        경직형 양마비 아동의 근육뼈대계 수술 후 체중지지 트레드밀 훈련이 대동작 기능과 균형에 미치는 영향

        To determine the effect of body weighted treadmill training on gross motor function and balance in children with spastic diplegia following musculoskeletal surgery for the lower limbs. The GMFCS level of these subject were II and III. Children with cerebral palsy between 5 and 10 years of age participated and were divided into 2 groups: those having undergone soft tissue surgery and those having undergone both soft tissue and bone surgery. Body weighted support treadmill was trained in both groups after 40-minute of neurodevelopmental treatment, each session was 10-minutes 3 time a week for 12weeks. Both groups showed significant statistical difference in Gross Motor Function Measure and Pediatric Balance Scale in post-training of 6weeks and 12weeks than pre-training(p<.05). There was significant difference in Gross Motor Function Measure between 6 weeks of pre- and post-training, 6weeks and 12weeks of post-training in each groups(p<.05). There was no statistical significant difference between 6 weeks of pre- and post-training, 6weeks and 12weeks of post-training. in each groups(p<.05). Therefore, body weighted treadmill training may be considered a beneficial intervention method to improve balance abilities of children with cerebral palsy. 본 연구는 다리의 근육뼈대계 수술 후 체중지지 트레드밀 훈련이 경직형 양마비 아동의 대동작 기능과 균형에 어떤 영향을 미치는지 알아보기 위해 실시하였다. 5~10세의 대동작 기능분류체계(GMFCS) II~III단계 뇌성마비 아동을 연구 대상자로 선정하여 두 집단(다리의 연부조직만 수술한 집단과 연부조직과 뼈를 모두 수술한 집단)으로 배분하였다. 두 집단 모두 40분간 신경발달치료를 마친 후 체중지지 트레드밀 훈련을 1회 10분, 주 3회, 12주 동안 적용하였다. 두 집단 모두 대동작 기능평가와 아동용 균형척도가 훈련 전 보다 훈련 6주후와 12주후에 통계적으로 유의한 차이가 있었다(p<.05). 대동작 기능평가는 훈련 전과 훈련 6주후, 훈련 6주후와 12주후 모두 두 집단 간의 유의한 차이가 있었지만 아동용 균형척도는 훈련 전과 훈련 6주후, 훈련 6주후와 12주후 모두 두 집단 간의 유의한 차이가 없었다(p<.05). 그러므로 다리의 근육뼈대계 수술 후 체중지지 트레드밀 훈련은 뇌성마비 아동의 대동작 기능과 균형을 향상시키는데 효율적인 중재 방법으로 고려될 수 있을 것이다.

