        Transarterial Embolization for Sporadic Renal Angiomyolipoma: Patient Selection and Technical Considerations for Optimal Therapeutic Outcomes

        Although renal angiomyolipoma (AML) is a benign tumor, treatment may be necessary occasionally because it can cause potentially life-threatening retroperitoneal hemorrhage. Transarterial embolization (TAE) is a safe and effective treatment option to prevent the hemorrhagic rupture of AMLs and relieve the symptoms caused by enlarged lesions or active bleeding. However, there is no clear consensus regarding the indications for prophylactic TAE in patients with sporadic renal AMLs. In urgent TAE for bleeding AMLs, there is a likelihood of incomplete embolization when the focus is on stabilizing the clinical symptoms. This pictorial essay discusses the patient selection and technical considerations to achieve optimal therapeutic effects as well as the follow-up findings after TAE.

        Intervention treatment such as balloon retrograde or anterograde transvenous obliteration has been used for management of refractory hepatic encephalopathy as well as gastric variceal bleeding. Recently, plug-assisted retrograde transvenous obliteration without a help of balloon was newly developed to treat these patients. Here, we report three cases suffering refractory hepatic encephalopathy who were treated with this new technique.

      • A Case of a Gallbladder Infarction after Transcatheter Arterial Chemoembolization, Recovered with Only Medical Treatment

        Transcatheter arterial chemoembolization (TACE) is suitable treatment method of hepatocellular carcinoma (HCC) for patients with decreased underlying liver function or unresectable disease. But this procedure can induce some embolic events. We report a case of a gallbladder infarction caused by the embolic event in TACE procedure. A 61-year-old man had been going to undergo 6th TACE for marginal recurrence of HCC. After the procedure, gallbladder infarction occurred and was detected by abdominal computed tomography (CT) scan for response evaluation of the treatment at out-patient-clinic. Then, he was treated with oral antibiotics and conservative therapy at out-patient clinic. In 6th week after the event, abdominal CT scan showed improved state of gallbladder.

        Mycophenolic Acid Trough Concentration and Dose Are Associated with Hematologic Abnormalities but Not Rejection in Kidney Transplant Recipients

        Background: Little is known regarding the safe fixed dose of mycophenolic acid (MPA) for preventing biopsy-proven acute rejection (BPAR) in kidney transplant recipients (KTRs). We investigated the correlation of MPA trough concentration (MPA C0) and dose with renal transplant outcomes and adverse events. Methods: This study included 79 consecutive KTRs who received MPA with tacrolimus (TAC) and corticosteroids. The MPA C0 of all the enrolled KTRs was measured, which was determined monthly by using particle-enhanced turbidimetric inhibition immunoassay for 12 months, and clinical data were collected at each time point. The clinical endpoints included BPAR, any cytopenia, and BK or cytomegalovirus infections. Results: No differences in MPA C0 and dose were observed between KTRs with or without BPAR or viral infections under statistically comparable TAC concentrations. MPA C0 was significantly higher in patients with leukopenia (P = 0.021) and anemia (P = 0.002) compared with those without cytopenia. The MPA dose was significantly higher in patients with thrombocytopenia (P = 0.002) compared with those without thrombocytopenia. MPA C0 ≥ 3.5 µg/mL was an independent risk factor for leukopenia (adjusted odds ratio [AOR], 3.80; 95% confidence interval [CI], 1.24–11.64; P = 0.019) and anemia (AOR, 5.90; 95% CI, 1.27–27.51; P = 0.024). An MPA dose greater than the mean value of 1,188.8 mg/day was an independent risk factor for thrombocytopenia (AOR, 3.83; 95% CI, 1.15–12.78; P = 0.029). However, an MPA dose less than the mean value of 1,137.3 mg/day did not increase the risk of BPAR. Conclusion: Either a higher MPA C0 or dose is associated with an increased risk of cytopenia, but neither a lower MPA C0 nor dose is associated with BPAR within the first year of transplantation. Hence, a reduced MPA dose with TAC and corticosteroids might be safe in terms of reducing hematologic abnormalities without causing rejection.

        사물인터넷(internet of things, IoT)의 시대가 도래하면서 실시간으로 주변 환경을 감지하고 주기적으로 모니터링할 수 있는 모바일/웨어러블 센서에 대한 관심이 커지고 있다. 이에 따라 가볍고 유연한 유기반도체를 이용한 센서 개발이 활발하게 이루어지고 있으며, 특히 유기반도체 기반 가스센서는 기존 무기반도체 가스센서에 비해 화학반응에 민감하고 비교적 낮은 온도에서도 작동 가능하다는 장점을 가지고 있어 크게 주목받고 있다. 따라서 본 기고문에서는 유기반도체 기반 가스센서의 감지 원리를 살펴보고 이에 따른 유기반도체 기반 가스센서의 민감성(sensitivity), 선택성(selectivity), 유연성 향상에 관한 연구 동향을 논의하고자 한다.

