      • 보건의료조직에 있어서 CRM 마케팅과 QI 활동의 적용에 관한 연구

        temp CRM의 장점은 원하는 고객에 대해서 마케팅 노력을 기울일 수 있고 그 결과로 마케팅 비용을 줄이고 효율을 높일 수 있는 것이다. 그리고 마케팅 전략 수립에 대한 방법의 변화와 그 수행방법의 변화가 CRM에 있기 때문에 여기에 따른 절차와 문화, 조직구조까지 변화 시킬 수 있다. 이를 방지하기 위하여 CRM교육이 필요하며, 그 중요성을 인식하고 실행하여야 한다. 그리고 경영환경에 적합한 마케팅 전략 수립이 필수적이다. CRM의 정확한 이해와 적합한 수행이 있어야만 병원경영의 목표달성에 도움이 된다. 그러나 CRM자체의 문제가 없는 것도 아니다. 무엇보다 전문 인력이 부족하면 시스템 구축과 마케팅이나 컨설팅 인력을 확보하여 프로젝트의 부실화를 방지하여야 한다. 다음은 실업자들의 업무패턴을 파악하고 사용자의 편의를 고려한 적용이 있어야 한다. 고가의 비용을 투입하여 CRM을 실행하였다 해도 사용자 교육의 미비가 생기면 효율이 감소될 것이다. 역시 CRM의 적용에 있어서 호환이 되지 않으면 통합비용으로 별도의 비용이 지출해야 하므로 비용의 증가를 감수하여야 한다. 환자의 의료의 질에 대한 만족은 환자의 주관적 개념으로 동일한 의료서비스에 대하여 환자들은 저마다 다르게 의료의 질을 평가하게 된다. 대부분의 환자들은 자신의 욕구와 의료의 질에 대한 기대를 가지고 병원을 방문하여, 그것을 의료제공자가 충족시키는 정도가 환자의 만족도를 결정하게 된다. 의료의 질은 의료서비스에 대한 만족과 밀접한 관련이 있다. 의료의 질과 만족은 강하게 연관이 있으면서도 양자는 분명히 다르다. 의료의 질에 대한 평가와 만족도는 매우 일치도가 높다고 볼 수 있다. 이것은 의료의 질적 수준이 높으면 환자만족도도 높다는 것을 의미한다. 따라서 본 연구는 병원의 환자관리를 위한 CRM 마케팅의 전개를 실증적으로 역설하고 이의 도입을 통한 효과와 문제점, 이에 대한 대응방안을 제시하고, 모범적인 CRM운영 사례를 제시함과 동시에 효과적인 의료의 질 경영을 구현하고 이것이 환자의 의료서비스 만족도를 높이는 결과인 것을 모색하는 것이 본 연구의 목적이다.

        민영의료보험이 의료이용에 미치는 영향 : 국내 실증적 연구의 고찰

        The arguments exist that private health insurance(PHI) policy holders tend to use the health care services more than non-policy holders due to their little out-of-pocket spending, resulting in the adverse effects on the finances of National Health Insurance. This study aims to increase the objective understanding of the issue and to draw a direction of further research, by reviewing the articles, reports and statistics which examined the effects of purchasing PHI policies on health care utilization. Significant differences in healthcare utilization, except for the very partial increase of utilization in outpatient settings, have been not found. The similar trends of the results have existed in a few previous studies which tried to control the endogeneity of medical use and health insurance with latent variables which affect the decision on medical use and health insurance. However, we can not exclude the potential change of healthcare utilization patterns because the portion of the insured of indemnity PHI is becoming rapidly larger in the market. For further research, we should try to obtain the objective information of subjects' past medical history, health status, health related behavior, and income affecting purchase of PHI and utilization of healthcare services. And the efforts of controlling the endogeneity of medical use and health insurance with latent variables which affect the decision on medical use and health insurance, are very considerable.

        노년기에 따른 미충족 의료 현황 및 관련 요인

        This study investigated the prevalence of unmet needs for health care among Korean adults and related factors. Using data from the 2012 Korea Health Panel, 1,896 adults aged 65 and over and two age groups(Young-Old(66-74) and Old-Old(65≤)) were analyzed to identify these factors. Logistic regression analysis was used to examine the main factors associated with unmet medical needs. According to the results of this study, the factors influencing unmet medical needs with regard to economic factors were Subjective Health Status(young-old), Disability(young-old), Activity Limitation(young-old), Education Level(old) and Economic Activity(old). The factors influencing unmet medical needs were factors of attitude, information, and disease. factors were Disability(young-old), Activity Limitation(young-old), Education Level(old) and Economic Activity(old). Therefore, further research that investigates unmet needs depending on age group in the elderly would suggest helpful policy implications.

