다기관 기능부전을 초래한 치명적인 삼일열 말라리아 증례

삼일열 말라리아는 국내에서 재유행하고 있는 감염질환으로, 일반적으로 양성의 임상경과를 거친다. 저자들은 특이한 질병력이 없이 건강하였던 젊은 성인이 국내에서 삼일열 말라리아에 걸린 후 다기관기능부전과 폐출혈으로 사망한 증례를 경험하였다. 환자의 중증 임상경과를 설명할 만한 특이한 인자가 발견되지 않았다. Vivax malaria is a re-emerging infectious disease in Korea with cumulating 19,416 cases as of December 2003 since 1993. Vivax malaria is generally a benign disease with few severe complications. Even in the worldwide literature, there are only small number of case reports on severe complications. We experienced a fatal vivax malaria in a previously healthy young adult with severe multi-organ failure and fatal massive pulmonary hemorrhage. The patient presented with abdominal pain and shock. There were splenic hemorrhage, disseminated intravascular coagulation, oliguric acute renal failure requiring dialysis, and pulmonary hemorrhage leading the patient finally to death. The initial parasitemia was 2,046/μL. Thin smear and PCR revealed only Plasmodium Vivax. There was no explainable cause of death except for the only serological suggestion of Mycoplasma pneumoniae infection without correlating clinical evidence. This is a first case of fatal vivax malaria in Korea since its re-emergence in 1993. Further case monitoring is needed to define whether this is a index case or an isolated one.

<P>Calbindin-D<SUB>9k</SUB> (CaBP-9k) is a cytosolic calcium-binding protein that is induced by estrogenic compounds possibly through estrogen receptors. We compared CaBP-9k mRNA expression in the uterus with uterotrophic response in immature rats exposed to methoxychlor (MC), an environmental chemical with estrogenic activity. MC was orally or subcutaneously administered to 3-week-old female Sprague-Dawley rats for 3 days. The weights of the uterus and vagina significantly increased in the oral treatment group at a dose of 50, 100 and 200 mg/kg, but those of the subcutaneous (SC) treatment group only increased at 200 mg/kg. Northern blot analysis showed that CaBP-9k mRNA expression was significantly induced in a dose-dependent manner at doses of 50, 100 and 200 mg/kg/day in the oral treatment group. SC administration of MC induced significant expression at only a dose of 200 mg/kg/day; this was similar to the uterotrophic response. MC has an estrogenic effect on the uterus as shown by the increase in weight and induction of CaBP-9k mRNA expression, which were much greater following exposure via oral gavage than via the SC route. The strong correlation between the results of <I>in vivo</I> uterotrophic assay and CaBP-9k mRNA expression suggests that CaBP-9k mRNA expression in the rat uterus may be used as an early gene marker for detection of the estrogenic effects of putative environmental chemicals.</P>

The purpose of the present study is to verify the structural relationship among system quality, information quality, service quality, perceived ease of use, perceived usefulness, satisfaction, intention on continuous use of mobile learning in cyber university. For this study, W cyber university in Korea was chosen to conduct web survey. The subjects were 283 students who participated in W``s cyber university courses. A hypothetical model was composed of system quality, information quality, service quality, perceived ease of use and perceived usefulness as exogenous variables, satisfaction and intention on continuous use of mobile learning as endogenous variables. The result of this study through structural equation modeling analysis is as follows:First, information quality only affect satisfaction, Second, perceived ease of use, perceived usefulness and satisfaction significantly affect intention on continuous use of mobile learning. These results imply that information quality should be considered for the design and development of mobile learning contents. Also, perceived ease of use, perceived usefulness and satisfaction is important to enhance intention on continuous use of mobile learning. This study proposes strategies for successful mobile learning in cyber university.

Aims: Phenotypic and functional characteristics of tumor-infiltrating CD8 T-cells (CD8 TILs) has not been well studied in the context of heterogeneity of exhausted T-cells and patient stratification in hepatocellular carcinoma (HCC). Methods: We identified distinct T-cell exhaustion profiles determined by PD-1 expression levels on CD8 TILs from 48 HCC patients undergoing surgical resection by multi-color flow cytometry and RNA sequencing. Results: CD8 TILs from HCC patients significantly overexpressed immune checkpoint receptors such as PD-1, TIM-3 and LAG-3. CD8 TILs could be further stratified into PD-1<sup>hi</sup>, PD-1<sup>int</sup> and PD-1<sup>neg</sup> subpopulations, and TIM-3 and/or LAG-3 were exclusively expressed on PD-1<sup>hi</sup>, not PD-1<sup>int</sup>, subpopulation. RNA expression profiles were distinct among PD-1<sup>hi</sup>, PD-1<sup>int</sup> and PD-1<sup>neg</sup> CD8 TILs and a gene module of T-cell exhaustion was significantly enriched in PD-1<sup>hi</sup> CD8 TILs. HCC patients could be classified into two distinct subgroups based on the proportion of PD-1<sup>hi</sup> CD8 TILs; High PD-1 expressers and Low PD-1 expressers. High PD-1 expressers had more severely dysfunctional CD8 TILs with exhaustion profiles that were observed in PD-1<sup>hi</sup> CD8 TILs, compared to Low PD-1 expressers. High PD-1 expressers were associated with aggressive biologic tumor features. Combination blockades of Tim-3 or LAG-3 in addition to PD-1 blockade further enhanced proliferative responses of CD8 TILs from High PD-1 expressers. Conclusions: CD8 TILs displayed heterogeneous exhaustion profiles determined by differential PD-1 expression levels in HCC patients. A certain subgroup of HCC patients expressing multiple immune checkpoint receptors appears to be eligibly indicated for immune checkpoint inhibitors for effective reinvigoration of anti-tumor immunity.
