        전담간호사의 역할갈등 적응과정에 대한 근거 이론적 접근

        Purpose: The purpose of this research is to disclose the particular experiences of Clinical Nurse Specialist (CNS) with role conflict in Korea. Methods: This research utilized grounded theory approach in order to study the experiences CNS with role conflict, to disclose the basic socio-psychological process of them and to develop a substantive theory with regards to these finding. The author had in-depth interviews and participant observations with them until they were saturated. Total 8 CNS participated in this study. Results: As a result, 72 concepts, 24 subcategories and 13 categories were generated and paradigm model was proposed as follows. The causal conditions were 'role ambiguity,' 'high achievement motive.' The central phenomenon was 'being indecisive' for CNS. The contextual conditions that affected indecision were 'new experiences', 'unexpected workload'. The intervening condition was 'comparing others,' 'communicating,' 'pursuing knowledgement,' 'pending system.' The action/interaction, which are strategic or routine responses of the participants was 'aggressive coping,' 'passive coping.' According to consequences, those who discovered 'being indecisive' with role conflict of CNS can select 'distinct role,' or 'resignation.' The core category of this study was the 'establishing role.' Conclusion: As a nursing administration, it is needed to improve CNS' role and position by developing system, and as a nursing education, there will be prepared for CNS' job education program.

      • 발달장애인 평생교육센터의 운영현황 및 발전방안 연구 - 서울특별시를 중심으로 -

        본 연구는 서울시 발달장애인 평생교육센터의 운영현황을 분석하고 향후 발달장애인 평생교육센터의 발전적 방향을 모색하기 위해 혼합연구방법을 활용하여 수행되었다. 양적 조사에서는 서울시 16개 발달장애인 평생교육센터 181명의 종사자를 대상(전수조사)으로 수집된 설문자료를 분석하였고, 질적 조사에서는 센터장 4명을 대상으로 진행한 초점집단인터뷰 녹취 록을 토대로 내용분석을 실시하였다. 양적 조사 결과에서는 센터의 기본 운영현황(시설, 대상, 교사, 수업현황 등) 및 종사자의 센터운영에 대한 인식(인력비율, 직원교육 및 슈퍼비전 현황 및 필요도, 향후 센터 이용대상 등)을 제시하였고, 질적 조사에서는 보다 구체적인 센터운영의 현황 및 문제점, 발전 방안 등의 결과를 도출하였다. 이와 같은 연구 결과를 토대로 본 연구에서는 발달장애인 평생교육센터 운영의 발전방안을 다음과 같이 제언하였다. 첫째, 발달장애인 평생교육센터의 고유한 기능 수행을 위해 인적 지원체계의 변화와 개선이 필요하다. 둘째, 발달장애인 평생교육센터의 효과적 운영을 위해 물리적 환경의 최소기준이 재정비되어야 한다. 마지막으로 발달장애인 평생교육센터가 발달장애인의 자립을 위한 센터로서 역할을 하기 위해서는 명확한 지침에 따른 기능정립이 필요하다. This study was conducted by using a mixed-methods study to analyze the operation status of the Seoul lifelong education center for people with developmental disabilities and to find the developmental direction of the lifelong education center for people with developmental disabilities in the future. In the quantitative survey, the survey data collected by targeted (complete enumeration survey) to 181 employees of 16 Seoul lifelong education center for people with developmental disabilities, were analyzed. And in the qualitative survey, the content analysis was conducted based on the recorded transcripts of the focus group interview conducted by targeted to the four chiefs of the center. In the quantitative survey, the basic operation status of the center (the facility, object, teacher, class status, etc.) and the employees perception of the center operation (the manpower ratio, status and necessity of the staff education and supervision, the object of using center in future, etc.) were presented, and in the qualitative survey, the results of the more specific status and problems of center operation, and development plans were drawn. Based on the study results like those, in this study, made suggestions for the development plan of operation of the lifelong education center for people with developmental disabilities as follows. First, it is necessary to change and improve the human support system for the performance of the unique functions of the lifelong education center for people with developmental disabilities. Second, the minimum standard of the physical environment should be reorganized for the effective operation of the lifelong education center for people with developmental disabilities. Lastly, it is necessary to set up the function according to clear guidelines for the lifelong education center for people with developmental disabilities to play a role as the center for developmental disabilities self-reliance.

