        뇌졸중 상지기능 평가(Manual Function Test)의 정상인 표준치에 관한 연구

        This study was designed to get normative data of normal subjects by MFT(Manua1 Func-tion Test). The subjects were 95 normal adults. The results are as follows: 1. Mean scores of left and right MFS(Manua1 Function Score) were respectively 97.43 and 98.13. 2. MFS was higher on right upper extremity function than on left upper extremity function. 3. MFS was not significantly different between male and female. 4. Both left and right MFS (Manual Function Score) was significantly negative correlation with age.

        Objectives: We investigated muscle force from the upper limb lifting resistance test to conform the objectivity in manual muscle test. Methods: We made standard method in upper limb lifting resistance test to compare with experiment method switching the lower limb position left & right. And resistance forces of upper limb of subject were checked to inspector with closing eyes. Results: 1. The lifting resistance of right upper limb was stronger when the lower limb of right and left were abducted. 2. The lifting resistance of right upper limb was weaken when the lower limb of right and left were adducted. 3. The lifting resistance of right upper limb was weaken when the lower limb of right and left were elevated. Conclusions: As the above results, the deltoid muscle force checked in the upper limb lifting resistance test is affected by the location of lower limbs, it suggested that the muscle force of some part in the body will be affected by the other parts. It will be useful to understand the symmetry principle of body in muscle function.

        The Relationship between Cognitive Therapeutic Exercises and Basal Ganglia Function in Patients with Basal Ganglia Region Stroke; a Pilot Study

        Purpose This study was aimed that the cognitive therapeutic exercises (CTE) benefit on motor function in patients with basal ganglia stroke. Methods The subjects assigned to a right hemiplegia group (RHG) and left hemiplegia group (LHG) and performed the CTE 5 times a week for 4 weeks. Manual functional test (MFT), Korean-Modified Barthel Index (K-MBI) and Sensory Function Test (SFT) were used to assess motor functions of basal ganglia. Results There were significant differences in MFT in non-hemi side (MFT_nh), K-MBI and SFT in the RHG and in K-MBI in the LHG. Conclusion The subjects with the RHG more were improved in most assessments than that of the LHG. As a whole, the CTE influenced the basal ganglia functions.

        웹 페이지 수행기능분석과 점검 우선순위를 활용한 모델기반 웹 취약점 점검

        In general, Web vulnerability checking is preferred manual method to automatic method because of its accuracy. However, due to limit of inspection’s speed, only some screens in the target web-site are selected for sampling inspection. However, there is no separate selection(sampling) criterion for selecting the screen to be checked when checking the web vulnerability by sampling. Therefore, the selection of the target screen depends entirely on the web vulnerability Inspector. And, It is happen the problems that the results of the checking is different for the same web-site depending on the inspector. In this paper, to solve these problems, we classify the functions according to the function of each page and classifying according to the characteristics of the web-site, an inspection model is created using the performance function. Through the created inspection model, a check case can be created so that it can be used as a criterion for selecting an objective inspection target page. As a result of checking the actual website in the proposed method, it was confirmed that there is no deflection by the inspector in the identification of the screen to be checked. 일반적으로 웹 취약점 점검은 그 정확성 때문에 수동방식을 많이 선호하나, 점검 속도의 한계 등으로 대상 웹 사이트 내 일부 화면만을 샘플링으로 선정하여 점검한다. 그런데 샘플링으로 웹 취약점 점검을 할 때, 점검 대상화면을 선정(샘플링)하기 위한 별도의 선정(샘플링)기준이 없어, 대상 화면의 선정을 전적으로 웹 취약점 점검자에 의존하게 되어, 점검자에 따라 동일한 웹 사이트에 대해 점검 결과가 달라지는 문제가 발생한다. 본 논문에서는 이러한 문제(점검자에 따라 동일한 웹 사이트에 대해 점검 결과가 달라지는 문제)를 해결하기 위해 점검모델을 생성하여 점검대상화면을 선정(샘플링)하는 방법을 제시하고자 한다. 본 논문에서 제시하는 방법은 점검대상 웹 사이트의 개별 화면을 수행하는 기능에 따라 분류하고, 웹 사이트 특성에 따라 분류한 후 수행기능 등을 이용하여 점검모델을 생성한다. 생성된 점검모델을 통해 점검Case를 만들어 객관적인 점검대상화면 선정 기준으로 활용할 수 있도록 한다. 논문에서 제안한 방식으로 실제 웹사이트를 점검한 결과 점검대상 화면의 식별에서 점검자에 의한 편향이 발생하지 않는 것을 확인할 수 있었다.

