        디지털 덴티스트리 임상활용 및 기대효과에 기반한 직무스트레스 : 임상치과위생사의 관점 조사

        본 연구는 임상치과위생사를 대상으로 일반적인 특성과 디지털 덴티스트리 임상활용 및 기대효과에 대해 살펴보고, 이를 토대로 이들의 디지털 덴티스트리 역량 강화를 통한 직무스트레스 감소방안에 도움이 되는 기초자 료를 제공하고자 실시하였다. 연구대상은 일부 J지역 임상치과위생사이며, 설문조사를 실시하였다. 연구결과 전반 적 직무스트레스에 가장 큰 영향요인은 디지털 덴티스트리 임상활용도이었으며, 모형의 설명력으로 비교했을 때 디지털 덴티스트리 임상활용 시 기대효과가 가장 큰 영향요인이었다. 전반적 임상활용 기대효과가 낮을수록 전반 적인 직무스트레스가 높았다. 디지털 덴티스트리 임상활용 직무부담 및 불안정이 낮을수록 디지털 덴티스트리 관 련 교육참여 경험이 있는 경우 직무스트레스가 낮았다. 따라서 향후 임상치과위생사를 위한 성공적인 직무수행을 지원하는 프로그램 개발 및 직무스트레스 관리방안이 모색되어야 한다. This study examines the general characteristics, clinical use of digital dentistry, and expected effects of clinical dental hygienists, and provides basic data helpful in reducing job stress by strengthening their digital dentistry capabilities. It was carried out to do this. The subjects of the study were some clinical dental hygienists in J area, and a survey was conducted. As a result of the study, the most influential factor on overall job stress was the clinical use of digital dentistry, and when compared to the explanatory power of the model, the expected effect of clinical use of digital dentistry was the most influential factor. The lower the expected effect of overall clinical use, the higher the overall job stress. The lower the job burden and instability in clinical use of digital dentistry, the lower the job stress was in those who had experience participating in digital dentistry-related education. Therefore, in the future, program development and job stress management measures should be sought to support successful job performance for clinical dental hygienists.

      • Fundamental understandings of mesh process and various formats of digital data in digital dentistry

        Clinicians manage three-dimensional (3D) data in routine practice in the era of digital dentistry. Fundamental knowledge of digital data beyond the user’s manual provided by the companies is needed to understand and handle the 3D data in clinics. The 3D data used in the clinic are acquired by scanners and processed using a step-by-step procedure. A point cloud is an initial form of raw data. The data are then filtered, and a triangle mesh is generated. The meshes could be exported in various file formats. This fundamental knowledge would be helpful for clinicians in daily practice.

        “Digital Dentistry” is the most important issue in dental care field, especially on dental implant restoration. Recently, CAD/CAM technics have been used for dental implant design. But it is not effective as expected because many analogue procedures still remain on it. Therefore, it is needed to research about how to reduce user input from dental implant design. In this paper, we suggest an algorithm of automatic transformation of implant crown on dental CAD system. It translates, rotates, scales an implant crown automatically using dental implant library and user-picked 4 points. This algorithm can reduce time consume of dental implant design procedure and the results are well enough to use in field.

        디지털 치의학과 치과용 디지털 장비에 대한 치과 분야 종사자들의 인식 조사

        The purpose of this survey was to assess the perceptions of dental personnel concerning digital dentistry and dental digital equipment. The survey was conducted from July to August 2019 in Busan and Gyeongnam area. Three sets of questionnaires were designed to target dental professionals and staff, dental industry personnel, and dental students. The questionnaires were composed of best-choice questions, multiple-choice questions, and questions with the five-point Likert scale. Frequency analysis, descriptive statistics, and one-way ANOVA were used for data analysis. A total of 141 dental personnel participated in the survey: 92 dental professionals and staff, 29 dental industry personnel, and 20 dental students. The time saving was the most frequent response to the benefits of digital dentistry in all job categories. The main barrier to dental digital equipment introduction was the high cost. Although the evaluation of the economic efficiency of dental digital equipment was not positive, sixty-seven percent of dental professionals and staff having no digital equipment responded that they were considering purchasing dental digital equipment. They were most interested in intra-oral scanners. In conclusion, dental personnel were positively aware of the utility of dental digital equipment and interested in introducing new equipment. However, high initial costs and uncertain economic efficiency were found to be the main barriers to using dental digital equipment.

