        ^(99m)Tc-HMPAO SPECT를 이용한 뇌 허혈성 질환의 진단

        A new radiopharmaceutical, technetium-99m hexamethylpropyleneamine oxime(^(99m)Tc-HM-PAO), has been reported to cross the blood-brain-barrier and to distribute in brain in porportion to regional blood flow. Measurement of cerebral blood flow with single photon emission computed tomography (SPCET) was performed using ^(99m)Tc-HMPO in 46 patients and clinical analysis was done. Decreased regional cerebral perfusion was observed in 41 cases. Moreover eight cases of them who had normal X-ray C-T scan revealed positive finding on SPCET. We porpose that ^(99m)Tc-HMPO SPCET seems to be a useful diagnostic method for evaluation of ischemic cerebral lesions.

        해면정맥동내 속목동맥과 정맥구조에 대한 미세해부학적 연구

        The description of the carotid artery and venous structure in the cavernous sinus, because of their size and location, was very short of even absent in the most anatomical textbooks. But, since the report of a direct surgical approach to the cavernous sinus by Parkinson in 1965, there has been an increasing number of reports of successful direct surgical approaches to the cavernous sinus. With the introduction of microsurgery and the developement of new techinques and surgical approach, a more accurate knowledge of vascular anatomy of the cavernous sinus is not only of theoretical academic interest, but may also have implications. To better study this vascular anatomy, eighty cavernous sinuses from fresh cadavers of adult subjects were dissected under the operating surgical microscope. A red solution of resin was injected to facilitate the dissection, and twenty cavernous sinuses were studied, based on serial sections in the coronal planes for the study of the venous structure of the cavernous sinus. The results of this study were summarized as follows. 1) The most common branch of internal carotid artery in the cavernous sinus was the meningohypophyseal trunk, the largest intracavernous branch, which was presented in 100% of our specimens, the inferior cavernous artery, in 96.3%, and capsular artery, in 13.8%. 2) The ophthalmic artery arised within the paraclinoid portion of the internal crotid artery in 33.8%. In 3.8%, it entered the floor of the optic canal through a foramen in the bone. But there was no intracavernous origin of the ophthalmic artery. 3) The cavernous sinus seemed to be a venous pathway, an irregular network of venous channel, not a trabeculated sinus.

        해면정맥동의 외측벽과 뇌신경의 주행에 대한 미세해부학적 연구

        The cavrnous sinus is a extradural venous pathway enclosed by leaves of dura and periosteum located on either side of the sella turcica, and has connections with cerebrum, cerebellum, brain stem and orbit. It contains the internal carotid artery, the abducens nerve, and the sympathetic nerves, and its lateral wall contains the third and fourth cranial nerves as well as the first and sometimes the second division of the fifth cranial nerve. These connections and the relationships make the sinus of special interest to neurosurgeons. However, the descriptions of the cavernous sinus and its contents show great variation in books and journals. To better define this anatomy, in this report, eighty cavernous sinuses from fresh cadavers were studied in detail using surgical microscope with special attention to the relationships important in surgical approaches on the intracavernous structures, and twenty cavernous sinuses were studied, based on serial sections in the coronal planes for the study of the microanatomical structures. The results of this study were summarized as follows: 1) The average distance between the posterior clinoid process and the entrance of the 3rd, 4th, 5th, 6th cranial verve into the cavernous sinus were 5.99±1.69, 12.00±2.83, 15.44±2.38, 17.85±3.12㎜. 2) The lateral wall of the sinus was composed of two layers: a deep layer is formed by the sheathes of nerve Ⅲ, Ⅳ, and Ⅵ with a reticular membrane and some dural fold, and the deep layer was less defined and more irregular and imcomplete. 3) The average length of the Parkinson's triangle was 14.76±3.40㎜(superior), 18.54±3.96㎜(inferior), 6.33±2.14㎜(posterior). 4) There were many variations in the origin and course of the sixth nerve. Among these, calssic pattern which originated and run all its course to orbit as a single trunk is 67.5%.

        백서 후궁 절제술 모델에서 Anti-adhesion Barrier Gel의 유착 방지 효과

        Objective:The authors compare peridural scar formation and adhesion with and without the use of Antiadhesion Barrier Gel(Adba) in an animal model of laminectomy. Methods : Forty-five Sprague-Dawley rats underwent a two level lumbar laminectomy. The Adba was applied to randomly assigned 30 rats around the dura. Remaining 15 rats underwent same operation without the use of Adba. The rats were sacrificed 2, 4, 8 weeks after surgery by 15 numbers. A gross anatomic assessment of scar formation was done using microdissection by an observer blinded to treatment. Amount of scar formation and tenacity were compared between experimental and control group by a numerical rating system. The histological comparing was also performed. Results : The amount of scar tissue and tenacity were reduced grossly and histologically at postoperative 2, 4, 8 weeks in animal model using Adba. Adba material was absorbed around 4 weeks of postoperative period in model. No special inflammatory reaction was observed, and the healing of wound was not affected by Adba. Conclusion : Adba significantly reduces the amount of scar formation and tenacity in rat laminectomy model without impacting the healing of operation wound and other complications.         대공부와 상위경추부 종양에 대한 수술적 치료

        Objective : The authors reviewed 51 patients of spinal cord tumor located at the craniovertebral junction and high cervical area to determine which factors influenced resectability, complications, and postoperative outcomes. Methods : Radiological examinations, clinical data, and operation notes were evaluated, and additional follow-up information was obtained from outpatient examinations. The mean follow-up period was 3.5 years Results : The most common neurological findings were motor deficit(72.5%) and sensory change(47.1%). There was no clinical finding that is considered as pathognomic. Meningioma(18 cases, 35.3%) was the most common tumor in this region. Total removal was achieved in 45 patients. There was no surgical mortality. Immediate postoperative motor weakness were encounted in 11 cases(21.6%) which improved in long term followup except two patients. Conclusions : The location and relationship of the tumor to surrounding struncture determine its resectability. Postoperative results were related to the preoperative neurological status and pathological findings.

        뇌낭미충증에 대한 임상고찰

        Cerebral cysticercosis is unfortunately a not uncommon disease in Korea, which was a hardly manageable parasitic infestation on the central nervous system. After the introduction of the brain CT scan, diagnosis and treatment of cerebral cysticercosis were improved markedly. The author analyzed 94 cases of cerebral cysticercosis who had been admitted to the Department of Neurosurgery and Neurology, Yonsei University Medical School from July 1972 to June 1983. Of the 94 patients, 52 operations were performed in 47 patients. The analysis and evaluation were made with particular emphasis on brain C-T scan finding.

        Abdominal pseudocysts have been recorded infrequently as a complication of intraperitoneal shunting. Authors have experienced two cases of subphrenic pseudocyst as a complication of ventriculoperitoneal shunt, which were treated with revision of shunt. First patient had medulloblastoma with hydrocephalus. The other one was a patient who had subarachnoid hemorrhage and hydrocephalus from Sturge Weber syndrome. It's diagnosis and treatment were discussed with review of literatures.

        A therapy to treat injuries to the central nervous system is, to date, a major clinical challenge. The devastating functional consequences they cause in patients have encouraged many scientists to search, in animal models, for a repair strategy that could, in the future, be applied to humans. To highlight the current understanding of traumatic spinal cord injury, we provide information regarding state-of-the-art care for the acute spinal cord injured model, and explore future treatments aimed at neural preservation and reconstruction. This article also reviews recent phase I clinical trials in neural and nonneural cell transplantation, stressing that the transition from basic science to clinical applications.
