        류마티스관절염 환자의 골절 위험요인에 대한 메타-분석

        Objective: This study was conducted by using a meta-analysis to calculate the mean effect sizes of the risk factors for fracture of patients with rheumatoid arthritis (RA) and to perform tests for the significance of the calculated mean effect sizes. Methods: Eleven studies that directly examined the relationships between fracture and risk factors were selected from 179 related studies identified from PubMed, MEDLINE, COCHRAN and CINHAL with the key words being `rheumatoid arthritis`, `fracture` and `risk factors`. Results: The mean effect size of age on fracture was significant. The mean effect sizes of steroid use and the duration of RA on fracture were also significant. However, all of these effect sizes (age, the use of steroid and the duration of disease) were small (bar D=0.15, 0.16 and 0.12, respectively). The mean effect size of the BMD at the lumbar spine was significant and the effect of the BMD at the L-spine was of medium size (bar D=0.47). The mean effect sizes of the other risk factors such as disability, disease activity, the BMD at the femur neck, BMI and a smoking habit were not significant. Conclusion: From the study results, it was noted that the efforts to prevent fracture of RA patients should focus on patients with an older age, the patients who used steroid, the patients with a longer duration of RA and the patients who present with low BMD at the lumbar spine.

        유방암 환자의 항암화학요법으로 인한 말초신경병증: 관련요인 및 건강 관련 삶의 질에 미치는 영향

        Purpose: This study aimed to identify factors associated with the occurrence of chemotherapy-induced peripheral neuropathy (CIPN) and to study its impact on health-related quality of life (HRQoL) in patients with breast cancer. Methods: In total, 137 women with breast cancer who had undergone more than one cycle of chemotherapy were recruited for this descriptive study from the outpatient department of a university hospital in Incheon, Korea. Data were collected using a self-reported questionnaire, which included the European Organization for Research and Treatment of Cancer (EORTC) QLQ-CIPN20 and EORTC QLQ-C30. Results: The mean CIPN score between patients was 13.57 on the sensory scale, 15.87 on the motor scale, and 25.06 on the autonomic scale. Of the studied socio-demographic, disease and treatment, and health behavior-related factors, only the chemotherapy regimen was significantly associated with CIPN (t = 2.50, p= .013). Taxane-based chemotherapy was significantly related to higher CIPN scores. Regression analyses revealed that CIPN was a factor that was significantly influential on HRQoL, adjusting for socio-demographic and clinical factors. Conclusion: This study suggests that oncology nurses need to be aware of the increased risk of CIPN in patients with breast cancer undergoing taxane-based chemotherapy. In addition, interventions for alleviating CIPN may be required to improve HRQoL among these patients.

        통풍 환자를 위한 웹기반 자가-관리 중재 프로그램이 통풍 관련 지식, 치료제 복용 순응도 및 자가-관리에 미치는 효과

        Purpose: This study was conducted to examine the changing patterns of knowledge related to disease, medication adherence, and self-management and to determine if outcomes were more favorable in the experimental group than in the comparison group through 6 months after providing a web-based self-management intervention. Methods: A non-equivalent control group quasi-experimental design was used and 65 patients with gout, 34 in experimental group and 31 in comparison group, were selected from the rheumatic clinics of two university hospitals. Data were collected four times, at baseline, at 1 month, 3 months, and 6 months after the intervention. Results: According to the study results, the changing patterns of knowledge and self-management were more positive in the experimental group than in the control group, whereas difference in the changing pattern of medication adherence between two groups was not significant. Conclusion: The results indicate that the webbased self-management program has significant effect on improving knowledge and self-management for middle aged male patients with gout. However, in order to enhance medication adherence, the web-based intervention might not be sufficient and other strategies need to be added.

        동맥 경화증 위험요인들에 대한 정준상관 분석

        The purpose of this study was to examine the relationship among variables which are categorized by two group : the first group consists of obesity, serum lipids, and blood pressure which were measured by physical index or physiological index, and the second group consists of demographic variables and variables related to lifestyle that is known to influence to first group. The canonical correlation analysis was conducted with the data collected from 400 male clients who visited one university hospital located in Inchon in a period, from may 1996 to December 1996, for physical examination. According to the results, blood pressure and HDL of the first group were shown to have strong relationship with age, alcohol, smoking, exercise of the second group. And total cholesterol, triglyceride, systolic, BMI of the first group were shown to have strong relationship with income status, education, exercise of the second group. And all of the serum lipids (total cholesterol, triglyceride, HDL), BMI, and diastolic were shown to have relationship with education, smoking, alcohol. It should be noted that fat rate, which was one of the indices for obesity, was not significant in any of the canonical variates. From the comparative study results on which combination of indices of obesity, serum lipids and blood pressure was related to either alcohol or smoking more, it can be seen that smoking was more highly related to two blood pressure indices such as systolic and diastolic, and that alcohol was highly related to serum lipid indices such as HDL, total cholesterol, triglyceride. Also investigation on what differences exist between the combination of variables showing high relationship to systolic and the combination of variables showing high relationship to diastolic was conducted. Systolic turned out to have relationship with total cholesterol, triglyceride, obesity (fat rate), income status, education, exercise. And diastolic was related to HDL-c, total cholesterol, tryglyceride, obesity (BMI), education, smoking, alcohol. From this results, it could be seen that the combination of variables which have high relationship to systolic and diastolic have different patterns. Lastly, the investigation on what relationships exist between serum lipids and demographic variables/lifestyle variables was conducted. It turned out that HDL-c had high relationship with age, education, income status, alcohol, and that all of serum lipids (total cholesterol, triglyceride, HDL) were shown to have high relationship with income status, education, age, exercise. From these results, it could be seen that the demographic variables were more influential both on HDL and on all of serum lipids than lifestyle variables were.

