        Neurofibromatosis Type 2와 무관하게 발생된 척추 경막내 다발성 신경초종 - 증 례 보 고 -

        Most extracranial schwannomas are solitary, but neurofibromas are frequently associated with other manietations of neurofibromatosis. Schwannomas that occur within the context of neurofibromatosis tend to be multiple, but multiple schwannomas without manifestation of neurofibromatosis type 2 are very rare. The authors report a very rare case of multiple spinal intradural schwannomas in the absence of neurofibromatosis Type 2 maniestations. A 40-year-old man suffered from longstanding low back pain and left side sciatica which was treated with two stage operations. MRI showed multiple intradural mass lesions extending from L1 vertebral segment to S1 vertebral segment. There were no clinical and radiological manifestations of Type 2 neurofibromatosis. Histologically confirmed diagnosis was schwannoma.

        아토피 피부염 환자에서 한방 추출물이 포함된 외용제품에 대한 임상 효과

        Background : Atopic dermatitis is a chronic and recurrent allergy eczema, called as 'Tae-yul'. It is a characteristic dermatitis, diagnosed from baby to adult, that carries severe itching. Objectivd : The aim of this trial is to examine the effect and safety of Medibebe in patients with atopic dermatitis. Methods : We made a comparison between before and after the use this product. Volunteers who satisfied the requirements were enrolled in the study. Degrees of severity of atopic dermatitis were measured by SCORAD index. Results : After 4 weeks' use of Medibebe, SCORAD index showed a decrease of $20.0{\pm}10.2$(95% C.I: 17.1-22.9) compared with when it was not applied, which is a statistically meaningful change(p-value<0.0001). Subjective pruritus and Sleeping loss reported after both 2 and 4 weeks' use of Medibebe decreased in a statistically meaningful way(p-value<0.0001) compared with before the application. but, 2 cases out of all 50 subjects for the clinical trial were excluded on the way. Conclusion : As a result of applying the 4 items of Medibebe(cream, lotion, soap, and body cleanser) to patients with atopic dermatitis and watching the progress, it is concluded and considered that these cleansing and moisturizing products can be used by atopic patients safely and effectively with almost no side-effect.

        3차원 수치해석을 이용한 지진 시 수직구-터널 접속부 동적 거동 분석

        3D time history analysis was performed on vertical shaft-tunnel connection to provide insight into the dynamic stress-strain behavior of the connection considering the effects of soil layers, periodic characteristics and wave direction of earthquakes. MIDAS GTS NX based on FEM (Finite Element Method) was used for this study. From this study, it is revealed that the maximum displacement occurred at the upper part of the connection when the long period seismic waves propagate through the tunnel direction in soft ground. Also, stress concentration occurs due to different behaviors of vertical shaft and tunnel, and the stress concentration could be influence for safety on the connection. The results of this study could be useful for the seismic performance design of vertical shaft-tunnel connection. 수직구와 수평 터널 주위 지층 변화, 지진파 방향성이 접속부의 동적 거동 특성에 미치는 영향을 연구하였다. 유한 요소 기반의 MIDAS GTS NX 프로그램을 이용한 3차원 시간이력 해석을 통해 검토하였다. 연구결과 연약지반에서 장주기의 지진파가 수평 터널 방향으로 발생하였을 때 접속부 상부에서 최대 변위가 발생하는 것으로 나타났다. 또한 수직구와 수평 터널의 서로 다른 거동 및 지층 경계면의 영향으로 인해 응력집중 현상이 발생하여 문제가 될 수 있음을 확인 할 수 있었다. 연구 결과는 수직구와 수평 터널의 접속부 구조물 내진설계를 위한 기초자료로 활용 될 수 있을 것으로 기대된다.

