The dramatic increase in the prevalence of obesity and its accompanying comorbidities are major health concerns in Korea. Obesity is defined as a body mass index (BMI) ≥25 kg/m2 in Korea. Current estimates are that 32.8% of adults are obese: 36.1% of men and 29.7% of women. The prevalence of being overweight and obese in national surveys is increasing steadily. Early detection and the proper management of obesity are urgently needed. Weight loss of 5-10% is the standard goal. In obese patients, control of cardiovascular risk factors deserves the same emphasis as weight-loss therapy. Since obesity is multifactorial, proper care of obesity requires a coordinated multidisciplinary treatment team, as a single intervention is unlikely to modify the incidence or natural history of obesity.
Testosterone Protects Pancreatic β-cells from Apoptosis and Stress-Induced Accelerated Senescence

Purpose: Androgens are steroid hormones that are very important in the sexual development and the maintenance of male reproductive system, and also have diverse actions in non-reproductive tissues, including potent antioxidant capacity. Type 2 diabetes mellitus is caused by tissue insulin resistance and insufficient insulin secretion from the pancreatic β-cells. The progressive decline of pancreatic β-cells in diabetes is closely related with the severity of disease. We wanted to know whether dihydrotestosterone (DHT) can protect insulin secreting pancreatic β-cells from apoptosis and accelerated senescence induced by oxidative stress. Materials and Methods: Cultured INS-1 cells were used. Various concentrations of H2O2 were applied to exert oxidative stresses. The degrees of apoptosis, accelerated senescence, and the changes of the expressions of related signaling molecules after the application of DHT were analyzed by CCK-8, p16 expression, SA-β-Gal staining, reverse transcription polymerase chain reactions and Western blots. Results: The application of H2O2 significantly increased (p<0.05) the degree of senescence and apoptosis of cultured INS-1 β-cells. DHT not only showed anti-oxidant protective capacity, but also significantly reduced (p<0.05) the degree of accelerated senescence. Conclusions: DHT effectively protects pancreatic islet INS-1 β-cells from H2O2 induced oxidative stress.
Multiple Gastrointestinal Stromal Tumors: Clinicopathologic and Genetic Analysis of 12 Patients

Multiple gastrointestinal stromal tumors (GISTs) are extremely rare and usually associated with type 1 neurofibromatosis and familial GIST. The aim of this study was to investigate the clinical, phenotypic, and genetic characteristics of multiple GISTs to gain insights into their underlying pathogenesis and clinical behavior. Forty-seven paraffin blocks of multiple GISTs from 12 patients were analyzed. Genomic DNA was extracted from the tumor and normal mucosa and mutations for 4 exons of KIT gene and 3 exons of PDGFRA gene were determined. Among 12 patients with multiple GISTs, 5 were sporadic, 2 were familial with germline mutations of KIT gene, and 5 were associated with type 1 neurofibromatosis. All but 1 sporadic and familial multiple GISTs showed mutations of KIT gene shared by the same mutation on each GIST mass within a patient. But in 1 sporadic case, different types of KIT mutations were observed. Two familial multiple GIST cases showed diffuse involvement of the gastrointestinal tract with diffuse hyperplasia of interstitial cell of Cajal. Multiple GISTs associated with type 1 neurofibromatosis were located in the jejunum and harbored no mutations of KIT or PDGFRA. Different types of KIT gene mutation found in our case raise a possibility that recurrence of GISTs within a gastrointestinal tract may have a chance to be a rare occurrence of multiple primary GISTs instead of true recurrence. Multiple GISTs show unique clinical, phenotypic, and genotypic characteristics that are dependent on the particular underlying mechanisms, but the overall prognosis is favorable regardless of the numbers or phenotype of GISTs.

Background/Aims: The incidence of duodenal tumors has increased by health surveillance. However, preoperative diagnosis of subepithelial duodenal tumors remains difficult because of the wide variety of pathologies and the location of the tumors. We analyzed endoscopic, radiological, and pathological features of subepithelial benign duodenal tumors (BDTs), which were treated by surgical resection. Methods: Five patients with subepithelial BDTs treated by surgical resection were analyzed retrospectively. We compared the preoperative and postoperative diagnosis and evaluated the clinical presentations, endoscopic and radiological findings, surgical treatments, pathological results, and outcomes of these patients. Results: All the patients underwent successful surgical resection. There were two cases of gastrointestinal stromal tumors (GISTs) treated with segmental duodenectomy, one case of carcinoid tumor treated with antrectomy, one case of gangliocytic paraganglioma treated with ampullectomy, and a lipoma removed by mass excision. The two GISTs were in the duodenal third and fourth segment close to the pancreas, and it was difficult to exclude pancreatic tumors by imaging studies. All the patients remained healthy for more than three years. Conclusions: Subepithelial BDTs are rare and difficult to diagnosis. Awareness and preoperative diagnosis of subepithelial BDTs can lead to minimally invasive treatment, including endoscopic or local surgical resection. Korean J Pancreatobiliary 2014;19(1):18-25
양파껍질 추출물이 함유된 물간법으로 굴비 제조의 최적 조건 확립