        뇌졸중 환자의 몸통 안정화 훈련과 병행한 상지 접근이 상지 기능 및 직업 복귀에 미치는 영향

        Purpose The purpose of this study was to investigate the effect of upper limb function and satisfaction of work ability based on trunk stability training after stroke. Methods In this study, 25 hospitalized patients were subjected to upper limb approach combined with trunk stabilization training for 13 patients and general occupational therapy for 12 patients. In this study, the difference in satisfaction with upper limb function and satisfaction of work ability before and after intervention was evaluated using stroke Manual Function test (MFT) and Work Ability Index (WAI). Results There was a significant difference in the comparison of MFT and WAI before and after intervention in the experimental group (p< .05). In the control group, there was no significant difference between MFT and WAI (p >.05). After the intervention, there was a significant difference in WAI between the two groups (p<.05). Conclusion Through this study, upper limb approach combined with trunk stability training for stroke patients is thought to help improve satisfaction of work ability and upper limb function.

        보상작용 억제와 피드백을 제공한 가상현실 치료가 만성 뇌졸중 편마비 환자의 상지기능에 미치는 영향

        The purpose of this study was to investigate the effects of virtual reality (VR) therapy with compensation inhibition and feedback (CIF) on upper extremity function in chronic stroke patients. Seven chronic stroke patients participated in this study, which was a randomized controlled trial with a crossover design. Self upper extremity exercise, conservative VR therapy, and VR therapy with CIF were performed for one hour per session, 5 times per week, over a 3 week period. The main outcome measures involved range of motion (ROM) including shoulder, elbow, and wrist joints, a Manual Function Test (MFT), and a Motor Activity Log (MAL). Data were calculated as posttest and pretest changes in every session and were analyzed using Friedman and Wilcoxon signed-rank tests at p<.05. The results were as follows: 1) Statistically significant increase in ROM measurements of shoulder and elbow joints were seen with VR therapy with CIF compared to VR therapy and self upper extremity exercise (p<.05), whereas no significant increasing was noted for the wrist joint (p>.05). 2) Statistically significant increase in the MFT was seen with VR therapy with CIF compared with VR therapy and self upper extremity exercise (p<.05). 3) VR therapy with CIF also resulted in statistically significant increase in both activity of use (AOU) (p<.05) and quality of movement (QOM) (p<.05) on the MAL test when compared with VR therapy and self upper extremity exercise, respectively. In conclusion, VR therapy with CIF was more effective than conservative VR therapy and self upper extremity exercise in improving the upper extremity function in hemiplegic patients with chronic stroke.

        편마비 환자의 상지기능 평가에 관한 연구

        We assessed the function of the both hands and arms of the hemiplegic patients by MFT(Manua1 Function Test) and ADL score of the hemiplegic patients by FIM(Func- tional Independence Measure) and investigate its significance. The subjects of this study were 34 patients who had been hospitalized Rehabilitation Hospital, Yonsei University College of Medicine. The results are as follows ; 1. MFS(Manua1 Function Score) of both uninvolved upper limbs and involved upper lim- bs were significantly correlated with total FI M score. 2. The shoulder abduction, touch hand to the back , pegboard sub-items of MFT on the uninvolved upper limbs were significantly correlated with total FIM score. 3. The all sub-items of MFT on the involved upper limbs were significantly correlated with total FIM score. 4. The function of both uninvolved upper limbs and involved upper limbs by MFT were significantly correlated with self care, mobility, locomotion. According to the above results, MFT shows the functional status of the hemiplegic patients relatively well.

