Objective: In 2011, School of Medicine, University of California, Irvine was among the first schools to implement a 4-year ultrasound curriculum. We aimed to find the point-of-care ultrasound (POCUS) utilization pattern among University of California, Irvine alumni. Methods: We surveyed University of California, Irvine alumni from the class of 2011 and beyond. Survey questions included POCUS reliance, frequency of use, and comfort with image acquisition and interpretation compared with peers. The primary outcomes were self-reported comfort and reliance on POCUS. Results: We received 93 responses from 624 surveyed alumni (response rate, 14.9%), of which 87 were analyzed. Although 46 respondents (52.9%) reported more reliance on POCUS, three (3.4%) relied on it less than their peers. At the same time, 72 (82.7%) and 67 (77.0%) felt more comfortable than their colleagues in obtaining and interpreting POCUS, respectively. No respondents felt less comfortable obtaining or interpreting POCUS than their peers. The frequency of POCUS use correlated directly with the frequency with which POCUS changed the responder’s case management (rho, 0.860; P<0.001). POCUS reliance also correlated with respondents’ comfort level in obtaining (rho, 0.321; P<0.001) and interpreting (rho, 0.378; P<0.001) POCUS results. Conclusion: University of California, Irvine graduates had higher reliance on POCUS than peers in their respective specialties. Their POCUS findings frequently changed their case management.

In previous model- and full-scale studies, high-amplitude vertical vibrations of mast-arm trafficsignal structures have been shown to be due to vortex shedding, a phenomenon in which alternatingly shed,low-pressure vortices induce oscillating forces onto the mast-arm causing a cross-wind response. When thefrequency of vortices being shed from the mast-arm corresponds to the natural frequency of the structure, aresonant condition is created causing long-lasting, high-amplitude vibrations which may lead to the fatiguefailure of these structures. Turbulence in the approach flow is known to affect the cohesiveness of vortexshedding. Results from this full-scale investigation indicate that the surrounding terrain conditions, whichaffect the turbulence intensity of the wind, greatly influence the likelihood of occurrence of long-lasting,high-amplitude vibrations and also impact whether reduced service life due to fatigue is likely to be ofconcern.

While research in Vietnam, like in many Asian countries, points to cultural constraints and testing systems as barriers to English language education, increasing attention has been given to the influence of teachers’ beliefs and practices on learners’ communicative second language (L2) development. This paper extends this research by reporting on a study which investigated the influence of contextual factors on the cognition and decision-making of five Vietnamese novice EFL teachers in using the national English language textbook series. This textbook series, in line with Vietnam’s language policy, is underpinned by Communicative Language Teaching (CLT) principles. Data were collected through interviews, classroom observations and stimulated recall interviews with teachers from two high schools in Vietnam. The results indicate that the teachers modified the textbook activities in ways that limited opportunities for student oral interactions in English. The teachers explained their more conservative practices as a response to their perceptions of their students’ limited motivations and capacities. As an area of research that has received little investigation in the EFL literature, the teachers’ prioritization of their students’ capacities and motivations beyond other considerations, despite their own enthusiasm for a more communicative approach, is an important finding.

Objective Hip fractures are associated with significant morbidity and mortality. Ultrasound-guided peripheral nerve blocks are a safe method to manage pain and decrease opioid usage. The pericapsular nerve group (PENG) block is a novel, potentially superior block because of its motor-sparing effects. Through training, simulation, and supervision, we aim to determine whether it is feasible to perform the PENG block in the emergency department. Methods Phase 1 consisted of emergency physicians attending a workshop to demonstrate ultrasound proficiency, anatomical understanding, and procedural competency using a low-fidelity model. Phase 2 consisted of a prospective, observational, feasibility study of 10 patients with hip fractures. Pain scores, side effects, and opioid usage data were collected. Results The median pain score at time 0 (time of block) was 9 (interquartile range [IQR], 6.5–9). The median pain score at 30 minutes was 4 (IQR, 2.0–6.8) and 3.5 (IQR, 1.0–4.8) at 4 hours. All 10 patients required narcotics prior to the initiation of the PENG block with a median dosage of 6.25 morphine milligram equivalents (MME; IQR, 4.25–7.38 MME). After the PENG block, only 30% of the patients required further narcotics with a median dosage of 0 MME (IQR, 0–0.6 MME) until operative fixation. Conclusion In this feasibility study, PENG blocks were safely administered by trained emergency physicians under supervision. We demonstrated data suggesting a trend of pain relief and decreased opiate requirements, and further investigation is necessary to measure efficacy.

The current study was focused on the induction of apoptotic effects of resveratrol along with the combination treatments of nutlin-3 and transforming growth factor-β (TGF-β) against the human ovarian cancer cell line A2780/CP70. To determine the extent of apoptosis following the above-mentioned treatments, we assessed the execution of apoptotic events that proceed via caspase activation and cytochrome c release. We estimated the caspase-3 and -9 activities using a direct enzymatic assay that measures the cleavage of synthetic peptide substrate (N-acetyl-Asp-Glu-Val-Asp-p-nitroanilide). Our experiments showed an increase in caspase-3 and -9 activities in the cells that were treated with the combination of resveratrol (5μM) with nutlin-3 (5μM) or TGF-β (1μg/mL). Since activation of procaspase-3 by caspase-9 requires the release of cytochrome c into the cytoplasm, we measured the levels of cytochrome c in the cytoplasm by western blot experiments. The data indicated a considerable increase in caspase-3 and cytochrome c levels when cells were treated with drugs for 24 hours. Experiments with 4,6′-diamino-2-phenylindole dihydrochloride (DAPI) staining also confirmed the induction of apoptosis in all the above-mentioned treatments done at 24 and 48 hours.
