The invasive fungal infections are frequent in patients with neoplastic disease and result in a significant morbidity and mortality. In the last decade, the progress in antineoplastic chemotherapy and the more aggressive approach in the treatment of cancer patients gave rise to increase the number of patients predisposed to invasive fungal infections. With a brief reveiw of literature, we report one case of invasive aspergillosis involving lung, brain and thyroid gland, which was diagnosed by stereotaxic aspiration of brain and needle aspiration of thyroid gland in 21-year-old male patient who had taken the chemotherapeutic agents for the treatment of relapsed malignant lymphoma.

Because castration-resistant prostate cancer (CRPC) does not respond to androgen deprivation therapy and has a very poor prognosis, it is critical to identify a prognostic indicator for predicting high-risk patients who will develop CRPC. Here, we report a dataset of whole genomes from four pairs of primary prostate cancer (PC) and CRPC samples. The analysis of the paired PC and CRPC samples in the whole-genome data showed that the average number of somatic mutations per patients was 7,927 in CRPC tissues compared with primary PC tissues (range, 1,691 to 21,705). Our whole-genome sequencing data of primary PC and CRPC may be useful for understanding the genomic changes and molecular mechanisms that occur during the progression from PC to CRPC.

<P><B>BACKGROUND</B></P><P>Genomic profiling of tumors has contributed to the understanding of colorectal cancer (CRC), facilitating diagnosis, prognosis and selection of treatments, including targeted regimens. A report suggested that a 19-gene-based risk classifier (TCA19) was a prognostic tool for patients with stage III CRC. The survival outcomes in patients with stage IV CRC are still poor and appropriate selection of targeted therapies and immunotherapies is challenging.</P><P><B>AIM</B></P><P>To assess clinical implication of TCA19 in patients with stage IV CRC, and to identify TCA19 with involvement in immune-oncology.</P><P><B>METHODS</B></P><P>A retrospective review of the medical records of 60 patients with stage IV CRC was conducted, assessing clinicopathological variables and progression-free survival (PFS). TCA19 gene expression was determined by quantitative polymerase chain reaction (qPCR) in matched normal and tumor tissues taken from the study cohort. Expression of potential immune-oncology regulatory proteins and targets was examined by immunohistochemistry (IHC), western blot, immunofluorescence staining in tissues from a validation cohort of 10 patients, and in CRC cell lines co-cultured with monocyte <I>in vitro</I>.</P><P><B>RESULTS</B></P><P>In the patients with TCA19 score higher than the median, the PFS rates of eight patients who received the targeted regimens were significantly higher than the PFS rates of four patients who received 5-fluorouracil-based regimen (<I>P</I> = 0.041). In multivariate analysis, expression of signaling lymphocytic activation molecule family, member 7 (SLAMF7) and triggering receptor expressed on myeloid cells 1 (TREM1) was associated with PFS in the 60-patient cohort. After checking another 10 validate set, the expression of the IHC, the level of real-time qPCR, and the level of western blot were lower for SLAMF7 and higher for TREM7 in primary and metastatic tumors than in normal tissues. In CRC cells expressing SLAMF7 that were co-cultured with a monocytic cell line, levels of CD 68 and CD 73 were significantly lower at day 5 of co-culture than at day 0.</P><P><B>CONCLUSION</B></P><P>The TCA19 score might be prognostic for target-regimen-specific PFS in stage IV CRC. Down-regulation of SLAMF7 and up-regulation of TREM1 occur in primary and metastatic tumor tissues.</P>

