A 43 years old female patient who had been diagnosed as having valvular heart disease but had not received any treatment invited and admitted due to progressive dyspnea. She was diagnosed as having aortic and mitral valve stenosis and regurgitation. Neurologic symptoms developed suddenly therefore, surgery was performed. In the operation field, there were many fungating tissue around the mitral valve annulus and left atrial wall. After operation, no neurologic symptoms were observed and pathologist revealed that fungating tissue was papillary fibroleastoma. The patient recovered and was followed in outpatients department.

In these days, SNS(Social Network Services) have been one of the biggest issues and leaded lots of changes in our society. The changing way of communication and participation is bring about the movement of power in many areas. The consumers have had a bigger power than before so the firms need to listen to consumers’ requirements. In politics, politicians have more tried to meet the voters’ needs than before because voters could more easily act collectively through the new IT devices. The purpose of this study is to suggest utilization methods of SNS due to the changes in our societ, case study with the case frame we developed, and several strategies for efficient management of social network services. In order to reach the purpose, we review papers and reports related to SNS and search its success cases in companies and public sectors.

In this paper, we propose a novel visualization technique to explain the predictions of deep neural networks. We use knowledge distillation (KD) to identify the interior of a black-box model for which we know only inputs and outputs. The information of the black box model will be transferred to a white box model that we aim to create through the KD. The white box model will learn the representation of the black-box model. Second, the white-box model generates attention maps for each of its layers using Grad-CAM. Then we combine the attention maps of different layers using the pixel-wise summation to generate a final saliency map that contains information from all layers of the model. The experiments show that the proposed technique found important layers and explained which part of the input is important. Saliency maps generated by the proposed technique performed better than those of Grad-CAM in deletion game.

Finding the critical path (or the longest path) on acyclic directed graphs, which is well-known as PERT/CPM, the ambiguity of each acr's length can be modeled as a range or an interval, in which the actual length of arc may realize. In this case, the min-max regret criterion, which is widely used in the decision making under uncertainty, can be applied to find the critical path minimizing the maximum regret in the worst case. Since the min-max regret critical path problem with the interval arc's lengths is known as NP-hard, this paper proposes a heuristic algorithm to diminish the maximum regret and the computational experiments shows the proposed algorithm contributes to the improvement of solution compared with the existing heuristic algorithms.

Background: Traumatic cardiac injury is very rare but mortality is very high when the diagnosis and management are delayed. We reviewed our case retrospectively. Material and Method: From March 1995 to July 2003, 17 patients were diagnosed as having traumatic cardiac rupture. Five patients were stabbed, seven patients were motor vehicle accidents, four patients had fallen down, and the cause was unknown in one patient. Emergency operations were done and six patients were operated under CPB. Result: Four patients died during or after operation. The mean ICU stay period was 3.86±3.35 days and the mean hospital stay was 18.27±14.99 days. No mortality was observed in those whose vital signs were stable in the operating room. Conclusion: Preoperative vital status was very important and thoracic traumatic patient should be suspected as having cardiac injury. 배경: 외상에 의한 심장 손상은 드물게 발생하지만 진단 및 치료가 지연되었을 경우 치사율이 매우 높다. 이에 본원에서는 외상성 심장 손상 환자를 대상으로 후향적 연구를 진행하였다. 대상 및 방법: 1995년 3월부터 2002년 7월까지 본원 응급실을 통하여 내원한 외상성 심장 손상 환자 대상으로 하였다. 총 17명의 환자 중 흉기 등에 의한 자상이 5예, 둔상 중에서 교통사고로 인한 손상이 7예, 작업장 등에서 추락한 경우가 4예, 원인을 알 수 없는 경우가 있었다. 모든 예에 있어서 응급 수술을 진행하였고, 6예의 경우 심폐기를 이용한 수술을 진행하였다. 결과: 수술 후 4예의 사망이 있었다. 수술 후 모든 환자는 중환자실로 이송하였고 평균 중환자실 재원기간은 3.86 3.35일, 평균 총재원기간은 18.27 14.99일이었다. 응급실에 도착하여 수술의 진행까지 생명 징후가 비교적 안정적이었던 경우 사망한 예는 없었다. 결론: 수술 전의 환자의 상태는 환자의 예후에 직접적인 영향을 주는 중요한 인자이기 때문에 흉부외상 환자에게 있어서 심장손상 여부의 판단은 매우 중요하다.
