Surgical Treatment for Papillary Thyroid Carcinoma in Japan: Differences from Other Countries

Papillary thyroid carcinoma (PTC) is the most representative carcinoma among thyroid malignancies. The treatment strategy, especially surgery, in Japan traditionally differs from that in other countries, including Korea. Total thyroidectomy has been less frequently adopted, but lymph node dissection has been more actively performed in Japan than in other countries. Based on our data, total thyroidectomy is not necessary for low-risk patients, while it is, of course, mandatory for patients with high-risk features. Prophylactic central node dissection may not prolong patients’ survival, but we routinely perform it because reoperation for recurrence to this compartment is troublesome. In the past, Japanese endocrine surgeons actively performed prophylactic lateral node dissection, but indications are narrowing. However, it may be better to perform prophylactic modified radical neck dissection for patients exhibiting certain characteristics to reduce the rate of lymph node recurrence. I hope that surgical strategies in Japan and other countries will fuse with each other in order to identify the best treatments for PTC patients throughout the world.

This study aimed to profile the microflora in infected root canals before and after root canal treatment using cultureindependent methods. Six infected root canals in singlerooted teeth with periapical lesions from five subjects were included. Quantification of total bacteria was performed by real-time PCR with primers targeting 16S rRNA genes. PCR products with universal 16S rRNA gene primers were cloned and partially sequenced, and bacterial identification at the species level was performed by comparative analysis with the GenBank database. The concentration of extracted DNA before treatment was higher than that after root canal treatment, although the difference was not statistically significant. Sequence analysis revealed that oral bacteria such as Fusobacterium, Streptococcus, Olsenella, and Pseudoramibacter detected in cases before root canal treatment disappeared after treatment. These results suggest that the root canal microflora are distinct before and after root canal treatment, and that treatment changes the microflora in both quantity and quality.
Model-Based Software Validation for Automotive Control Systems

Virtual hardware-in-the-loop simulation (VHILS) simulation was proposed to validate real-time control software. The VHILS simulator integrated entire mechatronic control systems and dealt with multiple technology domains of mechanical, electronic, control and software. A processor simulator ran control software codes in binary format and analyzed accurately their real-time behavior. The VHILS concept was applied to an automotive adaptive cruise control system (ACCS), and driver maneuvering, vehicle dynamics, micro controller operation and CAN communication were modeled. The modeling efficiency of the serial data communication and the data exchange between the CAN model and the multi-domain simulation were identified as the primarily causes of longer computational time. By carefully designing the simulator interface and the CAN model, we successfully built a VHILS for the ACCS that agreed accurately with experimental results and yielded a practical turn-around time.

Purpose: To evaluate the effects of phosphodiesterase type 5 inhibitor (PDE5i) on urinary continence recovery after bilateral nerve-sparing radical prostatectomy (BNSRP). Materials and Methods: Between 2002 and 2012, 137 of 154 consecutive patients who underwent BNSRP in our institution retrospectively divided into 3 groups that included patients taking PDE5i immediately after surgery (immediate PDE5i group, n=41), patients starting PDE5i at an outpatient clinic after discharge (PDE5i group, n=56), and patients taking no medication (non-PDE5i group, n=40). Using self-administered questionnaires, the proportion of patients who did not require incontinence pads (pad-free patients) was calculated preoperatively and at 1, 3, 6, 12, 18, and 24 months after BNSRP. Severity of incontinence was determined based on the pad numbers and then compared among the 3 groups. Results: Proportions of pad-free patients and severity of incontinence initially deteriorated in all of the groups to the lowest values soon after undergoing BNSRP, with gradual improvement noted thereafter. The deterioration was most prominent in the immediate PDE5i group. As compared to the non-PDE5i group, both the PDE5i and immediate PDE5i groups exhibited a better final continence status. Conclusions: PDE5i improves final continence status. However, administration of PDE5i immediately after surgery causes a distinct temporary deterioration in urinary incontinence.

