        자침세포성 빈혈 치험 1 예

        N/A Spur cell anemia is intense hemolytic anemia associated with alcoholic liver cirrhosis characterized by spiculated erythrocytes due to a striking increase in cholesterol content and in cholesterol to phospholipid ratio. Several attempts including splenectomy, plasmapheresis and calcium antagonists, have been tried to correct this anemia. Presented here is a case of a 60-year-old man with alcoholic liver cirrhosis who revealed jaundice and severe hemolyic anemia. The peripheral blood smear demonstrated spurred erythrocytes which are caused by serum factor. The cross incubation test, that is, spur cells with normal serum and normal red blood cells with the patient's serum, revealed reversability of the spur cells. The benefits of plasmapheresis were equivocal, while a calcium antagonist, flunarizine, was administered with discernible benefits. The authors suggest that spur cell anemia should be suspected in alcoholic liver cirrhosis with severe jaundice and anemia refractory to transfusion and that the calcium antagonist, flunarizine, is useful to treat it.

        2015 개정 교육과정에 따른 중학교 교육과정의 탄력적 편성·운영 실태와 시사점 탐색

        본 연구의 목적은 2015 개정 교육과정을 중심으로 중학교 교육과정의 탄력적 편성·운영 실태와 시사점을 탐색하는 데 있다. 이를 위하여 본 논문은 중학교 단계에서 교육과정 탄력성의 필요와 의미를 구체화하는 한편, 2015 개정 교육과정에 포함된 네 가지 조항(교과(군)별 20% 증감, 선택 교과 개설, 자유학기 활동, 학교스포츠클럽 활동)을 중심으로 중학교 교육과정의 변화와 학교 현장의 의견을 살펴보았다. 연구 방법으로는 전국 845개 중학교의 학교별 교육과정 담당교사 1명씩을 대상으로 설문조사를 실시하였다. 연구 결과, 첫째 교과(군)별 20% 증감 운영과 관련하여서는, 90% 이상의 중학교에서 수업 시수 편성·운영 시 20% 증감 지침을 적용하고 있는 것으로 나타났고, 가장 많은 학교에서 시수를 증배 운영하는 교과는 체육인 반면, 감축 운영하는 교과는 국어로 나타났다. 둘째, 선택 교과와 관련하여, 2015 개정 교육과정에 제시된 선택 과목 중 한문의 선택비율이 여전히 가장 높았고, 진로와 직업, 보건, 중국어, 일본어 등이 그 뒤를 이었다. 중학교 교사들은 2015 개정 교육과정에 제시된 선택 과목의 변경 필요성 및 시수 확대의 필요성에 대해 불필요하다는 인식이 우세하였다. 셋째, 자유학기 활동과 관련하여, 설문 참여 중학교의 70% 정도에서 자유학기를 두 학기 이상 운영하고 있으며, 일반 학기의 교과와 창의적 체험활동의 틀을 중심으로 자유학기 활동을 조정·통합할 필요가 있다는 인식이 높았다. 마지막으로, 학교스포츠클럽 활동과 관련하여, 교사들은 학교스포츠클럽 활동과 체육 교과 및 자유학기 활동 중 예술·체육 활동 간의 중복 문제를 해결할 필요성에 대한 인식이 매우 높았다. 본 연구는 이러한 분석 결과를 토대로 중학교 교육과정의 실질적인 탄력성을 높이기 위한 시사점을 제시하였다. This study examines main changes in middle school curriculum through the lens of curriculum flexibility granted in the 2015 Korean National Curriculum. In particular, it attends to the four provisions in the national curriculum; 1) adjustment of instructional hours within 20% across subjects, 2) elective courses, 3) activities in Free Semester Program, 4) Sports Club Activities. To explore how these provisions affect the middle school curriculum, this study makes use of survey data collected from 845 middle schools. Main finding indicates that more than 90% of schools have adjusted the number of instructional hours; instructional hours for physical education have been increased most, while those for Korean language have been decreased most. In terms of elective courses, Chinese characters was offered in the largest number of schools. Respondents replied that enough types of elective courses are avaliable in middle school and thus they do not have to be expanded. On the other hand, they replied that several activities in Free Semester Program need to be integrated with those in regular semesters, considering the overlap between the two. Similar reponses were also found regarding the relation among Sports Club Activities, physical education, and Free Semester Program.

