        6,000 m 고도에 해당하는 급성·평압 저산소 조건에서 키네시오 테이핑 적용이 고유수용성 감각기능에 미치는 영향

        Purpose: It is known that there is impairment in the responses of ther opprioceptive sense function at high altitudes. However, the effect of Kinesio taping (KT) on the response of joint position proprioception to acute normobaric hypoxia remains unclear. The purpose of this study is to evaluate the effect of KT on proprioceptive sense function in a normobaric hypoxic condition corresponding to an altitude of 6,000 m. Method: In a single-blind, balanced crossover study, 18 healthy voluneters (22.7 ± 1.8 years) were randomly allocated to receive either Kinesio taping + hypoxia (KH), hypoxia (HP), or control (CT) with a 1-week interval. For the assessment of proprioceptive sense function,a ctive joint position sense (AJPS) and passive joint position sense (PJPS) were measured at a knee angle of 5°5 of flexion from the maximal passive exte-n sion. KT was applied to different regions of the knee extension muscle group. The acute normobaric hypoxic condition was maintained for 40 minutes using a low oxygen gams ixture. All data were analyzed using twoway ANOVA with repeated measures. Results: The absolute angular error of AJPS in KH and HP was significantly greater than that in CT at posttest (8.0 ± 4.3 and 8.8 ± 6.0, respectively, vs. 2.5 ± 1.3, P<0.01). The absolute angular error of PJPS in KH and HP was significantly greater than that in CT at post-test (7.1 ± 5.5 and 6.8 ± 2.2, respectively, vs. 2.2 ± 1.6, P<0.01). However, the absolute angular error was not significantly different between KH and HP. Conclusion: These results suggest that acute hypoxia impairs the proprioceptive sense function of the knee, and hypoxia-induced damage to proprioceptive sense function is not affected by Kinesio taping.

        상부요로계에 대한 내비뇨기 시술에서 예방적 항생제 사용과 시술 후 감염성 합병증 발생에 관여하는 위험인자

        Purpose: We investigated a retrospective analysis of infectious complication and its risk factors after endourologic procedures of the upper urinary tract, focusing on antibiotic prophylaxis (AP). Materials and Methods: We studied AP in 488 upper urinary tract examinations or treatments. Procedures included ureteral stenting, percutaneous nephrostomy, retrograde pyelography, and diagnostic ureteroscopy. We calculated that the prevalence and kind of AP and the incidence of febrile infectious complications with respect to each procedure and attempted to find the risk factors. Results: AP was used in 456 (93.3%) cases totally. Antibiotics of quinolone were the most popular medication. There are 17 (3.7%) febrile infectious complication cases after procedure, the most common procedure was percutaneous nephrostomy in 7 cases (6.0%). Bacteriuria, hydronephrosis, whether or not ureteral stent, or nephrostomy, or Foley catheterization was applied prior to examination or treatment, and were independent risk factors for infectious complications (<0.05). Conclusions: The data suggest that the risks of infectious complications should be evaluated carefully and AP should be recommended according to those risks before the procedures.

        대사증후군 백서모델에서 음경해면체의 발기능 및 조직학적 변화

        Purpose: We evaluated the changes of the erectile function and histology of the corpus cavernosum in a rat model of metabolic syndrome. Materials and Methods: We used male spontaneous hypertensive rats (SHRs) as an experimental group (n=6) and Wistar-Kyoto rats as a control group (n=6). The SHRs were fed with a high fat diet, but the Wistar-Kyoto rats were fed with a normal fat diet for 12 weeks. All the groups were then checked for body weight and various biochemiclal parameters. To investigate penile erection, the intracavernosal pressure (ICP), mean arterial pressure (MAP) and cGMP level of the corpus cavernosum were recorded for all the groups. Serial sections of the penis were used to perform Masson's trichrome staining and immunohistochemistry for determining the TGF-β1 expression. Results: We confirmed that metabolic syndrome was induced in the experimental group by the significant difference of the various biochemical parameters. This wasn’t clear.)As a result of erectile function, the ICP/MAP ratios were checked as 51.0±7.5% and 31.0±5.5%, respectively, for the control and experimental groups. So the ICP/MAP ratio of the latter was markedly decreased compared with the former and the cGMP level of the corpus cavernosum was the same for both groups. On Masson's trichrome staining, the number of smooth muscle cell was decreased and the collagen fibers with an irregular, distorted arrangement were increased in the experimental group. The immunoreactivity for TGF-β1 tended to increase in the experimental group. These histological findings revealed that fibrosis of the corpus cavernosum occurred in the experimental group. Conclusions: Our results demonstrate that metabolic syndrome is harmful to erectile function and it leads to histological changes of the corpus cavernosum according to a rat model of metabolic syndrome

