      • Angioplasty로 치료한 Spontaneous brachial artery thromboembolism

        Acute arterial ischemia due to an embolism is associated with a high mortality rate and poor prognosis. Percutaneoustransluminal angioplasty (PTA) was first described by Dotter and Jukins in 1964 and subsequently modified by Gruentzig and Hoff in 1974. PTA has proved a safe and effective treatment for focal atherosclerotic disease of the aorta and its major extremity braches. In this case, we had experienced a case of spontaneous brachial arterial occlusion disease, successfully treated with percutaneous angioplasty. A 77-yr-old woman experienced a severe right arm pain and tingling sensation with cyanosis. Angiographic examination demonstrated a total radial arterial occlusion and complete obstruction. Here, we report a case with good result, which were treated with PTA with review of the literature.

        Outcome of neonatal palliative procedure for pulmonary atresia with ventricular septal defect or tetralogy of Fallot with severe pulmonary stenosis: experience in a single tertiary center

        Purpose: The present study aimed to evaluate progression and prognosis according to the palliation method used in neonates and early infants aged 3 months or younger who were diagnosed with pulmonary atresia with ventricular septal defect (PA VSD) or tetralogy of Fallot (TOF) with severe pulmonary stenosis (PS) in a single tertiary hospital over a period of 12 years. Methods: Twenty with PA VSD and 9 with TOF and severe PS needed initial palliation. Reintervention after initial palliation, complete repair, and progress were reviewed retrospectively. Results: Among 29 patients, 14 patients underwent right ventricle to pulmonary artery (RV-PA) connection, 11 palliative BT shunt, 2 central shunt, and 2 ductal stent insertion. Median age at the initial palliation was 13 days (1-98 days). Additional procedure for pulmonary blood flow was required in 5 patients; 4 additional BT shunt operations and 1 RV-PA connection. There were 2 early deaths among patients with RV-PA connection, one from RV failure and the other from severe infection. Finally, 25 patients (86%) had a complete repair. Median age of total correction was 12 months (range, 2-31 months). At last follow-up, 2 patients had required reintervention after total correction; 1 conduit replacement and 1 right ventricular outflow tract (RVOT) patch enlargements. Conclusion: For initial palliation of patients with PA VSD or TOF with severe PS, not only shunt operation but also RV-PA connection approach can provide an acceptable outcome. To select the most proper surgical strategy, we recommend thorough evaluation of cardiac anomalies such as RVOT and PA morphologies and consideration of the patient's condition.

        This study is a development of 245kV 40kA GIS(Gas Insulated Switchgear) with motor-drive mechanism. The Ultra high voltage GIS applicates an interception technology of hybrid arc extinguishing method and Motor-Direct-Drive-Mechanism for operation type. We will develope a compact GIS for construction of an intelligent substation by an applicated IT technology.

      • Platelet-to-lymphocyte ratio is related with the severity of OSA syndrome and hypertension

        Background: Chronic intermittent hypoxia, platelet activation and inflammation are the related mechanisms of pathogenesis in obstructive sleep apnea syndrome (OSAS), which may link to the risk of cardiovascular disease (CVD). The aim of this study was to evaluate the relationship of platelet- to-lymphocyte ratio (PLR), a new biomarker showing systemic inflammation, and platelet distribution width (PDW), an indicator of platelet activation, with the severity of OSAS assessed by polysomnographic parameters. Methods: This was a cohort study in which patients who had undergone a full night polysomnography for diagnosis of OSAS were recruited. According to apnea-hypopnea index (AHI) scores, individuals inwhom AHI was less than 5 were recruited as the control group, those in whom 5-15 as the mild OSAS group, those in whom 16-30 as the moderate OSAS group, and those in whom AHI was greater than 30 as the severe OSAS group. The presence of hypertension, diabetes mellitus, current smoking, and dyslipidemia were defined as CVD risk. Results: A total of 290 patients were involved in the study. There was significant linear increase of age, body mass index (BMI), systolic blood pressure and male proportion according to the higher AHI groups. PLR and PDW were also significantly different among the groups, and were significantly associated with AHI (r=0.417 for PLR and r=0.227 for PDW, all p’s<0.001) and Epworth sleepiness scale (ESS) (r=0.160 for PLR and r=0.189 for PDW, all p’s<0.05). Multivariate regression analysis showed that high AHI (≥9.2) (adjusted odds ratios [OR] 5.03, 95% confidential interval (CI) 1.67-15.2, p=0.004) and PLR≥159 (adjusted OR 2.81, 95% CI 1.34-5.91, p=0.006) were significant independent predictors of the hypertension. Conclusions: PLR and PDW were related with the severity of OSAS. Moreover, PLR might be useful as a systemic inflammation biomarker to predict hypertension in OSAS patients.       • Energy Efficiency Analysis of Internally and Externally Insulated Apartment Buildings

