        Tc-99m DISIDA SCAN에서 deconvolution 방법을 이용한 간암 환자의 수술 후 잔여 간 기능의 정량적 평가

        Surgical removal of hepatic cancerous tissues have been widely performed due to its early detection. However, a patient can not survive if excessive hepatic tissues were removed. Therefore, quantitative evaluation of remaining hepatic function after surgery is a really important factor for surgeon. the currently used ICG Rmax and Lidocaine clearance tests have disadvantages such as tedium, complexity, and inability to estimate remaining hepatic function after surgery. While HEF has advantages such as simplicity, quickness, nonivasiveness, and quantification, its reliability has been doubtful. Thus, the program for calculation of HEF has been developed from serial gamma camera image data. And we compared the reliability of HEF with ICG Rmax and Lidocaine clearance test using 6normal and 18 abnormal rabbits with damaged livers. The correlation coefficient of HEF to ICG Rmax and MEGX was 0.91, 0.94, respectively. I was also found that the HEFs of normal and abnormal hepatic tissues was higher than 100% and lower than 80%, respectively. Thus we confirmed that HEF can be a good indicator distinguishing between abnormal tissues and normal ones. Finally, we could conclude that patients would survive if both the pre-and the post-operative HEF were greater than 60%.

        임피던스 심장기록기의 개발과 응용

        The thermodiluton is a standard method to measure cardiac output in clinical medicine. However it has many disadvantages such as expensive instrument and measurement, limited number of measurement, pain, safety problem, and side-effect due to insertion of catheter into heart. Electrical Impedance Cardiography has no such disadvantages and that it can continuously monitor stroke volume, contractility of cardiac muscle, and systolic time interval (STI) as well as cardiac output. While this impedance technique has been widely used and vigorously studied adroad, it is not introduced yet in Korea. Thus an Impedance Cardiograph has been developed in order to introduce this new technique. Its accuracy also has been verified by simultaneous measurement of cardiac output with the thermodilution technique. Finally changes of cardiac function during exercise were also measured. 현재 임상에서 환자의 심박출량(cardiac output)을 측정하는데 널리 쓰이는 열희석법(thermodilution)은 카테터(catheter)의 심장주입에 따르는 위험성, 부작용 및 고도의 기술요구와 측정장비 및 비용의 고가, 측정 횟수의 제한, 환자의 고통등 여러 가지 문제점을 내포하고 있다. 한편 Electrical Impedance Cardiography는 이러한 열의석법의 단점들을 해결하고 나아가서 열희석법과는 달리 계속적으로 심박출량 뿐만 아니라 박동량(stroke volume) 및 심근육의 수축력등 심장의 기계적 기능을 감시할 수 있는 방법으로서, 외국에서는 널리 쓰이고 있고 이에 대한 연구도 활발한 상태이나 국내에서는 거의 보급이 안되어 있는 설정이다. 그리하여 본 연구에서는 이 새로운 분야의 국내 보급을 위하여 시제품을 완성하고 임상에서의 표준방법인 열희석법과 동시에 측정하여 기기의 정확성을 확인하였으며 운동 중에 운동부하 증가에 따른 심장기능의 변화도 측정하였다.

        임피던스 방법을 이용한 체지방 측정

        Bioelectrical Impedance method for measuring human body composition is based upon the principle that the electrical conductivity of the fat-free mass (FFM) is far greater than that of the (at. Deter- mination o( impedance was nlade in 30 healthy and obese men aged $40.9{\pm}14.7$yr (mean${\pm}$SD) and 25 healthy and obese women aged $44.0{\pm}10.2$ yr. A commercial impedance body fat analyzer was used with a four electrode arrangement that introduced a painless constant current(ImA at 50 kHz) into the body. Linear relationships were found between impudence values and FFM(r=-0.786) and between weight and FFM(r=0.837). Signigicant increases in the correlation coefficients were ob served when the predicator Hta/z was regressed against FFM(r=0.912) where Ht Is height and Z Is impedance. A linear regression equation, FFM=0.586(Htf/z) +0.317(Weight) -1.674(r=0.977), was found. The correlation coefficient of % body fat between the impedance and infrared methods was 0.898. These data Indicate that the bioelectrical impedance technique is a reliable and valld approach for the estimation of human body composition. This method Is safe, noninvasivr, prouides rapld measure menu, requires little operator skill and subject cooperation, and Os portable. Finally measurement of percent body fat was made possible using both the regression equation and a developed impedance measuring device which measures impedance between wrist and ankle.

