      • Effect of gel spinning accompanied with cross-linking using boric acid on the structure and properties of high-molecular-weight poly(vinyl alcohol) fiber

        <P>To enhance the thermal stability of poly(vinyl alcohol) (PVA) fiber, the fiber was prepared from the gel spinning of high molecular weight (HMW) PVA by using dimethyl sulfoxide/water (8/2, v/v) as a solvent, accompanied with the cross-link by boric acid (B-PVA). In addition, the structure and properties of the B-PVA fiber were compared with those of the HMW PVA fiber obtained by using the same spinning system without cross-linking (NB-PVA). Through a series of experiments, it turned out that cross-linking actualized by an optimum amount of boric acid (0.3 wt % based on PVA) and zone drawing caused significant changes in the properties of HMW PVA gel fiber. That is, cross-linking increased thermal degradation temperatures at each degradation step and amounts of final residues, resulting in improving thermal properties of the PVA fiber. On the contrary, it was found that in the case of the B-PVA fiber, some broadening of the original PVA unit cell occurred, which was identified by the peak shift to lower angle in X-ray diffractogram.         우리나라 노인여성의 골다공증성 척추골절로 인한 경제적 부담 추계

        Objectives : To estimate the economic burden of osteoporotic vertebral fracture (VF) from a societal perspective. Methods : From 2002 to 2004, we identified all National Health Insurance claims records for women ${\geq}50$ years old with a diagnosis of VF. The first 6-months was defined as a "clearance period," such that patients were considered as incident cases if their first claim of fracture was recorded after June 30, 2002. We only included patients with ${\geq}$ one claim of a diagnosis of, or prescription for, osteoporosis over 3 years. For each patient, we cumulated the claims amount for the first visit and for the follow-up treatments for 1 year. The hospital charge data from 4 hospitals were investigated to measure the proportion of the non-covered services. Face-to-face interviews were conducted with 106 patients from the 4 study sites to measure the out-of-pocket spending outside of hospitals. Results : During 2.5 years, 131,453 VF patients were identified. The patients had an average of 3.38 visits, 0.40 admissions and 6.36 inpatient days. The per capita cost was 1,909,690 Won: 71.5% for direct medical costs, 20.6% for direct non-medical costs and 7.9% for indirect costs. The per capita cost increased with increasing age: 1,848,078 Won for those aged 50-64, 2,084,846 Won for 65-74, 2,129,530 Won for 75-84and 2,121,492 Won for those above 84. Conclusions : Exploring the economic burden of osteoporotic VF is expected to motivate to adopt effective treatment options for osteoporosis in order to prevent the incidence of fracture and the consequent costs.

      • Multiple occurrence of recurrent nodular fasciitis after surgical extirpation: masquarding as in-transit metastasis of sarcomatous presentation

        Nodular fasciitis (NF) is a rare benign fibrous tumor related with a proliferative reactive process arising mostly from the muscular fascia or deep subcutaneous region. It distinctively appeared as solitary nodule in young-aged patient with rapid growth, which gives its other name as pseudosarcomatous fibromatosis. A 16-year old boy visited our clinic for the palpable mass on the left shoulder. The initial lesion had already been partially removed by the skin biopsy at local clinic. The biopsy specimen showed only fragments of fibrous mass consisting of collagenous stroma with spindle-shaped cells, but no atypia or mitotic figure. Surgical excision was performed, of which the surgical specimen showed invasion of spindle shaped cells involving subcutaneous layer interlacing with conspicious collagenous fiborus stroma without malignant features. After 3 months, two nodules reappeared near the operation sites and the following biopsy results showed the same as before. We decided to observe the remaining lesions and there had been no significant changes for 5 months. Multiple lesional occurrence is as quite uncommon feature of NF as the recurrence after surgical treatment. Herein, we report a rare case of multiple recurrence of NF after surgical extirpation, which presented with in-transit metastasis of sarcoma-like feature.

        Erosive adenomatosis of the nipple (EAN) is a rare benign neoplasm characterized by the erosion of the nipple forming crust with serous or bloody discharge. Although there is no pronounced link between its nature course with the malignant transformation, the erosive deformity is directly related to the recurrent functional discomfort and cosmetic problem, resulting in deterioration of the quality of life. A 22-year old woman visited the breast clinic for her persistent deformity of the left nipple. She underwent a core-needle biopsy of a breast mass, which was incidentally found during a breast examination. However, no additional abnormalities were found except for fibrocystic change, and then visited to our clinic for evaluation of the skin lesion. The physical examination revealed serous and bloody erosion on top of the nipple with loss of half of its normal structure. The skin biopsy showed hyperplasia of the tubular structure composed of layers of cuboidal cells, but no atypia or pleomorphism. Then she received four times of cryosurgery every six-weeks, and after 6 months the nipple almost completely healed only remaining a tiny slit-like hole. Although the first choice of treatment for EAN is local excision, it is not quite feasible approach in young-aged female patients given the unsatifactory cosmetical outcomes and the anxiety for invasive procedure. Cryotherapy could be one of the acceptable option for the treatment of EAN.