        경직형 양마비 아동의 과제지향훈련 시 무게조끼 적용이 대동작 수행력과 균형 능력에 미치는 영향 : 무작위배정 위약비교 연구

        Objective : The purpose of this research is to find clinical effects of application of weighted vest during task-oriented training focused on gross motor performance and balance abilities of children with spastic diplegia. Methods : 34 subjects were divided by simple random sampling into two groups; experimental group (male : 9, female : 8, average age : 8.12) and placebo group (male : 9, female : 9, average age : 7.53). Both two groups underwent to 40 minute intervention, twice a week for 12 weeks. The intervention was task-oriented training focused on facilitating closed kinematic chain and multi-joint functional movement pattern. During the training, the experimental group received loaded-resistance weighted vest and placebo group also received weighted vest but without loaded-resistance. Participants in both groups underwent 8 to 10 reps of the task-oriented training and there were 3 minutes break time between tasks. There were pre-test of gross motor performance and balance abilities, and two times of post-tests were performed upon 6 weeks and 12 weeks after the intervention completed. And in final, an additional follow-up test was performed 12 weeks after the evaluation was finished in order to find any difference between the two groups over time. Results : There was significant difference in Gross Motor Performance Measure (GMPM) between two groups. It is found that average score of the experimental group increased more than the placebo group after 6 weeks and 12 weeks intervention (p<.05). There was significant difference in Pediatric Berg’s Balance Scale (PBS) between two groups. It is found that average score of the experimental group increased more than the placebo group after 6 weeks and 12 weeks intervention (p<.05). Conclusion : Based on the results in this study, it is proposed that application of weighted vest into task-oriented training to facilitating closed kinematic chain and multi-joint movement can improve gross motor performance and balance abilities of children with cerebral palsy 목적 : 본 연구는 경직형 양마비 아동에게 과제지향훈련을 시행할 때 무게조끼 적용이 대동작 수행력과 균형 능력에 미치는영향을 알아보며 뇌성마비 아동을 치료하기 위한 중재방법으로 임상적으로 유용한지 제안하고자 시도되었다. 연구방법 : 연구 대상자를 단순 무작위 표본추출법으로 실험군(남자 : 9명, 여자 : 8명, 평균연령 : 8.12세)과 위약군(남자: 9명, 여자 : 8명, 평균연령 : 7.53세) 각각 18명씩 배분한 후 두 중재군 모두에게 1회 40분, 주 2회 과제지향훈련을12주 동안 실시하였다. 실험군은 무게저항을 제공하는 무게조끼를 착용하고, 위약군은 무게저항 없이 무게조끼만 착용하였다. 두 중재군 모두에게 닫힌 운동사슬과 다관절의 기능적인 움직임 패턴을 촉진하는 과제지향훈련을 각각 8~10회실시하며 과제 사이에 약 3분간 휴식 하였다. 두 중재군의 대동작 수행력, 균형 능력에 대한 사전검사는 중재 전에측정하였고, 사후검사는 중재 6주후와 12주후 총 2회 시행하였으며 마지막으로 추적검사는 실험종료 12주후에 실시한후 두 중재군 사이의 시간경과에 따른 변화량을 분석하였다. 결과 : 대동작 수행평가는 중재 6주후와 12주후에는 실험군이 위약군보다 평균 점수가 더 증가하였으며 유의한 차이가나타났다(p<.05). 아동용 균형척도는 중재 6주후와 12주후에는 실험군이 위약군보다 평균 점수가 더 증가하였으며 유의한 차이가 있었다(p<.05). 결론 : 그러므로 닫힌 운동사슬과 다관절의 기능적인 움직임을 촉진시키는 과제지향훈련 시 무게조끼 적용은 경직형 양마비 아동의 대동작 수행력과 균형 능력을 향상시키는 유용한 중재 방법으로 고려될 수 있다.

        Development of a Hibiscus syriacus ‘Danna’ with Small and Vivid Red Eye Spot on Bright Pink Petals

        A new Hibiscus syriacus ‘Danna’ was developed at the National Institute of Forest Science in 2013. It was initially derived from an artificial cross in 2004, between H. syriacus ‘Hanŏl’, a seed parent, a Korean local cultivar with vigorous growth and H. syriacus ‘Hanŏl-tanshim’, as a pollen parent, with long bloom period. ‘Danna’ was preliminarily selected as ‘KFRI-HH03’ for the aesthetic and stable quality of its unique color of its flower and rapid growth in elongation in 2008. Trial evaluation was conducted for 6 years (2008 - 2013), meanwhile, all investigated traits were retained in rooted cuttings originated from it. ‘Danna’ had a bright pink flower with very small red eye zone. Its flower diameter is 111.3 mm; length of red eye spot on petals is 13.8 mm. Leaves were 71.3 mm long and 43.1 mm wide, and petiole length was 14.2 mm. Flowering period of the cultivar investigated in 2013 was 81 days. It was registered as a new cultivar at the Korea Seed & Variety Service in 2015 (Grant Number 5756). Its tree habit shows vigorous and fast growth and is upright, so it can be used as an ornamental tree or a street tree.