        기초간호자연과학의 인체구조와 기능 내용별 필요도에 대한 연구

        The purpose of this study was to define the content of requisite human structure and function knowledge needed for clinical knowledge of nursing practice. Subjects of human structure and function were divided into 10 units, and each unit was further divided into 21 subunits, resulting in a total of 90 items. Contents of knowledge of human structure and function were constructed from syllabus of basic nursing subjects in 4 college of nursing, and textbooks published by nurse scholars prepared with basic nursing sciences. The degree of need of 90 items was measured with a 4 point scale. The subjects of this study were college graduated 136 nurses from seven university hospitals in Seoul and three university hospitals located in Chonnam Province, Kyungbook Province, and Inchon. They have been working at internal medicine ward, surgical ward, intensive care unit, obstetrics and gynecology ward, pediatrics ward, opthalmology ward, ear, nose, and throat ward, emergency room, rehabilitation ward, cancer ward, hospice ward, and their working period was mostly under 5 years. The results were as follows : 1. The highest scored items of human structure and function knowledge necessary for nursing practice were electrolyte balance, blood clotting mechanism and anticoagulation mechanism, hematopoietic function, body fluid balance, function of plasma, and anatomical terminology in the order of importance. The lowest scored items of human structure and function knowledge necessary for nursing practice was sexual factors of genetic mutation. 2. The highest order of need according to unit was membrane transport in the living unit, anatomical terminology in movement and exercise unit, mechanism of hormone function in regulation and integration unit, component and function of blood in oxygenation function unit, structure and function of digestive system in digestive and energy metabolism unit, temperature regulation in temperature regulation unit electrolyte balance in body fluid and electrolyte unit, concept of immunity in body resistance unit, and genetics terminology in genetics unit. The highest order of importance according to subunit was membrane transportation in cell subunit, classification of tissues in tissue unit, function of skin and skin in skin subunit, anatomical derivatives of the skeleton subunit, classification of joints in joint subunit, an effect of exercise on muscles in muscle subunit, function of brain in nervous system subunit, special sense in sensory subunit mechanism of hormone function in endocrine subunit, structure and function of female reproductive system in reproductive system unit, structure and function of blood in blood unit, structure of heart, electrical and mechanical function in cardiovascular system unit, structure of respiratory system in respiratory system subunit, structure and function of digestive system in digestive system subunit, hormonal regulation of metabolism in nutrition and metabolism subunit, function of kidney in urologic system subunit, electolyte balance in body fluid, electolyte and acid-base balance subunit. 3. The common content of human structure and function knowledge need for all clinical areas in nursing was structure and function of blood, hematopoietic function, function of plasm, coagulation mechanism and anticoagulation mechanism, body fluid, electrolyte balance, and acid-base balance. However, the degree of need of each human structure and function knowledge was different depending on clinical areas. 4. Significant differences in human structure and function knowledge necessary for nursing practice such as skin and derivatives of the skin, growth and development of bone, classification of joint, classification of muscle, structure of muscle, function of muscle, function of spinal cord, peripheral nerve, structure and function of pancrease, component and function of blood, function of plasma, structure and function of blood, hemodynamics, respiratory dynamics, gas transport, regulation of respiration, chemical digestion of foods, absorption of foods, characteristics of nutrients, metabolism and hormonal regulation, body energy balance were demonstrated according to the duration of work. 5. Significant differences in human structure and function knowledge necessary for nursing practice such as classification of tissue, classification of muscles, function of muscles, muscle metabolism, classification of skeletal muscles, classification of nervous system, neurotransmitters, mechanism of hormone function, pituitary and pituitary hormone, structure and function of male reproductive organ, structure and function of female reproductive organ, component and function of blood, function of plasma, coagulation mechanism and anticoagulation mechanism, gas exchange, gas transport, regulation of respiration, characteristics of nutrients, energy balance, function of kidney, concept of immunity, classification and function of immunity were shown according to the work area. Based on these findings, all the 90 items constructed by Korean Academic Society of Basic Nursing Science should be included as contents of human structure and function knowledge.
