Coronary stent dislodgement is a rare complication of percutaneous coronary intervention. We report a rare case of dislodgement of two intracoronary stents. On withdrawal of two balloon catheters, one with a guide wire was mechanically distorted from the left main (LM) to the proximal left anterior descending artery (LAD) while the other was dislodged from the LM to the ostial left circumflex ar-tery. The stent in the LAD could not be retrieved into the guide catheter using a Goose neck snare, because it was caught on a previously deployed stent at the mid LAD. A new stent was quickly deployed from the LM to the proximal LAD, be-cause the patient developed cardiogenic shock. Both stents, including a distorted and elongated stent, were crushed to the LM wall. Stent deployment and crushing may be a good alternative technique to retrieving a dislodged stent.

This paper evaluated service life in a RC continuos beam with 2 spans considering loading condition, cold joint, and substitution ratio of GGBFS. Based on the previous test results for 91 and 365 days, changing ratio of diffusion coefficients with applied stress are normalized considering substitution of GGBFS and cold joint. For evaluation of service life in RC continuous beam with 2 spans, non-linear analytical model is adopted. The reduction of service life with cold joint and loading condition should be considered in maintenance even if GGBFS which shows excellent resistance to chloride ingress is used for the RC structures.

Background/Aim: The objective of this study was to compare the efficacy of lamivudine (LAM) and entecavir (ETV) in prevention of HBV reactivation in patients receiving systemic chemotherapy. Methods: A total of 554 non-hepatic cancer patients with HBsAg who underwent chemotherapy and received prophylactic antiviral therapy (454 with LAM and 100 with ETV) at the Samsung Medical Center between January 2008 and December 2010 were identified. Exclusion criteria were as following: late prophylaxis (not started within the first cycle of chemotherapy), any prior chemotherapy or/and antiviral therapy, follow-up less than 3 months, irregular medication, and chronic hepatitis B. Finally, clinical outcomes of 146 patients in LAM group and 40 in ETV group were compared. Hepatitis was defined as a 3 times increase in alanine aminotransferase (ALT) that exceeds 40 IU/L or an absolute increase of ALT to >100IU/L when compared with the baseline pre-chemotherapy value. Results: At baseline, there were no significant differences between two groups with regard to gender, aspartate aminotransferase, ALT, bilirubin, albumin, cancer type, purpose of chemotherapy, steroid use, and adjuvant radiotherapy. However, patients were younger (48.2 ± 9.4 vs. 51.5 ± 9.4, p=0.046), HBeAg seropositivity rate was higher (28.9% vs. 11.7%, p=0.010), and baseline HBV DNA level were higher [2401.0 IU/mL (<9->1.7x108) vs. 291.5 IU/mL (<9->1.7x108), p=0.006] in ETV group compared to LAM group. Five patients (2.7%) developed hepatitis: 4/146 (2.7%) and 1/40 (2.5%) in LAM and ETV group, respectively (p=1.000). Among them, 3 patients were accompanied by HBV DNA elevation (≥10-fold) only in LAM group (2.1% vs. 0.0%, p=1.000). During preemptive preemptive treatment, two in LAM group and one in ETV group developed hepatitis (1.4% vs. 2.5%, p=1.000). Severity of hepatitis was not different between two groups. Conclusions: LAM and ETV are comparable in efficacy in terms of prevention of hepatitis in HBsAg-positive patients receiving cancer chemotherapy.
Effects of loading conditions and cold joint on service life against chloride ingress

RC (Reinforced Concrete) members are always subjected to loading conditions and have construction joints when constructed on a big scale. Service life for RC structure exposed to chloride attack is usually estimated through chloride diffusion test in sound concrete, however the test is performed without consideration of effect of loading and joint. In the present work, chloride diffusion coefficient is measured in concrete cured for 1 year. In order to evaluate the effect of applied load, cold joint, and mineral admixtures, OPC (Ordinary Portland Cement) and 40%-replaced GGBFS (Ground Granulated Blast Furnace Slag) concrete are prepared. The diffusion test is performed under loading conditions for concrete containing cold joint. Investigating the previous test results for 91 days-cured condition and the present work, changing diffusion coefficients with applied stress are normalized considering material type and cold joint. For evaluation of service life in RC continuous beam with 2 spans, non-linear analytical model is adopted, and service life in each location is evaluated considering the effects of applied stress, cold joint, and GGBFS. From the work, varying service life is simulated under various loading conditions, and the reduced results due to cold joint and tensile zone are quantitatively evaluated. The effect of various conditions on diffusion can provide more quantitative evaluation of chloride behavior and the related service life.