<P>Our previous studies demonstrated that transplantation of human umbilical cord blood-derived mesenchymal stem cells (hUCB-MSCs) into the hippocampus of a transgenic mouse model of Alzheimer's disease (AD) reduced amyloid-관 (A관) plaques and enhanced cognitive function through paracrine action. Due to the limited life span of hUCB-MSCs after their transplantation, the extension of hUCB-MSC efficacy was essential for AD treatment. In this study, we show that repeated cisterna magna injections of hUCB-MSCs activated endogenous hippocampal neurogenesis and significantly reduced A관42 levels. To identify the paracrine factors released from the hUCB-MSCs that stimulated endogenous hippocampal neurogenesis in the dentate gyrus, we cocultured adult mouse neural stem cells (NSCs) with hUCB-MSCs and analyzed the cocultured media with cytokine arrays. Growth differentiation factor-15 (GDF-15) levels were significantly increased in the media. GDF-15 suppression in hUCB-MSCs with GDF-15 small interfering RNA reduced the proliferation of NSCs in cocultures. Conversely, recombinant GDF-15 treatment in both in vitro and in vivo enhanced hippocampal NSC proliferation and neuronal differentiation. Repeated administration of hUBC-MSCs markedly promoted the expression of synaptic vesicle markers, including synaptophysin, which are downregulated in patients with AD. In addition, in vitro synaptic activity through GDF-15 was promoted. Taken together, these results indicated that repeated cisterna magna administration of hUCB-MSCs enhanced endogenous adult hippocampal neurogenesis and synaptic activity through a paracrine factor of GDF-15, suggesting a possible role of hUCB-MSCs in future treatment strategies for AD.</P>

Background/Aims: When undergoing endoscopic submucosal dissection (ESD), patients with liver cirrhosis (LC) may suffer from a high risk of bleeding, bacteremia and tissue vulnerability. There have been few reports evaluating the effi cacy and safety of ESD in patients with LC. Methods: From January 2004 to March 2010, 23 patients with LC (cirrhosis group) underwent ESD for superficial gastric neoplastic lesions. The number of patients with a liver function in the Child-Pugh classes A and B were 20 and 3, respectively. The clinical outcomes and complications were compared with 69 patients without LC (control group) that were matched for age and sex. Results: The en bloc resection, R0 resection and en bloc plus R0 resection rates of the cirrhosis group were 82.6%, 91.3%, and 82.6%, respectively, and did not show significant differences from the rates of the control group. No local recurrence was found in either group during the follow-up period. The procedure length of time (41.0 vs 39.0 minutes), rate of bleeding (4.3% vs 7.2%) and rate of perforation (0.0% vs 1.4%) in the cirrhosis group were also comparable to the results from the control group. Conclusions: ESD was safely performed in patients with LC, and satisfactory outcomes were achieved with high en bloc and R0 resection rates for superfi cial gastric neoplastic lesions. The ANSI N13.32 recommends that a study of the angular response of a dosimeter be carried out once, although no pass/fail criterion is given for angular response. Gamma dose equivalent conversion and angular dependence factors were calculated by using MCNP code for the case of ANSI N13.32 extremity phantoms(finger and arm) at the depth of 7㎎/㎠. Those extremity dosimeters were assumed to be irradiated form both monoenergitic photons and ISO X-ray narrow beams. These calculated gamma dose equivalent conversion and angular dependence factors were compared to B. Grosswendt's result calculated by using X-ray beams. The result showed that the dose equivalent conversion factors of this study agreed well with that of B. Grosswendt for all energies within 2% except &% in the case of the low energies. In the case of angular dependence factors comparison, they agreed within 3%. It was shown that angular dependence factors of the finger phantom decreased as the horizontal angle of the phantom increased for the ISO X-ray beams less than 60keV. For the higher energy X-ray beams range they decreased slightly around 40 degree, but then increased from this energy to 90 degree.

Neutron room scattering corrections that should be made when neutron detectors are calibrated with a D₂O moderated 252Cf neutron source in the center of a calibration room are considered. Such room scattering corrections are dependent on specific neutron source type, detector type, calibration distance, and calibration room configuration. Room scattering corrections for the responses of a thermoluminescence dosimeter and two different types of spherical detectors to neutron source in the Radiation Calibration Laboratory (RCL) neutron calibration facility at the Korea Atomic Energy Research Institute(KAERl) were experimentally determined and are presented. The measured room scattering results are then compared with theoretical results calculated by predicting room scattering effects in terms of parameters related to the specific configuration. Agreement between measured and calculated scattering correction is generally about 10% for three kinds of detectors in the calibration facility.