        A 72-year-old male patient was referred to the hematologic department because of persistent hypereosinophilia, splenomegaly and chronic fatigue; these existed for 3 months. A presumptive diagnosis of a myeloproliferative variant of hypereosinophilic syndrome was made on the basis of anemia, splenomegaly, bone marrow hypercelluarity, increased serum tryptase and vitamine B12. Fip 1 like 1-platelet-driven growth factor receptor alpha fusion gene and platelet-driven growth factor receptor beta gene rearrangement analysis with RT PCR were negative. In initial therapeutic approach with prednisolone, hydroxyurea, and imatinib mesyalte, blood eosniophilia did not improve and transformation into chronic eosinophilic leukemia observed in the peripheral blood smear. In the case herein reported, therapeutic response was not achieved with cytosine arabinoside chemotherapy. (Korean J Asthma Allergy Clin Immunol 2007;27:61-65)

        Tsutsugamushi disease (Scrub typhus) is an acute, febrile illness caused by Orientia tsutsugamushi, which is transmitted to humans through chigger bites. Leptospirosis, a febrile disease caused by various pathogenic Leptospira, and is acquired by exposure to contaminated water and soil. Both diseases have been the most common acute febrile diseases in the autumn in Korea for many years. Concomitant leptospirosis and scrub typhus is quite rare. We report a case of a coinfection with leptospirosis and scrub typhus in a 51-year-old male who presented with fever, abdominal pain and acute dyspnea. The patient was diagnosed with as acalculous cholecystitis, acute respiratory distress syndrome, and septic shock caused by the infection. This is the first case report of a coinfection with leptospirosis and scrub typhus in Korea. (Tuberc Respir Dis 2007; 63: 378-381)

        Background: In recent epidemiologic data, occurrence of asthma or recurrent bronchitis was associated with Toxocara seroprevalence in children. Objective: The aim of this study was to evaluate the nonspecific bronchial responsiveness in patients with toxocariasis. Method: Seventy-one patients (mean 52.7 yr, M : F=48 : 23) who visited our hospital with eosinophilia caused by toxocara larval infestation during the period from September 1, 2003 through March 30, 2006 were evaluated. In this study we have examined bronchoalveolar larvage (BAL), methdcholine bronchoprovocation test (MBPT), and pulmonary function test for lung evaluation. Diagnosis of toxocariasis was made with ELISA that using the secretory-excretory antigen from T. canis larvae. Result: Toxocariasis patients had asthma like symptoms; cough, dyspnea, and sputum. All patients had increased percentage of eosinophil and absolute eosinophil count (mean 26.8%, 3,191 cells/uL) in peripheral blood. Serum IgE and eosinophil cationic protein levels were increased (mean 1,723 U/mL, mean 181 ng/mL). It was found that 60.5% of toxocariasis patients had bronchial hyperreactivity (mean PC20 11.95 mg/mL). The mean of eosinophils in peripheral blood and BAL fluid were significantly higher in positive MBPT patients than in negative MBPT patients. Conclusion: These results showed that bronchial hyperresponsiveness was frequently manifested in adults with toxocariasis and eosinophilia in peripheral blood, and that lung may play a significant role in the bronchial hyperresponsiveness.         개회충증의 핵심과 진단의 딜레마