        자동 보정 주파수 의존형 근관장 측정기의 개발

        Among the apex locators, the frequency dependent type is more accurate and convenient to use than others, But the accuracy of the apex locator is still influenced by the presence of various electrolytes used in root canal treatments. In this study, we have developed a frequency dependent electronic apex locator minimizing the influence of the electrolytes on the measurement of root canal lengths. It was also confirmed that two frequencies of 500Hz and 100kHz are optimal for the measuring impedance compare with commercial product used(400Hz and 8kHz)a and there were no differences in accuracy among the three different types of the waveforms; sinusoidal, triangular, and rectangular waves(p>0.05). Impedance ratio of the two different frequencies represents the position of the file in root canal, and the voltage difference of two signals represents the status of the fluid in the root canal. As a result of compensation using the voltage differences, the errors were decreased on the average from +0.54mm to +0.18mm in $H_2O_2$ solution (p<0.01), and from -0.33mm to -0.01mm in NaOCl solution(p<0.01). The accuracies based on ${\pm}$0.5mm, in $H_2O_2$ and NaOCl solutions were improved with the automatic compensation from 71.1% and 91.1% to 82.2% and 100% respectively.

        인덕턴스 호흡감시 시스템의 개발

        The impedance pneumography which is widely used in monitoring respiration is simple to use and noninvasive, but it is sensitive to motion artifacts and insensitive to detect obstructive apnea. A 3-channel respiratory inductive plethysmography (RIP) developed in this study detects inductance change of the inductance band induced by cross-sectional area change of thorax or abdomen as one breathes. It was confirmed that RIP was less sensitive to various motion artifacts but more sensitive to detection of obstructive apnea than impedance pneumography.

        각종 전자파에 의한 인체의 노출 : 역학조사를 위한 전자파 측정

        Although air, water, and noise pollutions have been widely recognized, electromagnetic forces (EMF) hazard has been rarely recognized as a pollution and very little studies has been done in this country. Thus, in this study electromagnetic forces radiated by various home appliances, office machines, and communication equipments were measured and so were several places radiating strong EMF such as subway stations and electric substations. Among the home appliances microwave oven radiates lots of magnetic field and microwaves, and electric mattress does strong magnetic field. In video game room strong magnetic and considerable electric fields were measured. It was observed strong magnetic field inside of electric powered train and very strong magnetic and electric fields on some platforms. Hand-phone and car-phone radiate very hazardous level of microwaves to brain and that they rapidly come into wide use. In this study data base for various electric machines and places radiating strong EMFs were constructed and could be used for future epidemiological studies.

        新羅 眞智王代의 王權强化와 彌勒信仰

        Maitreya(彌勒) was believed since the reign of King Beopheung(法興王) when Buddhism was officially accepted but it was from King Jinheung(眞興王) that the faith was regularly established and from King Jinji(眞智王) that it was transmitted in recording.<br/> The figure involved with faith in Maitreya in the reign of King Jinji is Jinjasa(眞慈師). he believed in Maitreya which had a character of traditional faith(傳統信仰). He recommended Misirang(未尸郞) as a Maitreya disciple to King Jinji and made him an elite youth of Hwarangdo(花郞徒).<br/> Misirang has a character of traditional faith, his being expressed as Maitreya indicates that traditional faith in fused into Buddhism, However, Misirang becoming an elite youth complained of the politics of King Jinpyeong(眞平王) and disappeared.<br/> Maitreya becoming popular in the reign of King Jinji resulted from trying to consolidate royal powers, In this process, King Jinji appointed Buddhist priests like Jinjasa, Bojang(寶藏) and Hyejang(慧藏) and pushed for the policies solidifying royal powers with a help of Buddhist influence.

