This paper presents a new jetting dispenser driven by a piezoelectric actuator at high operating frequency to provide very small dispensing dot size of adhesive in modern semiconductor packaging processes. After describing the mechanism and operational principle of the dispenser, a mathematical model of the structured system is derived by considering behavior of each component such as piezostack and dispensing needle. In the fluid modeling, a lumped parameter method is applied to model the adhesive whose rheological property is expressed by Bingham model. The governing equations are then derived by integrating the structural model with the fluid model. Based on the proposed model, dispensing performances such as dispensing amount are investigated with respect to various input trajectories.

Purpose: The purpose of this study was to identify factors influencing the Radiation Protection(RP) behaviors of endoscopy nurses during endoscopic interventional radiology. Methods: A total of 188 endoscopy nurses working at 30 tertiary or general hospitals participated in this questionnaire-based study. The questionnaire included items on general and job related characteristics, RP knowledge, RP attitude, RP behavior, self-efficacy, and safety climate. Data were collected through online surveys from March 22 to April 10, 2019. Results: Multivariate analysis revealed that RP attitude (β=.65, p<.001), safety climate (β=.12, p=.035), self-efficacy (β=.14, p=.009), and existence of RP protocols (β=.11, p=.038) were significant predictors of better RP behavior. Conclusion: The findings showed that the RP behavior of endoscopy nurses was at high levels and the continuing education for endoscopy nurses and development of a radiation safety management education program were important to improve RP behavior.

Objective: To investigate the clinical, laboratory, radiologic characteristics, and joint involvement of rheumatoid arthritis (RA) in Korean. Methods: We retrospectively reviewed initial medical records of 210 patients diagnosed as rheumatoid arthritis in Seoul, Sanggye, and Ilsan Paik Hospital, Inje University. Evaluation included age, sex, disease duration, duration of morning stiffness, joint involvement, the number of tender and swollen joints, rheumatoid factor, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and bony erosion in simple X-ray. Results: In total 210 patients, mean age was 50.1±13.5 years, and male to female ratio was 1 to 5.6. Mean duration of arthritis was 51.8±72.6 months and positive rate of rheumatoid factor was 66.7%. Mean time interval between symptom onset and first visit was 14.1±19.1 months. Bony erosion rate was 15.3% in early RA patients, who had been diagnosed as RA within 1 year. The order of frequency of joint involvement was as the follows; metacarpophalangeal (MCP) joints, wrist joints, proximal interphalangeal (PIP) joints, metatarsophalangeal (MTP) joints, ankle joints, and knee joints. It was noted that the joints on the right and left sides behaved in like fashion. This pattern was not different with early RA. The titer of rheumatoid factor showed poor correlations with variables of clinical activity (ESR, CRP, duration of morning stiffness, and number of tender and swollen joints). Conclusion: In Korea, clinical manifestations and joint involvement of RA patients were not different from other published studies. The most commonly involved joints are MCP joints, wrist joints, PIP joints, and MTP joints.

Renal manifestations of systemic sclerosis are proteinuria, hypertension, azotemia and renal crisis. Mild proteinuria is common, but the nephrotic syndrome occurring in association with systemic sclerosis has been rarely reported. The majority of nephrotic syndrome cases are caused by glomerulonephritis in patients with an overlap syndrome of SLE and systemic sclerosis. This report suggests that the nephrotic syndrome can be an infrequent sequel of systemic sclerosis alone without glomerulopathy. A 38-year old woman was suffering from visual loss and headaches. She had Raynaud`s phenomenon and thickened skin on nearly whole body. Urine protein was 4,950 mg/day and serum albumin level was 2.7 g/dL. The result of renal biopsy was renovasculopathy-compatible with systemic sclerosis.