Study Design: Controlled laboratory study. Purpose: This study aimed to evaluate the efficacy of platelet-rich plasma (PRP) stored at room temperature (RT), frozen, or after freeze-drying. Overview of Literature: PRP enriches tissue repair and regeneration, and is a novel treatment option for musculoskeletal pathologies. However, whether biological activity is preserved during PRP storage remains uncertain. Methods: PRP was prepared from blood of 12 healthy human volunteers (200 mL/person) and stored using three methods: PRP was stored at RT with shaking, PRP was frozen and stored at –80°C, or PRP was freeze-dried and stored at RT. Platelet counts and growth factor content were examined immediately after preparation, as well as 2, 4, and 8 weeks after storage. Platelet activation rate was quantified by flow cytometry. Results: Platelet counts were impossible to determine in many RT samples after 2 weeks, but they remained at constant levels in frozen and freeze-dried samples, even after 8 weeks of storage. Flow cytometry showed approximately 80% activation of the platelets regardless of storage conditions. Almost no growth factors were detected in the RT samples after 8 weeks, while low but significant expression was detected in the frozen and freeze-dried PRP. Over time, the mean relative concentrations of various growth factors decreased significantly or disappeared in the RT group. In the frozen group, levels were maintained for 4 weeks, but decreased significantly by 8 weeks (p <0.05). The freeze-dried group maintained baseline levels of growth factors for the entire 8-week duration. Conclusions: Freeze-drying enables PRP storage while maintaining bioactivity and efficacy for extended periods.

Japan has started a project of Quasi-Zenith Satellite System (QZSS) since FY2003. QZSS will be highly useful not only for communications but for complement of the modernized GPS in urban canyon and mountainous area with its high visibility brought out by its inclined orbits. National Institute of Information and Communications Technology, Japan (NICT) is to develop a Space-borne Hydrogen Maser (SHM) and time management system, and carry out experiments using them. A breadboard model (BBM) of the SHM was developed to overcome the issues such as size, weight and life time, which are required in space environment. Basic design was carried out for time management system to achieve nano second level time transfer. The design and the manufacturing of the engineering models (EM) of the on-board equipment have been carried out since 2004. The first satellite is planned to be launched in FY 2008.

Sc2C2@C82(II) and Sc2C2@C82(III) have been synthesized and chromatographically isolated and characterized by using UV-Vis-NIR absorption spectroscopy. Solid-state 13C and 45Sc NMR on Sc2C2@C82 metallofullerenes provide the rotational activation energy of Sc atoms encapsulated in C82 fullerenes which is lower than that in solution due to crystal field effects on the rate of the rotation.

Machining of steel-based alloy (STAVAX) and tungsten carbide (WC) is very challenging due to their superior hardness. Severe wear in the machining of the die materials is the dominant problem. Texturing on the rake face of the tool and ultrasonic elliptical vibration cutting (UEVC) may resolve this issue and could prolong tool life. The patterns on the rake face of the tool entrap the chip and debris created in machining and prevent abrasion. During UEVC the flank face of the tool does not contact the workpiece and the rake face of the tool helps the chip evacuate from the machining area when the tool disengages with the workpiece. Thus, UEVC reduces cutting force and tool wear compared to conventional cutting. This paper investigates the cutting performance and tool wear in machining of the die materials with textured polycrystalline diamond (PCD) tools under UEVC. The linear patterns parallel or perpendicular to the chip flow are engraved on the rake face of the PCD tools with the focus ion beam (FIB) process. Orthogonal cutting experiments are carried out on ultra-precision machine tools with and without UEVC. Machining under UEVC remarkably reduces the cutting and thrust force and improves tool life. For machining of STAVAX with the textured PCD tool, the linear patterns alone do not improve the cutting, but they improve the cutting when they are combined with UEVC. In machining of WC, the linear pattern becomes effective by slightly decreasing the thrust force and friction coefficient when UEVC is not applied.
Negative Muon Capture on Nitrogen Oxide Molecules

The characteristic muonic X-ray measuring system for low pressure gas sample was constructed to investigate the initial process of muonic atom formation. Low background characteristic muonic X-ray spectra were obtained for neon (1.0 bar) and nitrogen mono-oxide samples (0.99 bar) by muon irradiation with 19 MeV/c. The deviation of characteristic muonic X-ray intensity patterns both for muonic nitrogen and oxygen atoms in nitrogen mono-oxide sample between our low pressure experiment and the previous high pressure experiment was found. Muon capture probability was also determined and compared with the previous work and empirical estimations.