        대상포진 척수염을 동반한 대상포진 치험

        Herpes zoster is a relatively common disease caused by Varicella-Zoster virus and characterized by a vesicular dermatomal rash and a variety of neurologic manifestations. These neurologic symptoms include herpes zoster neuralgia, myelitis, encephalitis, cranial arteritis, segmental and rarely polyradi- culitis. This report is a case of a 57-year-old female with herpes zoster paresis affecting the arm and hand. Herpetic pain was much relieved after continuous cervical epidural blockade. However, fifty days after onset of ailment, she complained motor paralysis. EMG of muscles innervated by the left Ca and T1 roots revealed high frequency denervation potentials and large amplitude polyphasic motor units. We recommended physiotherapy and aggressive exercise therapy of the hand. After eighty days of therapy, denervated spontaneous activities disappeared on EMG. Clinically, strength of muscles had nearly recovered to 75% of normal strength in left hand intrinsics

        결핵예방 행동의도의 영향 요인

        The new case rate of tuberculosis (Tbc) in Korea has increased over the past several years. The present study’s purpose was to identify influencing factors of and investigate the mediator on Tbc preventive behavioral intention in college students. Descriptive correlational and cross-sectional survey design was employed. The data were statistically analyzed by using the descriptive and inferential statistics. Sobel test was used to identify mediator’s significance. A convenience sample of 490 subjects recruited from two universities in Gyeongsangbuk-do and Jeollabukdo, Korea. The mean age of subjects was 21.15±1.60. The subjects was showed relatively low level of Tbc preventive behavioral intention with mean score of 3.02±0.47. The overall model significantly explained 24.9% of variance in Tbc preventive behavioral intention. Among predictors, the residential types, exercise, tuberculin skin test, behavioral beliefs, attitude, and subjective norm were significantly associated with Tbc preventive behavioral intention. Attitude was mediator between behavioral beliefs and Tbc preventive behavioral intention. Therefore, when developing the strategies for reinforcing intention toward Tbc preventive behaviors, the interventions for providing behavioral beliefs, improving attitude, and enhancing subjective norm should be considered. These findings might provide a theoretical basis to develop of effective strategies for enhancing Tbc preventive behavioral intention in college students.

        마우스 유방암 모델에서 5-Aza-2'-deoxycytidine의 암줄기세포 유지 억제 효과

        비정상적 DNA메칠화는 암 발생에 있어 중요한 역할을 한다. 최근 연구에 의하면 암줄기세포 유지에 있어 DNA과메칠화가 연관되어 있다고 보고하고 있다. 따라서 본 연구는 4T1 유방암 실험모델에서 demethylating agent인 AZA 처리에 따른 후성유전적 변화가 암줄기세포의 유지 및 증식에 있어 어떠한 영향을 미치는지 조사 하였다. 4T1 세포에서 AZA 처리에 따른 tumorsphere 형성이 감소 하는 것을 in vitro 실험을 통해 관찰 하였고, in vivo 실험에서는 줄기세포 조절 유전자들(Oct-4, Nanog. Sox2)의 발현이 감소 되는 것을 확인 하였다. 본 연구 결과로 볼 때 4T1 유방암 실험모델에서 AZA에 의한 후성유전적 변화는 줄기세포 조절 유전자(SRG)들의 발현을 조절하면서 암줄기세포 특성을 변화시켜 암줄기세포의 증식 및 유지를 억제 할 것으로 사료된다. 향후 이러한 DNA 메칠화억제를 항암치료에 응용하면, 암줄기세포를 파괴함으로써 암의 재발 및 악성화를 효과적으로 제어 할 수 있을 것으로 사료된다. Aberrant DNA methylation plays an important role in the development of cancer. It has been reported recently that DNA hypermethylation is involved in the maintenance of cancer stem cells. The present study was designed to test the hypothesis that the demethylating agent, 5-aza-2‘-deoxycytidine (AZA), can inhibit the potential for maintenance of cancer stem cells. To validate this hypothesis, we used 4T1 syngeneic mouse models of breast cancer. The AZA pre-treated 4T1 cells showed a dramatic inhibition of tumorsphere formation, compared to their counterparts in vitro. In addition, the AZA treatment significantly suppressed the expression of stem regulator genes, such as oct-4, nanog and sox2, compared to counterparts in vivo. Therefore, selective inhibition of DNA methylation may be useful for stem-specific cancer therapy.

