Annual Financial Impact of Thyroidectomies for Nodular Thyroid Disease in China

A large proportion of patients with thyroid nodules in China undergo thyroidectomy in order to get confirmatory histology diagnosis. The financial impact of this modality remains to be investigated. To evaluate rationality of performing thyroidectomy without a routine FNA preoperatively from the economic perspective, we conducted a retrospective, observational study of all archival thyroidectomies with records of cost per stay (CPS), cost per day (CPD) and length of stay (LOS) from 2008 to 2013 in the First Affiliated Hospital of Nanjing Medical University. We compared all the parameters between cancer and non-cancer thyroidectomies. We recruited 6, 140 thyroidectomies with valid records of CPS, CPD and LOS in this period. The CPS of cancer thyroidectomy was significantly higher than non-cancer thyroidectomy. The percentage of cancer thyroidectomy increased from 26.5% to 41.6%. The percentage of annual cost of cancer thyroidectomies rose from 30.2% to 45.2%. The LOS for cancer and non-cancer thyroidectomy decreased while the CPD increased in the past six years. The estimated national cost in 2012 for all thyroidectomies would be USD 1.86 billion with USD 1.09 billion for non-cancer thyroidectomies. We have witnessed great improvement in the healthcare for patients with thyroid nodules in China. However, given limited healthcare resources, currently thyroid FNA for more precise preoperative diagnosis may help to curb the rapidly increasing demand in healthcare costs in the future for nodular thyroid disease in China.

Background: Increasing evidence from animal, epidemiological and clinical investigations suggest that dietary anthocyanins have potential to prevent chronic diseases, including cancers. It is also noteworthy that human epidermal growth factor receptor 2 (ErbB2) protein overexpression or ErbB2 gene amplification has been included as an indicator for metastasis and higher risk of recurrence for breast cancer. Materials and Methods: The present experiments investigated the anti-metastasis effects of black rice anthocyanins (BRACs) on ErbB2 positive breast cancer cells in vivo and in vitro. Results: Oral administration of BRACs (150 mg/kg/day) reduced transplanted tumor growth, inhibited pulmonary metastasis, and decreased lung tumor nodules in BALB/c nude mice bearing ErbB2 positive breast cancer cell MDA-MB-453 xenografts. The capacity for migration, adhesion, motility and invasion was also inhibited by BRACs in MDA-MB-453 cells in a concentration dependent manner, accompanied by decreased activity of a transfer promoting factor, urokinase-type plasminogen activator (u-PA). Conclusions: Together, our results indicated that BRACs possess anti-metastasis potential against ErbB2 positive human breast cancer cells in vivo and in vitro through inhibition of metastasis promoting molecules.
A new reconfigurable liquid-metal-antenna-based sensor

In this paper, a new sensor chip with frequency reconstruction range of 2.252 GHz ~ 2.450 GHz is designed and fabricated. On this basis, a self-designed "T-shaped" shell is added to overcome the disadvantage of uneven deformation of the traditional steel shell, and the range of the sensor chip is expanded to 0 kN ~ 96 kN. The liquid metal antenna is used to carry out a step-by-step loading test, and the relationship between the antenna resonance frequency and the pressure load is analyzed. The results show that there is a good linear relationship between the pressure load and the resonant frequency. Therefore, the liquid metal antenna can be regarded as a pressure sensor. The cyclic loading and unloading experiments of the sensor are carried out, and different loading rates are used to explore the influence on the performance of the sensor. The loading and unloading characteristic curves and the influence characteristic curves of loading rate are plotted. The experimental results show that the sensor has no residual deformation during the cycle of loading and unloading. Moreover, the influence of temperature on the performance of the sensor is studied, and the temperature correction formula is derived.

The structure, elastic and electronic properties of MoAlB have been studied under high pressure by using first principles. The optimized lattice parameters, elastic constants, bulk moduli, and shear moduli agree well with experimental and other theoretical values. The relationships of the lattice constants with pressure indicated an anisotropic behavior. The calculated elastic constants satisfy the mechanical stability conditions. Our calculated Vickers hardness of MoAlB is 11.56~GPa, which agrees with the experimental data and confirms its having a relatively softer nature than the other borides. The Cauchy pressure, the Poisson's ratio and the $G/B$ value suggest that MoAlB is a brittle solid under ambient conditions, but the brittleness decreases with increasing pressure. The band structure and the density of states reveal the metallic feature of MoAlB, and the energy levels shift toward both sides of the Fermi level with increasing pressure. The Mulliken population and density of states suggest a mixture bonding nature of covalence and metallicity in MoAlB.

