Lidocaine으로 Retrobulbar block時 發生한 過敏反應 1例

This report shows a fatal hyperergy occurring during retrobulbar block using lidocaine on a 42 years old male patient. When we injected 60㎎ of lidocaine into the retrobulbar space, following signs were noticed in one minute: hypertension, convulsion, dyspnea and loss of consciousness. In addition to the above symptoms, cardiac arrest developed in 2∼3 minutes and did not respond to any resuscitative effort. It is considered that 60㎎ of lidocaine for retrobulbar block can induce hyperergy and cardiac arrest. The clinical picture and treatment of such reaction is discussed with particular attention to the importance of close observation of all anesthetic patients whether under general or local anesthesia.

차세대 통신 시스템에서 공통적으로 요구하는 소형화, 광대역화, 집적화추세에 따라 RF소자는 광대역화, 소형화, 집적화를 향해 발전하고 있으며 안테나 기술도 현재 추세에 발맞추어 소형화, 능동집적화, 광대역화를 통하여 개발 방향을 모색하고 있다. 본고에서는 현재 추세인 소형 안테나, 능동 안테나와 더불어 최근 관심이 집중되고 있는 임펄스 통신에 이용될 수 있는 초광대역 안테나 기술에 대해 소개한다. The technical trends of the next generation wireless communication are miniaturization, broad-band operation, and integration. Thus, in accordance with this trend, RF devices and components are being developed in such way, and same with the antenna. In this paper, technology of small antenna, active antenna, and ultra wideband antenna, which can be applied to impulse communication system, are introduced.

In this paper, spatial amplifier using wideband printed dipole antennas is proposed for spatial power combining. The input and output printed dipole antennas have wide bandwidth of 22 %(5.44 ㎓∼6.75 ㎓) and 26.3 %(5.2 ㎓∼6.78 ㎓), respectively. Gain of the designed amplifier is 9.5 ㏈ at 6 ㎓. As a result, the proposed spatial amplifier using wideband printed dipole antennas and amplifiers shows that the gain is 16.8 ㏈ at 6 ㎓ and 3-㏈ gain bandwidth is 1.04 ㎓.

레이다의 빔패턴은 무반향챔버와 같은 특정 시설에서 측정되며, 물리적으로 큰 레이다는 운용환경에서 빔패턴 측정가능한 시설로 이동이 어렵고 비용이 많이 소모된다. 만약 레이다가 실제 환경에 존재하는 지형지물이나 천체를 이용하여 빔패턴의 특성을 확인할 수 있다면, 비용 측면에서 효과적인 레이다 운용유지 능력을 제공할 수 있다. 태양은 어느 위치에서나 상시 존재하는 천체이고, 레이다에서 열잡음 형태로 측정 가능하므로 모니터링에 유용하나, 주변 전파 환경에서 수신되는 간섭신호의 영향으로 성능 점검 자동화에 어려움이 있다. 따라서 본 논문에서는 이러한 문제를 극복하기 위해 자동 점검 기법 및 간섭 제거 방법을 적용하여 실험하고, 실제 환경에서 시험한 결과를 제시한다. A beam pattern of a radar is measured at a specific facility such as an anechoic chamber, and it is difficult and expensive to move a physically large radar from an operating environment to the facility. If a radar can measure beam pattern characteristics with landmark or celestial bodies in an operating environment, it can provide a cost effective maintenance. Sun is a celestial body which exists in an operating environment, and sun noise is easy to monitor automatically. However, interferences interfere with the automatic detection of noise. Therefore, in this study, we implemented and tested an automatic sun noise monitoring method and an effective interference suppression method in a radar operating environment.
손가락굽힘근 힘줄의 통풍결절 침착에 의해 발생한 손목터널증후군

Carpal tunnel syndrome is the most common peripheral entrapment neuropathy. We report on the first Korean case of carpal tunnel syndrome induced by tophaceous deposition in flexor digitorum tendons of a patient with chronic gout. A 63-year-old male suffered from numbness, decreased sensation over both median nerve distribution, and weakness of thenar muscle for 3 years. Physical examinations revealed positive Tinel’s sign and Phalen’s test and thenar qjmuscle atrophy was found on both hands. In nerve conduction study, there was no action potential of the sensory and motor of the bilateral median nerve. Ultrasonography showed increased cross-sectional area of median nerve due to tophaceous deposition in flexor digitorum tendons in the carpal tunnel. Dual-energy computed tomography showed diffuse multifocal green color coding tophaceous deposition within the carpal tunnel. His neuropathic symptoms improved after injection of triamcinolone into the carpal tunnel and administration of oral medication including non-steroidal anti-inflammatory drugs and colchicine. <P>While azo dyes have been widely used in dye industry, the azo dyes have been seldom applied as sensitizers to dye sensitized solar cells. In this study, new metal-free organic sensitizers, ST and AZ, which are same structures except bridging units, were synthesized and evaluated. ST containing stilbene as bridging unit gave higher energy conversion efficiency than AZ containing azo group as bridging unit. As a result of density functional theory (DFT) calculations, ST displayed more localized frontier molecular orbitals at lowest unoccupied molecular orbital (LUMO) states than AZ.</P>

To evaluate the diagnostic significance of ECG criteria for left atrial enlargement, we performed standard 12 lead ECGs and M mode echocardiogram in 75 patients. The ECGs of patients were analysed. Twenty seven of the patients had left atrial enlargement and 48 had a normal left atrial dimension as determined by echocardiography. To evaluate diagnostic significance, we used the sensitivity and specificity of ECG criteria for left atrial enlargement. The results were as follow ; 1) The single most effective electrocardiographic criteria for left atrial enlargement was criteria 2(duration of the negative phase of the P wave in lead V₁>40ms). 2) Criteria 5(notched P wave in any standard lead with peak to peak duration) had very low sensitivity, but high specificity. 3) Combining 2 or more of the electrocardiographic criteria did not substantially enhanced diagnostic accuracy more than criteria 2.