        신라 중대 초 당제의 수용과 정비

        신라는 520년 율령을 반포하였는데 17관등과 백관의 공복, 골품제도 등 상고기부터 시행되어 오던 관습과 제도, 법제 등 고유하고 전통적인 부분을 많이 포함하고 있었다. 이후에도 새로운 법령과 법률이 계속해서 만들어졌는데, 특히 진흥왕대와 진평왕대에 중앙의 행정관부와 새롭게 복속한 지방민들에 대한 규정이 대폭적으로 확대되었는데, 이것은 520년에 반포된 율령의 관련 조목에 편입되면서 새로운 조문들이 증가하였다. 642년 대야성이 함락된 이후 김춘추는 왕권 강화와 지배체제의 개편을 구상하고, 적극적인 대당외교를 추진하여 당제를 수용하였다. 그리고 신라가 당제를 수용한 시기는 648년 김춘추가 당에 파견된 이후부터라고 할 수 있다. 그는 651년부터 정국운영의 구상을 적극적으로 추진하였는데, 이것은 당 의관과 연호를 사용하는 한화정책의 추진과 집사부의 설치를 중심으로 하는 관제정비의 실시로 나타났다. 당제의 수용은 이후 골품제도의 변화를 가져오는 계기로 작용하는 등 신라 사회에 큰 영향을 주었다. 당제의 정비는 신문왕대부터 새롭게 변화하여 성덕왕대의 활발한 대당외교와 경덕왕대의 지명과 관부 명칭의 변경으로 이어졌는데, 특히 중대의 전제왕권과 한화정책은 상호 깊은 관련을 맺고 있다. 주목되는 것은 신라가 당제를 수용하였음에도 불구하고 중앙관부는 당 중앙 관부의 핵심인 3성6부제를 실시하지 않고 이전부터 존재하였던 집사부를 중심으로 운영되었다는 사실이다. 이것은 신라가 당제를 수용하여 관료제를 추진하면서도 한편으로는 진골귀족의 영향력이 작용하고 있었기 때문으로 보이며, 이러한 의미에서 신라 중대에 당제의 정비과정은 일정한 한계를 지녔다고 할 수 있다. 그러나 829년 집사부가 집사성으로 바뀌고 여러 관부들이 새롭게 설치되었던 하대에는 변화된 모습을 보이면서 이후 태봉과 고려의 관제정비에도 일정한 영향을 끼치고 있다. Silla proclaimed Yulryeong(율령) in 520, which included its inherent and traditional contents such as 17 official ranks and official uniforms of all the government officials and Bone rank system etc. it had enforced before. Even thereafter, new laws and regulations were enacted continuously, among which regulations on central administrative departments and newly subjugated local people were drastically increased. After Daeya Castle(대야성) was fell in 642, Kim Chun-chu(김춘추) planned the strengthening of royal authority and the reorganization of ruling system and actively promoted diplomacy policy toward Tang(대당외교) and accepted Tang"s system(당제). It can be said that Silla accepted Tang system since Kim Chun-chu was dispatched to Tang in 648. He pushed the Chinese assimilation policy(한화정책) using Tang"s dress and era name starting from 651 and carried out the modification of government systems(관제정비) such as establishment of Jipsabu(집사부) etc. The acceptance of Tang system took a chance to bring about the change of Bone rank system(골품제도) and had a great influence on Silla society. The maintenance of Tang system was newly changed from the time of King Sinmun and connected to active diplomacy to Tang at the time of King Seongdeok and the change of names of place and official departments at the time of King Gyeongdeok, especially, the authoritarian royal power and the Chinese assimilation policy in the Middle period(중대) have a close relation each other. It is noted that though Silla accepted Tang"s system, it did not establish the key of Tang"s central official departments, the Three Ministries-Six Departments System(3성6부제), but it was operated centering on Jipsabu which had existed before. However, in the Late period(하대) when Jipsabu was changed to Jipsasung(집사성) and many official departments were newly established, it changed and had a certain effect on the modification of Taebong(태봉) and Goryeo dynasty(고려)"s government systems.
