Proliferation of abnormal hematopoietic cells with impaired differentiation, regulation and programmed cell death leads to leukemia. AML(acute myeloid leukemia) is a malignancy with malfunction of myeloid hematopoietic cells with acute behavior. The oral manifestations of the disease are posterior palate hemorrhage, gingival bleeding and gingival ulceration as a result of infection by normal oral flora and gingival infiltration by leukemic cells. A 49-year-old male patient was referred from local dental clinic. The patient was diagnosed with AML FAB M1 (acute myeloid leukemia French-American-British classification M1 myeloblastic leukemia without maturation). The oral infection focus was removed by a conservative treatment. 2 days after the dental treatment, the patient underwent chemotherapy. At 8-month follow-up, the overall outcome was excellent. Oral manifestations of AML are often the first indications of the malignancy. Therefore it is essential for dentists, especially oral and maxillofacial surgeons, to be aware of the diagnostic signs and complications associated with leukemia for better diagnosis and subsequent treatment and management.

Introduction: This study evaluated nerve recovery through retrospective study of patients with lingual nerve damage. Patients and Methods: The patients who visited Seoul National University Dental Hospital for an injury to the lingual nerve from April 1988 to August 2009 were enrolled in this study (n=41). The relevance of various factors including the causes of damage, age, etc. was analyzed by the subjective improvement based upon questionnaires and the clinical records. The evaluation variants were a subjective assessment and neurosensory examination composed of the direction, contact threshold, two-point discrimination, pin prick, thermal discrimination and current perception threshold. Results: The causes of lingual nerve damage were an extraction of the lower third molar (75.6%), local anesthesia (9.7%), incision and drainage (4.88%), trauma (2.44%). The evaluation of subjective prognosis exhibited no difference in sensory improvement depending on the cause, age and gender. Based upon the subjective evaluation, 44.7% of patients showed sensory improvement. The first hospital visit from injury was shorter in the group showing subjective improvement (3.41 months) than those showing no improvement (5.24 months) (P=0.301). Thirty six out of 41 patients were treated with only conservative therapy and 5 patients were treated by surgical intervention. Neurosensory examinations revealed improvement, although not statistically significant, and the degree was higher in the subjectively improved group. The contact threshold discrimination showed the highest correlation with subjective improvement (P=0.069). Most of the sensory recovery was gained within 12 months and the degree of improvement at the tip of the tongue was higher than that of the dorsum (P<0.001). Conclusion: The damaged lingual nerve improved at a rate of 44.7% and mostly within 12 months after the incident. There was no difference between the subjective prognosis and neurosensory examination depending on the cause of damage, age and gender, whereas the contact threshold discrimination was the best variant that reflected the subjective prognosis statistically.

Orthognathic surgery is designed to correct problems of the jaw and face and restore facial harmony. The limitations of orthognathic surgery occur at all steps of the surgical workflow: preoperative planning, simulation, and operation. Many studies have shown the accuracy and advantages of 3 dimensional computer-assisted program for orthognathic surgery. The purpose of this paper is to introduce the accuracy of the maxillary repositioning in patients who underwent orthognathic surgery using a 3 dimensional computer assisted surgery program. The reliability of computer guided orthognathic surgery using splint and surgical guide need to be improved further. The 3 dimensional computer assisted analysis seems to be more precise to interpret than twodimensional analysis. High-precision planning of orthognathic surgery has predictable results. Three-dimensional computer assisted orthognathic surgery has the following advantages : planned surgical movement is possible, splints guide with CAD/CAM technology; and increase predictable results .Computer assisted simulation surgery ensures accuracy during surgery, thereby facilitating predictable results. It may provide solution that enables surgeon to perform planned surgery more accurately.

Cervicofacial subcutaneous emphysema is defined as the abnormal introduction of air into the subcutaneous tissues of the head and neck. It is mainly iatrogenic and traumatic in origin. Our two case reports are also due to the same cause, but the features of the trauma and the site of the dental treatment are different from the existing reports. A 29-year-old man visited our hospital with facial swelling and pain after experiencing facial trauma in a soccer game. Another 55-year-old woman visited with similar symptoms after replacement of her maxillary anterior fixed prosthesis. In the two cases presented, subcutaneous emphysema was gradually treated with no complications during antibiotic prophylaxis and supportive care. In this paper, we report two cases of traumatic and iatrogenic subcutaneous emphysema and their diagnoses, etiologies, complications, and treatments based on a literature review.

Temporomandibular disorders (TMD) are challenging to treat despite various existing methods. Stress on the temporo- mandibular joint (TMJ) leads to an accumulation of reactive oxygen species, promoting inflammation and cartilage degra- dation. Resveratrol, a natural polyphenol, inhibits inflammatory pathways and suppresses chondrocyte apoptosis, making it a promising candidate for TMD treatment. This review analyzes six studies with 3 kinds of animals on the intra-articular application of resveratrol for treating arthritis. This paper aims to elucidate the potential of resveratrol administrations into the articular cartilage to improve severity of arthritis, providing a foundation for future research and clinical investigations.

MRONJ (medication related osteonecrosis of the jaw) is a disease characterized by necrosis of the jaw that occurs after administration of anti-resorptive or anti-angiogenic drugs. However, etiology and effective treatment methods of MRONJ are not yet clearly identified. In this study, a systematic review was conducted to investigate the effects of pentoxifylline and tocopherol on MRONJ. A total of 5 clinical studies using pentoxifyllie and tocopherol in MRONJ patients among the papers registered in Pubmed/medline were reviewed. Pentoxifyllie and tocopherol were used for therapeutic purposes in 32 patients, and the mean age was 68 years for men and 74.21 years for women. Of the 32 patients, 20 patients were stage 2,11 patients were stage 3, and one patient was stage 0. In the most cases, the triggering factor was extraction. All patients complained of pain in the lesion area before drugs administration. The average follow-up period was 10.6 months. After drugs administration, 29 patients (90.6%) showed improvement in symptoms in terms of bone exposure and pain. Administration of pentoxifylline and tocopherol in MRONJ may improve symptoms of pain and bone exposure. However, it seems that additional studies on the appropriate use and dose, timing and duration of administration are needed.

This paper reviewed prolotherapy as an approach to treating temporomandibular joint disorders (TMD), focusing on temporomandibular joint subluxation and hypermobility. Prolotherapy, commonly used in orthopedics, involves injecting substances like dextrose or lidocaine to promote healing in musculoskeletal conditions. Clinical studies in which this prolo- therapy was used for TMD were reviewed. Total 13 papers involving 372 patients (Male 90, Female 282) were examined, considering factors such as dextrose concentration (10%~50%), injection frequency (one to four times), injection period (one to four weeks) and injection sites. The assessment primarily focused on pain reduction and changes in maximum mouth opening after prolotherapy. In all papers, a consistent trend of pain reduction after prolotherapy was observed. Regarding maximum mouth opening, there were 6 cases of decreased range and 7 cases of increased range. With prolotherapys rec- ognition as a new medical technology for TMD, it emerges as a potential alternative treatment, highlighting its significance in the field.
