        천포창 환자의 치과치료시 고려사항: 증례 보고 및 문헌고찰

        To treat pemphigus patients who suffer from autoimmune blistering disease, clinicians should apply special consideration to them. Since oral cavity is composed of mucosa membrane, dental treatment could irritate these patients’ mucosal surface and intensify the disease. In this case, 41-year old female with general pemphigus vulgaris got #25 fixed prosthodontics treatments. This case report suggests some dental treatment considerations for mucocutaneous vesiculobullous disease. 수포성 자가면역 질환에 걸린 천포창 환자의 치료함에 있어 임상의는 세심한 주의를 기울여야 한다. 치과치료로 인해 구강 내 점막이 자극되면 천포창은 심화될 수 있다. 이에 우리는 41세 천포창 환자의 고정성 보철물 수복 사례를 통해 천포창 환자에 있어 임상 치료시 주의점에 대해 알아보고자 한다.

        치근단 방선균증: 증례보고와 문헌고찰

        Actinomycetes is a type of normal inhabitants of the oral cavity and is an anaerobic bacteria mainly distributed on the root surface in contact with the gingival sulcus. A. israelii, a gram-positive bacterium, is normally present in carious teeth and calculus with the highest frequency. In the environment such as traumatic injury, apical infection, and pericoronitis that occurs around a partially impacted tooth, the nature changes to infectivity and causes an acute inflammatory response. For the treatment of actinomycosis, large doses of penicillin are administered for at least 10 days after surgical drainage and debridement. The apical lesions on radiographs can sometimes appear as malignant lesions, so differentiation is necessary and biopsy is essential. Therefore, in this classroom, actinomycosis was diagnosed and reported in the biopsy of the apical lesion of a tooth that had been extracted due to severe dental caries, and the literature review was conducted.

        Mandibular metastasis of cholangiocarcinoma: A case report

        Tumors metastasizing from distant regions to the oral and maxillofacial region are uncommon, comprising only 1%–2% of all malignancies. Cholangiocarcinoma is a malignancy that arises from cholangiocytes, which are epithelial cells that line the bile ducts. These cancers are difficult to diagnose and have a poor prognosis. In this paper, we report a rare case of mandibular metastasis of cholangiocarcinoma diagnosed at the primary site and discuss the radiographic findings observed in this case.

        Study about Dental Extraction of Patients Receiving Antiplatelet Treatment

        The population of patients with antiplatelet treatment is expanding globally with the rising prevalence of cardiovascular disease and increasing use of percutaneous coronoary interventions. While antiplatelet agents have revolutionized the management of atherosclerotic disease and its thromotic complications, the potential of bleeding remains an inherent risk. Dentists are more likely to stop antiplatelet therapy before dental extraction because they think that the patient is at high risk for bleeding. However stopping or altering antiplatelet therapy may expose such patients to the risk of a thromboembolic event such as thromboembolism, myocardial infarction, or cerebrovascular accidents particulary in patients wirh drug-eluting stent. In this study,we report 3cases that were perfomed dental extraction in patients receiving single or dual antiplatelet therapy without immediate and late postextraction bleeding and reviewed the literature on dental extraction in patients receiving antiplatelet therapy. We concluded that dental extractions may be safely performed in patients receiving single or dual antiplatelet therapy when appropriate local hemostatic measures are taken, thus averting thrombotic risk of temporary antiplatelet discontinuation.