        Digital workflow를 활용한 ‘All-on-4’ 임플란트 지지 고정성 보철물 수복 증례

        구치부 심한 골소실을 보이는 완전 무치악 환자에게 이공 전방에 4개 이상의 임플란트를 식립 후 전악 수복을 하는 ‘All-on-X’ 임플란트 지지 고정성 보철 수복을 고려해 볼 수 있다. 과거에는 이러한 임플란트 고정성 보철 수복시 특별한 기준점 없이 술자의 판단에 의존하여 잔존 치조제에 임플란트를 식립하고 보철 수복 단계에선 이미 식립된 임플란트에 맞춰서 보철물을 제작하는 방식이었지만, 최근 디지털 치의학의 발전으로 진단, 수술, 보철 제작 등 모든 과정에서 계획적이고 예지력 있는 치료가 가능해졌다. 디지털 총의치, 임플란트 서지컬 가이드, 안면스캔 그리고 복제 의치를 통한 인상 채득 등의 디지털 기술들을 사용하여 복잡한 기공 과정을 줄이고, 임시 수복물의 심미와 기능을 최종 보철물로 효과적으로 이행할 수 있다. 상기 환자는 상악 디지털, 하악 ‘all-on-x’ 임플란트 고정성 보철물 수복환자로, 진단부터 최종 수복까지 디지털 기술을 활용하여 기능과 심미성을 만족시키는 보철물을 제작할 수 있었기에 이를 보고하고자 한다. In the case of fully edentulous patients with severe alveolar bone resorption, the consideration of ‘All-on-X’ implant-supported fixed prosthesis after placing four or more implants in the anterior maxilla is possible. Recent advancements in digital dentistry have enabled systematic and predictable treatment in all phases, including diagnosis, surgery, and prosthesis fabrication. By incorporating digital dentistry techniques such as digital complete denture, implant surgical guides, facial scanning into the conventional restoration process, it is possible to reduce the complexity of the prosthesis fabrication and effectively achieve the transition from provisional prosthesis to definitive prosthesis in terms of both aesthetics and function.

        여러 보철 치료 술식에 따른 바른 스캐닝 과정과 구강스캐너의 활용

        With the development of digital dentistry, various intra oral scanners which acquire intraoral image without conventional impression taking and stone pouring steps have been introduced. Fixed dental prostheses such as inlay, onlay, crown, and bridge fabricated by CAD/CAM technique combined with digital impressions is getting popular due to the recent rapid progress of digital impression taking system. In comparison with traditional prosthetic procedure, the advantages of intraoral image acquiring and CAD/CAM technique are as follows; the omission of conventional impression materials, reduced workflow step, and increased efficiency by online communication with clinic and laboratory. This review article covers some opinions about the suitable scanning procedures for the various prosthodontic treatments and the utilization of digital intraoral scanner and CAD/CAM system.

        단일 환자에서 디지털 및 전통적인 워크플로우로 제작한 두 쌍의 총의치를 장착하여 비교평가한 증례 보고

        Application of digital technology in dental treatments can simplify the treatment process and reduce patient discomfort. In regards of digital complete dentures, several commercial systems are available. However, these systems are not being routinely applied in Korea. In this case report, comparative analyses were performed regarding the two sets of complete dentures fabricated in digital and conventional workflow in a single patient. Complete dentures made with digital workflow were able to form an ideal occlusion because customized teeth were used. Compared to that, there were also limitations such as lower retention of mandibular dentures, so we would like to report this. 치과치료에 디지털 기술을 접목함으로써 치료 과정을 단순화하고 환자의 불편감을 줄여줄 수 있다. 디지털 기술을 활용한 총의치의 경우 여러 상업적인 시스템들이 나와 있으나 아직 국내에서는 보편적으로 사용되고 있지 않다. 본 증례 보고에서는 단일 환자에서 디지털 및 전통적인 치료 술식으로 제작한 두 쌍의 총의치를 장착한 뒤 이에 대한 환자의 만족도와 유지력, 안정성, 적합도를 비교하였다. 디지털 치료 술식으로 제작한 총의치는 개인화되어 디자인된 밀링된 치아를 이용했기 때문에 이상적 교합을 형성할 수 있었으며, 디지털 치은 디자인을 통해 심미성 또한 확보하여 적절한 기능적, 심미적 임상결과를 보였으나, 전통적 제작 방식에 비해 하악 의치의 유지력이 떨어지는 등의 한계 또한 존재하였기에, 이에 대하여 보고하고자 한다.

        A Case of Full-mouth Rehabilitation using a Digital twin Based on Multi-source Data in a Patient with Vertical Dimension Loss due to Multiple Tooth wear and Fractures

        Abrasion, fracture, and absence of multiple teeth can occur owing to various causes. In the case of generalized wear, the inherent vertical dimension is lost, and tooth-supported or implant-supported fixed prosthetic restorations are considered as treatment alternatives. Any treatments planned on an articulator should be reproduced in patients without any errors. After verification, the final prosthesis should be fabricated. In the traditional method, this process is complicated, with theoretical and technical challenges to be overcome by practitioners. However, with the development and introduction of digital workflows as a treatment method, full-mouth rehabilitation is no longer a burdensome and challenging procedure. In this case report, full-mouth rehabilitation was performed in a 56-year-old male patient with loss of vertical dimension using a digital workflow different from the conventional method. Digital data, such as from intraoral and facial scans, CBCT, and jaw motion data, were acquired and used. Full-mouth rehabilitation was performed through case analysis, treatment plan establishment, prosthesis design, and digital manufacturing in digital twins created through the integration of various digital data.

        Creating protective appliances for preventing dental injury during endotracheal intubation using intraoral scanning and 3D printing: a technical note

        Digital dentistry has influenced many dental procedures, such as three-dimensional (3D) diagnosis and treatment planning, surgical splints, and prosthetic treatments. Patient-specific protective appliances (PSPAs) prevent dental injury during endotracheal intubation. However, the required laboratory work takes time, and there is the possibility of tooth extraction while obtaining the dental impression. In this technical report, we utilized new digital technology for creating PSPAs, using direct intraoral scanners and 3D printers for dental cast fabrication.