        <P>A Gram-stain-negative, strictly aerobic, non-motile, non-spore-forming, short rod-shaped and moderately halophilic bacterial strain, CAU 1105T, was isolated from soil on reclaimed tidal land in Modo, Republic of Korea, and its taxonomic position was investigated using a polyphasic approach. Strain CAU 1105T grows optimally at a temperature of 37?C at pH?7 in the presence of 3?% (w/v) sea salt. Based on 16S rRNA gene sequence similarity analyses, the novel isolate was assigned to the genus Palleronia within the class Alphaproteobacteria and showed the highest 16S rRNA gene sequence similarity with Palleronia marisminoris B33T (95.4?%). Strain CAU 1105T contained ubiquinone-10 as the only respiratory quinone and C18?:?1ω7c as the major cellular fatty acid. The DNA G+C content of strain CAU 1105T was 64.3?mol%. On the basis of phenotypic differentiation, phylogenetic and chemotaxonomic data, strain CAU 1105T represents a novel species of the genus Palleronia, for which the name Palleroniasoli sp. nov. is proposed. The type strain is CAU 1105T (?=?KCTC 42298T?=?NBRC 110740T). An emended description of the genus Palleronia is also provided.</P>

        마음챙김명상 프로그램과 자비수행을 추가한 마음챙김명상 프로그램의 효과비교: 만성근골격계 통증 환자를 중심으로

        이 연구는 순수 마음챙김명상 프로그램과 자비수행을 추가한 마음챙김명상 프로그램을 만성 근골격계 통증환자들에게 적용하여 통증, 삶의 질, 정서에 미치는 영향을 보고자 했다. 총 7회기의 프로그램에 각 집단 10명씩 무선할당하여 1시간 30분의 마음챙김명상 프로그램에, 자비수행을 추가한 집단은 20여분의 자비수행 교육과 실습을 겸했다. 분석결과 마음챙김명상 집단은 통증, 우울, 상태불안이 유의하게 감소했고, 자비수행을 추가한 마음챙김명상 집단은 모든 척도에서 유의한 효과를 보였다. 자비수행을 추가한 집단이 순수 마음챙김명상 집단에 비해 더 긍정적인 효과가 있는지 확인한 결과 수용, 탈중심화, 삶의 만족예상에서 더 큰 변화량을 보였다. 두 집단 모두 추적 검사에서 상당부분 효과가 유지되었다. Background: Choric Musculoskeletal pain patients suffer from emotional distress such as depression and anxiety and also experience much more discomfort in their lives. The purpose of this study was to compare the effects of a Mindfulness meditation group therapy and Mindfulness meditation group therapy added Compassion meditation on pain, quality of life, and psychological features in Chronic Musculoskeletal pain patients. Methods: 20 participants received 7 weekly sessions in mindfulness meditation training program, which take about 90 minutes each. Only Compassion Meditation group received 20 minutes added program. Measures included Visual Analog Scale (VAS), Quality of life (WHOQOL-BRIF), Center for Epidemiologic Studies Depression Scale (CES-D), State-Trait Anxiety Inventory-Y (STAI-Y), Acceptance and Action Questionnaire-16 (AAQ-16), Experiences Questionnaire (EQ), Life Satisfaction Expectancy scales (LSES). All the variables were measured at three time points: pre, post, and follow up implementation. Results: The t-test results of the participants before and after the Mindfulness Meditation group showed statistically significant improvement in their Pain (t=2.64, p<.05), Depression (t=5.579, p<.05), and State Anxiety (t=2.223, p<.05), but not in another positive psychological features. Added Compassion Meditation group showed statistically significant improvement all of measures (Pain: t=3.180, p<.01, Quality of life: t=4.781, p<.01, Depression: t=2.208, p<.01, State Anxiety: t=3.838, p<.01, Trait Anxiety: t=3.208, p<.01, Acceptance: t=-2.635, p<.05, Experience: t=-3.041, p<.01, Life satisfaction: t=-2.23, p<.05). The follow-up implementation showed the persistence of beneficial changes. Conclusions: Added compassion meditation is more effective than mindfulness meditation for chronic musculoskeletal pain. There are greater benefits for alleviating pain, improving quality of life and other psychological features that can be gained from performing added compassion meditation rather than doing mindfulness meditation solely.