        Effects of Family-Centered Training on Functional Balance and Activities of Daily Living in Children with Cerebral Palsy

        This study was conducted to examine the effects of the application of familycentered training over the 12-week period by the pediatric physiotherapist on functional balance and activities of daily living in children with cerebral palsy. Among the 26 children with cerebral palsy as the subjects were allocated to the experimental and control group. Both groups were subjected to neurodevelopmental treatment by the pediatric physiotherapist. The experimental group, participated in family-centered training program 3 times a week over a period of 12 weeks for the total of 36 sessions, functional balance and activities of daily living were verified through intergroup comparison. There was no significant difference between the outcomes prior to training and after 6 weeks of training, (p>.05) Pediatric Berg's Balance Scale(PBS) and Functional Independence Measure for children(Wee-FIM) increased significantly from those measures after 6 weeks to those after 12 weeks of training (p<.05). Therefore, these results suggest that on family-centered training on children with cerebral palsy has beneficial effects on functional movements and physical activities.

        단심이 크고 독특한 홍단심계 무궁화 ‘근형(槿亨)’육성

        A new Hibiscus cultivar ‘Geunhyeong’ wasdeveloped at the Korea Forest Research Institute in 2011. It was initially bred from a artificial cross in 2003, between H. syriacus × paramutabilis cv. Shichisai, a seed parent, aJapanese hybrid cultivar with unique flower color and longbloom period, and H. syriacus cv. Kaeryangtansim, as apollen parent, a tetraploid cultivar with vigorous growth. ‘Geunhyeong’ was preliminarily selected as ‘KFRI-SK16’ forthe aesthetic and stable quality of its unique color of its flowerand rapid growth in elongation in 2006. Trial evaluationwas conducted for 5 years (2007 ~ 2011), meanwhile, allinvestigated traits were retained in rooted cuttings originatedfrom it. ‘Geunhyeong’ had a bright pink flower with large,strong red eye zone. Mean of the flower diameter was108.6 mm, length of red eye spot on petals was 30.3 mm,and strong red radiation lines were extended from the edgeof the red eye zone to more than three quarters of the wayto the edge of the flower. Leaves were 71.1 mm long and33.5 mm wide, and petiole length was 12.9 mm. Floweringperiod of the cultivar investigated in 2011 was 82 days. Itwas registered as a new cultivar at the Korea Seed &Variety Service in 2013 (Grant Number 4358). Its tree habitshowed vigorous and fast upright growth, so it can be usedas an ornamental tree or a street tree. 무궁화 ‘근형’은 2003년 무궁화속 종간교잡 품종으로 꽃색이 독특하고 개화기간이 긴 ‘七彩(무지개)’(H. syriacus× paramutabilis cv. Shichisai)를 모본으로, 4배체 무궁화로수세가 강건한 ‘개량단심’(H. syriacus cv. Kaeryangtansim)을 화분친으로 인공교배 후 양묘된 실생묘로부터 선발되었다. 2006년 1차 선발 후 형질 안정성 검정을 수행한 뒤, 2011년 ‘KFRI-SK16’를 최종 선발하여 ‘근형’으로명명하였다. 해당 품종은 직립형으로 가지가 곧추 서며연간 신초 길이생장이 평균 64.0cm이고 5년생 삽목묘평균 수고가 189.2cm에 달하므로 교목형으로 유도하여 가로수 또는 조경수로 이용하기 적당하다. 전체적인 꽃의 형태는 깔대기형으로 평균 직경이 108.6mm에 이르는 홍단심계 홑꽃이며, 꽃잎 길이/너비 비율은 약 1.84로 꽃잎이 다소 겹치는 I-b형이다. 꽃잎색은 RP65B으로 보라색이 섞인 분홍색이며, 진한 붉은색의 단심과 단심선이 꽃잎 길이의 약 3/4까지 매우 넓고 길게 퍼져 있다. 2011년조사된 개화 시작일은 7월 10일, 개화 종료일은 9월 30일로 총 개화기간은 82일에 달하였다. 잎의 전체적인 모양은 다소 긴 형으로 잎몸의 길이/너비 비율은 2.12이며, 엽병 길이는 12.9mm로 긴 편이다. 잎몸의 열편은 없거나 미미하고 잎몸 가장자리 물결 정도는 중간으로 결각이 적은편이다.

