        액션러닝 기반 건강사정 수업 운영의 간호대학생의 건강사정에 대한 자신감, 건강사정 지식, 비판적 사고능력, 수업 만족도에 대한 효과

        Purpose: The purpose of this study was to identify the effects of the action learning approach on the self-confidence and knowledge of health assessment, critical thinking ability and class satisfaction in students taking health assessment courses. Methods: This non-equivalent control group pretest-posttest study enrolled 127 nursing students as participants, with 64 and 63 in the experimental and control group, respectively. These two groups attended 33 hours (2 or 4 hours per week for 11 weeks) of action learning and traditional classes, respectively. Differences in the dependent variables between the two groups were compared before and after the intervention using independent t-test. Results: The action learning group reported significantly greater self-confidence in health assessment (t=5.10, p<.001) and critical thinking ability (t=2.23, p=.027) than the control group. There was no significant difference in knowledge of health assessment or class satisfaction between two groups (p>.05). Conclusion: These findings indicate that action learning is an effective intervention for enhancing self-confidence and critical thinking ability in nursing education.

        외국인 환자의 의료서비스 만족도, 재방문 의사, 추천 의사에 영향을 미치는 요인

        Background: This study aims to analyze quality of and satisfaction with Korea medical services and identify factors affecting medical service satisfaction, revisit, and recommendation intention among international patients. Methods: Secondary analysis of survey data conducted by Korea Health Industry Development Institute from June 10th to July 17th in 2013 was done using multiple regression and logistic regression analysis. The 191 international patients from 9 medical institutions in Seoul were enrolled. Results: The results showed that international patients were satisfied with 85.6 points out of 100.0 points. International patients appraised higher in staff service rather than other services. Factors influencing medical service satisfaction were gender, religion, medical specialty, length of stay, and quality of medical services. Quality of medical service explained 29.8% of medical service satisfaction and especially, `doctor`s care` and `communication and patient respect` were significantly related to medical service satisfaction. Medical specialty had a significant influence on revisit intention. There were no statistically significant influencing factors of recommendation intention. Additionally, more satisfied patients were associated with higher revisit and recommendation intention. Conclusion: This study implies that quality of medical services is a critical factor for patient satisfaction and that satisfaction with medical services is an important factor for increasing revisit and recommendation intention among international patients. In addition, health care providers should consider cultural differences to enhance satisfaction with medical services for international patients. Therefore, multidimensional strategy is required to strengthen the cultural competency of healthcare providers.

        병원환경에 대한 가상현실을 활용한 수업이 간호대학생의 병원환경 지식, 학업적 자기효능감, 학습몰입, 교육만족도, 학업성취도에 미치는 효과

        Purpose: This study examined the effects of virtual reality (VR)-based fundamental nursing education on nursing students’ knowledge of the hospital environment, academic self-efficacy, learning flow, educational satisfaction, and academic achievement. Methods: In total, 137 nursing students participated in this non-equivalent control group pretest-posttest study, with 69 and 68 in the experimental and control groups, respectively. Differences in the dependent variables between the two groups before and after the intervention were compared using the independent-sample t-test and Mann-Whitney U test. Results: The VR intervention group showed significantly higher knowledge of the hospital environment (t=3.88, p<.001), educational satisfaction (z=-3.82, p<.001), and academic achievement (z=-2.54, p=.011) than the control group. Conclusion: These findings indicate that VR-based education is an effective intervention for improving knowledge of the hospital environment, educational satisfaction, and academic achievement in nursing education.

