants' revisions had significantly improved in terms of both holistic and analytic measurements; furthermore, significant differences were also found in the final versions revised after the teacher feedback using the same analysis. Additionally, the current study closely examined the participants' samples to illustrate the different ways of revisions made by the students, and to show the differences between the peer and teacher feedback. This study clearly shows that using different kinds of feedback methods (oral and written; peer and teacher) in the appropriate writing stages may lead to improvement of Korean EFL learners' writing. 본 연구는 EFL 쓰기 교실에서 사용할 수 있는 효율적이고, 효과적인 피드백 방법에 대해 연구를 하였다. 이 연구의 참여자는 한국의 한 대학교에서 고급영어작문을 선택한 24명의 대학생이다. 학습자들은 소그룹별로 한 가지의 동일한 주제를 놓고 자신의 의견을 주장하는 작문 (argumentative essay)을 작성하도록 하였고, 그 이후, 교사가 제시해준 동료 수정을 위한 피드백 확인 리스트 (peer feedback form)를 바탕으로 구두 (oral) 와 문자언어 (written)로 동료 피드백 (peer feedback)을 받았다. 그 이후, 교사가 구두와 문자언어로 교사 피드백 (teacher feedback)을 제시하였다. 즉, 학습자들은 총 세 번의 과정을 거치며 본인의 작문을 완성 하였다. 처음에 초고를 쓰고, 그 다음, 동료 수정을 받아 2차 수정본을 쓰고, 마지막으로, 교사수정을 통해, 완성본을 썼다. 세 편의 글은 모두 총체적 점수와 분석적 점수로 평가하였다. 분석적 점수는 전부 여섯 가지 항목으로 분석하였다. t-검정 결과, 첫 번째 초고와 동료 피드백을 받고 수정한 두 번째 초고사이에 유의한 차이가 있었고, 또한, 교사피드백을 받고 작성한 2차 수정본과 완성본 사이에서도 유의한 차이가 있었다. 그리고, 이 연구는 학생들이 실제적으로 어떤 부분을 어떻게 수정을 했는지에 대해서도 질적으로 예시를 제시하였다. 이 질적인 예시를 통해 동료 피드백과 교사 피드백이 학습자들의 작문에 어떤 영향을 미쳤는지도 보여 주었다. 마지막으로, 이 연구는 적절한 쓰기활동의 단계에 다양한 피드백 방법의 사용이 한국 EFL 학습자들의 쓰기활동에 향상을 가져올 수 있다고 하는 교육적인 시사점을 제시하였다.

만성 신부전 환자의 선단 궤양

Calciphylaxis has been described as a rare condition in patients with end-stage renal disease and secondary hyperparathyroidism. Selye et al. First coined the term calciphylaxis to describe soft-tissue calcification and cutaneous necrosis. The common manifestation include painful digital necrosis and medial calcification of small and medium sized vessels. Calciphylaxis is a clinical diagnosis and is characterized by painful, violaceous, mottled skin lesions. Radiographic studies reveal calcification of median and small vessels under the knee but vascular calcification is not specific for calciphylaxis. Histolopathologic findings show a markedly reduced lumen, secondary to the fibrin deposition and inflammation in and around the outer media with an outer ring of calcification which leads to progressive ischemic tissue necrosis. Kidney transplantation and parathyroidectomy is recommened but it is controversial. We present the case of a 64-years-old man with chronic renal failure as well as painful ischemic necrosis on the lower extremity. The pertinets literature is reviewed.