        뇌하수체졸중의 임상적 고찰

        Objectives : Pituitary apoplexy is a well-described clinical syndrome resulting from pituitary hemorrhage, hemorrhagic infarction, or infarction, almost invariably occurring in the presence of an adenoma. We analyzed pituitary apoplexy with an emphasis on clinical presentation, pathology and predisposing factors. Methods : We reviewed 35 histologically proven pituitary adenomas, operated from January 1995 to August 1999, to select 8 cases which showed clinical or operative findings compatible with pituitary apoplexy. These patients were analyzed in terms of symptom and sign, hormonal status, and predisposing factors, pathologic findings. Results : Among 35 surgically treated tumors of the pituitary gland, 8 cases(23%) were diagnosed as pituitary apoplexy. The pathologic findings revealed hemorrhage(7 cases) and infarction(1 case) of pituitary adenomas. One case had predisposing factor of appendectomy. The most common presenting symptom and sign were sudden severe headache and visual disturbance. Conclusion : We treated pituitary apoplexy surgically and obtained good outcomes. Pituitary apoplexy due to massive infarction of the pituitary gland is very rare condition but surgical treatment by trans-spheniodal surgery showed a good result.

        개방형 무선 통신망 구조를 위한 복합 보안 프레임워크 설계

        다음은 요약문 입니다. 여러 종류의 무선 통신망들에 대한 통합화된 서비스에 대한 수요가 요구되고 있다. 이러한 요구는 기존의 유선망과 무선 통신망의 통합 배치를 통하여 가능하게 되었다. 각각의 무선 통신망들은 기존이 망들에 비해 고유의 특징을 가지고 있다. 개방형 무선 통심 환경 하에서의 주요한 문제 중의 하나가 보안성에 관련된 문제이다. 개방형 환경 하에서 통합된 환경의 보안성을 실현하기 위해서는 현실에서 실제적으로 적용 가능한 통합 보안 구조를 개발하여야 한다. 따라서 본 논문에서는 이러한 요구조건을 만족하기 위해서 고려해야 하는 사항들을 분석하였다. The increasing demand of users for various wireless communication services has lead to the development with uniting co-existence of different and wireless networks. Each one of the these wireless networks has its own unique characteristics, as compared to other networks. An integrated security mechanism is one of the key issue in the open wireless network. To realize overall security mechanism in open wireless architecture, we should provide practical workable framework for the realization of the integrated security challenges. In this paper, we analysed integrated security platform and considerations based on the security concept.

        포화 압축 벤토나이트 내 기체 이동 현상 관측을 위한 기체 주입 시험

        In the disposal environment, gases can be generated at the interface between canister and buffer due to various factors such as anaerobic corrosion, radiolysis, and microbial degradation. If the gas generation rate exceeds the diffusion rate, the gas within the buffer may compress, resulting in physical damage to the buffer due to the increased pore pressure. In particular, the rapid movement of gases, known as gas breakthroughs, through the dilatancy pathway formed during this process may lead to releasing radionuclide. Therefore, understanding these gas generation and movement mechanism is essential for the safety assessment of the disposal systems. In this study, an experimental apparatus for investigating gas migration within buffer was constructed based on a literature review. Subsequently, a gas injection experiment was conducted on a compacted bentonite block made of Bentonile WRK (Clariant Ltd.) powder. The results clearly demonstrated a sharp increase in stress and pressure typically observed at the onset of gas breakthrough within the buffer. Additionally, the range of stresses induced by the swelling phenomenon of the buffer, was 4.7 to 9.1 MPa. The apparent gas entry pressure was determined to be approximately 7.8 MPa. The equipment established in this study is expected to be utilized for various experiments aimed at building a database on the initial properties of buffer and the conditions during gas injection, contributing to understanding the gas migration phenomena.

