      • Risk and protective factors for salutogenesis of adolescents with Moyamoya disease

        Aim(s): To identify the risk and protective factors affecting salutogenesis in adolescents with Moyamoya disease based on Antonovsky’s salutogenesis theory. Method(s): This study used a qualitative descriptive design. We interviewed adolescents diagnosed with Moyamoya disease who were hospitalized or undergoing treatment at the outpatient clinic of X hospital in Seoul from July 4, 2019 to October 10, 2019. We also interviewed their parents and experts with experience in treating adolescents with Moyamoya disease. Data analysis was performed according to the process of deductive content analysis based on Antonovsky’s salutogenesis theory. Result(s): This study identified risk and protective factors affecting salutogenesis in adolescents with Moyamoya disease. Risk factors were closely related to problems with friends, parent relationships, and academic work. Protective factors were identified as social and emotional support through interpersonal relationships, positive attitudes of adolescents to the disease, ability to understand and predict patterns of symptoms, increasing strength to resolve the state of tension, and developing available resources in emergencies. Particularly, families of adolescents plays an important role in maintaining the health of adolescents with Moyamoya disease. Conclusion(s): The factors affecting salutogenesis in adolescents were identified by reflecting on the characteristics of the developmental stage of adolescents and the symptoms and situations that are unique to adolescents. In order for adolescents with Moyamoya to acquire and maintain health, it is important for them to recognize available resources and use them appropriately to successfully manage the state of tension caused by stress. Based on the results of this study, we suggest the development of interventions to maintain and promote the health of adolescents with Moyamoya disease.

        Dynamic Patterns of PTK7 Protein Expression in Adult Mouse Tissues

        PTK7 belongs to a subgroup of receptor protein tyrosine kinases with inactive catalytic activity of the protein tyrosine kinase(PTK), which is known to be important for signal attenuation, cell adhesion, and regulation of planar cell polarity. In this study, to decipher possible biological roles of PTK7, we examined the tissue distribution and localization of PTK7 protein in adult mouse tissues. PTK7-positive signals were detected in variety of tissues and cells in multiple systems, including respiratory, digestive, urinary and reproductive organs, as well as liver and pancreas. These expression profiling data imply various physiological roles of PTK7 in multiple organ systems. Interestingly, among various tissues examined, no specific signal was detected in the heart and muscles. Taken together, dynamic patterns of protein expression in adult tissues strongly suggest PTK7 may play important roles during organogenesis and histogenesis.

      • Pre-treatment neutrophil-to-lymphocyte ratio is associated with improved overall survival in patients with gynecological cancer treated with immunotherapy

        Objective: The purpose of this study was to determine whether the pretreatment neutrophil-lymphocyte ratio at the time of initiation of immune checkpoint inhibitors (ICIs) in patients with recurrent gynecological cancer has an effect on progression free survival (PFS) and overall survival (OS). Methods: A retrospective, single-center cohort study was conducted using data between March 1, 2002, and February 18, 2021 at Samsung Comprehensive Cancer Center, Seoul. We identified patients who received ICIs for recurrent gynecological cancer (ovarian, endometrial, cervical, and other cancers). The absolute serum neutrophil and lymphocyte ratio (NLR) was calculated from the complete blood count laboratory results performed before the first cycle of ICIs. Based on the median value of the NLR ratio, analysis was performed by dividing into 2 groups as high or low NLR. Results: Among 250 patients, 207 patients were included in the analysis, excluding patients with unknown pre-treatment NLR ratio or follow-up loss. 103 patients were assigned to the group with a low NLR, and 104 patients were assigned to the group with a high NLR ratio. There were statistically significant differences between the two groups in terms of body weight, BMI, prior radiation therapy, ECOG, total number of ICIs, and prior antibiotics use before ICIs (p<0.05). In the univariate analysis according to NLR, the Kaplan-Meier curve showed significantly longer overall survival (OS) in patients with low NLR compared with high NLR (p=0.17). In Cox multivariable analysis, NLR, ECOG, and total number of ICIs were found to affect Especially, low NLR group demonstrated survival gain about 3 times compared to the high group (HR 3.26; 95% CI 1.89 5.62; p<0.001). Conclusion: Pre-treatment high NLR showed significant improved. OS in patients with ICIs for recurrent gynecological cancer. It is thought that pretreatment NLR can be used as a predictive marker to predict survival after ICIs in patients with recurrent gynecological cancer.

      • [FCT 8] The efficacy of acellular dermal matrices in scar revision surgery: experimental and clinical study

        We hypothesized that acellular dermal matrix (ADM) graft along with scar revision may act as a dermal regeneration substitute, and it will improve skin color, texture, humidification, and elasticity. We implanted acellular dermal matrix in subcutaneous layer of incision area during scar revision surgery for the successful treatment of various scars. In clinical study, we excised scar tissue measuring length and width of the scar (n=22), implanted the ADM underneath and closed the defect with appropriate approximation. Functional skin values of scar revision area and adjacent normal skin were evaluated in terms of skin elasticity, humidification, transepidermal water loss, and color. Also, patient satisfaction was assessed at postoperative 6 months with visual analog scale in terms of cosmetic satisfaction, pain or itching sensation and scar hardness. Patient`s survey revealed that patients were satisfied with the postoperative results showing 7.8 ± 1.1 of overall VAS score in all factors. Humidification of scar indicated that the score was 1.03 ± 0.3 (scar site per adjacent normal skin) at preoperative period and 1.52 ± 0.95 at 6 months. ADM could be adequate for use as a tissue remodeling substitute, and scar revision technique using ADM implantation is satisfactory in terms of high patient satisfaction and significantly improved humidification.

