Objective : To report the method and efficacy of the site specific-defects repair in the endopelvic fascia and uterosacral ligament suspension for the correction of posthysterectomy vaginal vault prolapse. Methods : This study was performed in 3 patients with posthyterectomy vaginal vault prolapse who underwent the surgery for the reconstruction of endopelvic fascia and the correction of vault prolapsed vagina by using uterosacral ligament suspension at the department of Obstetrics and Gynecology in Moon-Hwa Hospital. Among them, 1 patient was treated by laparotomy, and 2 patients were treated by laparoscope. Results : The age of patients ranged from 49 to 67 years. The mean operation time was 111 minutes (105-120 minutes). The status of postoperative vagina was normal in all 3 patients, who were discharged 9 days after operation without symptoms. No complication or recurrence were found in them despite long-term follow-up for 24 months, 12 months, 7 months, respectively after operation. One patient has been treated with HRT therapy. Conclusion : The reconstruction of the defect of endopelvic fascia and uterosacral ligament fixation may be very safe, effective, and functional in the correction of vaginal vault prolapse. This study reports the treatment of vaginal vault prolapse using uterosacral ligament within Korea for the first time.

Purpose: Long-term results following bariatric surgery compared to conventional treatments has never been reported in morbidly obese Korean patients. This study aimed to evaluate the long-term efficacy of bariatric surgery in morbidly obese Korean patients compared to conventional medical treatments. Methods: In this multicenter retrospective cohort study, we reviewed 137 obese subjects between January 2008 and February 2011 with a body mass index (BMI) > 30 kg/m2 who had more than 5 years of follow-up clinical data after bariatric surgery (surgery group, n = 49) or conventional treatment (conventional treatment group, n = 88). Anthropometric data and the status of comorbidities were compared between the 2 groups. Results: The median follow-up period was 72.1 months (range 19.3–109.7 months). At the last follow-up, the surgery group showed a greater amount of total weight loss than the conventional treatment group (24.9% vs. 2.8%, P < 0.001). The prevalence of diabetes and hypertension significantly decreased in the surgery group, while the conventional treatment group showed a marked increase in these comorbidities. In the surgery group, Roux-en-Y gastric bypass and sleeve gastrectomy achieved comparable long-term weight loss (26.5% vs. 22.4%, respectively; P = 0.087). Conclusion: In the long-term, bariatric surgery achieved and maintained significantly greater weight reduction, as well as a decrease in obesity-related comorbidities, than did conventional medical therapy in morbidly obese Korean patients

Background: While physical inactivity is suggested to have a link to metabolic syndrome, the association of metabolic syndrome with physical activity in patients with COPD or restrictive lung diseases remains controversial. Objective: We aimed to explore the relationship of metabolic syndrome with physical activity according to the presence of airway diseases in Korean adult population. Methods: Using the data from Korean National Health and Nutrition Examination Survey (KNHANES) in 2008-2012, we enrolled 17,094 subjects who aged over 40 years and had spirometric data. spirometry pattern and presence of metabolic syndrome were checked for classification. Physical activity was evaluated by questionnaire. Results: After adjusting for age, gender, body mass index, smoking status, energy intake, percentage of carbohydrates in energy intake, household income, high-risk drinking frequency, and education, multivariate logistic regression analysis found that physical inactivity was significantly associated with metabolic syndrome (odd ratio [OR], 1.347; 95% confidence interval [CI], 1.187 - 1.529; P-value < 0.001). Whereas the association was also significant in subjects with normal (OR, 1.348; 95% CI, 1.167 - 1.558) and restrictive pattern of spirometry (OR, 1.412; 95% CI, 1.019 - 1.957), there was no significant association of physical inactivity with metabolic syndrome in those with obstructive airway disease (OR, 1.228; 95% CI, 0.813 - 1.856). Conclusion: The association of physical inactivity with metabolic syndrome might vary with the presence of airway disease.

Friction-type reinforcing members (FRMs) developed for the purpose of enhancing the wind-resistant performance of transmission towers are tested experimentally. The FRMs, in the middle of which slotted bolted connections (SBCs) are installed, are placed on the outside of tower legs, and provide additional damping and stiffness to the tower structure under bending deformations. First, the SBCs used in the FRMs are tested for various frictional sliding interface conditions. Second, the FRMs are installed on a 1/2 scale plane tower substructure and cyclic loading tests are conducted. Energy dissipation capacity and effects from local deformations of the FRMs and joint slips are investigated. From the test results, remarkable energy dissipation capacity, reaching to the 2.4 times of that before the installation of the FRMs, is observed. However, the local deformation of the SBC and joint slip should be prevented for more reliable design of the FRMs.

Initial data from a multicusp ion source developed for buckminsterfullerene (C(60)) cluster ion production are reported in this article. A C(60)(+) beam current of 425 nA and a C(60)(-) beam current of 200 nA are obtainable in continuous mode. Compared to prior work using electron impact ionization, the multicusp ion source provides at least two orders of magnitude increase in the extractable C(60)(+) beam current. Mass spectra for both positive and negative bismuth cluster ions generated by the multicusp ion source are also included.

This paper deals with a method for dynamic characteristic analysis considering core losses in transverse flux linear machines (TFLMs) with solid cores. This paper focuses on how to calculate the core losses of solid cores and how to apply the core losses to the dynamic simulation. The magnetic field characteristics, which are used for the core loss calculation and dynamic simulation, are calculated by using a 3-D equivalent magnetic circuit network analysis method. The accuracy of the proposed core loss calculation and dynamic simulation methods are examined by comparing the input currents of the dynamic simulation model with the measured input current of two TFLM prototypes.