      • 하지 혈류제한 운동이 근육크기와 근력에 미치는 영향

        Purpose : The purpose of this study was to investigate thigh muscle-bone CSA and leg strength during low-intensity exercise program with leg blood flow restriction by external compression to reduce muscle outflow. Methods : Eighteen health students gave informed written consent to participate in this investigation. An occlusion cuff was attached to the proximal end of the leg so that blood flow was reduced during the training. The training was conducted one times a day, three times a week, for 8 weeks using one sets of 30 minutes. The training program performed to squat with standing, lunge with standing and heel raise with one leg standing. Measurements of thigh muscle-bone CSA(cross-sectional area) and leg strength were evaluated pre and post-training. Statistical evaluation of these data was accomplished utilizing a paired t-test by SPSS 12.0 program for windows. Significance level was set at p <.05. Results : All data are reported as means and standard deviations(SD) for all variables. The result of the study is followed; After the training, muscle-bone CSA, gluteus maximus m, quadriceps m, hamstring m of both legs were significantly improved but not calf muscle(p<.05). There was no significant difference of change quantity between muscle-bone CSA and leg strength in Lt. and Rt. side. But the variation in leg muscle strength of Rt. leg(dominant) was much more increased than Lt. leg(non-dominant) after 8 weeks training. Conclusion : Low-intensity training with leg blood flow restriction offers a potentially useful method for improving leg muscle strength. .

        장애아동에게 제공된 의료재활서비스의 의료보건기관별 가족중심 접근법 차이에 대한 조사연구

        The purpose of this study was to investigate the extent of medical rehabilitation services provided to children with disabilities and their caregivers by family-centered approach. In this study, a short version of the MPOC-20 was used to evaluate the care rehabilitation service, or therapeutic intervention, of a child with a disability provided by a health care provider over the past year. As a result of this study, there were significant differences among health care institutions in the past one year, including those providing direct medical rehabilitation services to the disabled children and their families and those with management/support team. Also, the satisfaction of the family-centered medical rehabilitation service of disabled children’s guardians was highest in disabled welfare centers, and the lowest in child care rooms in hospitals. Therefore, centers including the medical rehabilitation service therapist and management/support team staff in the hospital in-patient clinic should increase the participation of families of children with disabilities in therapeutic intervention and strengthen their capacity to care for children with disabilities through a family- centered approach. 본 연구의 목적은 장애아동과 그 보호자에게 제공된 의료재활서비스가 어느 정도 가족중심적 접근법으로 적용되는지 의료보건기관별 차이를 알아보고자 하는 것이다. 장애아동 보호자에게 지난 1년 동안 의료보건기관으로부터 제공받은 장애아동 의료재활서비스 즉, 치료적 중재에 대해 어떻게 생각하고 있는지를 단축형 MPOC-20 한글판을 사용해서 평가하였다. 본 연구결과 지난 1년간 장애아동과 그 가족에게 직접적인 의료재활서비스를 제공하는 사람들과 관리/지원팀 직원이 포함된 센터 모두 의료보건기관별로 유의한 차이가 있었다. 또한, 장애아동 보호자의 가족중심 의료재활서비스의 만족도는 장애인복지관이 가장 높았고, 병원 내 아동치료실이 가장 낮은 것으로 나타났다. 그러므로 병원 내 아동치료실의 의료재활서비스 치료사와 관리/지원팀 직원을 포함한 센터에서는 가족중심적 접근법을 통해 장애아동의 가족들의 치료적 중재 참여도를 높이고, 장애아동을 돌보는데 필요한 역량을 강화시켜야 한다.
