        시멘트형 Polished Versys HeritageⓇ 대퇴 스템의 2-5년 추시 결과

        Purpose: Surface finishing of a cemented femoral stem is a subject of controversy even though contemporary cementing techniques have improved results. Versys Heritage femoral stem was used with a contemporary cementing technique to determine the outcome of using a polished surface. Materials and Methods: Between October 2000 and August 2003, 131 primary hybrid hip arthroplasty procedures were performed in 117 patients. The mean age at the time of the index arthroplasty was 52 years and 9 months (24-78 years). The average follow up period was 49 months (range, 24-71 months). All the hips were evaluated clinically by the Harris hip score and radiologically by the cement grade, osteolysis as well as stress shielding. Results: At the final follow up, the Harris hip score for all patients had improved from preoperative 59.1 (range, 17-91) to post operative 92.8 (range, 78-100). The cement grade was measured using Barrack's method. Of 131 hips, 75, 54 and 2 were grade A, grade B and grade C1 at the final follow up, respectively. None of the hips showed a C2 or D grade. There was no evidence of aseptic loosening or osteolysis on the femoral side during follow up. 45% of stems showed grade 1 and 2 stress shielding according to Engh's criteria. Conclusion: In this study, the Polished Versys Heritage femoral stemⓇ showed excellent results at the short to mid term follow up period. However, a longer-term follow-up study will be needed to clarify the implications of the femoral prosthesis surface finish or design.

      • 골종양 치료에 있어서 사지 구제술식의 임상적 결과

        During the last decade, the use of primary amputation has decreased, and a larger number of patients are being treated with resection and reconstruction with limb preservation. The advantage of chemotherapy, improvements in diagnostic imaging methods and newer techniques for reconstruction have made limb salvage an alternative to amputation for most patients with malignant bone tumor. Clinical results and functional results of the limb salvage operation of bone tumors treated at the Department of Orthopaedic Surgery, Guro Hospital, Korea university was reviewed. Eighteen bone tumors(seven giant cell tumors and eleven malignant bone tumors.) were studied over a period of five years. The limb salvage group included fourteen endoprothesis, two pasteurized autograft, one near total scapulectomy and one rotationplasty. Functional evaluation was performed according to the Enneking's modified system. The results were as followed; 1. At a mean follow-up of thirty two months (15-77 months), fourteen(83%) of the eighteen patients with limb salvage procedures had been continuously disease free. 2. There are no local recurrence but deep infection developed in two patients and three patients with distant metastasis. 3. Thirteen(87%) of the fifteen patients showed above 60% of functional results. 4. Fourteen(93%) of the fifteen patients were satisfied with the limb salvage procedures.

        Total Hip Arthroplasty: Past, Present, and Future. What Has Been Achieved?

        In the 1960s, Sir John Charnley introduced to clinical practice his concept of low-friction total hip arthroplasty (THA). Although early designs were plagued by poor performance and even failure, there have been steady advances in implant designs, biomaterials, surgical techniques and an understanding of the biomechanical restoration of the hip; these advances have contributed to improvements in implant survival and clinical outcomes of THA in the past three decades. With improved wear resistance and mechanical reliability, a potential to last for at least 25 to 30 years are now available for THA. In this review, we focus on the evolution of THA and review current controversies and future directions of this procedure based on a single surgeon’s 29-year of experience at a single institution.

      • 스테로이드성 괴사에 있어서 혈관 및 지방변화에 관한 실험적 연구

        To elucidate the pathogenesis of steroid-induced necrosis of the bone, the author performed a histologic study on femoral head, lung, kidney, and liver in the rabbits. Of twenty two rabbits, twelve were administered a weekly dose of five milligrams of methylprednisolone (group Ⅰ), eight were administered a weekly dose of eight milligrams of methylprednisolone (group Ⅱ) and two were employed as a control. Each, two rabbits were sacrificed at one, three, four, eight, ten and twelve weeks after administration in group Ⅰ and another two rabbits were sacrificed at one, three four and eight weeks in group Ⅱ. Those histologic changes mainly observed on hemorrhage, vascular and fatty change, osteoporosis, and necrosis of bone were studied. The results obtained are as follows. 1. Fatty changes of the femoral head showed increasing the size of fat cell at one week after initial administration and were progressively increased until four weeks, but size of fat-cell began to decrease at eight weeks. Vascular changes of the femoral head were noted at three weeks and necrosis of vascular wall at eight weeks. Finally avasrular necrosis of the femoral head were found at ten and twelve weeks in group Ⅰ. 2. Fatty changes of the liver reavealed at one week and the changes were progressively aggravated with portal and central venous congestion and dilatation until eight weeks in group Ⅱ. Necrosis with hepatitis were noted at ten and twelve weeks in group Ⅰ and at eight weeks in group Ⅱ. 3. Fatty changes of the lung were not noted in group 1 and 2 through experimental periods. Vascular changes of the lung showed subintimal edema, endothelial hyperplasia, medial hypertrophy and vacuolar degeration at three weeks, subintimal edema and fibrosis at ten weeks and twelve weeks in group Ⅰ and endateritis at four and eight weeks in group Ⅱ. 4. Fatty changes of the kidney showed fat vacuoles in the proximal tubules at four and twelve weeks in group Ⅰ, one and four weeks in group Ⅱ. Vascular changes of the kidney showed mucinous degeration and edema of medial layer in arterioles and small artery at four weeks, erdateritis and subintimal fibrosis at eight weeks in group Ⅰ and hydrophic dgeneration at four weeks in group Ⅱ. With the above results it is suggested that vascular changes proceed or associated with fatty change in steroid induced necrosis.

        Assessment of Korea’s Orthopedic Research Activities in the Top 15 Orthopedic Journals, 2008–2017

        Background: Bibliometrics is increasingly used to assess the quantity and quality of scientific research output in many research fields worldwide. This study aims to update Korea’s worldwide research productivity in the field of orthopedics using bibliometric methods and to provide Korean surgeons and researchers with insights into such research. Methods: Articles published in the top 15 orthopedic journals between 2008 and 2017 were retrieved using the Web of Science. The number of articles, citations and h-index (Hirsch index), funding sources, institutions, and journal patterns were analyzed. Results: Of the total 39,494 articles, Korea’s contribution accounted for 5.6% (2,161 articles), ranking fifth in the world in the number of publications. Korea ranked sixth (with 29,456) for total citations worldwide but ranked 17th (13.64) in terms of average citation per item and 14th (55) in terms of h-index. Korea showed the most prolific productivity in the field of sports medicine and arthroscopy. The institution that produced the highest number of publications was Seoul National University (n = 386, 17.9%). Conclusions: Orthopedic research in South Korea demonstrated high productivity in terms of the number of publications in high-quality journals between 2008 and 2017. However, total citations and average citations per article were still relatively low. Efforts should be made to increase citation rates for further improvement in research productivity in the field of orthopedics.
