      • Spontaneous vaginal hemorrhage caused by choriocarcinoma : Necessity for the early diagnosis and treatment

        Hemorrhagic choriocarcinoma lesions may lead to life-threatening conditions, but an early diagnosis contributes to a potential cure. We report a case of an woman with choriocarcinoma, that occurred within one month after full term delivery. The 25-year-old patient with gravida 1, para 1 was admitted in our hospital for massive vaginal bleeding and dry cough. Radiological investigation found a 8.4cm mass in enlarged uterus and also multiple nodules in both lungs. Her serum β-hCG level was 747,682 mIU/mL. An emergency surgery was performed to control heavy vaginal bleeding. The patient was early diagnosed as choriocarcinoma FIGO stage III and the score of the Prognostic Scoring Index modified by the WHO was 12. Multiagent chemotherapy, the EMACO regimen(etoposide, actinomycin D, methotrexate, vincristine and cyclophosphamide) was administered. After the seventh chemotherapy session, her serum β-hCG level was <5 mIU/mL. The diagnosis of choriocarcinoma may be difficult, especially in the setting with the limit of medical resources. The early application of various diagnostic techniques such as x-ray, computed tomography is helpful for evaluating the patient´s condition.

        Population Genetic Structure and Evidence of Demographic Expansion of the Ayu (Plecoglossus altivelis) in East Asia

        Plecoglossus altivelis (ayu) is an amphidromous fish widely distributed in Northeastern Asia from the East China Sea to the northern Japanese coastal waters, encompassing the Korean Peninsula within its range. The shore lines of northeastern region in Asia have severely fluctuated following glaciations in the Quaternary. In the present study, we investigate the population genetic structure and historical demographic change of P. altivelis at a population level in East Asia. Analysis of molecular variance (AMOVA) based on 244 mitochondrial control region DNA sequences clearly showed that as the sampling scope extended to a larger geographic area, genetic differentiation began to become significant, particularly among Northeastern populations. A series of hierarchical AMOVA could detect the genetic relationship of three closely located islands between Korea and Japan that might have been tightly connected by the regional Tsushima current. Neutrality and mismatch distribution analyses revealed a strong signature of a recent population expansion of P. altivelis in East Asia, estimated at 126 to 391 thousand years ago during the late Pleistocene. Therefore it suggests that the present population of P. altivelis traces back to its approximate demographic change long before the last glacial maximum. This contrasts our a priori expectation that the most recent glacial event might have the most crucial effect on the present day demography of marine organisms through bottleneck and subsequent increase of effective population size in this region.

      • Novel pendrin inhibitor attenuates lipopolysaccharide-induced acute lung injury in mice

        Background: Pendrin is encoded by SLC26A4 and its mutation leads to congenital hearing loss. Additionally, pendrin is up-regulated in inflammatory airway diseases such as chronic obstructive pulmonary disease, allergic rhinitis, and asthma. In this study, the effects of a novel pendrin inhibitor, YS-01, were investigated in an LPS-induced acute lung injury (ALI) mice model, and the mechanism underlying the effect of YS-01 was examined. Methods: LPS (10 mg/kg) was intranasally instilled in wild type (WT) and pendrin-null mice. Lung injury parameters were assessed in the lung tissue and bronchoalveolar lavage fluid (BALF). Pendrin levels in the BALF of 41 pneumonia/ARDS patients and 25 control (solitary pulmonary nodule) patients were also measured. Results: LPS instillation induced lung injury in WT mice but not in pendrin-null mice. YS-01 treatment dramatically attenuated lung injury and reduced BALF cell counts and protein concentration after LPS instillation in WT mice. Proinflammatory cytokines and NF-κB activation were suppressed by YS-01 treatment in LPS-induced ALI mice. However, the protective effects of pendrin inhibitor lost after SCN- instillation. Furthermore, pendrin expression was upregulated in pneumonia/ARDS patient compared to that in control patient BALF (mean, 24.86 vs. 6.83 ng/mL, p < 0.001). Conclusions: A novel Pendrin inhibitor, YS-01, suppressed lung injury in LPS-induced ALI mice and our data provide a new strategy for the treatment of inflammatory airway diseases including sepsis-induced ALI.

        Effect of Denosumab on the Change of Osteoclast Precursors Compared to Zoledronate Treatment in Postmenopausal Women with Osteoporosis