        <P>Apartment buildings are the most common type of residential buildings in Korea, where hot water heating pipes are installed in the floor structures. An internal insulation system (IIS) has been applied to the outside walls of most Korean apartment buildings, so there are many cases in which the layer of insulation is disconnected by structural components at the wall-slab joints. These joints become thermal bridges where heat transfer increases. An external insulation and finish system (EIFS) is a possible solution to this problem. In this study, the surface temperature distributions of actual apartment buildings with an IIS and an EIFS were investigated with an infrared thermal imaging camera. Annual heat losses and gains through the wall-slab joints with an IIS and an EIFS were calculated by three-dimensional transient heat transfer simulations in which hot water heating was excluded and included, respectively, in order to evaluate not only the heating and cooling loads, but also the heating efficiency when the building is actually heated. The results show that the amount of heat transfer through the wall-slab joints with an IIS is considerable, even though additional insulation is installed under the slab, and the EIFS significantly reduces the heating and cooling loads and improves the heating efficiency.</P>

        Self-blast circuit breakers utilize the energy dissipated by the arc itself to create the required conditions for arc quenching during the current zero. The high-current simulation provides information about the mixing process of the hot PTFE cloud with SF? gas which is difficult to access for measurement. But it is also hard to simulate flow phenomenon because the flow in interrupter with high current, SF?-PTFE mixture vapor and complex physical behavior including radiation, calculation of electric field. Using a commercial computational fluid dynamics(CFD) package, the conservation equation for the gas and temperature, velocity and electric fields within breaker can be solved. Results show good agreement between the predicted and measured pressure rise in the thermal chamber.

        GSK3B induces autophagy by phosphorylating ULK1

        Unc-51-like autophagy activating kinase 1 (ULK1), a mammalian homolog of the yeast kinase Atg1, has an essential role in autophagy induction. In nutrient and growth factor signaling, ULK1 activity is regulated by various posttranslational modifications, including phosphorylation, acetylation, and ubiquitination. We previously identified glycogen synthase kinase 3 beta (GSK3B) as an upstream regulator of insulin withdrawal-induced autophagy in adult hippocampal neural stem cells. Here, we report that following insulin withdrawal, GSK3B directly interacted with and activated ULK1 via phosphorylation of S405 and S415 within the GABARAP-interacting region. Phosphorylation of these residues facilitated the interaction of ULK1 with MAP1LC3B and GABARAPL1, while phosphorylation-defective mutants of ULK1 failed to do so and could not induce autophagy flux. Furthermore, high phosphorylation levels of ULK1 at S405 and S415 were observed in human pancreatic cancer cell lines, all of which are known to exhibit high levels of autophagy. Our results reveal the importance of GSK3B-mediated phosphorylation for ULK1 regulation and autophagy induction and potentially for tumorigenesis.