        Toxocariasis is an endemic parasitic infection and one of the most common causes of peripheral blood eosinophilia in Korea. The clinical manifestation is atypical, which makes it difficult to distinguish toxocariasis from other clinical conditions. Serologic tests for helminthic or protozoal infections frequently show positive results in general population and measurement of serum specific immunoglobulin G (IgG) antibody against Toxocaria canis (T. canis) using Enzyme-linked immunosorbent assay (ELISA) has a problem of cross-reactivity with those helminthes or protozoas in Korea. To avoid problems of cross-reactivity with other helminthes or protozoas and ensure the diagnosis of toxocariasis, western blotting procedure with excretory-secretory antigens from T. canis larvae was recommended.         N/A Background and objective : The cysteinyl leukotrienes are bioactive lipid mediators that contribute to the pathophysiologic condition of asthma and rhinosinusitis. We tested whether the leukotriene receptor antagonist ONO-1078(Pranukast) had steroid sparing effect on mode- rate to severe asthmatics with chronic rhinosinusitis. Methods : Eighteen asthmatic patients with chronic rhinosinusitis who required more than 800 mcg/day of budesonide inhalation for the adequate control of asthma symptoms were recruited for this study. For the first 4 weeks, patients were treated with high dose (800-1200 mcg/day) budesonide inhalation. For the next 4 weeks, the dose of budesonide inhalation was decreased by 400 mcg/day and oral ONO-1078 (900mg/day) was administered. FEV1 was evaluated every 2 weeks. And PC20 on methacholine challenge. serum eosinophil cationic protein and blood eosinophil count were measured every 4 weeks. Diary cards were completed with morning and evening PEFR and symptom scores for asthma and rhinosinusitis during the treatment periods. Results : Despite the reduction of the dose of inhaled corticosteroid by 400mcg/day, FEV and PEFR did not. decrease with the addition of oral ONO-1078. The symptom scores of asthma and rhino-sinusitis did not change, and the need for β-agonist did not increase. Conclusion : These results suggest that ONO-1078 might have steroid sparing effect in moderate to severe persistent asthmatics with chronic rhinosinusitis who required high dose nhaled budesonide to control asthma symptoms.         구강 편평세포암종에서 E-cadherin과 ${\beta}-catenin$의 발현과 임상병리학적 특징

        Changes in cell adhesion molecules are associated with infiltration and metastatic progression of cancer. Reduced expression of E-cadherin and ${\beta}-catenin$ complex in some carcinomas has been reported. The changes in the expression in oral squamous cell carcinoma (OSCC) is not fully understood and it also remains undetermined whether the expression of these adhesion molecules in metastatic lesions differs from that in the primary lesions. In the present study, therefore, we immunohistochemically examined the expression of E-cadherin and ${\beta}-catenin$ in 45 primary OSCCs and 19 metastatic lymph nodes. We compared the expression of these molecules between primary and metastatic lesions and investigated the correlation between the expression and clinicopathologic parameters. The expression of E-cadherin and ${\beta}-catenin$ was reduced in 35/45 (78.2%), 14/45 (31.2%) of primary tumors respectively, but 18/19 (94.7%) and 17/19 (89.4%) of lymph nodes showed preserved expression. The reduced expression of the E-cadherin was associated with lymph node metastasis, invasive mode and marginal status but no significant relationship was not found with ${\beta}-catenin$. In conclusion, the loss of E-cadherin and ${\beta}-catenin$ complex function is associated with progression of OSCC and suggest that the expression of this complex will be a supplementary prognostic tool.

        주파수 선택적 레이돔 결함요소 해석을 위한 반-무한배열구조 해석 방안에 관한 연구

        본 논문에서는 전자기 주기구조인 주파수 선택 구조를 레이돔에 적용하는 과정에서 발생하는 결함요소를 해석하기 위해 반-무한배열구조 해석방법을 제안하였다. 3D 전자기 해석 소프트웨어를 이용하여 N×N 유한배열 구조를 해석 하게 되면 많은 해석시간과 메모리가 소요되는 반면 제안한 반-무한배열구조 해석을 이용하면 적은 계산시간으로 전자기성능을 비교적 정확하게 예측할 수 있다. 주기구조해석과 반-무한배열구조의 해석 결과를 비교하여 ±3% 이내의 오차를 확인하였다. 주기적으로 배열된 패턴의 중심부에 발생한 결함요소에 대해 해석과 측정결과를 비교하였으며, 일부 해석과 측정결과의 오차는 존재하지만, 성능변화 지점에서 동일한 경향성과 관심대역에서 유사한 성능저하를 확인하였으며, 반-무한배열구조 해석을 이용하여 전자기 주기구조 내 발생하는 결함요소의 해석 가능성을 확인하였다. In this paper, a semi-infinite array analysis method is proposed to analyze the defects that may occur during the fabrication of the frequency selective radome. In the analysis of N×N finite array structure using 3D analysis software, much analysis time and memory are required, whereas the semi-infinite array structure analysis proposed in this paper can predict relatively accurate electromagnetic performance while reducing the analysis time. In comparison with the results of the periodic simulation, the simulation of the semi-infinite array structure confirmed the error within ±3%. To verify the results of the simulation, the results were compared with the measured results, and the same tendency at the point of performance change and similar performance degradation at the band of interest were investigated. Using the proposed semi infinite array structure analysis, it is confirmed that the analysis of defects in the electromagnetic periodic structure is possible.
