Mononeuritis multiplex is known to occur in many illness including certain types of systemic vasculitis, ANCA has been described in association with Wegener's granulomatosis, polyarteritis nodosa, and pauci-immune necrotizing crescentic glomerulonephritis, since it was first reported in 1982 by Davis et al in patients who had necrotizing glomerulonephritis with no immune deposits. ANCA may well be useful in distinguishing cases mediated by vasculitis from those produced by other processes, especially early in the course of a disease. We report a case of mononeuritis multiplex due to ANCA associated vasculitis. We diagnosed vasculitis neuropathy by sural nerve biopsy and angiography. The patient was treated with prednisolone and oral cyclophosphamide. Her subjective symptoms and objective signs were improved. Subsequent examination of ANCA was negative.
뇌경색증 환자 및 급성심근경색증 환자에 있어서 식후 High Density Lipoprotein Subfraction 과 Triglyceride 의 관계에 대한 연구

9 healthy subjects, 15 patients with cerebral infarction and J2 patients with acute myocardial infarction were studied in order to investigste whether. 1) Poat-prandial HDL2/HDL3, ratio reflects the degradation of triglyceride-rich lipoproteins in normal individuals. 2) HDL2/HDL3 decrease in patients with cerebral infarction and those with acute myocardial infarction. HDL,HDL2,HDL3 and triglyceride were evaluated in serum. Blood samples were collected in fasting state and 4 hours after breakfast. The results were: 1) Post-prandial HDL2/HDL3, has negative correlation with post-prandial triglyceride in normal control group. 2) In patients with acute myocardial infarction, the post-prandial HDL2/HDL3, ratio was lower and post-prandial triglyceride was higher than those of control group significantly(p$lt;0.05). 3) In patients with cerebral infarction, post-prandial HDL, HDL, and HDL2/HDL3, ratio were lower and post-prandial triglyceride was higher than those of control group significantly(p$lt;0.05). These results suggest that: 1) Post-prandial HDL2/HDL3, ratio may reflects the degradation of triglyceride-rich lipoproteins. 2) Decroased removal of post-prandial triglyce-ride may be a risk factor in acute myocardial infarction and cerebral infarction.