        갑상선 유두암에서 BABA 내시경 갑상선 절제술에 대한 임상적 고찰

        Purpose: The aim of this study was to evaluate the operative feasibility and safety of endoscopic thyroidectomy via bilateral axillo breast approach (BABA) compared to conventional thyroidectomy in papillary thyroid carcinoma (PTC) patients. Methods: From July 2009 to November 2010, patients underwent BABA endoscopic thyroidectomy (ET group; n=41) or conventional open thyroidectomy (OT group; n=61) for PTC. Clinical and pathologic characteristics of patients, operation time, post-operative complications, cosmetic satisfaction and thyroglobulin (TG) level were analyzed retrospectively. Results: The mean age of the patients was 40.05±9.58 years (range 25∼61 years) and 46.21±13.68 years (range 19∼79 years) for the ET and OT group, respectively. The operative extent in the ET group did not include advanced thyroid cancer or lateral neck dissection. The size of the tumor was 0.78±0.59 cm (range 0.1∼3.00 cm) and 1.54 ±1.05 cm (range 0.3∼6.00 cm) for the ET and OT group, respectively. Extrathyroidal extension and number of retrieved lymph nodes were significantly higher in the OT group. Postoperative radioactive iodine ablation was performed on 25 patients (72.43%) in the ET group and 48 patients (78.69%) in the OT group. There was no abnormal uptake on radioactive iodine scans in the iodine-treated patients and no significant differences in postoperative off-T4 TG levels between the two groups. There were no significant differences in operative time, amount of drainage, postoperative hospitalization period, hypocalcemia, and vocal cord palsy between the two groups. Cosmetic results of ET group were rated as excellent in a 3-month postoperative questionnaire by 25 (72.43%) of 35 patients. Conclusion: Endoscopic thyroidectomy via the bilateral axillo breast approach can be a feasible and effective option for PTC in selected cases. (Korean J Endocrine Surg 2011;11:158-163)

        추출 방법에 따른 자소엽 추출물의 항산화 효과 비교

        자소엽(perilla frutescens)은 꿀풀과(Labiatae)에 속하며 널리 알려져 있는 약용 식물이다. 본 연구에서는 자소엽을 물, 열수, 초음파 추출 방법으로 추출하여 항산화 효과를 비교하고 가장 효과적인 추출방법을 제시하고자 한다. 물, 열수, 초음파 처리를 통해 각각의 자소엽 추출물을 제조 하였고, DPPH 라디칼 소거능 및 총 페놀 함량을 통해 항산화 효과를 검증하고, 인간 간세포인 HepG2에 대한 세포 독성효과와 hydrogen peroxide(H2O2)로 유도된 산화적 스트레스로부터 간세포 보호효과를3-[4,5-dimethylthiazole-2-yl]-2,5-diphenyltetrazolium bromide(MTT) assay로 확인하였다. 자소엽 초음파 추출물은 5000 ㎍/mL농도에서 69.07%의 DPPH 라디칼 소거능을 나타내며 물, 열수 추출물과 비교하여 우수한 항산화 효과를 나타내었다. 또한 총 페놀 함량 측정 결과 51.60±1.06 mg GAE/g extract 로서 물, 열수 추출물 보다 높은 총 페놀 함량을 확인하였다. 그러나 산화적 스트레스에 의한 세포 보호효과는 미비하였다. 본 연구를 통해 추출 방법에 따른 항산화 효과의 차이를 확인하였으며, 우수한 항산화 효과를 나타낸 자소엽 초음파 추출물을 이용하여 추가적인 연구가 필요 할 것으로 사료된다. Perilla frutescens (P. frutescens) is one of evergreen shrubs belonging to the Labiatae and is grown wildly in Korea. This study was carried out to evaluate the anti-oxidative effects of Perilla frutescens Extracts by Extraction Methods (water, heating and sonication). Anti-oxidative effects were measured using 1,1-diphenyl-2-picrylhydrazyl (DPPH) radical scavenging activity and total phenol content. Cell viability and hepatoprotective effects were identified by 3-[4, 5-dimethylthiazol-2-yl]-2, 5-diphenyltetrazolium bromide (MTT) assay. Among various extracts, P. frutescens extracts by sonication showed the highest DPPH radical scavenging activity at 5000 ㎍/mL. Total phenolic content in P. frutescens extracts by sonication was 51.60±1.06 mg GAE/g extract. However, P. frutescens extracts did not show hepatoprotective effects. This study identified anti-oxidative effects of P. frutescens extracts by sonication, and it would be necessary to perform further studies of P. frutescens extracts by sonication.
