An unusual polypoid laryngeal tumor has been studied with light and electron microscopy. In light microscopic section, there were two components: a squamous cell carcinoma and a sarcomatoid spindle cell population. Transitional zone between these two components was found. The electron micrographs demonstrated the presence of slender bundles of tonofilaments in many tumor cells. Desmosomes were present, but few in number and poorly developed. Findings suggest this laryngeal tumor as a spindle cell variant of squamous cell carcinoma.

저자들은 최근 39세된 여자로서 3차례에 걸친 개복 수술과 방사선 요법을 시행 받은 후 4년이 지나 복벽에서 발생한 비교적 특징적인 육안 및 광학현미경적 조직소견을 보인 악성 섬유 조직구종 1例를 경험하였다. 육안적으로 종괴는 외표면이 피부로 덮혀있고 그 중앙부위가 결손되면서 결절을 형성하고 절단면에서 구형을 정하는 종양조직으로 백색 및 연한 황색을 띤 균질성의 어육(fish-flesh)의 소견을 보였다. 광학 현미경적으로 난형 혹은 다각형의 조직구양 세포들과 방추형 세포들이 긴밀히 섞이면서 다발을 이루거나 storiform상 배열을 하며 때로는 거대세포와 이형성 핵분열을 관찰할 수 있었고 종양조직내에 산재한 염증세포들과 부분적으로 출혈 및 괴사의 소견도 볼 수 있었다. 저자들은 본 증례를 보고함에 있어 특히 악성 섬유조직구종의 병원론(pathogenesis)과 그 발생기전에 대해서 또한 예후를 결정하는 주요 인자들에 대해 중점을 두었으며 문헌 고찰과 함께 고안하였다. 환자는 술후 6개월이 지난 현재 복벽의 수술창은 깨끗하고 재발의 소견이 없으나 앞으로 계속 재조사(follow-up)를 받을 예정이다. Malignant Fibrous Histiocytoma (MFH) has become a well known malignant tumor of soft tissue since its description by O'Brien and Stout, and Stout and Lattes proposed as having a common origin from the tissue histiocyte, but there are different oopinion that MFH is a neooplasm of undifferentiated mesenchymal cell with binary differentiation into histiocytoid and fibroblastic cells. Irrespective of its precise histogenesis, the tumor contains both fibroblast-like and histiocyte-like cells in varying proportions, accountiing in part for its broad morphologic spectrum. The major microscopic characteristics include fibrogenic areas with spindle cells(fibroblast-like) arranged in a focal storiform apttern, clusters or sheets of histiocyte-like cells, benign and malignant giant cells, foam cells, inflammatory cells, scattered mitotic figures are frequently observed. This report presents a case of malignant fibrous histiovytoma arising in the previous laparotomy scar of low abdominal wall, where had been previously irradiated, of 39-year-old female patient who had had history of total abdominal hysterectomy with left salpingo-oophorectomy for cervical carcinoma on February 24, 1977, radiation therapy carcinoma of right ovary on April 18, 1978, and after that Co therapy for 27 days, total 5400 rad, till June 15, 1978, and right hemicolectomy with ileo-colostomy for postoperative intestinal obstruction on November 11, 1978. It is our aim in this report to discuss histogenesis and to emphasize on the development of prognostic criteria of MFH.

This study was carried out to investigate the mechanisms of acinar cell deletion, leading to the pancreatic atrophy of rat pancreas after experimental duct ligation. Fifty-seven male Sprague-Dawley rats, maintained on a stock diet, weighing 200gm, in average, were divided into 2 experimental groups. Group 1. Control group. Six rats. Abdominal cavity was opened and closed without further treatment. Group 2. Fifty-one rats. Animals were treated with partial ligation of the pancreatic ducts according to the procedure developed by Hultquist followed by sequential sacrifices at: 1 hour (3 rats), 3 hour (3 rats) 6 hours (6 rats), 12 hours (3 rats) and 24 hours (8 rats); 2 days (8 rats), 3 days(3 rats), 4 days (3 rats), and 5 days(5 rats); 1 week (3 rats), 2 weeks (3 rats), 2 weeks (3 rats) and 8 weeks (3 rats); after ligation. Distal part of the pancreas of the animals after partial ligation was extirpated and examined by both light and electron microscopy. The results obtained were as follows: Light microsopically, noted were and interstial edema and focal necrosis of the pancreatic tissue along with fine vacuolization and depletion of the zymogen granules in the acinar cell cytoplasms and condensation of the acinar cell nucleus. These changes were observed by 2 days after ligation. At about the same time, one can observe the dense body, identified to be apoptotic body, in the acinar cells which were found to be decreased in quantity. By 5 days affer ligation, no recognizable acinar cells left in the collagenous stroma exdept intercalated ducts. Conspicuous stroma except intercalated ducts. Conspicuous stromal hyalinization, thereafter. Electron microscopically (TEM and SEM), nuclear condensation and margination toward the nuclear membrane was noted by 6 hours after duct ligation. By 24 hours sporadic membrane-bounded apoptotic bodies appeared in the acinar cells, the number of which reaching to the peak by 3 days after ligation. These apoptotic bodies were found to be phagocytosed by either intraepithlial mononclear phagocytes or adjoinging acinar cells. It can be concluded, therefore: That orderly remodeling of pancreatic exocrine tissue during atrophy is effected by rapid deletion of acinar cells by apoptosis.