        Implantable Port System을 이용한 복합부위통증증후군 치험 1예 -증례 보고-

        The complex regional pain syndrome(CRPS) exhibit symptoms such as: abnormal skin color, temperature change, abnormal pseudomotor activity, edema. If CRPS is not treated appropriately at acute stage, then the affected extremity may become a useless, painful appendage. Treatment of CRPS by sympathetic blockade may be achieved by repeated intravenous regional guanethidine blocks, repeated anesthetic sympathetic blocks, surgical sympathectomy or oral sympatholytic therapy. We treated 29-year-old male patient with CRPS of left upper extremity by continuous cervical epidural blockade. Due to wound infection and dislocation of the epidural catheter, we inserted an implantable port system to inject the mixture of local anesthetics and small amount of morphine. After 10 months of treatment, patient was cured of symptoms and signs of CRPS and was able to resume a normal life.

        직장-가정생활 부정적 상호전이과정과 여성 친화적 기업문화가 유연근무제도 수요에 미치는 영향

        Currently, Korean working women have been struggling to achieve work-family balance. The company actively adopt flexible work scheduling to help working mothers achieve work-family balance. However, little research has been done on "Why". Current study investigates the Korean working mother's demands for flexible work scheduling. Negative work-family spillovers results in increased demands for flexible working scheduling, while negative family-work spillovers has no impacts. Women-friendly organizational cultures reduce working mothers' demands for flexible work scheduling. Contrary to working mothers, negative work-family interactions and family-friendly organizational cultures fail to provide any effects on flexible scheduling demands. Therefore, it is necessary to consider organizational cultures and gender differences when building and establishing flexible work scheduling. 최근 유연근무제도는 근로자의 일과 가정의 양립을 지원하는 방안으로 사회적 관심을 모으고 있다. 본 연구는 직장과 가정생활의 부정적 상호전이 과정과 여성 친화적 기업문화가 근로자의 유연근무제도의 수요에 미치는 영향과 성별 효과를 살펴보고자 하였다. 한국여성정책연구원이 조사한 여성 관리자패널 데이터를 활용하여 여성 관리자 748명과 남성 관리자 481명을 대상으로 각 변수들의 영향력을 분석하였다. 연구결과에 의하면, 기혼 여성 관리자들의 직장생활이 부정적으로 가정생활에 전이되면 유연근무제도의 필요성이 증가되었다. 반면, 가정생활이 직장으로 부정적으로 전이되는 것은 유연근무제도의 필요성에 별다른 영향을 미치지 못하였다. 여성 친화적 기업문화에서는 기혼 여성 관리자의 유연근무제에 대한 필요성이 줄어든다. 남성 관리자의 경우, 직장-가정생활의 부정적 상호전이과정 그리고 여성 친화적 기업문화는 그들의 유연근무제도 수요에 유의미한 영향을 미치지 못하였다. 유연근무제도의 수요에 대한 남녀의 차이는 성 역할에 대한 사회적 가치와 기업문화에 기인한다고 볼 수 있다. 유연근무제도가 근로자의 일과 가정의 양립 지원의 도구로 활용되기 위해서는 기업문화가 중요한 변수임이 확인되었다.
