The main idea behind this study is to fabricate a cost-effective and eco-friendly source of electrical energy for battery-less IOT devices, sensors and small medical/electronic wearable devices, as per the UN’s sustainable development goal SDG 7 (affordable and clean energy). In this study, an electrical energy generation device triboelectric nanogenerator (TENG) is designed and fabricated from fish fin, which is biocompatible, non-toxic, and treated as waste material in the fish market. The fish fin isolated from the waste of the fish is converted to the dielectric layer of the TENG, which is further utilized to convert ambient mechanical energy into useful electrical energy. The thermal stability of the material also has been tested using TGA, and it was observed that it was stable to work up to 100°C. Other biomaterial combinations of egg shell membrane, dog hairs, and tree cotton are also tested in this study. The output of the fish fin based fabricated TENG is utilized to power up 65 green LEDs connected in the series, which is equivalent to 130 V of voltage, and the current of 1.1µA was measured across a 1MΩ resistor. This output of the TENG was used to power up a small digital watch. The fabricated device can also be used as sensor to detect human motion.

<P>A supramolecular self-assembly of finite tapes of lattice water molecules with the coordinated water molecules of hydrated metal ions forms 2D cationic layers of water molecules in the coordination complex [Ni(H<SUB>2</SUB>O)<SUB>6</SUB>][Ni(Pydi)<SUB>2</SUB>(H<SUB>2</SUB>O)<SUB>2</SUB>]·4H<SUB>2</SUB>O (Pydi = 2,5-pyridinedicarboxylate). It reveals yet another unique mode of the cooperative association of water molecules. Objective Hip fractures are associated with significant morbidity and mortality. Ultrasound-guided peripheral nerve blocks are a safe method to manage pain and decrease opioid usage. The pericapsular nerve group (PENG) block is a novel, potentially superior block because of its motor-sparing effects. Through training, simulation, and supervision, we aim to determine whether it is feasible to perform the PENG block in the emergency department. Methods Phase 1 consisted of emergency physicians attending a workshop to demonstrate ultrasound proficiency, anatomical understanding, and procedural competency using a low-fidelity model. Phase 2 consisted of a prospective, observational, feasibility study of 10 patients with hip fractures. Pain scores, side effects, and opioid usage data were collected. Results The median pain score at time 0 (time of block) was 9 (interquartile range [IQR], 6.5–9). The median pain score at 30 minutes was 4 (IQR, 2.0–6.8) and 3.5 (IQR, 1.0–4.8) at 4 hours. All 10 patients required narcotics prior to the initiation of the PENG block with a median dosage of 6.25 morphine milligram equivalents (MME; IQR, 4.25–7.38 MME). After the PENG block, only 30% of the patients required further narcotics with a median dosage of 0 MME (IQR, 0–0.6 MME) until operative fixation. Conclusion In this feasibility study, PENG blocks were safely administered by trained emergency physicians under supervision. We demonstrated data suggesting a trend of pain relief and decreased opiate requirements, and further investigation is necessary to measure efficacy.
Fibrolamellar variant of hepatocellular carcinoma presenting during pregnancy

The Fibrolamellar variant of Hepatocellular Carcinoma (FLHCC) is a rare form of liver cancer that presents in the 3<SUP>rd</SUP> decade of life, is rarely associated with cirrhosis or chronic Hepatitis B/C virus infection, and usually presents with normal serum alpha-fetoprotein (AFP) levels. FLHCC presenting during pregnancy is extremely rare, with only 4 cases reported. We present a case of FLHCC in pregnancy and discuss the dilemmas in management. A 26 year-old primigravida, 26 weeks of gestation presented with a month’s history of obstructive jaundice secondary. Investigations revealed a mass in the left lateral segment of the liver with extension down the left hepatic duct into the common bile duct. Following an emergency caesarean section at 31 weeks, she underwent a left hepatectomy with extrahepatic bile duct excision. The postoperative course was uneventful. Histopathology showed FLHCC. In conclusion, liver tumors presenting during pregnancy should be managed in a multidisciplinary setup with facilities for neonatal intensive care. Management depends on the presumed pathology, period of gestation and family preferences.
