        선별적 파상풍 예방 처치를 위한 Tetanos Quick Stickⓡ의 유용성

        P u r p o s e: Tetanus is still one kind of major health problem in many countries, so tetanus prophylaxis is very important. However medical interview and wound description are not always enough to determine the tetanus prophylaxis. Thus, we assessed the utility of Tetanos Quick StickⓇ test for selective tetanus prophylaxis in the emergency department. M e t h o d s: From September 17, 2003 to October 4, 2003, 180 patients were studied for 14 days. Tetanos Quick Stick Ⓡ and ELISA (enzyme-linked immunosorbent assay) were performed with 180 samples from 180 patients. R e s u l t s: The Tetanos Quick StickⓇ had a sensitivity of 79.6%, a specificity of 99.2%, a positive predictive value of 97.7 %, a negative predictive value of 91.9%, and an accuracy of 93.3%. C o n c l u s i o n: The results of our study revealed that the Tetanos Quick StickⓇ test is useful for selective tetanus prophylaxis in the emergency department.

        한국인의 항파상풍 항체의 정성적 조사

        Background: In Korea, vaccination against tetanus began in the mid-1950s. At present, there is a routine immunization schedule with diphtheria, pertussis, and tetanus (DPT) vaccines at 2,4,6 month after birth and booster at 15-18 month and 4-6 years. However, further prophylactic programs according to guidelines of Center for Disease Control do not exist due to the absence of tetanus toxoid. And we have usually used only passive immunization, tetanus immunoglobulin, as a prophylactic regimen for injured patients. In this study, we aimed to determine the tetanus antibody titers levels of Koreans and point out problems of prophylactic programs after a routine immunization schedule. Methods: This study was conducted with 112 healthy adults from August 29, 2003 to September 19, 2003. No volunteers had unstable vital signs, or had taken any medication for chronic illnesses. A questionnaire was used for the volunteers to assess their knowledge about tetanus vaccination schedule, and the whole blood was withdrawn from each volunteer to determine qualitative anti-tetanus antibody titers using TQS (Tetanus Quick Stick). TQS had been introduced as a rapid, easy method to determine the immune status. Results: Of the 112 studied subjects, only 13 (11.6%) were found to have protective levels of anti-tetanus antibody titers. There was no association between quantitative antibody titers and age, sex, educational status, economy, place of birth or residence, military service, and vaccination. Conclusion: Although tetanus is a preventive disease by the antibodies developed as a result of vaccination and its occurrence is not very common, it is associated with a rather high mortality rate. Recently, there has beena tetanus immunization shortage in Korea caused by an absence of tetanus toxoid. The effect of a tetanus shortage makes the tetanus antibody values go under the protective level for most adults. We recommend that tetanus prophylaxis in Korea should be performed following the guidelines of Center for Disease Control and Prevention.

        한국인의 항파상풍 항체 역가

        Inspite of immunization, TETANUS remains a disease with a high morbidity and mortality rate. Tetanus has been still occurred more frequently in developing countries where immunization programs are inadequate. In Korea, there is a routine immunuzation schedule with diphtheria, pertussis, and tetanus(DPT) vaccines at 2,4,6 month after birth at 18 month. But further prophylactic programs are not exist. Moreover, we have usually used only passive immunization, tetanus antitoxin, as a prophylactic regimen for injured patients instead of guidelines of Center for Disease Control. The purpose of this study was to assess the baseline tetanus immunity state and to suggest the proper immunization schedule and adequate prophylactic guideline for injured patients in Korea. One hundred thirty one trauma patients from emergency department of Severance hospital from September to November of 1993 were included. Antietanus antibody titers of the patients were analyzed by ELISA assay. The mean antitetanus antibody titer was 0.728 ±0.236IU/mL. The titers were diminished rapidly with age. In pediatric group(age under 15), the mean titer was 3.251 ±0.426 IU/mL and in geriatric group(age over 64), the mean titer was 0.007 ±0.002 IU/mL which was below the minimal protective antibody titer(0.01 IU/mL). While all of the pediatric patients have protective antibody titer, only 69.2% of young adults(15-39, n=65), 56.7% of adults(40-64, n=30), and 57.2% of geriatrics(>65, n=14) have protective antibody titer. Our results suggest that tetanus prophylaxis is unnecessary in the age under 14. In other age groups, however, our data strongly support the need of prophyulaxis.
