Eagle's syndrome is the term given to symtomatic elogation of the styloid process or mineralization of the stylohyoid or stylomandibular ligament. Since ossification of stylohyoid ligament at cadeva was fist described by De Manchetics in 1652 and Weinlecher described clinical symptom which produced by elongated styloid process and osteotomy of styloid process in 1872, Clinical symptom which include sensation of a foreign body on the pharynx, dysphagai, dysphonia, referred pain, and mouth opening disturbance was termed by Eagle as Eagle's syndrome. then, case reports of Eagle's syndrome are presented. in these cases, the patient's chief complaints included periauricular radiating pain, mouth opening disturbance, foreign body sensation, dysphagia, tenderness on the neck. Through vairous X-ray examination and palpation of tonsillar fossa, elongted styloid process were confirmed. Under the general anesthesia there were successfully removed out via transoral approach technique, described by Eagle. after resented styloid process, they were freed from the symptoms without further complication. Therefor we reported these cases treated by surgical resection of styloid process with good results.

Background: Macrophage migration inhibitory factor (MIF) is a proinflammatory cytokine and significantly elevated in patients with AD. The MIF gene polymorphisms such as -173 G to C and -794 CATT5-8 MIF have been implicated in AD. Objectives: The aim of this study is to evaluate the relationship between -173 G to C and -794 CATT5-8 MIFgene polymorphisms and total plasma immunoglobulin E (IgE) level in AD patients. Methods: We performed polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) analysis from 178 patients with AD and 80 controls. The total plasma MIF level was determined by ELISA. Logistic regression analyses were performed. Results: Compared with GG homozygotes of MIF-173G/C polymorphism, odds ratio (OR) for CC homozygotes was 9.3 (p=0.048), and OR for GC heterozygotes was 2.5 (p=0.031). Compared with MIF G/5-CATT haplotype, OR for MIF C/5-CATT haplotype was 9.7 (p=0.009), and OR for MIF C/7-CATT haplotype was 4.5 (p=0.005). The IgE level was highly associated with age and sex among AD patients with the MIF -794 7-CATT polymorphism (p=0.043), while MIF C/7-CATT haplotype was associated with decreased IgE level (p=0.036). Conclusion: Our results shows that the MIF promoter polymorphisms in -173 C allele and MIF C/5-CATT and MIF C/7-CATT haplotype are associated with increased risk for AD. In contrast, the -794 7-CATT locus and MIF C/7-CATT haplotype was associated with decreased total plasma IgE level in AD patients.
Cutaneous malignant mesothelioma

Malignant mesothelioma is a rare malignant tumor that arises from pleural, pericardial or peritoneal mesenchymal cells. Though extremely rare, cutaneous mesothelioma mayoccur through regional spread, distant metastasis, or direct extension within surgical scars. Clinically, cutaneous lesions present as violaceous papules, nodules, or plaques. Histologically, mesothelioma are divided into epithelial (60%), sarcomatoid (15%), and mixed types (25%). These histological features may simulate metastatic adenocarcinoma, a primary adnexal neoplasm or angiosarcoma. Immunohistochemistry can aid in diagnosis, as the malignancy yields positive staining for calretinin, WT-1, Cytokeratin 5/6, mesothelin, and podoplanin. A 66-year-old man presented with asymptomatic, solitary, palpable, skin-colored, subcutaneous mass on the Rt. flank detected 3 days ago. He had been previously diagnosed with mesothelioma of the Rt. chest wall 3 years prior and undergone mass excision along with adjuvant chemotherapy. A follow up abdomen CT showed a small nodule on the right abdominal wall, which was referred to our department for biopsy. Histopathological findings were remarkable for neoplastic, epithelial cells exhibiting irregular, tubular and papillary growth pattern in the dermis. The cells were positive for calretinin, cytokeratin 5/6, and wt-1 and negative for MOC-31. This to our knowledge is the first report of cutaneous malignant mesothelioma in Korea.

Acquired generalized lipodystrophy is a rare disease in which a significant amount of adipose tissue is lost from a relatively large region of the body. The disease typically manifests during childhood and adolescence. Metabolic abnormalities, including insulin resistance, diabetes mellitus and hypertriglyceridemia develops in the early course of the disease. A 26-year-old woman with a 12 year history of type 2 diabetes mellitus and hyperlipidemia visited our outpatient clinic with a nonhealing diabetic foot ulcer. She was noticed to have fat loss from the face at 11 years of age. Over the next few years, it progressed to involve the trunk and extremities. Her hair became curly, and she developed diabetes mellitus, hypertriglyceridemia, and hepatomegaly at 14 years of age, and was placed on insulin therapy. On physical examination, her height was 1.60 m and weight was 30.0 kg. Her appearance was muscular with marked loss of fat from the face, trunk, and extremities. Her mother and younger sister had the same appearance. Diagnosis of lipodystrophy was made. Herein, we report a rare case of acquired generalized lipodystrophy in a 26-year-old female.

Background: Balneotherapy is widely used for the management of inflammatory skin diseases, but the mechanism is partly understood. Korea has abundant resources of hot spring(HS) and among them are sea salt type of Na(Ca)-Cl. We previously reported that different HS water affected the secretion of proinflammatory cytokines from HaCaT cells. Objectives: We evaluated effect on proinflammatory cytokine secretion in primary keratinocytes(KCs) of three different sea salt type HS water, i.e., Haeundae, Magumsan, and Seokmodo HS area, selected by salt concentration gradient. Methods: To know whether primary KCs express Toll-like receptors(TLRs), we isolated mRNA from KCs and evaluated the expression of TLR1 to 10 by RT-PCR. Next, we investigated the effect of three different HS waters on theexpression of cytokines(IL-8, TNF-α, IL-1β, IL-6, and GM-CSF) in the primary KCs under TLR1, 2, 3, 5, and TLR6 stimulation by ELISA. Results: The treatment of HS waters induced suppression of proinflammatory cytokines under TLR stimulation in the primary KCs. Cytokine secretion from primary human KCs to TLR3 ligand was the most prominent. Decrease of IL-8, TNF-α, IL-1β, IL-6, or GM-CSF secretion under TLR stimulation were observed to all HS waters. Conclusion: Three different sea salt type HS water treatment affected the secretion of proinflammatory cytokine production from KCs under TLR stimulation. These parameters and protocols can be used to evaluate anti-inflammatory effect of sea salt type HS water.