      • Mobiletype - Moyamoya Adolescent Salutogenesis: A Usability Test

        Aim(s): Moyamoya disease (MMD) is a rare, chronic, and progressive intracranial arteriopathy. It is critical for individuals with MMD to have the capability of managing their symptoms. This study aimed to 1) develop a mobile application (APP) titled ‘Moyamoya Healthy Youth’ that can help adolescents with MMD to improve their health, and 2) conduct the usability test of this app. Method(s): The app development was guided by Antonovsky (1987)’s Salutogenesis Theory. The app’s usability was evaluated by the Intervention Mapping Protocol. We reviewed prior studies regarding MMD and relevant interventions. We performed individual or focus group interviews targeting adolescents with MMD, parents, and healthcare providers. Additionally, we conducted a survey to identify MMD symptoms. Based on the findings, healthcare professionals (pediatric nursing professors, MMD specialists) and app developers (app development consultant, programmer, user interface designer) created the ‘Moyamoya Healthy Youth’ app. Subsequently, pediatric nurse practitioners and pediatric nursing professors, high school students, and app development professionals used the app themselves and evaluated the app"s easiness and usefulness. Results(s): Considering availability, accessibility, and visibility, this app had been developed from August to November 2020. The app"s system structure was employed a server/client system using HTTP and Web API. The server configuration used the MySQL database and Apache Tomcat/Spring Framework. The client configuration was performed by Android SDK. This app has been uploaded to Google Play. The main screen of the app consisted of ‘Diary’, ‘Daily Report’, and ‘Cheer Up Message’. In terms of the usability test, the mean scores of the easiness and usefulness of using the app were 4.42 (SD = 0.34) and 4.52 (SD = 0.22), respectively, which could be regarded as favorable levels. Conclusion(s): This study verified the usability of the ‘Moyamoya Healthy Youth’ app. This app could contribute to the enhanced self-management skills of adolescents with MMD.

      • Impact of different adjuvant treatment approaches on survival in high-risk endometrial carcinoma: a multicenter study

        Objective: The purpose of this study is to compare the oncological outcomes according to adjuvant treatment in patients with high risk endometrial cancer. Methods: A retrospective study was conducted using data between January 1, 2000, and December 31, 2020 at two tertiary centers in Korea (Samsung Medical Center) and Taiwan (National Taiwan University Hospital). Patients with high risk endometrial cancer were eligible. The definition of high risk referred to when final pathology showed grade 3 endometrioid or non-endometrioid with any stages or grade 1-2 endometrioid with stage III or IV. In terms of adjuvant therapy, we categorized it into systemic chemotherapy (CT), radiotherapy (RT), or combination of chemotherapy and radiotherapy (CRT). Propensity score matching was done based on the pathologic stage and cell type after surgery. Results: Total 519 patients with high risk endometrial cancer were eligible in which 176 patients, 200 patients, and 143 patients received RT, CT, and CRT, respectively. In all patient groups, disease free survival and overall survival were statistically significantly higher in the RT group. Advanced stage (stage III-IV) patients who underwent the CRT were improved survival. After propensity score matching, 83 patients in each treatment group were finally analyzed. There was no difference in survival with adjuvant therapy after matching in all patients group, early stage and advanced stage patients. Although statistical significance was not reached after matching, a high survival rate was achieved when CRT was performed in advanced stage patients. CRT was further analyzed by dividing it into concurrent CT and RT, sequential (RT followed by CT, or vice versa), PORTEC3, and Sandwich method, and there was no difference in the survival curve. Conclusion: Although all high-risk endometrial cancer patients were included, survival gains were no different for each stage and treatment group. In stage III and IV groups, it is considered that there is a survival gain in the CRT.

        Lower limb compartment syndrome by reperfusion injury after treatment of arterial thrombosis post-laparoscopic radical hysterectomy and pelvic lymph node dissection for cervical cancer

        Compartment syndrome is a clinical condition associated with decreased blood circulation that can lead to swelling of tissue in limited space. Several factors including lithotomy position, prolonged surgery, intermittent pneumatic compressor, and reperfusion after treatment of arterial thrombosis may contribute to compartment syndrome. However, compartment syndrome rarely occurs after gynecologic surgery. In this case, the patient was diagnosed as compartment syndrome due to reperfusion injury after treatment of arterial thrombosis, which occurred after laparoscopic radical hysterectomy and pelvic lymph node dissection for cervical cancer. Despite its rarity, prevention and identifying the risk factors of complication should be performed perioperatively; furthermore, gynecologist should be aware of the possibility of complications.