        치과 병원 응급실에 내원한 환자의 주소에 따른 분류와 연구

        The rise of medical knowledge and awareness of the importance of dental and stomatognathic system increase the patients who visit dental emergency room. The chief complaints of patients who visited the emergency room varies from a pain, trauma, hemorrhag. The purpose of this study was to classify dental emergency patients by chief complaint and to analyze in indiviual group and to provide more effective emergency dental care. This study was carried out with 1129 patients visiting emergency room of the dankook university dental hospital from 7/2011 to 6/2012. Dental emergency patients was classified trauma, pain, hemorrhage group by chief complaint and studied mothly, the day of the week, time, age distribution and cause of the individual group. The chief complaint of visiting emergency room were trauma 660 people (58.5%), pain 347 people (30.7%), hemorrhage 96 (8.5%), other 26 patients (2.3%). The monthly distribution was observed in May (12.8%), March (10.5%), September (10.2%). The trauma patients were frequent in the spring and early summer but painful patients visited in September (12.7%) and May (11.8%) in March (11.2%). The peak age group was 20 to 29 years(20.9%), followed by 0 to 9 years(19.4%), 40 to 49years (15.2%,). In trauma group the peak age was under the age of 10 (31.7%), followed by 10 to 19 years (18.8%), 20 to 29 years (17.1%) . However, in pain group, peak age was 20 to 29 years (26.8%) followed by 40 to 49 years(21.3%), 30 to 39 years (19.6 %). The most common cause of trauma were subluxation(16.5%),laceration13.7%), uncomplicated crown fracture(12.05%) and in pain group was pulpal origin(46.1%), followed by periodontal origin(20.7%), post op pain(8.9%). Undefined pain or neuralgia were 7.9%. The most common cause of hemorrhage was post extraction(66.7%), post operation(16.7%), spontaneous bleeding due to periodontitis(12.5%). In conclusion, the trauma, pain were different in monthly distribution and the peak age of patients. Dental emergeny doctor should understand pattern of indiviual emergency group and perform proper diagnosis and treatment for more effective emergency care.

        Histopathologic Comparison of Osteomyelitis, Osteoradionecrosis, Medication-Related Osteonecrosis of the Jaw

        Osteonecrosis is defined as non-vital bone tissue as a result of abnormal process of osseous healing, and is caused by several reasons such as infection, radiation, and medication. Osteomyelitis, osteoradionecrosis, and medication related osteonecrosis of the jaws (MRONJ) which have necrotic bone in common are confused clinically due to similar symptoms and radiographic findings, and are difficult to diagnose definitively. Because each disease represents a separate clinical progress and requires a different treatment approach, it is very important to distinguish each disease. The aim of this study was to analyze the histopathologic features of osteomyelitis, osteoradionecrosis, MRONJ and to understand their different pathogenesis.

        Sevoflurane 흡입진정에서 어려운 기도관리를 위한 LMA 활용

        Although sevoflurane sedation have the advantage of the reduction of anxiety and pain relief, difficult airway is attributed to increased agitation, tachycardia, desaturation, ventilation difficulty, sedation failure. In the sedation for dental treatment, we should pay more attention to the airway management because dental procedures take place in the mouth of airway unlike any other area. The layngeal mask airway (LMA) has become commonly used device for airway management during anesthesia for relatively short procedures, such as minor oral surgery and dental procedures. It can be inserted without use of a larygoscope and muscle relexants, and insertion is easy to achieve and generally takes less time than endotracheal intubation. The LMA is an excellent barrier against aspiration of saliva, blood within the surgical field but should not be used in patients at risk of aspiration In this study, we reported that after a failure of airway management in inhalation sedation, we performed the short-emergency dental treatment successfully, using a laryngeal mask.

        치과진료실에서 발생한 약물 관련 아나필락시스 (Anaphylaxis): 증례 보고

        Anaphylaxis is rare event in dental clinics and there are few case reports of anaphylaxis related to prescription drugs in dental clinic. A 53-year-old woman with a rhinitis and drug allergy history had a severe anaphylactic reaction after taking prescription drugs in dental clinic. The patient had cutaneous manifestations such as angioedema, generalized urticaria and respiratory symptoms such as dyspnea, wheezing, cardiovascular symptom such as hypotension and central nervous symptom such as drowsy. The patient received emergency care, including epinephrine administration in the emergency room and her symptoms improved. The purpose of this paper is to report a case of anaphylaxis related with oral medications prescribed by dentist and to discuss the risk factors for drug-induced anaphylaxis that dentists should be aware of.
