S-398 : 말기 신부전 환자에서 Valacyclovir 사용 후 발생한 중추 신경계 부작용 2예

Valacyclovir is a prodrug of acyclovir, transformed to acyclovir by first-pass metabolism in the liver. In recent years, valacyclovir has largely replaced acyclovir in the treatment of herpesvirus infections, because it is more effective by oral administration. Valacyclovir is used increasingly to treat herpes zoster, although neuropsychiatric symptoms [valacyclovir neurotoxicity (VAN) or acyclovir neurotoxicity], may accompany use of this drug. This paper reports two cases of neurotoxicity of valacyclovir in a patients with end-stage renal disease who was undergoing maintanence hemodialysis. A 68-year-old male, receiving hemodialysis three times a week, prescribed valacyclovir 1000 mg/day on every dialysis day, for herpes zoster of left thigh . After he took his second dose orally, he developed lethargy and disorientation. His mental status progressed to semicoma. Daily conventional hemodialysis was started. After three sessions of hemodialysis, the patient became alert A 64-year-old male who was being treated with hemodialysis was admitted for dysarthria and euphoria. He took 1,000 mg of valacyclovir twice for hepes zoster of fourth level of thoracic nerve. After two days of treatment with oral valacyclovir, he had several neurologic symptoms as mentioned above. We performed daily hemodialysis and after three session of hemodialysis, he fully recovered. In conclusion, valacyclovir can induce life-threatening neurotoxicity, especially in ESRD patients with appropriate dose reduction. But which can be effectively managed by hemodialysis.
The Effect of Private Brands’ Service Quality on Brand Attitude

Purpose: This study analyzes service quality of Pivate Brand (PB) productsto provide some academic and practical implications . Research design, data, and methodology: The focus is on how service quality of No Brand, which has recently received much attention, affects brand attitude and, to confirm whether consumers’ prior knowledge of PB moderates this effect. A total of 167 men and women in their 20s who have experience using No Brand were surveyed. Hypothesis was verified by using hierarchical regression analysis. Results: a) Tangibles, reliability, empathy, and assurance of service quality for No Brand have a positive effect on brand attitude; b) The moderating effect of prior knowledge is only statistically significant in the context of reliability. Conclusion: This study provides academic and practical implications for establishing differentiated PB strategies. It highlights the effects of service quality and consumer prior knowledge on brand attitude. This suggests that the sub-dimension of PB service quality has a discriminatory effect on the brand attitude of No Brand, and that consumer's prior knowledge of the product influences reliability of service quality. Therefore, consumer's prior knowledge is important in order to increase trust.

NIPS 시스템은 중성자 핵반응 결과 방출되는 즉발 감마선을 정량적으로 측정하는 장치이며 고체 및 액체 폐기물 중 존재하는 다양한 원소를 비파괴적으로 분석할 수 있는 장점이 있다. 본 연구에서는 NIPS 시스템에 이용된 고순도반도체 검출기의 계측효율을 Ba 및 Eu 방사성 동위원소 선원과 Cl(n, ) Cl 핵반응 시 발생되는 즉발감마선을 이용하여 80 keV에서 8 MeV까지 넓은 영역에 대하여 구하였다. Cl(n, ) Cl 핵반응을 이용한 고에너지 감마선의 계측효율은 즉발감마선의 방사능 값을 정확히 알 수 없기 때문에 저 에너지 영역에서 정확히 알고 있는 검출기 효율곡선에 규격화시켜 전 에너지 영역에서의 효율보정곡선을 구하였다. 또한 KCl 표준용액에 Cf 중성자 선원을 조사시켜 표준용액으로부터 방출되는 즉발 감마선을 고순도반도체 검출기로 측정하고 광대역 계측효율 곡선을 이용하여 수용액 시료에서의 평균 열중성자 속을 예측하였다. NIPS 측정시스템은 주변 재료 물질의 핵반응으로 방출되는 감마선 background를 줄이기 위해 두 개의 고순도반도체 검출기를 이용한 동시계수 장치가 고안되었으며, 동시계수 모드에서의 계측효율도 함께 고려되었으며, 표준선원을 이용하여 전 계수 또는 동시계수모드에서의 background에 대한 측정감도를 비교하였다.다. Neutron induced prompt gamma-ray spectroscopy(NIPS) system measures the prompt gamma-ray emitting by the interaction of a neutron with various materials. This system will be of great benefit to scientists worldwide, since it provides the non-destructive measurement of many element in either solid or liquid wastes. In this study, the full-energy-peak (FEP) efficiency calibration for a HPGe detector was constructed in the -ray energy range from 80 keV to 8 MeV, using Ba and >TEX>$^{152}Eu$ RI sources and $ ^{35}Cl(n, )^{36}Cl$ thermal neutron captured reaction. The FEP efficiency curve for the higher energies using the $^{35}Cl(n, )^{36}Cl$ reaction was normalized with the curve obtained from the RI sources, since the accurate activity of its prompt -ray is unknown. The average thermal neutron flux was theoretically calculated using the FEP efficiency curve for the KCl standard solutions. The NIPS system equipped with a - coincidence setup with two n-type coaxial HPGe detectors was considered in order to reduce the interfering -ray background. The FEP efficiency curve for the - coincidence system was also obtained for full energy range. The performance of the normal and coincidence NIPS system was tested by comparing signal-to-noise ratio in each mode using the reference sample.e.
A CHANGE OF SCALE FORMULA FOR GENERALIZED WIENER INTEGRALS

Cameron and Storvick introduced change of scale for-mulas for Wiener integrals of bounded functions in the Banach alge-bra S of analytic Feynman integrable functions on classical Wiener space. Yoo and Skoug extended this result to an abstract Wiener space. Also Yoo, Song, Kim and Chang established a change of scale formula for Wiener integrals of functions on abstract Wiener space which need not be bounded or continuous. In this paper, we investigate a change of scale formula for generalized Wiener inte- grals of various functions on classical Wiener space.
Risk Classification of Vessel Navigation System using Correlation Weight of Marine Environment

Various algorithms and system development are being required to support the advanced decision making of navigation information support system because of a serious loss of lives and property accidents by officer's error like as carelessness and decision faults. Much of researchers have introduced the techniques about the systems, but they hardly consider environmental factors. In this paper, We collect the context information in order to assess the risk, which is considered the various factor of the sailing ship, then extract the features of knowledge context, which is to apply the weight of correlation coefficients among data in context information. We decide the risk after the extract features through the classification and prediction of context information, and compare the value accuracy of proposed method in order to compare efficiency of the weighted value with the non-weighted value. As a result of experience, we know that the method of weight properties effectively reflect the marine environment because the weight accurate better than the non-weighted.
