Gemcitabine 투여환자의 혈관통 실태와 임상요인에 따른 혈관통 차이

Purpose: This study was performed to examine vascular pain due to gemcitabine and according to clinical factors. Methods: The survey was performed with 525 cancer patients visiting chemotherapy infusion room in one general hospital. The data were collected via self-reported questionnaire, researchers observation, and reviewing medical records. Data were analyzed descriptive statistics, ttest, Kruskal-Wallis test, Tukey test using ranks, Jonckheere-Terpstra test, and Spearman correlation analysis were used. Results: The mean scores of the intensity and onset time of vascular pain were 3.06±2.16 and 8.13±13.13 min, and each other were negative correlaton. Degree of intensity of vascular pain had a significant difference on gender (p=.003), age (p=.004), weight (p=.019), Body mass index (BMI) (p=.005), Body surface area (BSA) (p=.030), infusion time (p<.001), dose (p=.027), dose per minute (p<.001), the number of administered gemcitabine (p=.005), combination chemotherapeutic drug (p=.013) and tumor type (p=.002). Degree of onset time of vascular pain had a significant difference on infusion time (p=.044), combination chemotherapy (p=.001) and injection site (p=.001). Conclusion: Patients administrated gemcitabine were experiencing various vascular pain, and vascular pain had a difference on clinical factors. Therefore, oncology nurses should be considered significant clinical factors to implement effective interventions to patients administered gemcitabine.

Purpose: The purpose of this study was to examine the effect of a group coaching program(GCP) on depression, anxiety, and hope in women breast cancer patients undergoing chemotherapy. Methods: A total of 152 patients were enrolled and randomly assigned to an experimental group receiving the GCP (N=76) and an untreated control group (N=76). Data collection consisted of three measurements of depression, anxiety, and hope (pre, post, and 3weeks later). The intervention was a one-time GCP consisting of 5 subparts for 30~40 minutes for a group of 3~4 people, conducted by a single nurse. Results: The scores of depression, anxiety, and hope changed significantly over time (p〈.001, p〈.001, p〈.001). Depression, anxiety, and hope changes were significantly different between the experimental and control groups (p〈.001, p〈.001, p〈.001). There was a difference in depression, anxiety, and hope among the groups according to the implementation of the GCP (p〈.001, p〈.001, p〈.001). In the experimental group, depression, anxiety, and hope level were significantly different between the pre- and post-test (p〈.001), pre- and 3weeks later-test(p〈.001), but not in the control group. Conclusion: The GCP for women breast cancer patients receiving chemotherapy was effective in reducing depression and anxiety, and increasing hope.
Decoding With Hypothesis Testing: A Near ML Decoding Scheme for MIMO Systems

<P>A near maximum-likelihood (NML) scheme for the decoding of multiple-input-multiple-output (MIMO) systems is addressed by incorporating the technique of hypothesis testing in the searching procedure. The proposed decoding scheme selects the best node based on the node metric, determines one child node of the best node via hypothesis testing, and connects the best node with some sibling nodes of the child node. From simulation results, it is confirmed that the proposed scheme has a lower computational complexity than other NML decoders and that the performance difference between the proposed and maximum-likelihood schemes is negligibly small.</P>

Background: FDG-PET has been suggested to have a supplementary role in localizing recurred sites of differentiated thyroid carcinoma. This study was performed to show whether FDG-PET is feasible as an alternative diagnostic modality for patients with I-131 scan negative thyroid carcinoma by verification of post-surgical pathology findings. Methods: Eighteen patients of papillary thyroid carcinoma (M:F=4:14, age 41±16 year) who had total thyroidectomy and I-131 ablation therapy were included. All patients showed negative I-131 scan on therapeutic dose but they were suspected as disease recurrence because of elevated serum Tg or anti-Tg Ab during follow-up periods. FDG-PET was performed, and then cervical lymph node dissection on either side or both sides of the neck was done according to FDG-PET results. Results: A total of 77 cervical lymph node groups were dissected in 18 patients; internal jugular chain 49, spinal accessory 9, jugulodigastric 5, anterior jugular 4, paratracheal 3, supraclavicular 2, and oters 5. Forty eight lymph node groups revealed metastatic papillary carcinoma on pathology and their largest diameter ranged from 0.4 to 7.0cm (1.2±0.7cm). All patients had at least one malignant lymph node group. FDG-PET detected 37 among 48 malignant lymph nodes (sensitivity 77%), and their count ratio ranged 1.7-31.1 (6.1±6.3). Among the 30 malignant lymph nodes less than 1cm, FDG-PET detected 20 lymph nodes. Of the 29 lymph node groups without malignant cells, FDG-PET was also negative in 24 groups (specificity S3%). Positive predictive value of FDG-PET on I-131 scan negative differentiated thyroid carcinoma was 88%; negative predictive value was 69%. Conclusion: FDG-PET has been confirmed as a valuable diagnostic modality to detect cervical lymph nodes of differentiated thyroid carcinoma who are suspicious for recurrence but with negative I-131 scan, by pathologic findings (J Kor Soc Endocrinol 14:520-530, 1999).

이 논문에서는 새로운 다상수열과 그것을 만드는 방법을 제안하고, 그 수열들의 상관성질을 살펴본다. 이 수열은 정수 덧셈과 나머지 셈만으로 쉽게 만들 수 있고, 상관성질도 좋다. 주파수 선택적이고, 시간 선택적이지 않으며, 느리게 바뀌는 나카가미 감쇄채널에서 덧셈 흰빛 정규잡음이 있을 때, 제안한 수열들을 쓴 유사동기 부호분할 다중접속 시스템의 성능을 분석한다. In this paper, new polyphase sequences and a sequence generation method are suggested. These sequences are found to have good correlation properties. Since the suggested generation method consists only of integer sums and modular techniques, sequence generation is also easy. The performance of the sequences is investigated when used in QS-CDMA systems under frequency selective, time nonselective, slow Nakagami fading channel with additive white Gaussian noise.