        의료서비스경험자의 보건의료제도 인식도와 영향 요인

        Objectives : This study was performed to investigate health care system recognition and influential factors using the data from the “2017 Health Care Experience Survey”. Methods : Data on 7,000 participants in the Health Care Experience Survey were drawn and statistically examined using a t-test, ANOVA, and multiple regression analysis. Results : First, the significant factors of health care service satisfaction were education, income, region, chronic diseases, unmet medical needs, satisfaction with doctors and institutions, and the health care system's reliability and importance. Second, the influential factors of willingness to pay additional health insurance premium were age, occupation, income, health status, chronic diseases, unmet medical needs, satisfaction with health care institutions, limit to utilization of medical services, necessity of health care reform, and the health care system's reliability, satisfaction, importance. Conclusions : Since the additional burden for improving the health care has been negative to the socially disadvantaged, there should be efforts to provide stable health care funding for financial stability of the health insurances by considering public opinions and reaching social consensus.

        의료서비스경험에 따른 외래 의료서비스 만족도와 영향 요인

        Objectives: This study investigated outpatient health care satisfaction and influential factors using data from the “2018 Health Care Experience Survey”. Methods: Data on 6,705 participants in the Health Care Experience Survey were statistically examined using a t-test, ANOVA, and multiple regression analysis. Results: First, the level of satisfaction among outpatients was generally positive, and satisfaction among older people was high. Second, the significant factors of outpatient overall satisfaction were gender, consultation time, waiting time, reason for choosing institution, satisfaction with doctors, nurses, institutions, and the therapeutic result. Third, influential factors of intention to recommend were education, institution type, reason for choosing institution, satisfaction with doctors, nurses, institutions, and the therapeutic result. Conclusions: In order to improve the overall satisfaction of outpatients, the treatment method should be improved. For example, the doctor should devote more time for and engage in sufficient conversation with the patient, the nurse should be polite, and patients should be given easy-to-understand explanations.

        민간의료보험이 의료 이용에 미치는 영향 : 한국의료패널(2008-2010)을 이용한 Two-Stage 분석

        This paper examined the effects of private health insurance(PHI) on the health care utilization among the Korean. The used data was the three waves of Korea Health Panel (2008, 2009, 2010), and the number of subjects was 13,951 persons. Authors employed two-stage least square panel model where the instrument variables for controlling for endogeneity of PHI were number of insurance planner per 100,000 in resident area and whether subject worked on financial profession. The results showed that healthcare expenditure of outpatients who purchasing PHI was higher than that of outpatients without PHI, and there was no difference in admission between the two groups. This article recommended the Korean government to monitor the effects of PHI on the health care utilization in order to improve the efficiency of health care finance.

        텍스트마이닝을 활용한 보건의료산업학회지의 토픽 모델링 및 토픽트렌드 분석

        Objectives : The purpose of this study was to analyze representative topics and topic trends of papers in Korean Society and Health Service Management(KSHSM) Journal. Methods : We collected English abstracts and key words of 516 papers in KSHSM Journal from 2007 to 2017. We utilized Python web scraping programs for collecting the papers from Korea Citation Index web site, and RStudio software for topic analysis based on latent Dirichlet allocation algorithm. Results : 9 topics were decided as the best number of topics by perplexity analysis and the resultant 9 topics for all the papers were extracted using Gibbs sampling method. We could refine 9 topics to 5 topics by deep consideration of meanings of each topics and analysis of intertopic distance map. In topic trends analysis from 2007 to 2017, we could verify ‘Health Management’ and ‘Hospital Service’ were two representative topics, and ‘Hospital Service’ was prevalent topic by 2011, but the ratio of the two topics became to be similar from 2012. Conclusions : We discovered 5 topics were the best number of topics and the topic trends reflected the main issues of KSHSM Journal, such as name revision of the society in 2012.

        보건의료산업학회지 게재논문(2007-2018년)의 연구동향 분석 : 치위생분야를 중심으로

        Objectives: The study’s purpose was to analyze the research trends and future directions of dental hygiene papers. Methods: In order to search for papers for this study, a survey was conducted using the Korean Journal of Health Service Management publications listed on The Korean Society of Health Service Management’s website. Results: A total of 76 articles were published in the dental hygiene study. Its main trend was having oral health problems with 32 episodes. The most common word used in the Journal of Health Industry for the past 12 years is “health”. Conclusions: Considering the results of the analysis of the Korean Journal of Health Service Management, the development of convergence research in health care and dental hygiene should be explored.