        간호사 확보수준이 수술한 암환자의 원내 사망 및 감염에 미치는 영향

        본 연구의 목적은 의료기관 간호사 확보수준이 수술한 암환자의 원내 사망과 원내 감염에 어떠한 영향을 미치는지 알아보고자 하였다. 이차자료를 이용하였으며, 2012년 상반기에 260개 의료기관에서 수술 후 사망률이 높은 6개 암으로 수술받은 암환자 24,510명을 대상으로 하였다. 단순 로지스틱 회귀분석과 일반화추정방정식(GEE) 모형을 이용하여 분석하였다. 환자 및 의료기관 특성(기관 종별 유형, 소재지, 수술건수)을 보정 후, 사망 가능성은 간호사 확보수준 0-1등급에 비해 2-3등급 군(odds ratio [OR], 1.46; 95% confidence interval [CI], 1.00-2.11), 6-7등급 군(OR, 3.28; 95% CI, 1.87-5.74)에서 더 높은 것으로 나타났다. 병원 감염이 발생할 가능성은 간호사 1인당 침상수가 하나씩 증가할 때마다 증가하여, 0-1등급 군에 비해 2-3등급 군은 6.63배(95% CI, 3.00-14.62), 4-5등급 군은 5.79배(95% CI, 1.88-17.78), 6-7등급 군은 8.4배(95% CI, 1.82-38.84) 높은 것으로 나타났다. 낮은 간호사 확보수준은 환자안전과 의료의 질을 반영하는 지표인 높은 수술 후 원내 사망과 감염발생과 관련이 있었다. 이는 적절한 간호사 확보 수준은 수술 후 암 환자 결과와 관련 있음을 보여준다. 수술한 암환자의 의료의 질 향상과 환자 안전을 위해서는 적절한 간호사 수가 확보되어야 하며, 이를 위한 정책 유지가 필요하다. This study was conducted to examine the influence of the nurse staffing level on the level of in-hospital death and infection in cancer patients who received surgery. Secondary data were used and the subjects of this study were 24,510 patients who received surgery for six types of cancer with a high postoperative mortality rate in the first half of 2012 at 260 hospitals. Simple logistic and GEE multiple logistic regression analyses were used. After adjusting for the patient and hospital characteristics, a greater likelihood of dying was found in the nurse staffing level 2-3 group (odds ratio [OR], 1.46; 95% confidence interval [CI], 1.00-2.11) and in the level 6-7 group (OR, 3.28; 95% CI, 1.87-5.74) compared to the level 0-1 group. The likelihood of in-hospital infection increased with each additional bed per nurse, being 6.63 times higher (95% CI, 3.00-14.62) in the level 2-3 group, 5.79 times higher (95% CI, 1.88-17.78) in the level 4-5 group, and 8.4 times higher (95% CI, 1.82-38.84) in the level 6-7 group, as compared to the level 0-1 group. A lower nurse staffing level was associated with higher in-hospital death and infection levels. This shows that an appropriate nurse staffing level is associated with superior postoperative cancer patient outcomes. Policies for providing adequate nurse staffing should be maintained for the sake of ensuring improved care quality and patient safety.

        플립러닝을 활용한 기본간호학 수업이 간호대학생의 자기주도적 학습능력, 문제해결능력, 수업만족도에 미치는 효과

        Purpose : This study seeks to examine the effects of using flipped learning in fundamentals of nursing classes on self-directed learning ability, problem-solving ability, and class satisfaction among nursing students. Methods : A one-group pretest-posttest design was utilized with the participants, involving 62 nursing students. The data were analyzed using descriptive statistics and paired t-test using SPSS/WIN 24.0. Results : The scores on self-directed learning and problem-solving abilities significantly increased after flipped learning classes (t=6.93, p<.001; t= 16.49, p<.001, respectively). Students’ class satisfaction was lower than 3.5 at 3.38 points. Conclusion : The findings indicate that flipped learning is an effective intervention for improving self-directed learning and problem-solving abilities in nursing education. It is imperative to enhance the faculty s competency to apply various teaching methods to increase students’ class satisfaction.

        휴대폰 동영상 촬영을 활용한 교육이 간호학생의 핵심기본간호술 수행 자신감, 성취도, 실습 만족도에 미치는 효과

        Purpose: This study was conducted to investigate the effects of core basic nursing education using cellular phone video recordings on self-confidence in performance, achievement, and practice satisfaction. Methods: The research design was a nonequivalent control group modified and non-synchronized design. The participants were assigned to either the experimental (n=60) or control group (n=67). During one semester, the experimental group received training for 10 selected core basic nursing skills using cellular phone video recording. The control group, also for one semester, received training with traditional practices. Self-confidence in performance and practice satisfaction were measured using a questionnaire, and achievement was evaluated by professors at the conclusion of the semester. Collected data were analyzed using t-test and Pearson correlation coefficients. Results: Self-confidence in performance increased significantly after training in the experimental group (t=7.94, p<.001). The experimental group showed significantly higher self-confidence in performance (t=2.39, p=.018) and achievement (t=2.40, p=.018) compared to the control group. However, the difference in practice satisfaction was not statistically significant. Conclusion: The results indicate that cellular phone video recording is effective in improving self-confidence in performance and achievement of core basic nursing skills.

      • 폐암 환자의 의료 이용에 영향을 미치는 요인

        The purpose of this study was to explore the utilization of health care of patients with lung cancer in Korea and identify determinants of these patients` health care utilization. Methods: This was a descriptive analytical study. The national medical fees claims data of patients with lung cancer were used. Using SPSS Statistics 20, the x2-test and logistic regression were performed to determine the factors influencing health care utilization. Results: There were significant differences by sex, age, disease type, stage, comorbidity index, region of institutions, and type of institutions in the utilization of surgical procedures; by age, disease type, stage, comorbidity index, region of institutions, and type of institutions in the utilization of chemotherapy; and by age, stage, comorbidity index, region of institutions, and type of institutions in the utilization of radiotherapy. Conclusion: The findings of this study suggest that democratic and clinical characteristics of patients as well as institutional characteristics affect health care utilization of patients with lung cancer. Additional research is needed to determine the factors influencing health care utilization of patients with lung cancer.

