소아에서 편도절제술 시행시 진통효과 : Tramadol과 Meperidine의 비교

Background: Pain is major problem regarding quality of life in children undergoing tonsillectomy. Preemptive analgesia by medicine given before commencement of surgery is a new recommended for relief of pain during and after operation. We compared intra-and postoperative analgesic and recovery characteristics of tramadol, meperidine given at induction of anesthesia in 40 ASA I or II children undergoing tonsillectomy with or without adenoidectomy. Materias and Methods: Fourty children aged 4-7 years undergoing tonsillectomy with or without adenoidectomy were randomly assigned to receive either tramadol 2 mg/kg (group 1) or meperidine 1 mg/kg (group 2) at induction of anesthesia. Thiopental sodium (5 mg/kg) and rocuronium (0.6 mg/kg) were used for the induction of anesthesia, and it was maintained with sevoflurane in N₂O/O₂50/50 via an endotracheal tube. Pain assessment was done by facial pain scale. Agitation scores, time to recovery of spontaneous respiration and the incidence of postoperative nausea and vomiting. Heart rate and mean arterial pressure was recorded at regular intervals. Results: Facial pain scale scores were increased in group 1 at 10 and 20 min in recovery room. There was no difference for agitation scores in the two groups. Intraoperative mean arterial pressure (15th min) were found to be higher in group 1. The time to recovery of spontaneous respiration was delayed with group 2 compared with group 1. The incidence of nausea and vomiting was not statistically differed between groups. Conclusions: Meperidine was more effective for pain relief than tramadol after tonsillectomy in children.

Individual identification is an important part in medicolegal field especially in mass disaster. At July, 27, 1989, KAL KE-803 was crashed on landing at Tripoli International Airport, Liba. The plane was caught in fire and sixty eight Koreans were sacrified. The majority of victims were severely charred and injured. The authors examed all dead bodies and successfully identified all the cases through visual, anthropological, odontological, radiological and pathological methods including fingerprint and blood typing.

The well dispersed ZAO nanoparticle with 25?30 nm in diameter was prepared by an ultrasonictemplate method. The morphologies, structures, and photoelectricity of the ZAO nanoparticles were analyzed by TEM, XRD, UV-Vis, etc. The thermochemistry behaviors of the precursor were studied by TG-DSC. The composite ¯lm with semipermeable structure made from ZAO nanoparticles and collodion can be explored ¯rstly. The photocatalytic properties of the ZAO nanoparticles and composite ¯lm were investigated. The composite ¯lm can be applied in the sewage treatment ¯eld.

Background: Panax ginseng Meyer (P. ginseng), a herb distributed in Korea, China and Japan, exerts benefits on diverse inflammatory conditions. However, the underlying mechanism and active ingredients remains largely unclear. Herein, we aimed to explore the active ingredients of P. ginseng against inflammation and elucidate underlying mechanisms. Methods: Inflammation model was constructed by lipopolysaccharide (LPS) in C57BL/6 mice and RAW264.7 macrophages. Molecular docking, molecular dynamics, surface plasmon resonance imaging (SPRi) and immunofluorescence were utilized to predict active component. Results: P. ginseng significantly inhibited LPS-induced lung injury and the expression of proinflammatory factors, including TNF-a, IL-6 and IL-1b. Additionally, P. ginseng blocked fluorescence-labeled LPS (LPS488) binding to the membranes of RAW264.7 macrophages, the phosphorylation of nuclear factor-kB (NF-kB) and mitogen-activated protein kinases (MAPKs). Furthermore, molecular docking demonstrated that ginsenoside Ro (GRo) docked into the LPS binding site of toll like receptor 4 (TLR4)/myeloid differentiation factor 2 (MD2) complex. Molecular dynamic simulations showed that the MD2-GRo binding conformation was stable. SPRi demonstrated an excellent interaction between TLR4/MD2 complex and GRo (KD value of 1.16 × 10<SUP>-9</SUP> M). GRo significantly inhibited LPS488 binding to cell membranes. Further studies showed that GRo markedly suppressed LPS-triggered lung injury, the transcription and secretion levels of TNF-α, IL-6 and IL-1β. Moreover, the phosphorylation of NF-kB and MAPKs as well as the p65 subunit nuclear translocation were inhibited by GRo dose-dependently. Conclusion: Our results suggest that GRo exerts anti-inflammation actions by direct inhibition of TLR4 signaling pathway.