        Background: A rapid increase in bone turnover and bone loss has been observed in response to the discontinuation of denosumab. It led to an acute increase in the fracture risk, similar to that observed in the untreated patients. We aimed to investigate the effect of denosumab on osteoclast (OC) precursor cells compared to that of zoledronate. Methods: The study compared the effects of denosumab (60 mg/24-week) and zoledronate (5 mg/48-week) over 48 weeks in postmenopausal women with osteoporosis. From patients’ peripheral mononuclear cells, CD14+/CD11b+/vitronectin receptor (VNR)- and CD14+/CD11b+/VNR+ cells were isolated using fluorescent-activated cell sorting, representing early and late OC precursors, respectively. The primary endpoint was the changes in OC precursors after 48 weeks of treatment. Results: Among the 23 patients, 11 were assigned to the denosumab group and 12 to the zoledronate group (mean age, 69 years). After 48 weeks, the changes in OC precursors were similar between and within the groups. Serum C-terminal telopeptide of type I collagen levels were inversely correlated with OC precursor levels after denosumab treatment (r=-0.72, P<0.001). Lumbar spine, femur neck, and total hip bone mineral density (BMD) increased in both groups. Lumbar spine BMD increased more significantly in the denosumab group than in the zoledronate group. Conclusions: Denosumab and zoledronate treatments induced similar changes in OC precursors. During denosumab treatment, old age and suppressed bone turnover were associated with increased OC precursor cell populations. Further validation studies with prospective designs are required.

        Identification of a novel mutation in a patient with pseudohypoparathyroidism type Ia

        Pseudohypoparathyroidism type Ia (PHP Ia) is a disorder characterized by multiform hormonal resistance including parathyroid hormone (PTH) resistance and Albright hereditary osteodystrophy (AHO). It is caused by heterozygous inactivating mutations within the Gs alpha-encoding GNAS exons. A 9-year-old boy presented with clinical and laboratory abnormalities including hypocalcemia, hyperphosphatemia, PTH resistance, multihormone resistance and AHO (round face, short stature, obesity, brachydactyly and osteoma cutis) which were typical of PHP Ia. He had a history of repeated convulsive episodes that started from the age of 2 months. A cranial computed tomography scan showed bilateral calcifications in the basal ganglia and his intelligence quotient testing indicated mild mental retardation. Family history revealed that the patient's maternal relatives, including his grandmother and 2 of his mother's siblings, had features suggestive of AHO. Sequencing of the GNAS gene of the patient identified a heterozygous nonsense mutation within exon 11 (c.637 C>T). The C>T transversion results in an amino acid substitution from Gln to stop codon at codon 213 ($p.Gln213^*$). To our knowledge, this is a novel mutation in GNAS.

      • Anti-senescence and anti-inflammation effect of Pandanus conoideus Lamk through nitric oxide and reactive oxygen species

        Objective: This study is to investigate if red fruit oil (RFO, Pandanus conoideus Lamk), which regulates NO and reactive oxygen species (ROS) metabolism, suppresses the senescence and inflammatory reaction of endothelial cells. Methods: The antioxidant activity of RFO was evaluated using 2,2,-diphenyl-1-picrylhydrazyl (DPPH) assay. To evaluate anti-senescence effect, 2 mM H2O2 was treated to human umbilical vein endothelial cells (HUVEC). 84 genes related to oxidative stress were evaluated using RT2-profiler PCR array. For anti-inflammatory effect, 1 μg/mL lipopolysaccharide (LPS) was treated to murine monocyte. The level of NO, pro-inflammatory cytokines, and signaling proteins were measured using ELISA, real-time PCR, and western blot analysis. Results: DPPH assay showed that antioxidant potent of RFO was 78.5 times 1mM ascorbic acid. RFO promoted the activation of superoxide dismutase (SOD) which induced degradation of endothelin (ET-1) in HUVECs. RFO increased the levels of SOD, glutathione peroxidases 3 (GPx), and peroxiredoxins (TPx), whereas it decreased the ROS metabolism gene and the oxidative stress responsive genes. In macrophages, RFO reduced nitric oxide (NO), prostaglandin E2 (PGE2), ROS and significantly both protein and mRNA level of iNOS. RFO inhibited the translocation of NF-κB p65 subunit, phosphorylation of I-κB, Extracellular signal-regulated kinase (ERK), and c-Jun N-terminal kinase (JNK) in a dose-dependent manner. Conclusion: RFO inhibited the senescence of endothelial cell by superoxide anion/ROS through regulating the ROS metabolism gene and the inflammatory cytokines through NF-κB. To conclude, RFO could be candidate of clinical target for treating vascular senescence or inflammation.

        This research aimed to study on the characteristics of elderly facilities environmental affordance as strategies in response to the aged society. For this, among the documentaries about the aged society, four documentaries were selected. The spatial scenes of domestic and foreign elderly facilities in documentaries were divided according to the spatial category. Among the characteristics of spatial environment of the analysis cases, the 35 scenes picked out from the 123 scenes were utilized. The data were analyzed using the Murtha and Lee’s User Benefit Criteria. The analysis results were the following. Social Facilitation was particularly important at indoor spatial environment of the four affordances. Perceptual Maintenance and Social Facilitation for private spatial environment and Physiological Maintenance and Social Facilitation for common spatial environment were emphasized. Physiological Maintenance and Social Facilitation for management support spatial environment and Social Facilitation for corridor environment were emphasized. Perceptual Maintenance and Social Facilitation were emphasized in outdoor spatial environment. Therefore, for elderly facilities it is important to have Social Facilitation, Perceptual Maintenance and Physiological Maintenance, that support the environment.