목 적 : 본 연구는 어린 연령의 소아에서 선천성 면역력의 중요한 인자인 Mannose Binding Lectin(MBL2) 유전자의 다형성 및 일배체형의 분포를 심한 respiratory syncytial virus(RSV) 감염증 환자군과 정상대조군에서 분석함으로써, MBL2 유전자의 다양성이 소아 RSV 감염증의 발생과 예후에 기여하는지를 밝히고자 하였다. 방 법 : 1993년부터 2000년까지 7년 동안 서울대학교 어린이병원 소아과에서 RSV 감염증으로 치료받은 심한 RSV 감염증 환아 99명을 환자군, 건강한 성인 224명을 대조군으로 선정하였다. 유전형 분석은 선천성 면역의 중추적 역할을 하는 MBL2 유전자의 promoter -221 X/Y와 exon 1의 구조 변이의 분포를 환자대조군에서 비교하여 분석하였다. 결 과 : 환자군 99명의 중앙 연령은 9.5개월로, 6개월 미만 48명(49%), 6~24개월 사이 39명(39%), 그리고 24개월 이상이 12명(12%)이었다. 남아는 58명(59%)이었다. 환자군에서 MBL2 대립유전자 B다형성의 빈도는 유전형 AA 60(61%), 이형접합 AB 34(34%), 그리고 유전형 BB 5(5%)로 나타났다. 환자군의 promoter -221 X/Y 유전형의 분포는 유전형 YY 84(85%), 이형접합 XY 15(15%)이었으며, 유전형 XX는 발견되지 않았다. Exon 1의 구조 변이와 promoter 변이 모두 환자군과 대조군간에 차이가 없었다. MBL2 일배체형의 분포에도 두 군간에 차이가 없었다. 결 론 : 본 연구 결과, 한국인 소아에서 MBL2 유전자의 다형성과 일배체형이 심한 RSV 감염증에 기여하는 예후 인자임을 밝히지 못하였다. 향후 전향적으로 수집한 더 많은 수의 환자군과 적절한 대조군을 통한 연구가 시행되어야 할 것으로 생각한다. Purpose : We hypothesized that mannose binding lectin gene(MBL2), a key molecule of innate immunity, may contirbute to the development and the outcome of respiratory syncytial virus(RSV) disease in early childhood. This study was performed to investigate the genetic basis of polymorphisms and haplotypes of MBL2 for RSV disease severity in Korean children. Methods : Cases with severe RSV diseases are 99 children with severe RSV lower respiratory tract infections, who were admitted to the Seoul National University Children's Hospital through 1993~2000. The control subjects consisted of 224 anonymous healthy Korean blood donors. The frequency of promoter variant(-221, X/Y) and structural variant(codon 54) were compared between the case patient group and the control subject group. Results : The mean age of patients was 11.8 months; 49% were <6 months, 39% were 6-24 months and 12% were >24 months. In the cohort of cases of severe RSV diseases, the genotypic frequencies of structural variant in codon 54 were 61% for AA, 34% for AB, and 5% for BB. Those of the promoter X/Y variant were 85% for YY and 15% for XY. There were no significant differences in overall distribution of both structural and promoter variants between the cases and the control subjects. We did not observe statistical difference in the haplotypic frequencies of MBL2. Conclusion : Common variants of MBL2 gene most likely do not contribute to the risk for severe RSV diseases in Korean children. Further genetic association studies should be conducted in a larger propsectively recruited cohort of children with RSV infection.

This paper presents a new type of jetting dispenser for the integrated circuit (IC) fabrication and surface mount technology The proposed system is featured by the piezoelectric actuator and hydraulic magnification device. After describing structural component of the dispensing mechanism and its operation principle, both the fluid modeling and the hydraulic magnification modeling are undertaken with a lumped-parameter method based on the analogy of the fluid system and mechanical system. A mathematical governing equation is then derived by integrating the fluid model with the mechanical model of the driving piston and piezoelectric actuator. Subsequently, in order to achieve a desired dispensing amount, control algorithm adjusting duty cycle of the driving voltage is synthesized and control responses are presented in time domain.
류마티스관절염 환자에서 레플루노마이드와 메토트렉세이트 병합요법의 간독성 조사

Objective: Leflunomide is the newest disease-modifying anti-rheumatic drug (DMARD) that is known to have an equivalent clinical efficacy and tolerability to methotrexate (MTX). Previous studies reported that a co-treatment with MTX and leflunomide can induce additive clinical improvements in RA patients. However, a previous study also demonstrated a reversible elevation of the transaminase levels in up to 63% of patients administered a combination treatment of leflunomide and MTX. This study examined the hepatotoxicity of a combination treatment of MTX and leflunomide. Methods: From March, 2004, to February, 2006, 203 patients who had been treated in 3 rheumatology clinics, Goyang city, South Korea, were reviewed retrospectively. The data showed that 38.92% of patients scored higher than grade 1 hepatotoxicity and 6.90% of patients scored higher than grade 2 according to the NCI/NIH (National Cancer Institute/National Institutes of Health) Common Toxicity Criteria. Results: The median onset time of hepatotoxicity was 5.91 months after treatment. Leflunomide administration was stopped in 39 patients due to several adverse reactions. Among the 39 patients, hepatotoxicity was observed in only 20.51%, suggesting that the hepatotoxicity was not more frequent than expected. Hepatotoxicity did not increase in proportion to the dose of leflunomide and MTX, age, gender, and disease activity. Conclusion: These results indicate that a combined treatment of leflunomide and MTX can be used safely by monitoring the liver enzyme, particularly in the first six months.