        간호학생의 학업성취도와 임상수행능력 및 임상수행자신감의 관련성

        Purpose: The purpose of this study was to determine relationships between academic achievement, clinical competence, and confidence in the clinical performance of nursing students, and to identify factors that influence them. Methods: This was a descriptive correlation study. Data were collected from 118 nursing students at a nursing college in Seoul. One-way variance analysis, Pearsons correlation coefficient, and multiple linear regression analysis were used to examine relationships between academic achievement, clinical competence, and confidence in clinical performance. Results: Clinical competence was related to academic achievement in students’ previous semester and to students’ academic scores in a fundamental of nursing course. Clinical competence showed a weak positive correlation with academic achievement in students’ previous semester and academic scores in a fundamental of nursing course in both evaluations by the professor and students’ self-evaluations. However, confidence in clinical performance had no significant correlation with academic achievement. The factor affecting the clinical competence was academic scores in a fundamental of nursing course, and factors affecting confidence in clinical performance were health status, personality, major suitability, and class satisfaction in a fundamental of nursing skills course. Conclusion: These findings indicate that students with high academic achievement have better clinical performance, but confidence in clinical performance is not related to academic achievement.

        외국인 환자 만족도 측정 도구의 신뢰도와 타당도

        연구목적 : 본 연구는 한국보건산업진흥원에서 기 개발된 외국인 환자 만족도 측정 도구를 수정·보완하여 신뢰도와 타당도를 규명하고자 시도되었다. 연구방법 : 연구 대상은 서울·경기지역에서 국제진료센터를 운영하고 있는 9개 병원에서 외래 혹은 입원하여 진료를 받은 외국인으로 영어, 러시아어, 몽골어 중 한 개 언어에 능통한 자를 대상으로 하였다. 기 개발된 외국인 환자 만족도 측정 도구에 대하여 국내외 유사 도구 검토, 전문가 내용 타당도 검토 후 예비 문항을 개발하여 설문조사를 실시하였고, 신뢰도 및 타당도 분석 및 전문가 자문 과정을 거쳐 최종 도구를 개발하였다. 연구결과 : 최종 외국인 환자 만족도 측정 도구는 7개 항목, 39개 문항으로 구성되었다. 7개 항목은 `의사 진료`(5개문항), `정보 및 교육`(6개 문항), `병원생활 및 진료비`(8개 문항), `병원 편의`(5개 문항), `직원 서비스`(4개 문항), `의사소통과 환자 존중`(4개 문항), `접근성`(4개 문항)이며, `전반적 만족도`, `재이용 의사` 및 `추천 의사` 3개 문항의 총 39개 문항으로 확정하였다. 최종 7개 항목 36문항의 Cronbach`s alpha는 .95이며, 항목별 신뢰도는 `의사 진료` .83, `정보 및 교육` .88, `병원생활 및 진료비` .85, `병원 편의` .85, `직원 서비스` .77, `의사소통과 환자 존중` .86, 그리고 `접근성` .65이었다. 결론 : 이상의 결과를 종합하여 볼 때 국내 의료기관을 방문한 외국인 환자 만족도를 측정하는데 신뢰도와 타당도 높은 도구가 개발된 것으로 판단된다. 본 연구에서 개발된 외국인 환자만족도 측정 도구를 활용하여 외국인 환자의 경험을 측정함으로써 궁극적으로 외국인 환자에 대한 의료서비스의 질을 향상시키는데 기여할 것으로 생각된다. Objective: This study aimed to verify the reliability and validity of the international patient`s satisfaction inventory, which had been previously developed, by revising and supplementing it. Methods: Toward this end, the study selected foreign patients who had received in-patient or out-patient treatments in nine hospitals in Seoul and Gyeonggi-do which run an international care center and who are fluent in either English, Russian, or Mongolian. After reviewing other satisfaction inventories in Korea and other countries that are similar to the developed satisfaction inventory and examining its validity from specialists, the study developed preliminary questions and used them for a survey. After the reliability and validity analyses as well as a consultation with specialists, the study developed the final inventory. Results: The final international patient`s satisfaction inventory consisted of seven categories and 39 questions. The seven categories were “doctor`s care (five questions),” “information and education (six questions),” “hospital environment and financial aspect (eight questions),” “hospital convenience (five questions),” “staff services (four questions),” “communication and patient-respect (four questions),” and “accessibility (four questions),” while the 39 questions included “overall satisfaction,” “whether to use the service again,” and “whether to recommend the service to others.” Cronbach`s alpha coefficient of the 7 categories, 36 items was .95, and the reliability coefficient of “doctor`s care,” “information and education,” “hospital environment and financial aspect,” “hospital convenience,” “staff services,” “communication and patient-respect,” and “accessibility” was .83, .88, .85, .85, .77, .86, and .65, respectively. Conclusion: The results of this study showed that the previously developed international patient`s satisfaction inventory offered high reliability and validity in measuring the satisfaction level of foreign patients who visit medical facilities in Korea. It is believed that using the developed satisfaction inventory to measure the experience of foreign patients will ultimately contribute to the quality of medical services to foreign patients.