        리컴번트 각도가 운동과 회복기 중 뇌혈류와 심부담도에 미치는 영향

        Background: The cerebral blood flow velocity (CBFV) and rate pressure product (RPP) have been reported to benefit hemodynamics more during exercise in the recumbent position than during that in the upright position. However, it is unclear which angle is of the greatest benefit to hemodynamics during exercise in the recumbent position. This study aimed to evaluate the effect of the recumbent angle on CBFV and RPP during exercise. Methods: In a balanced crossover study, 15 healthy volunteers (age, 18.7±1.1 years) were asked to perform the bicycle exercise four times in the upright and at recumbent angles of 67° (R), 47°R, and 15°R, with weekly intervals between each condition. The exercise intensity was set initially at 50 W and increased by 25 W every 2 minutes up to 150 W. CBFV in the middle cerebral artery (MCA) was measured using transcranial doppler sonography. All data were analyzed using two-way repeated-measures analysis of variance. Results: CBFV at 15°R was significantly higher than that in the upright position and at 67°R at 10 minutes recovery (69±16 vs. 56±7 and 58±10 cm/s, respectively; P<0.05). The RPP was significantly lower at 67°R, 47°R, and 15°R than in the upright position immediately after exercise (176±23, 177±22, and 173±26, respectively, vs. 241±42 mmHg×beats/min×10-2; P<0.001). Conclusions: At an angle of less than 67°R, exercise increases CBFV and reduces RPP, relative to those during exercise in the upright position. This hemodynamic effect was most prominent at 15°R, where cerebral circulation was further increased in the recovery phase. 연구배경: 리컴번트 자세는 직립자세에 비해 운동 중 뇌혈류속도는 증가시키고 심근 산소소비량은 감소시킨다. 따라서 본 연구의 목적은 뇌혈류속도와 심근 산소소비량 향상에기여할 수 있는 최적의 리컴번트 각도를 조사하는 것이다. 방법: 본 연구에 참여한 15명의 건강한 성인 남성은 직립(upright, 90°), 67° 리컴번트(R), 47°R, 15°R와 같이 네 개의각도에 각각 교차되도록 무작위 배정되었다. 자전거 에르고미터 운동부하는 점진적으로 주었다. 뇌혈류속도는 뇌혈류초음파장비에 의해 측정되었으며, 심근 산소소비량은 분당심박수와 수축기 혈압을 이용하여 계산되었다. 측정된 모든데이터는 이원반복측정 분산분석을 이용하여 분석하였다. 결과: 뇌혈류속도는 회복기 10분에 15°R 집단이 upright, 67°R 집단에 비해 유의하게 높은 것으로 나타났다(69±16 vs. 56±7, 58±10 cm/s, P<0.05). 심근 산소소비량은 67°R, 47°R, 그리고 15°R 집단이 upright 집단에 비해 운동종료직후 유의하게 낮은 것으로 나타났다(176±23, 177±22, 173±26 vs. 241±42 mmHg×beats/min×10-2, P<0.001). 결론: 이 결과는 67° 이하의 리컴번트 자전거 운동은 뇌혈류속도를 증가시키고 심근 산소소비량을 감소시키지만 특별히15° 리컴번트 자전거 운동 종료 후 회복기의 뇌혈류를 더욱촉진시킬 수 있음을 시사한다.

        변혁적리더십이 조직몰입과 이직의도에 미치는 영향: 조직공성성의 매개효과

        This study aims to investigate the effects of transformational leadership on the organizational commitment and turnover intention. And the effects of organizational justice on the organizational commitment, turnover intention. Also, practical analysis has been done by employees in the hotel firms located in Seoul to cross-check how to affect mediating effects of organizational justice on the relationship between transformational leadership and organizational commitment, turnover intention. The findings of this study suggest that the transformational leadership make an influence on the organizational commitment and the turnover intention. And the organizational justice make an influence on the organizational commitment and the turnover intention. And also, the organizational justice affect mediating effects on the relationship between transformational leadership and organizational commitment, turnover intention. The ultimate results of this study bring up to set up the new strategy in ways on the organization management practices focused on the organizational justice, organizational commitment and turnover intention in the organizational management of the hotel firms to preempt competitive advantage. And the contributions and limitations of this research were discussed and the future possible researches were mentioned.