Improvement in the Image Quality and the Capacity of the Reversible Data Hiding

This paper proposes a reversible data hiding method in the spatial domain. A reversible data hiding method not only extracts hidden data from a stego image that conveys data but also restores the original image. The proposed method modulates pixel values adaptively and reversibly before an embedding process to improve the image quality of a stego image and simultaneously to increase the capacity. Simulation results show the effectiveness of the proposed method.
Application of Near Spectroscopy for Nondestructive Evaluation of Ginsenoside Content

The effective component Ginsenoside in Ginseng has been widely used to cure some hypochondriasis and be as supplementary medicines. There are many chemical analysis methods to measure the contents of Ginsenoside in Ginseng; however, all these methods have some shortcomings such as long time, environmental pollution and damaging the samples. In this paper, it is possible to use near infrared spectroscopy to measure the content of Ginsenoside in Ginseng without destruction. As the results, Rg1, Rb1, Re and T-Saponin of Ginsenoside can be measured with the accuracy of R(0.81) SEP (1.704 mg/g), R(0.74) SEP (1.211 mg/g), R (0.78) SEP (1.049 mg/g) and R(0.84), SEP(4.537 mg/g).

An electric scooter with a V-belt continuously variable transmission (CVT) driven by a permanent magnet synchronous motor (PMSM) has a lot of nonlinear and time-varying characteristics, and accurate dynamic models are difficult to establish for linear controller designs. A PMSM servo-drive electric scooter controlled by a novel hybrid modified recurrent Legendre neural network (NN) control system is proposed to solve difficulties of linear controllers under the occurrence of nonlinear load disturbances and parameters variations. Firstly, the system structure of a V-belt CVT driven electric scooter using a PMSM servo drive is established. Secondly, the novel hybrid modified recurrent Legendre NN control system, which consists of an inspector control, a modified recurrent Legendre NN control with an adaptation law, and a recouped control with an estimation law, is proposed to improve its performance. Moreover, the on-line parameter tuning method of the modified recurrent Legendre NN is derived according to the Lyapunov stability theorem and the gradient descent method. Furthermore, two optimal learning rates for the modified recurrent Legendre NN are derived to speed up the parameter convergence. Finally, comparative studies are carried out to show the effectiveness of the proposed control scheme through experimental results.

Because the wheel of V-belt continuously variable transmission (CVT) system driven by permanent magnet synchronous motor (PMSM) has much unknown nonlinear and time-varying characteristics, the better control performance design for the linear control design is a time consuming job. In order to overcome difficulties for design of the linear controllers, a hybrid recurrent Chebyshev neural network (NN) control system is proposed to control for a PMSM servo-driven V-belt CVT system under the occurrence of the lumped nonlinear load disturbances. The hybrid recurrent Chebyshev NN control system consists of an inspector control, a recurrent Chebyshev NN control with adaptive law and a recouped control. Moreover, the online parameters tuning methodology of adaptive law in the recurrent Chebyshev NN can be derived according to the Lyapunov stability theorem and the gradient descent method. Furthermore, the optimal learning rate of the parameters based on discrete-type Lyapunov function is derived to achieve fast convergence. The recurrent Chebyshev NN with fast convergence has the online learning ability to respond to the system's nonlinear and time-varying behaviors. Finally, to show the effectiveness of the proposed control scheme, comparative studies are demonstrated by experimental results.
