        당뇨병성 신경병증 환자에서 관찰되는 탈수초화의 전기생리학적 소견

        Background: Diabetic neuropathy is one of the most common neuropathies. Although pathologic studies show both segmental demyelination and axonal loss in diabetic neuropathy, the relative importance of segmental demyelination is debated. Conduction block (CB) is a physiologic hallmark of segmental demyelination. If segmental demyelination were a main pathology of diabetic neuropathy, CB should be common. We undertook this study to determine the prevalence of CB in diabetic patients. Methods: Fifty-two consecutive diabetic patients (M=30, F=22) were referred to EMG laboratory and underwent routine nerve conduction studies (NCS). CB was defined by two methods. One was > 20% drop in peak-to-peak amplitude and<15% change in negative-peak duration between proximal and distal stimulation sites. The other was>50% drop in the amplitude and area. Clinical findings, electrophysiological data, and effectiveness of immunomodulating therapy for some patients with CB were reviewed. Results: A total 326 nerves were studied. The criteria for 20% and 50% CB were met in 35 nerves in 19 patients and 7 nerves in 6 patients, respectively (prevalence=10.7%, 2.1%, respectively). Some patients with CB were treated with IVIG or steroid and had a good response. Conclusions: CB in diabetic neuropathy is not a common finding. The rarity of CB in diabetic neuropathy suggests that segmental demyelination is not a prominent part of the underlying pathology. The presence of CB and good responsiveness to immunomodulating therapy in diabetic neuropathy also suggest alternative or additional causes for neuropathy, such as chronic inflammatory demyelinating polyneuropathy.

      • 허혈성 뇌경색의 위험인자로서의 고인슐린혈증

        Background and Purpose : Insulin resistance and hyperinsulinemia may contribute to the development of atherosclerosis by a direct effect on the artery. Chronic hyperinsulinism has been recognized as a risk factor for coronary atherosclerosis. However, little is known about the role of hyperinsulinemia in ischemic stroke. We undertook this study to clarify whether high insulin level is a causative factor in the pathogenesis of ischemic stroke. Methods : Forty-eight consecutive ischemic stroke patients without diabetes mellitus and hypertension were divided into three groups, according to clinical presentations, brain imaging, transcranial doppler, and cerebral angiographies: large-artery atherosclerosis group (LAA, n=20), small-vessel occlusion group (SVO,n=16), and cardioembolism group (CE, n=12). Fasting blood sugar (FBS), fasting and 2-hour postprandial insulin, and lipid levels were analyzed. Data of each group were compared with those of age-matched control subjects (n=16). Results : No significant differences in age, sex, blood pressure, FBS, and smoking habit were noted among the three groups and the control. There was a significant difference in 2-hour postprandial insulin levels among LAA, SVO, CE and the control group(mean ±SEM : 66±10.6, 27±5.5, 30±4.4, 28±6.1μU/mL ,respectively; P〈0.001, ANOVA). 2-hour postprandial insulin levels were significantly higher in LAA compared with other three groups (ANOVA, Post Hoc Multiple comparison : Student-Newman-Keuls test). Conclusions:These findings suggest that increased postprandial insulin levels may be a potential pathogenetic factor for the development of large artery atherosclerosis.         Because of not yet-house built for sale and high-rate supply of house, building companies work for conversion to qualitative supply. Moreover a recent the revised regulations follow the trend of the times. The first objective of this study is understanding that the revised regulations recommend the character of design and then grasping the diversification of elevations by analyzing of high-rise apartment building since 1980' in Seoul. By 3-way(vertical, horizontal, urban corresponding aspects) analysis on experimental apartment, a primary material (an analysis table) was made. And elevation diagram was made by sampling of times-3 types. The diagram was made clear by analyzing of contemporary. And inquiring prove 1) decoration and 3-dimension, 2) articulation, 3) subdivision and maximization were founded. 1) was regarded high quality as primary countermeasure of the revised regulations. 2),3) are applied to the residential-commercial apartment. By the reason that three trends are mutual, they are likely to be one trend.