현재 대부분의 굴비제조는 마른간법을 이용한 방법이 사용되고 있다. 마른간법은 제조 과정인 섞기에 있어서 조기에 상처가 발생하기도 하여 제품의 품질을 저하시키는 단점이 있다. 따라서 물간법을 이용하여 식염농도별(20, 30%), 온도별(5, 25, 35℃), 시간별(1, 6, 12, 24 hr) 조기중의 수분과 염도의 변화 및 미생물의 변화를 살펴보았다. 물간법에 있어서 식염수의 농도가 높을수록 조기육의 탈수량도 많으며, 염장의 온도가 높을수록 식염의 조기육으로 삼투속도는 크게 나타나서 수분함량과 염도는 반비례하였다. 물간법에 있어서 식염농도 20%에서 25℃, 1시간 염장 처리하면 마른간법으로 제조되어 시판되는 굴비와 유사한 수분 및 식염함량을 얻을 수 있었다. 조기육에 대한 무게 변화를 측정한 결과, 식염농도 20%에서 24시간 후 5℃에서는 중량이 4% 정도 증가하고, 25℃에서는 변화가 없었으며, 35℃에서는 7%정도 감소하였다. 반면 식염농도 30% 경우 5℃에서 1%정도 중량 감소가 있었고, 25℃와 35℃에서는 각각 9%와 13%정도 감소를 나타내었다. 조기육의 미생물수는 식염농도 20%에 침지한 것이 식염농도 30%에서보다 1 log 이상 높게 나타났고, 저장온도가 높을수록 미생물수은 증가하였다. 침지액속의 미생물 수는 조기육보다 1 log 정도 낮은 값을 보여주었다. 양파껍질 추출물을 이용하여 조기에 처리한 결과 처리구가 무처리구 보다 명도(L값)와 황색도(b)값이 높게 나타났고, 양파껍질 추출물은 조기로부터 추출한 지방에 대하여 대조구와 비교하여 낮은 산가를 보여주었다. For the development of better Gulbi processing, brine salting method was applied for the Yellow croaker (Larimichthys polyactis). The changes of moisture contents, salt contents, and total microbial numbers in Yellow croaker were measured following different brine concentration (20, 30%), temperature (5, 25, 35℃), and soaking time (1, 6, 12, 24 hours) by brine salting method. Rate of salt penetration into Yellow croaker muscle increased as higher brine concentration and higher dipping temperature. When compared to commercial products of Gulbi by dry-salting method, the moisture and salt contents in Yellow croaker showed similar values after treated with 20% brine at 25℃ for 1 hour. The weight of Yellow croaker increased about 4% when immersed it in 20% brine at 5℃ for 24 hours. There was no weight change at 25℃ dipping temperature and reduced 7% of weight at 35℃ dipping temperature. At 30% brine concentration, the weight of Yellow croaker reduced 1%, 9%, and 13% on weight at 5℃, 25℃, and 35℃, respectively. Total microbial counts in Yellow croaker muscle soaked at 30% brine showed 1 log lower numbers than 20%. The muscles had about 1 log higher microbial numbers than the treated brine solution. An ethanol extract of onion peel added to brine for giving better color and for preventing oxidation on Gulbi lipid. The treated group showed higher L and b values on Gulbi surface as well as antioxidant effect on the extracted oil.
IL-12p40 Homodimer Ameliorates Experimental Autoimmune Arthritis

<P>IL-23 is the key cytokine that induces the expansion of Th17 cells. It is composed of p19 and p40 subunits of IL-12. The p40 subunit binds competitively to the receptor of IL-23 and blocks its activity. Our aim was to assess the preventive and therapeutic effect of the IL-12p40 homodimer (p40)<SUB>2</SUB> subunit in autoimmune arthritis animal models. In the current study, using IL-1R antagonist–knockout mice and a collagen-induced arthritis model, we investigated the suppressive effect of (p40)<SUB>2</SUB> on inflammatory arthritis. We demonstrated that the recombinant adenovirus-expressing mouse (p40)<SUB>2</SUB> model prevented the development of arthritis when given before the onset of arthritis. It also decreased the arthritis index and joint erosions in the mouse model if transferred after arthritis was established. (p40)<SUB>2</SUB> inhibited the production of inflammatory cytokines and Ag-specific T cell proliferation. It also induced CD4<SUP>+</SUP>CD25<SUP>+</SUP>Foxp3 regulatory T (Treg) cells in vitro and in vivo, whereas the generation of retinoic acid receptor–related organ receptor γt and Th17 cells was suppressed. The induction of Treg cells and the suppression of Th17 cells were mediated via activated STAT5 and suppressed STAT3. Our data suggest that (p40)<SUB>2</SUB> suppressed inflammatory arthritis successfully. This could be a useful therapeutic approach in autoimmune arthritis to regulate the Th17/Treg balance and IL-23 signaling.</P>
