열공망막박리 환자에서 유리체절제술 이후에 발생한 망막주름의 2예

Purpose: We report two cases of retinal folds developing after pars plana vitrectomy in patients exhibiting rhegmatogenous retinal detachment. Case summary: (Case 1) A healthy 52-year-old male visited our clinic complaining of blurred vision in his right eye. His visual acuity was 0.8 in that eye. Fundal examinations revealed upper retinal detachment and retinal tears at the 12 and 1 o’clock positions. He underwent pars plana vitrectomy with gas injection, and 1 week later, the retina was reattached. A retinal fold was detected at the 4 o’clock position; the fold extended for two disc diameters from the optic disc to the equator. The fold resolved spontaneously after 3 months. (Case 2) A 59-year-old male visited our clinic complaining of blurred vision in his right eye. His visual acuity was “counting fingers” in that eye. Fundal examination revealed a retinal tear at the 11 o’clock position and upper retinal detachment involving the macula. He underwent pars plana vitrectomy with gas injection. A retinal fold was detected in the temporal region of the disc running from the 7 o’clock position to the equator. Over 11 months of observation without treatment, optical coherence tomography (OCT) revealed that the retinal fold resolved. Conclusions: We report the first two Korean cases of spontaneous relief of retinal folds developing after vitrectomy, and the OCT patterns of the folds. 목적: 열공망막박리 환자에서 유리체절제술 이후에 발생한 망막주름의 2예를 보고하고자 한다. 증례요약: (증례 1) 52세 남성이 우안의 시력저하를 주소로 내원하였다. 우안의 교정시력은 0.8이었고 12시와 1시 방향의 망막 열공과 황반부를 침범하지 않은 상부 망막박리가 관찰되었다. 치료를 위해 유리체절제술 및 가스 주입술을 시행하였으며 망막은 재유착되었다. 유리체절제술 시행 1주일째 망막은 유착되었으나 시신경유두의 4시 방향으로 2시신경유두 직경 떨어진 부위로부터 경선방향으로 적도부까지 이어지는 망막주름이 형성되었다. 특별한 치료 없이 경과관찰 중에 안저소견에서 3달째에 주름이 자발적으로 호전된 소견이 관찰되었다. 이후 재발 없이 안정적인 경과를 보였다. (증례 2) 59세 남성이 우안의 시력저하를 주소로 내원하였다. 우안의 교정시력은 안전수지였고 11시 방향의 망막 열공과 황반부를 침범한 상부 망막박리가 관찰되었다. 유리체절제술 및 가스 주입술을 시행하였으며 망막은 재유착되었다. 유리체절제술 시행 10일째 망막박리의 폐쇄를 확인하였으나 시신경유두의 이측에서 7시 방향으로 적도부까지 이어지는 망막주름이 형성되었다. 경과관찰 중 빛간섭단층촬영에서 2주째에 동일한 위치에 망막주름이 관찰되었고 11달째 망막주름의 호전이 관찰되었다. 결론: 유리체절제술 이후에 발생한 망막주름의 자연호전과 빛간섭단층촬영 소견은 국내에 보고된 바가 없어 이를 보고하고자 한다.

옥천 변성대 남서부 지역 암석에서 산출되는 Phyllosilicate에 대한 광물학적 연구 : EPMA, BSE, TEM 연구 EPMA, BSE and TEM Study