        Indications of an Adaptive Response to X-ray in Lymphocytes of Radiographers

        Lymphocytes obtained from radiographers were irradiated with high-dose X-rays and analyzed for chromosome aberrations and micronuclei to examine whether an adaptive response to ionizing radiation can be induced. The study subjects were seven male radiographers and eight male controls. The frequency of chromosome aberrations was lower in the lymphocytes of radiographers than in those of the controls when the lymphocytes were irradiated with 1 Gy of X-rays (7.93 ± 0.78 in radiographers, 8.31 ± 0.53 in controls), whereas the frequency was higher in the lymphocytes of radiographers before the irradiation (1.36 ± 0.35 in radiographers, 0.13 ± 0.08 in controls). The same trend was observed for the frequency of micronuclei, but the difference was not statistically significant. The association with the frequencies of chromosome aberrations and micronuclei after 1 Gy of irradiation was significantly correlated in the radiographers and controls (Spearman`s correlation r = 0.5, p = 0.08). The reduced chromosome damage in the lymphocytes of the radiographers was correlated with the total cumulative dose (Spearman`s correlation, CA frequency, r = 0.7, p < 0.05; MN frequency, r = 0.5, p = 0.1). Thus, an adaptive response was observed in the radiographers, and therefore chronic occupational radiation exposure might act as an adapting dose.

      • Genome sequence of mungbean and insights into evolution within Vigna species

        Mungbean (Vigna radiata) is a fast-growing, warm-season legume crop that is primarily cultivated in developing countries of Asia. We constructed a draft genome sequence of mungbean to facilitate genome research into the subgenus Ceratotropis and to enable a better understanding of the evolution of leguminous species. The draft genome sequence covers 80% of the estimated genome, of which 50.1% consists of repetitive sequences. In total, 22,427 high confidence protein-coding genes were predicted. Based on the de novo assembly of additional wild mungbean species, the divergence of what was eventually domesticated and the sampled wild mungbean species appears to have predated domestication. Moreover, the de novo assembly of a tetraploid Vigna species (Vigna reflexo-pilosa var. glabra) provided genomic evidence of a recent allopolyploid event. To further study speciation, we compared de novo RNA-seq assemblies of 22 accessions of 18 Vigna species and protein sets of Glycine max and Cajanus cajan. The species tree was constructed by a Bayesian Markov chain Monte Carlo method using highly confident orthologs shared by all 24 accessions. The present assembly of V. radiata var. radiata will facilitate genome research and accelerate molecular breeding of the subgenus Ceratotropis.

        Clinical Characteristics of Microscopic Colitis in Korea: Prospective Multicenter Study by KASID

        Background/Aims: Microscopic colitis (MC) encompasses collagenous and lymphocytic colitis and is characterized by chronic diarrhea. In cases of MC, colonic mucosae are macroscopically normal, and diagnostic histopathological features are observed only upon microscopic examination. We designed a prospective multicenter study to determine the clinical features, pathological distribution in the colon and prevalence of MC in Korea. Methods: We prospectively enrolled patients having watery diarrhea no more than 3 times a day between March 2008 and February 2009. We obtained patient histories and performed colonoscopies with random biopsies at each colon segment. Results: A total of 100 patients with chronic diarrhea were enrolled for a normal colonoscopy and stool exam. MC was observed in 22 patients (22%) (M:F 1.2:1; mean age, 47.5 years). Of those 22 patients, 18 had lymphocytic colitis and 4 had collagenous colitis. The entire colon was affected in only 3 cases (13.6%), the ascending colon in 6 cases (27.2%), the transverse colon in 3 cases (13.6%), and the left colon in 3 cases (13.6%). More than 2 segments were affected in 7 cases (31.8%). Nonsteroidal anti-inflammatory drug-associated MCs were observed in 4 cases (18.2%), 3 of which showed improved diarrhea symptoms following discontinuation of the medication. Frequently associated symptoms were abdominal pain and weight loss. Autoimmune diseases were observed in 4 cases (18.2%). Half of the 22 patients with MC improved with conservative care by loperamide or probiotics. Conclusions: In a prospective multicenter study of Korean patients with chronic diarrhea, the frequency of MC was found to be approximately 20%, similar to the percentage observed in Western countries. Therefore, the identification of MC is important for the adequate management of Korean patients with chronic diarrhea. (Gut Liver 2011;5:181-186)