        횡복직근 유리피판을 이용한 유방재건술에서의 즉시 유두재건

        Immediate breast reconstruction in breast cancer patients is universalized and now with a wide variety of methods to choose from, we can select a breast reconstruction method according to the patient's condition. Among these methods, immediate breast reconstruction with TRAM free flap is the most commonly used. Nipple reconstruction is usually performed as a secondary procedure, reconstructed. Nipple is reconstructed with contralateral nipple composite graft or with local flap. Areola is reconstructed with skin graft and tattooing. Therefore, to reconstruct complete breast, two or more staged operations are needed and are troublesome to both the surgeon and the patient. If we could reconstruct breast mound and nipple at same time, we would reduce the operative stages and heighten the patient's satisfaction. The author performed delayed or immediate breast reconstruction with TRAM free flap and nipple reconstruction at the same time. If the TRAM flap was to situate in the whole of the breast or at the center of the breast mound, nipple was reconstructed with a local flap from the TRAM flap. If the TRAM flap was not situated in center of breast mound, nipple was reconstructed with a local flap from remnant breast skin. Immediate nipple reconstructions in breast reconstruction consisted total of 22 cases. Among these, delayed breast reconstruction were 5 cases and immediate breast reconstruction were 17 cases. According to patient's condition and mastectomy method, nipple reconstruction method was selected; nipple reconstruction with contralateral nipple composite graft(3 cases); nipple reconstruction with remnant breast skin(6 cases); nipple reconstruction from flap margin(10 cases); nipple reconstruction with prefabricated nipple on flap(3 cases). Malposition of the reconstructed nipple was the most common and serious complication(6 cases). The other complications were atrophy of the nipple(1 case), and necrosis(1 case). Reconstruction of the breast and nipple at the same time can reduce the need of a secondary operation and use remnant skin or redundant flap tissue maximally. On the other hand, it must be considered that position and shape of nipple could be deformed, because the nipple reconstruction is performed before the shape of reconstructed breast settles completely. Prudent attention is needed, because the danger of complication is higher than delayed nipple reconstruction.

        측파대 광 주입 락킹 기법을 이용한 밀리미터파 신호생성 및 실험적 분석

        In this paper, we have proposed sideband injection locking method, heterodyne technique, to generate millimeter-wave signal. Microwave signals in cellular broadband mobile communication networks and distributed networks can favorably be generated and distributed by optical techniques. In principle, these techniques have already been investigated for optical control of phase-array antennas, characterization of photo-detector and phase locking of millimeter-wave oscillators and now being applied to wireless communications. The generation and transmission of millimeter-wave radio signals by optical means is of interest for future pico-cell broadband mobile communication system, especially for systems operating at frequencies of tens of GHz applicable to LMDS. We experimented and analysed the generation of millimeter wave signal. 본 논문에서는 밀리미터파 대역의 신호 발생을 위한 헤테로다인 방식인 sideband injection locking 방법을 제안하고 실험적으로 분석하였다. 향후, 무선 이동통신, 초고속 광대역 통신망 등에 사용되어질 밀리미터파 신호는 전기적 요소의 한계로 광학적인 요소에 의해 신호를 생성하는 방법이 많이 연구되고 있으며 광학적 기술에 기초한 소자기술 등이 발전되고 있다. 따라서 본 논문에서는 향후 밀리미터파 대역에서 사용되어질 LMDS(Local Multi-point Distribution Service) 등의 시스템에서 수십기가 헤르츠급의 광원으로 응용 가능한 헤테로다인 방식을 이용한 밀리미터파 대역의 신호생성에 대한 이론적 방법 및 실험적 고찰을 통하여 검증하였다.

        Analyses of a Signal Traffic for Authentication in Mobile Sensor Network

        In this paper, we analyses of a traffic for authentication signaling in third generation mobile sensor network. In universal mobile telecommunication system, authentication functions are utilized to identify and authentication a mobile station and validate the service request network services. The authenticating parties are the authentication the serving general packet radio service support node access the authentication center to obtain the authentication with the mobile station. In this paper, we propose that the automatic cost-effective solution size of the authentication vector array.

        반도체 레이저의 광 주입을 이용한 혼동 통신망의 암호화 기법 분석

        We theoretically studied synchronization of chaotic oscillation in semiconductor lasers with chaotic light injection feed-back induced chaotic light generated from a master semiconductor laser was injected into a solitary slave semiconductor laser. The slave laser subsequently exhibited synchronized chaotic output for a wide parameter range with strong injection and frequency detuning within the injection locking scheme. We also analytically examined chaos synchronization based on a linear stability analysis from the view point of synchronization based on a linear stability analysis from the view point of modulation response of injection locked semiconductor lasers to chaotic light signal.