Muscovite, chlorite and biotite are observed as the common phyllosilicates that occur in the metapelites at the southwestern part of the Okchon Metamorphic Belt. These minerals are studied using polarized light microscopy, EPMA analyses, back scattered electron(BSE) image observation and transmission electron microscopy(TEM), and found to be intergrown or interlayered each orther. The three minerals can not be distinguished from one another with polarized light microscope due to the intergrowth. The scale of the intergrowth is various from a fine scale of less the 0.1㎛ to approximately 10㎛ is the BSE image. At the TEM scale, they are interlayered from an individual layer(∼10Å) to hundreds of Å. As a result, an apparently homogeneous grain shows an analysis of a mixture of more than two minerals in EPMA. The nonstoichiometry of such as even more prominent in an analysis of the area where interlayering(or intergrowth) is evidently observed by BSE. The interlayering of cholorite and muscovite(C/M) is common in the chlorite zone, whereas, the interlayering between chlorite and biotite(C/B) is commonly observed in the biotite and the garnet zones according to the metamorphic grade. This implies that chlorite is separated from C/M by the mineral reaction in the chlorite zone continued from diagenesis, and biotite forms from the chlorite through C/B by the reaction in the biotite and the garnet zones. This also implies that the mineral reactions in the metamorphism occur, in a strict sense, under disequilibrium state, rather than forming a homogeneous mineral by equilibrium reaction. Purpose: To measure changes in ocular biometrics required for calculating intraocular lens powers during cataract surgery on phakic eyes undergoing implantation of a collamer lens (AQUA ICL [STAAR Surgical Company, Monrovia, CA, USA] or an Artiflex® lens [Ophtec BV, Groningen, Netherlands]) to correct myopia. Methods: A total of 45 eyes of 23 patients who underwent implantation of iris-fixated or posterior chamber phakic intraocular lenses (pIOLs) for correction of myopia >−7D were evaluated using the euphakic mode of the IOL Master® 500 (Carl Zeiss Meditec AG, Jena, Germany) prior to pIOL implantation. After implantation, the axial length (AL) and anterior chamber depth (ACD) were measured 1 month postoperatively using both the euphakic and pseudophakic modes of the instrument. We compared differences between predicted IOL powers calculated using the Sanders-Retzlaff-Kraff/Theoretical (SRK/T) and Haigis formulae. Results: Seventeen eyes (37.8%) receiving ICL and 28 (62.2%) Artiflex® pIOL implants were included in the study. After pIOL implantation, ALs measured by the euphakic and pseudophakic modes of the IOL Master were significantly longer (p = 0.03, p < 0.0001) and ACDs significantly shorter (p < 0.0001, p < 0.0001, respectively) than preoperatively. The changes after surgery were less when the euphakic rather than the pseudophakic measurement mode was employed. The postoperative IOL powers predicted by the SRK/T (−0.03D, p = 0.023) and Haigis formulae (−0.06D, p = 0.001) were significantly lower that the preoperative values. However, the differences were small and did not influence IOL power selection. Conclusions: After pIOL implantation, AL changes were less when measured using the euphakic rather than the pseudophakic mode of the IOL Master. Although the ACDs differed significantly after pIOL implantation, the changes were too small to influence IOL power calculations. ACD measurements differed significantly from those of AL after IOL implantation. Thus, a pIOL implantation history may affect biometric findings during cataract surgery. 목적: 고도근시 환자의 근시교정을 위해 Implantable collamer lens (AQUA ICL, STAAR surgical company, Monrovia, CA, USA) 및 ArtiflexⓇ lens (Artiflex, Ophtec BV, Netherlands) 삽입술을 시행받은 눈에서 백내장수술 시 안내렌즈 도수 계산에 있어 필요한 생체계측값의 변화를 알아보고자 한다. 대상과 방법: -7D 이상의 근시 환자의 교정을 위해 후방유수정체 및 홍채고정 안내렌즈삽입술(ICL 또는 ArtiflexⓇ lens)을 시행받은 환자 23명의 45안구를 대상으로 안내렌즈삽입술 전에 IOL masterⓇ (IOL master 500, Carl Zeiss Meditec AG, Jena, Germany)의 유수정체안 설정(euphakic mode)으로 안축장과 전방깊이를 측정한 후, 술 후 한 달째 유수정체안 설정과 인공수정체안 설정(pseudophakic mode)으로 안축장과 전방깊이를 측정하여 수술 전후와 측정 설정 간의 차이를 비교분석하였다. Sanders-Retzlaff-Kraff/Theoretical (SRK/T) 공식과 Haigis 공식을 사용하여 측정한 인공수정체 도수 예측치의 차이를 비교 분석하였다. 결과: 45안구 중 17안(37.8%)이 ICL 삽입술을 받았고 28안(62.2%)이 ArtiflexⓇ lens 삽입술을 받았다. 유수정체 안내렌즈삽입술 후 유수정체안과 인공수정체안 설정 모두 안축장이 술 전에 비해 통계적으로 유의하게 길게 측정되었고(p=0.03, p<0.0001), 전방깊이의 측정에서도 두 가지 설정 모두에서 유의하게 짧은 것으로 나타났다(p<0.0001, p<0.0001). 안축장과 전방깊이의 측정값의 변화는 유수정체안 설정이 인공수정체안 설정에 비해 차이가 적었다. 인공수정체 도수의 예측값(predicted IOL power)은 SRK/T 공식과(-0.03D, p=0.023), Haigis 공식(-0.06D, p=0.001) 모두 술 전에 비해 유의하게 작게 계산되었으나, 인공수정체 도수 선택에 영향을 주지 않을 정도로 작은 차이를 보였다. 결론: 유수정체 안내렌즈삽입술 후 생체계측치 측정에 있어 IOL masterⓇ의 유수정체안 설정이 인공수정체안 설정보다 수술 전과 비슷하게 측정되며 통계적으로 유의미한 차이를 보이나 임상적으로 안내렌즈 도수계산에 영향을 미칠 정도는 아니었다. 전방깊이 측정은 안축장 측정에 비해 안내렌즈삽입술 후 보다 많은 차이를 보였다. 유수정체 안내렌즈삽인안에서 백내장수술 시 생체계측치의 차이가 있을 수 있음을 유념해야 할 것으로 생각된다.