        Geranylnaringenin (CG902) inhibits constitutive and inducible STAT3 activation through the activation of SHP-2 tyrosine phosphatase

        <P><B>Abstract</B></P> <P>The roles and significance of signal transducer and activator of transcription 3 (STAT3) in human cancers have been extensively studied and STAT3 is a promising therapeutic target for cancer drug discovery. During the screening of natural products to identify STAT3 inhibitors, we identified geranylnaringenin (CG902), which decreased luciferase activity in a dose-dependent manner. CG902 specifically inhibited STAT3 phosphorylation at Tyr-705 in DU145 prostate cancer cells and decreased the expression levels of STAT3 target genes, such as cyclin D1, cyclin A, and survivin. Notably, the knockdown of the SHP-2 gene by small interfering RNA suppressed the ability of CG902 to inhibit STAT3 activation and CG902 activated the phosphatase activity of SHP-2 through direct interaction with SHP-2 and induced the phosphorylation of SHP-2. The interactions between CG902 and SHP-2 were confirmed by pull-down assay using biotinylated CG902. The interactions were also further validated by the drug affinity responsive target stability (DARTS) and cellular thermal shift assay (CETSA). The inhibitory effect of CG902 on cell growth was confirmed using the DU145 mouse xenograft model. We propose that CG902 inhibits STAT3 activity through a mechanism that involves the interactions between CG902 and SHP-2, and the phosphorylation of SHP-2, which leads to SHP-2 activation in DU145 cells. CG902 is the first compound to regulate STAT3 activity via the modulation of SHP-2 activity, and our results suggest that CG902 is a novel inhibitor of the STAT3 pathway and an activator of SHP-2, and may be a useful lead molecule for the development of a therapeutic STAT3 inhibitor.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

        Clinical, endocrinological, and molecular features of four Korean cases of cytochrome P450 oxidoreductase deficiency

        Purpose: Cytochrome P450 oxidoreductase (POR) deficiency is a rare autosomal recessive disorder caused by mutations in the POR gene encoding an electron donor for all microsomal P450 enzymes. It is characterized by adrenal insufficiency, ambiguous genitalia, maternal virilization during pregnancy, and skeletal dysplasia. In this study, we investigated the clinical, hormonal, and molecular characteristics of patients with POR deficiency in Korea. Methods: This study included four patients with POR deficiency confirmed by biochemical and molecular analysis of POR. Clinical and biochemical findings were reviewed retrospectively. Mutation analysis of POR was performed by Sanger sequencing after polymerase chain reaction amplification of all coding exons and the exon-intron boundaries. Results: All patients presented with adrenal insufficiency and ambiguous genitalia regardless of their genetic sex. Two patients harbored homozygous p.R457H mutations in POR and presented with adrenal insufficiency and genital ambiguity without skeletal phenotypes. The other two patients with compound heterozygous mutations of c.[1329_1330insC];[1370G>A] (p.[I444Hfs*6];[R457H]) manifested skeletal abnormalities, such as craniosynostosis and radiohumeral synostosis, suggesting Antley-Bixler syndrome. They also had multiple congenital anomalies involving heart, kidney, and hearing ability. All patients were treated with physiologic doses of oral hydrocortisone. Conclusion: We report the cases of 4 patients with POR deficiency identified by mutation analysis of POR. Although the study involved a small number of patients, the POR p.R457H mutation was the most common, suggesting founder effect in Korea. POR deficiency is rare and can be misdiagnosed as 21-hydroxylase or 17α-hydroxylase/17,20-lyase deficiency. Therefore, molecular analysis is critical for confirmatory diagnosis.

      • Primary Pulmonary Leiomyosarcoma: Could It Be Cured by Radiation Therapy?

        Primary leiomyosarcoma of lung is extremely rare and often diagnosed as a mass on routine chest radiography. Although advances have been made in treatment protocols, leiomyosarcoma remains one of the more difficult soft tissue sarcomas to treat. Surgical resection is usually curative for small and well-differentiated sarcomas. For poorly differentiated and non-resectable tumors, chemotherapy and radiation therapy are used as neoadejuvant or palliative treatment options. Generally, leiomyosarcomas are known to be resistant to radiation therapy alone. The authors experienced a 68-year-old woman who was diagnosed leiomyosarcoma by routine chest radiography. Although disease progression was observed despite of chemotherapy, radiation therapy reduced the size of tumor. This paper describes the curative potential of radiation therapy for primary pulmonary leiomyosarcomas through a case report and literature review.