        Comparative Study of Implicit and Explicit Solvation Models for Probing Tryptophan Side Chain Packing in Proteins

        We performed replica exchange molecular dynamics (REMD) simulations of the tripzip2 peptide (betahairpin) using the GB implicit and TI3P explicit solvation models. By comparing the resulting free energy surfaces of these two solvation model, we found that the GB solvation model produced a distorted free energy map, but the explicit solvation model yielded a reasonable free energy landscape with a precise location of the native structure in its global free energy minimum state. Our result showed that in particular, the GB solvation model failed to describe the tryptophan packing of trpzip2, leading to a distorted free energy landscape. When the GB solvation model is replaced with the explicit solvation model, the distortion of free energy shape disappears with the native-like structure in the lowest free energy minimum state and the experimentally observed tryptophan packing is precisely recovered. This finding indicates that the main source of this problem is due to artifact of the GB solvation model. Therefore, further efforts to refine this model are needed for better predictions of various aromatic side chain packing forms in proteins.

        Effect of cell-penetrating peptide-conjugated estrogen-related receptor ${\beta}$ on the development of mouse embryos cultured in vitro

        Objective: Estrogen related receptor ${\beta}$ (Esrrb) is a member of the orphan nuclear receptors and may regulate the expression of pluripotencyrelated genes, such as Oct4 and Nanog. Therefore, in the present study, we have developed a method for delivering exogenous ESRRB recombinant protein into embryos by using cell-penetrating peptide (CPP) conjugation and have analyzed their effect on embryonic development. Methods: Mouse oocytes and embryos were obtained from superovulated mice. The expression of Oct4 mRNA and the cell number of inner cell mass (ICM) in the in vitro-derived and in vivo-derived blastocysts were first analyzed by real time-reverse transcription-polymerase chain reaction and differential staining. Then 8-cell embryos were cultured in KSOM media with or without $2{\mu}g/mL$ CPP-ESRRB protein for 24 to 48 hours, followed by checking their integration into embryos during in vitro culture by Western blot and immunocytochemistry. Results: Expression of Oct4 and the cell number of ICM were lower in the in vitro-derived blastocysts than in the in vivo-derived ones (p<0.05). In the blastocysts derived from the CPP-ESRRB-treated group, expression of Oct4 was greater than in the non-treated groups (p<0.05). Although no difference in embryonic development was observed between the treated and non-treated groups, the cell number of ICM was greater in the CPP-ESRRB-treated group. Conclusion: Treatment of CPP-ESRRB during cultivation could increase embryos' expression of Oct4 and the formation rate of the ICM in the blastocyst. Additionally, an exogenous delivery system of CPP-conjugated protein would be a useful tool for improving embryo culture systems.

        Effects of Drug-eluting Stents on Plasma C-reactive Protein Levels after Coronary Stenting: Comparison with Bare Metal Stents

        After coronary stenting, inflammatory mechanisms play a crucial role in the pathogenesis of neointimal proliferation and in-stent restenosis. Drug eluting stents(DES) have been shown to reduce in-stent restenosis in various studies. We investigated plasma C-reactive protein (CRP) levels after DES implantation and compared with those after bare metal stent(BMS) implantation. In a prospective single center study, we performed percutaneous coronary intervention with a single stent in 67 patients(n=21 in BMS group, n=46 in DES group) with clinically stable angina. Plasma CRP levels were determined before intervention and 48 hrs, 72 hrs and 2 weeks after coronary stenting. There were 54 men(80.6%) and the age of patients ranged from 37 to 75 years(mean, 59±9 years). There was no difference in clinical and angiographic baseline characteristics except that more diabetic patients were included in DES group(34.8% vs 9.5%, p=0.04) and that the stent diameter was larger in BMS group(3.4±0.5 mm vs 3.0±0.4 mm, p<0.01). The plasma CRP levels at 48(13.4±14.7 vs 5.9±4.9 mg/L, p<0.01) and 72 hrs(16.7±19.8 vs 5.4±3.9 mg/L, p<0.01) after stent implantation were significantly higher in BMS than DES group. DES showed significantly lower plasma CRP levels after coronary stent implantation compared to BMS. This may reflect potent effects of DES on acute inflammatory reactions induced by coronary intervention.
