Purpose: Intentional poisoning is a major public health issue in many parts of the world. This study was conducted to provide details regarding the epidemiology of intentional poisoning in a metropolitan emergency department and to identify the changing patterns and epidemiology of poisoning. Methods: A retrospective study was conducted to evaluate intentional poisoning of patients who visited the emergency department in a tertiary teaching hospital between 2001 and 2015. All intentional poisoning-related emergency department visits over three five year periods (2001-2005 (P1), 2006-2010 (P2) and 2011-2015 (P3)) were reviewed to investigate trends in intentional poisoning patients. Information regarding patient sex, age, time from episode to admission, psychiatric history, type of intoxicants, alcohol co-ingestion, gastric lavage, charcoal administration, any previous suicide attempts, need for hospitalization and death before discharge was reviewed. Results: A total of 1269 patients were enrolled in this study. The number of patients admitted during each period was P1=515, P2=439 and P3=315. Comparison of the three groups revealed significant differences according to age (p<0.001), psychiatric history (p<0.001), alcohol co-ingestion (p=0.013), gastric lavage (p<0.001), charcoal administration (p<0.001), need for hospitalization (p=0.044), repeated attempt (p<0.001) and type of intoxicants (p<0.001). Conclusion: The average age of intentional poisoning patients has increased. While the use of sedatives and multiple drugs increased, the use of pesticides and the antihistamine decreased.

Intraoral defects are increasingly reconstructed using free flaps. However, certain situations may render free flap reconstruction difficult. This study presents two cases with underlying comorbidities that underwent reconstruction of buccal defects involving the retromolar trigone (RMT), using a single-stage peninsula-shaped lateral tongue flap (pLTF). A 58-year-old woman and a 75-year-old man were diagnosed with left buccal squamous cell carcinoma and right buccal adenoid cystic carcinoma, respectively. The buccal defects extending to the RMT area were detected and covered with an ipsilateral posteriorly based pLTF after wide excision of the lesion. All flaps survived without any postoperative complications. Moreover, no tumor recurrences or functional problems were observed during the final follow-up. A single-stage pLTF is believed to represent a good alternative to free flaps in buccal and RMT defects.

Background: Covering a small defect on the anterior lower leg is challenging because less soft tissue is available. This study presents the application of a keystone flap (KF) with omega variation (OV) and Sydney melanoma unit modification (SMUM) for small- and moderatesized pretibial defects.Methods: This study included a series of six cases of anterior lower leg defect reconstruction with OV and SMUM KF technique by a single surgeon between December 2017 and June 2020. Records and photographs of the patients were collected and analyzed.Results: The defects’ sizes ranged from 1.5×2.5 to 6×8 cm. All defects were covered with OV KF, SMUM KF, or OV SMUM KF based on the hotspots of surrounding perforators. The flap sizes varied from 3×6 to 8×17 cm. Moreover, all flaps were completely viable. A marginal maceration developed in one case, which was mended with conservative dressing. Outcomes were favorable in all patients during the average follow-up period of 4.83±1.17 months.Conclusion: This study successfully covered small- and moderate-sized anterior lower leg defects with OV and SMUM KF. We therefore suggest the OV and SMUM KF techniques as good alternative modalities for anterior lower leg reconstruction.
최소자승법을 이용한 다수 베타 방출 핵종 혼합물의 방사능 분석

베타선 스펙트럼의 최대 에너지가 확실하게 구별되는 2개의 핵종만을 포함하는 혼합시료의 경우에는 최대 에너지가 다르다는 점을 이용하여 손쉽게 각 핵종의 방사능값을 측정할 수 있다. 그러나 3개 이상의 베타 방출 핵종이 포함된 혼합물에 대해서는 각 핵종의 스펙트럼이 서로 겹치기 때문에 이러한 방법으로 구해진 방사능값은 신뢰도가 떨어지게 된다. 따라서, 본 연구에서는 최소자승법을 이용하여 혼합물의 중첩된 베타선 스펙트럼을 각각 분리 정량분석할 수 있는 밥법을 제시하였다. 또한, 실제로 4개의 베타 방출 핵종 ^3H, ^14C, ^16Cl, 90Sr)이 혼합된 사료를 조제하여 본 분석법을 검증한 결과 최고치 Reference value)와 분석치가 7% 이내에서 잘 일치함을 보였다. It is possible to count and perform quench correction on two β-label samples so long as the maximum β-energies are sufficiently different. However, when the coventional technique is applied to the radioassay of a mixture of more than three nuclides, the reliability of the activiteis determined is considerably reduced, resulting from the large overlapping of liquid scintillation pulse height distributions of ezch nuclide. A technique that allows the activities of multiple β-labeled samples to be radioassayed was proposed by using the least square method. The technique was applied to mixture samples of ^3H, ^14C, ^16Cl, and ^90Sr. The analytical values were in good agreement with the reference values within 7% relstive error.

Carcinoid tumors are slowly growing rare neoplasms that arise from argentaffin cells. In general, gastric carcinoids are very rare, constituting only 0.7-2.5% of all gastrointestinal carcinoids and approximately 0.3% of all gastric tumors. Gastric carcinod tumor commonly presents as a single polypoid mass and the most frequent site is the antral cavity. Clinical diagnosis is often difficult because the typical carcinoid syndrome is uncommon and nonspecific gastrointestinal symptoms are the more frequent clinical feature of this disease. Preoperative diagnosis is possible by large and deep endoscopic biopsies of the gastric wall. This paper presents a case of malignant gastric carcinoid tumor and a brief review of pertinent literature. A 52 year old female visited our medical hospital with the symptom of epigastric pain and indigestion for several months. Upper G-I series and gastrofiberscope showed malignat ulcerative lesion in the lesser curvature of the antral cavity. She had been operated under the diagnosis of gastric adenocarcinoma, but pathological findings demonstrated argyrophilic granules in tumor cell.
초등학교 과학 교과서에서 사용되는 단위에 대한 아동들의 이해도

This paper aims to find out how did elementary students understand scientific units in science textbooks. The subjects were 191 students of the 6th grade from 7 elementary schools in 3 different areas, consisting of 70 from 4 village schools of, 64 from 2 town schools of Gyeongnam province, and 57 from one city school in Ulsan Metro City. A test was developed based on the analysis of scientific units in the science textbooks and teacher`s manuals constructed according to the 6th and 7th National Science Curriculum. The understanding of elementary students` on the scientific units(Temperature, Length, Weight, Volume, Speed, Plane Angle)were surveyed. The result are as follows: Regarding the temperature unit, the students generally well understand why to measure and how to read temperature, but had some problem in recording it, in confusion with the plane angle sign. As for the length unit, they obtained high scores in understanding the purpose of measuring length as well as recording and reading it. Which indicates that they are well aware of and use the unit appropriately. With respect to the weight unit, they got high scores in reading and recording weight, which means most students have no problem using the unit. However, it was found that they do not understand why to use the plate balance scale. The unit of speed is the most difficult one for children`s of science to understand, presumably, because it is an derived unit from two basic units. It is also assumed that the students got the highest score in the plane angle unit because they studied the unit immediately before the test. From the children`s understanding of science units above the teacher`s understanding and teaching methods presumed to play a major role for children to understand and use the science units properly.