        뇌졸중 환자의 양측 상지기능과 일상생활 수행능력의 연관성

        The purpose this study was to investigate the relationship between Upper extremity's function and Activities of Daily Living(ADL) in stroke patients. The participants were 112 stroke patients who underwent occupational therapy. Data were analyzed using descriptive statistics, Pearson's correlation coefficient, and multiple linear regression analysis. The results are as foolows. MFT of both unaffected upper limbs and affected upper limbs were significantly correlated with total MBI score. The all area of MFT on the affected upper limbs were significantly correlated with sub-item of MBI. And finger manipulation area of MFT on the unaffected upper limbs were significantly correlated with sub-item of MBI. Significant factors influencing MBI, both unaffected upper limbs and affected upper limbs total score. Significant factors influencing sub-items of MBI, the function of affected upper limbs by MFT were MBI all sub items exculsive bowel, bladder control and the function of unaffected upper limbs by MFT were personal hygiene, bathing, feeding, toilet, bowel & bladder control, chair/bed transfer of MBI sub items. Above results show that ADL is highly correlated with affected upper limbs and unaffected upper limbs function in the stroke patients.

        Comparative Study of Upper Limb Function according to Sensory Feedback Training Combine with 1 Hz Low-Frequency Repetitive Transcranial Magnetic Stimulation in Patients with Stroke

        Purpose The purpose of this study was to evaluate the effect of upper limb function and hand dexterity on group with sensory feedback training after 1 Hz LF-rTMS and group with sensory feedback training after sham LF-rTMS of stroke patient. Methods This study was conducted from January to June 2021 on 24 hospitalized stroke patients who satisfied the selection criteria. Patients were classified into an experimental group to which sensory feedback training was applied after 1 Hz LF-rTMS and a control group to which sensory feedback training was applied after sham rTMS, and the intervention was applied 3 times a week, 40 minutes a day, for a total of 4 weeks. Before and after the intervention, upper extremity function was evaluated in MFT and hand dexterity was evaluated in BBT. Results Upper limb function before and after the intervention was statistically significant (p <.00) in the experimental group, but not statistically significant in the control group (p >.05). Hand dexterity before and after intervention was statistically significant (p <.01) in the experimental group, but not statistically significant in the control group (p>.05). There was no statistically significant difference in the upper limb function and hand dexterity between the intervention groups (p>.05). Conclusion The large mean change after sensory feedback training in the 1Hz LF-rTMS intervention group suggests that TMS-based sensory therapy is more effective than the sham TMS group in which only sensory training was performed.

        고빈도 반복 경두부 자기자극이 뇌졸중의 운동 기능에 미치는 영향

        Objective: To evaluate the effects of high frequency repetitive transcranial magnetic stimulation (rTMS) of the affected hemisphere on the motor recovery and spasticity in chronic post-stroke hemiplegic patients. Method: Thirteen chronic stroke hemiplegic patients were randomized to receive real and sham rTMS. rTMS was carried out 10 times at a frequency of 10 Hz with 10 s stimulation followed by 50 s rest, totalling 1,000 stimulations to the affected primary motor cortex using an intensity of 100% of resting motor threshold of unaffected hemisphere. Median nerve H-reflex, modified Ashworth scale (MAS) at elbow and wrist, and manual function test (MFT) were measured at baseline and after 2 weeks of treatment. Results: High frequency rTMS resulted in increased H- reflex latency and decreased H-reflex amplitude and H/M ratio. Also MAS decreased and MFT score increased after 2 weeks of treatment. Conclusion: High frequency rTMS in the affected motor cortex might facilitate motor recovery and reduce spasticity in chronic stroke patients. (J Korean Acad Rehab Med 2010; 34: 168-173)
