Background: The brachial-ankle pulse wave velocity (baPWV) is a useful parameter to assess arterial stiffness. However, it is difficult to evaluate arterial stiffness in hypertensive patients because the baPWV is affected by the blood pressure itself. This study was designed to estimate the relationship between the change of the blood pressure parameters and the baPWV (ΔbaPWV) when hypertensive patients were subjected to an acute reduction of blood pressure. Methods: Thirty patients with essential hypertension and whose blood pressure was higher than 140/90 mmHg were enrolled. In all the patients, the blood pressure and baPWV were measured using an automatic waveform analyzer with the patients at a resting state. When the reduction of blood pressure was more than 10 mmHg after sublingual administration of nifedipine 10 mg, then the blood pressure and baPWV were measured again in the same manner and then they were compared with the baseline values. Spearman`s correlation and multiple linear regression tests were performed to estimate the relationship between the change of the blood pressure parameters (ΔSBP, ΔDBP, ΔMAP and ΔPP) and the ΔbaPWV. Results: The baPWV was significantly decreased shortly after the administration of nifedipine (1903.6±305.2 cm/sec vs. 1716±252.0 cm/sec, respectively, p<0.01). The ΔbaPWV was correlated with the ΔSBP (r=0.550, p<0.01), ΔDBP (r=0.386, p<0.05), ΔMAP (r=0.441, p<0.05), and ΔPP (r=0.442. p<0.05). On the multiple regression analysis, the ΔSBP was the only significant variable for predicting the ΔbaPWV, and the linear equation was ΔbaPWV=8.7×SBP-48. Conclusions: The baPWV is affected by the systolic blood pressure level to a large degree and careful attention must be paid to the blood pressure level when evaluating arterial stiffness with using the baPWV.

Topical photodynamic therapy (PDT) via topical 5-aminolevulinic acid (ALA) is potentially useful for the treatment of basal cell carcinoma. However, as a photosensitizer used in PDT, methyl aminolevulinate (MAL) can replace ALA, because MAL has more advantages than ALA. We treated two patients with basal cell carcinoma using MAL-PDT. Topical MAL-PDT may be a safe and effective treatment modality for basal cell carcinoma. (Korean J Dermatol 2008;46(6):796∼799)

Actinic cheilitis is a pathologic condition affecting mainly the lower lip caused by long term exposure of the lips to UV radiation in sunlight. Analogous to actinic keratosis of the skin, actinic cheilitis is considered as a precancerous lesion and may develop into squamous cell carcinoma. Photodynamic therapy (PDT) is a treatment modality involving the use of a photosensitizing agent, oxygen, and light of a specific wavelength to produce controlled cell death. Photodynamic therapy (PDT) using the new highly selective photosensitizer methyl aminolevulinate (MAL) (Metvix(R), Galderma, U.K.) is a promising new treatment modality for premalignant and malignant skin lesions. We report a case of actinic cheilitis treated with PDT using MAL, with satisfactory outcome in both clinical and pathological aspects. (Korean J Dermatol 2008;46(6):835∼838)
항공사진 분석을 통한 산불 피해지 임분 변화 추정(Ⅱ)

산불 피해지의 임분 변화 상태를 구명하기 위하여 동해안 납부 지역 중 과거 1980년대와 1990년대에 산불이 발생된 강원도 삼척, 경상북도 포항, 경상남도 울산 및 울주 등 4개소에 표본점을 설치하여 1974년, 1980년, 1992년, 2000년에 각각 촬영된 항공사진을 활용하여 임분 구성 인자들을 판독하였다. 이를 통해 산불 발생 전 · 후의 임상별 면적을 추정하고 임분고와 수관 밀도 등의 임분인자를 기 조제된 항공사진 임분 재적표에 적용, 산불 발생에 다른 축적 변화를 추정하였다. 임상별 면적변화는, 삼척지역은 침엽수립에서 활엽수림으로 임상이 변화하였고, 포항, 울산, 울주 지역은 침엽수림과 혼효림의 임상이 침엽수림, 활엽수림, 혼효림으로 세분화되었다. 임목축적의 경우 삼척의 리기다소나무 재생임분은 강원도 평균 축적과 비슷하게 나타났으나 포항 · 울산 · 울주 지역의 임상별 축적은 해당 조사구의 행정구역 평균축적보다 낮게 나타났다. 이는 산불피해 이후, 임분의 생장상태가 떨어지므로 임상의 축적에도 영향을 미치는 것으로 판단되었다. This study was conducted to understand changes of stand area and growing stock at fire-damaged forest stands in the southern paπ of East coast region. Sample points were set up at Samchuck(1 region) in Kangwon-do province, Pohang(1 region) in Kyungsangbuk-do province, Ulsan(1 region) and Ulju(1 region) in Kyungsangnam-do province where forest fire had been occurred in 1980’s and 1990’s. Forest stand components were interpreted using aerial photos that had been taken in the years of 1974, 1980, 1992 and 2000. Conifer stands before forest fire have been changes to broad leaves stands in Samchuck area. In the other areas forest following fire develop to conifer stand, broad leaved stand and mixed stand by site characteristics. Growing stocks of Pinus thunbergii stands are similar to the mean of growing stocks of the Pinus thunbergii stands in Kangwon-do province. However growing stocks in Pohang, Ulsan and Ulju area are lower than mean of growing stock of common forest in each city. It means that forest fire affact to growing stock by lower stand growth.
눈꺼풀테회전술과 눈꺼풀성형을 이용한 반흔성 눈꺼풀속말림의 치료

Purpose: To report the efficacy of tarsal marginal rotation with blepharoplasty in the management of cicatricial entropion of the upper eyelid. Methods: In 9 consecutive patients, 14 eyelids with moderate cicatricial entropion of the upper eyelid were enrolled in this study. All 14 eyelids underwent tarsal margin rotation with blepharoplasty. In cases where one eyelid was involved, the fellow eyelid underwent upperlid blepharoplasty at the same time. The mean follow-up period was 17.7 months (range, 11 to 53 months). Results: In all 14 upper eyelids, the eyelashes rotated away from the surface of the eye postoperatively. There was not recurrence in 13 eyelids and the effect of correction was maintained for the follow-up period. However, in one eyelid the cicatricial entropion recurred after months. Tarsal margin rotation was repeated and there was no recurrence for 6 months. Conclusions: Tarsal margin rotation with blepharoplasty appears to be effective in managing cicatricial entropion of the upper eyelid in both functional and cosmetic aspects.