Objective: To investigate whether the expression level of valosin-containing protein (VCP) is correlated with the prognosis of primary orbital mucosa-associated lymphoid tissue (MALT) lymphoma. Methods: VCP expression in 58 samples from primary orbital MALT lymphoma patients was determined by immunohistochemisty using monoclonal antibodies. Correlations between VCP expression level and prognosis were clarified by statistical analysis. Results: It was found that the percentage of VCP positive cells in samples of primary orbital MALT lymphoma ranged from 32% to 95%. The samples were divided into two groups (level 1 and level 2) according to the median value (45%) of the percentage of VCP positive cells. It was found that the expression level of VCP was significantly correlated with recurrence (P=0.003) and tumor size (P=0.008). At the same time, the 5-year disease-free and overall survival rate of patients of level 1 was significantly better than that of level 2 (P=0.001; P=0.032). There was no observed correlation between the expression level of VCP and other clinical features. Conclusion: VCP could be a useful marker for predicting the prognosis of primary orbital MALT lymphoma.

A new cyclopentanopyridine alkaloid, 3-hydroxy-5-methyl-5,6-dihydro-7H-cyclopenta[b]pyridin-7-one (1), together with 11 known aromatic compounds were isolated from the secondary metabolites of the halotolerant fungal strain Wallemia sebi PXP-89 in 10% NaCl. Their structures including the absolute configurations of (2S,3S)-1-(4-hydroxyphenyl)butane-2,3-diol (2), (2R,3S)-1-(4-hydroxyphenyl)butane-2,3-diol (3), and (S)-3-hydroxy-4-(4-hydroxyphenyl)-2-one (4) were elucidated by spectroscopic analysis and a modified Mosher’s method. Compound 1 exhibited antimicrobial activity against Enterobacter aerogenes with a MIC of 76.7 μM. The absolute configurations of compounds 2-4 were determined for the first time.

Background/Aims: Programmed death-1 (PD-1) expression was investigated in CD4+ and CD8+ T cells from hepatitis B virus (HBV)-infected patients at the chronic hepatitis B (CHB) infection, liver cirrhosis (LC), and hepatocellular carcinoma (HCC) stages. Methods: PD-1 expression in circulating CD4+ and CD8+ T cells was detected by flow cytometry. The correla-tions between PD-1 expression and HBV viral load, alanine aminotransaminase (ALT) levels and aspartate aminotrans-ferase (AST) levels were analyzed using GraphPad Prism 5.0. Results: PD-1 expression in CD4+ and CD8+ T cells was significantly increased in both the CHB group and advanced-stage group (LC plus HCC). In the CHB group, PD-1 expres-sion in both CD4+ and CD8+ T cells was positively correlated with the HBV viral load, ALT, and AST levels. However, in the LC plus HCC group, significant correlations between PD-1 expression and the clinical parameters were nearly absent. Conclusions: PD-1 expression in peripheral CD4+ and CD8+ T cells is dynamic, changes with HBV infection progression, and is related to HBV viral load and liver function, especially in CHB. PD-1 expression could be utilized as a potential clini-cal indicator to determine the extent of virus replication and liver injury. (Gut Liver 2014;8:186-195)
Wireless charger networking for mobile devices: fundamentals, standards, and applications

<P>Wireless charging is a technique of transmitting power through an air gap to an electrical device for the purpose of energy replenishment. Recently, wireless charging technology has significantly advanced in terms of efficiency and functionality. This article first presents an overview and fundamentals of wireless charging. We then provide the review of standards, that is, Qi and the Alliance for Wireless Power, and highlight their communication protocols. Next, we propose a novel concept of wireless charger networking that allows chargers to be connected to facilitate information collection and control. We demonstrate the application of the wireless charger network in user-charger assignment, which clearly shows the benefit in terms of reduced costs for users to identify the best chargers to replenish energy for their mobile devices.</P>

Abscisic acid (ABA) and hydrogen peroxide (H2O2) are important regulatory factors involved in plant development under adversity stress. Here, the involvement of H2O2 in ABA-induced adventitious root formation in cucumber (Cucumis sativus L.) under drought stress was determined. The results indicated that exogenous ABA or H2O2 promoted adventitious rooting under drought stress, with a maximal biological response at 0.5 μM ABA or 800 μM H2O2. The promotive effects of ABA-induced adventitious rooting under drought stress were suppressed by CAT or DPI, suggesting that endogenous H2O2 might be involved in ABA-induced adventitious rooting. ABA increased relative water content (RWC), leaf chlorophyll content, chlorophyll fluorescence parameters (Fv/Fm, ΦPS II and qP), water soluble carbohydrate (WSC) and soluble protein content, and peroxidase (POD), polyphenol oxidase (PPO) and indoleacetate oxidase (IAAO) activities, while decreasing transpiration rate. However, the effects of ABA were inhibited by H2O2 scavenger CAT. Therefore, H2O2 may be involved in ABA-induced adventitious root development under drought stress by stimulating water and chlorophyll content, chlorophyll fluorescence, carbohydrate and nitrogen content, as well as some enzyme activities.