        A 65-year-old man presented with rightward gaze palsy of both eyes and adduction palsy of the right eye. Healso showed difficulty in downward gaze of the right eye and bilateral upward palsy. These findings were consis-tent with horizontal and vertical one-and-a-half syndrome. Brain MRI showed multiple infarctions in the rightparamedian thalamus, upper mesencephalon, and pons. The horizontal one-and-a-half syndrome originate fromthe unilateral pontine lesion that may involve the paramedian pontine reticular formation (PPRF) and the adjacentmedial longitudinal fasciculus (MLF) on the side of the complete gaze palsy. The vertical one-and-a-half syn-drome may be due to the thalamo-mesencephalic lesion affecting the efferent tracts of the rostral interstitial nucle-us of MLF (riMLF) for upgaze and the descending fibers to the ipsilateral inferior rectus subnucleus and contralat-eral superior oblique subnucleus. We report the concurrence of the two distinct syndromes, horizontal and verticalone-and-a-half, in a patient with multiple brainstem infarctions.대한뇌졸중학회지제6권제1호2004년5월

        하와이, 빅 아일랜드의 푸우오오 용암에 대한 암석학적 연구

        푸우오오 용암분출은 1983년에 분출이 시작된 이래 현재까지 활동하고 있으며, 다양한 성분 변화와 (5.6~10.1 wt.% MgO), 지표분출 면적을 가지는(-2 ㎞³) 하와이의 역사적인 분출 중에 하나이다(Garcia et al., 1999). 다년간에 걸쳐 하와이 분출에 대한 연구가 수행되어 왔으나, 최근 수년 이내에 분출된 푸우오오 용암의 지질학적인 특성에 대한 연구 결과의 부재와 푸우오오 분출 역사를 통한 하와이 마그마 분출과정의 특성을 파악하고자 시공간적으로 채취한 시료를 바탕으로 광물과 암석의 조직과 화학조성연구를 수행하였다. 박편 상에서 단사휘석이나 사장석 반정은 드물고 감람석이 대부분의 반정을 차지한다. 이것은 푸우오오 지역이 마그마 분화 초기단계에 머물러 있다는 것을 지시한다. MgO를 상대로 주원소에 대해 도시해 본 결과 MgO가 증가함에 따라 TiO₂, Al₂O₃, Na₂O, SiO₂는 감소하는 경향을 보인다. 미량원소의 경우 K₂O값에 대한 Zr, Y, Sr 그리고 V 도표에서 P18과 P19시료가 타 용암에 비해 진화가 더 진행된 영역에 도시되고 있다. 2003년에서 2006년 분출년도 간에 높은 Ni 함량이 나타나는데, 이것은 동일 기원마그마의 유업에 의한 결과이다. The Puu Oo eruption in Hawaii since 1983 is one of the largest eruptions on Hawaii's volcanic history with prominent compositional variation (5.6-10.1 wt.% in MgO content). Although intense researches of Hawaiian eruption have been conducted for recent years, there is no up-to-date study on Puu Oo lavas that is erupting hot lavas today. in oder to obtain basic information on the geological characteristics of the eruption including any noticeable change in its petrological trend and magma dynamics, we applied several geological approaches such as field survey, systematic sampling, petrography, mineralogy, and geochemistry. Clinopyroxene and Plagioclase phenocrysts are rarely observed on the thin section, however Olivine crysts are much more obvious in the study area. It indicates that Puu Oo is early stage of magma differentiation. Variation diagram of whole rock composition shows that the elements such as TiO₂, Al₂O₃, SiO₂ and Na₂O decrease with increasing MgO. In the trace element Sr, Y, Zr and V versus K₂O, P18, P19 samples are plotted in primitive area Variations of the Ni contents during 2003-2006 may suggest a sudden change in magma composition probably caused by new magma injection.
